Intramuscular (also IM or im) injection is the injection of a substance directly into muscle. In medicine, it is one of several alternative methods for the administration of medications (see route of administration). Muscles have larger and more blood vessels than subcutaneous tissue and injections here usually have faster rates of absorption than subcutaneous injections or intradermal injections.Depending on the injection site, an administration is limited to between 2 and 5 milliliters of fluid. It is injected with the dominant hand using a quick, darting motion perpendicular to the patient's body at an angle between 72 and 90 degrees, as a faster injection is less painful. The needle is then stabilized with the nondominant hand while the dominant hand slides to the plunger to slowly instill the medication, as a rapid injection causes more discomfort. The CDC does not recommend the outdated practice of aspirating for blood to rule out injecting into a blood vessel. The needle is withdrawn at the same angle inserted. Using the "Z track" or zigzag technique is recommended, where the skin is pulled and held down to one side with the nondominant hand about an inch and after the needle is withdrawn the displaced skin is allowed to return to its normal position. This is to ensure that the medication does not leak back along the needle track. Gentle pressure is applied with a gauze but the site is not massaged to prevent forcing the medication into subcutaneous tissue.

Calcitonin can be used therapeutically for the treatment of hypercalcemia or osteoporosis. Quantification of calcitonin for pharmacokinetic purpose is challenging due to its fast elimination and low plasma concentration. Salmon calcitonin is structurally similar to human calcitonin but with greater potency. Recent developments in high resolution accurate mass spectrometry (HRAMS) have given bioanalytical scientists additional tools to overcome quantification challenges associated with large molecules. In this study, the use of HRAMS on the QExactive was evaluated for the quantification of calcitonin at very low pg/mL level.

Facts: Gladys Mensing took the drug metoclopramide for four years to help fight diabetic gastroparesis. She filed a lawsuit against the generic drug's manufacturers and distributors, contending that the drug gave her a severe neurological movpent disorder, tardive dyskinesia, but none of the generic drug's manufacturers and distributors made any effort to include warnings on the label. Metoclopramide manufacturers Pliva Inc. and Actavis Elizabeth LLC asked for the lawsuit to be thrown out, arguing that government regulations require thp to have the same label on metoclopramide as is on its brand-name equivalent, Reglan. Reglan did not have a warning about tardive dyskinesia while Mensing was taking metoclopramide. A federal judge on the U.S. District Court for the District of Minnesota agreed, saying the lawsuit was pre-ppted by the federal regulations requiring the two labels to match. But the U.S. Court of Appeals for the 8th Circuit overturned that ruling, holding that more should have been done to warn consumers about possible risks. Question: Can generic drug makers be sued for not warning about potentially dangerous side effects on their drug labels even when they follow federal rules that only require their labels to match those of their brand-name equivalents? Conclusion: No. The Supreme Court reversed the lower court order in an opinion by Justice Clarence Thomas. Justice Kennedy joined the opinion in part. "We defer to the FDA's interpretation of its CBE and generic labeling regulations," Justice Thomas wrote. Meanwhile, Justice Sonia Sotomayor dissented, joined by Justices Ruth Bader Ginsburg, Stephen Breyer and Elena Kagan. "We have traditionally held defendants claiming impossibility to a demanding standard: Until today, the mere possibility of impossibility had not been enough to establish pre-emption," Sotomayor wrote.
