Ovulation refers to the phase in a woman’s menstrual cycle during which she releases a mature egg from her ovarian follicles to into the oviduct. Women produce thousands of eggs in their lifetime that wait to be fertilised. When an egg is released from the ovary, it starts to journey through the fallopian tubes en route to the uterus, the egg can be fertilized. Once the egg leaves the ovary, it only lives for 12 to 24 hours during which time it can be fertilized by a sperm. number 2 Changes during ovulation The average menstrual cycle lasts 28-32 days. While the exact timing of ovulation can vary, it happens between day 11 and 21 of the cycle. A brain hormone called luteinizing hormone (LH) surges, triggering the release of the egg that's most ripe. At the same time, your cervical mucus changes to a slippery, egg-white consistency to help sperm make their way to the egg. number 3 Tracking ovulation Women can know when they will ovulate by tracking menstrual cycles. The first day of the last menstrual period through ovulation is called the follicular phase. The day of ovulation to the next period is called the luteal phase. The follicular phase can vary widely between women from lasting only a very short duration to one of the several weeks or even a month or more. The luteal phase is generally more consistent, usually lasting 12 to 16 days in length. Getting women with PCOS to ovulate can be tricky. Luckily, there are a number of options A common reason why women with PCOS don’t ovulate is insulin resistance. Methods to reduce insulin resistance can often get ovulation to occur. Three ways to reduce insulin resistance are weight loss, exercise and medications. The most commonly used medication in PCOS patients to reduce insulin resistance is called metformin. Fertility medications are also used to induce ovulation. Most people are familiar with a fertility pill called Clomid. However, many women with PCOS do not respond to Clomid. A better choice is another pill called letrozole. Injections of fertility medications can also be used to get ovulation to occur but be careful, there is a much greater risk for multiple pregnancy with injections. Finally, surgery on the ovary can be used to induce ovulation. This surgery is performed through laparoscopy and uses a laser or electric current to vaporize the cells in the ovary that are responsible for some of the hormone abnormalities in PCOS. One thing you might not be totally ready for: the keto diet wreaking havoc on your menstrual cycle—and possibly making your periods irregular. "The keto diet can affect women’s periods due to the rapid weight loss some women have on it," says Angela Chaudhari, M.D., gynecologist at Northwestern Memorial Hospital. "Rapid weight loss can alter your estrogen-progesterone balance and impact ovulation, therefore causing your periods to become more irregular." But period irregularity isn't your only cause for concern on the keto diet—your period could even stop altogether. "Women who lose weight rapidly or become very thin can become amenorrheic, meaning no periods, due to anovulation, or a lack of ovulation," says Chaudhari. Keep in mind, though, that this is likely due to rapid weight loss that may happen on the keto diet—but it's not exclusive to the keto diet. "This can also occur due to a very low-calorie diet, excessive exercise, or having a very low BMI," says Chaudhari. "This can cause some hormones in your brain that regulate ovulation to drop, and therefore prevent ovulation or delay ovulation leading to menstrual irregularity." Another period issue on the keto diet: the lack of carbs. "The evidence is very limited, but extreme carbohydrate restriction may lead to changes in the lutenizing hormone, a hormone released from the brain," says Chaudhari. When the body's functioning normally, the lutenizing hormone contributes to regular ovulation, says Chaudhari; but when carbs are restricted—and the lutenizing hormone is compromised—it may cause amenorrhea or hypomenorrhea (short, light periods). Is it all bad news? Can the keto do anything good for my cycle? Actually, it might: The keto diet may be helpful to women with polycystic ovarian syndrome (PCOS), a hormonal disorder that prevents the ovaries from developing or releasing eggs, according to the Office on Women's Health. That's because, with PCOS, excess insulin in a woman's body also increases her androgen and testosterone levels, which then limits her estrogen production and the body's ability to ovulate. The keto diet, however, can help decrease inflammation and insulin resistance, which can be to blame for higher than usual insulin levels seen in PCOS patients, says Alwahab. Losing weight on the keto diet can also help PCOS symptoms. And instead of focusing on a specific diet, like the keto diet, it's most important to find a sustainable and healthy diet that helps you maintain a healthy weight—and ultimately, a stable period, says Chaudhari. Polycystic ovary syndrome, or PCOS, is a common HORMONAL disorder affecting about 10% of all women of reproductive age. PCOS is diagnosed when AT LEAST 2 of the following symptoms are present: - IRregular periods due to MISSED ovulation. - excess male hormone (androgen) as evidenced by lab tests or physical signs, such as excess facial and body, severe acne, and baldness. - presence of numerous small fluid-filled cysts in the ovaries which can be seen as dark circles on an ultrasound image. This is the symptom that originally gave the condition its name but is NOT always present in PCOS patients. PCOS is highly heritable, but the inheritance pattern is complex, with multiple genetic factors implicated in the susceptibility to the disease. While the exact cause of PCOS is unknown, disturbances in a number of hormones are thought to be responsible. PCOS patients usually have EXCESS luteinizing hormone, LH, together with a relatively LOW level of follicle-stimulating hormone, FSH, and increased levels of insulin. An ovary contains hundreds of thousands of IMMATURE eggs, each of these is enclosed in a structure called a follicle. Each month, a number of these follicles develop, compete with each other; and one of them survives and gives rise to a MATURE egg that is released during ovulation. Follicle development is mediated by FSH, a pituitary hormone. In PCOS patients, FSH deficiency results in ARREST of follicular maturation: the follicles stop halfway through their development and become cysts. IMPAIRED follicular development means NO mature egg produced or released, hence the ABSENCE of ovulation. Insulin is a hormone produced by the pancreas and is necessary for consumption of blood glucose by the body’s cells. INcreased insulin level in PCOS patients is a result of the body compensatory response to insulin RESISTANCE associated with PCOS. Excess insulin, together with high levels of luteinizing hormone, induce and maintain overproduction of androgen by the ovaries. Common complications of PCOS include: infertility, miscarriage or premature birth, type 2 diabetes, obesity, cardiovascular diseases, mood disorders, and endometrial cancer. While the choice of treatment may depend on the patient’s individual concerns, treating insulin resistance is generally recommended for all women with PCOS. Life style changes such as exercise, dieting and weight loss, and medications such as metformin, can LOWER both insulin and androgen levels, thus reducing the risks of type 2 diabetes, and improving ovulation. Patients who want to get pregnant may also be prescribed anti-estrogen medications such as clomiphene. On the other hand, when fertility is not the goal of treatment, hormonal birth control, a combination of estrogen and progestin, is usually prescribed to regulate the menstrual cycle and reduce risks of endometrial cancer.

Estrogen is produced by your ovaries all month long, but can also be produced by your fat cells. You can enter early perimenopause in your 30’s. In this stage, it’s not unusual to skip ovulation a few times a year. The missed ovulation means there is less progesterone produced, which leads to a condition called estrogen dominance. A symptom of estrogen dominance is weight gain in the hips and thighs. Menopause and Belly Fat As you age, your period becomes increasingly irregular, and then stops when you reach menopause. At and beyond menopause, both estrogen and progesterone are low. Your body tries to get more estrogen from your fat cells by encouraging fat production. In this time, you would want to support with more cruciferous vegetables or iodine (sea kelp). Most women crave just before their period called PMS (pre-menstrual syndrome) and other emotional issue like moodiness, etc. In this phase they need more B-vitamins (nutritional yeast), vitamin D and vitamin C. B1 is the most important. But make sure you get a food based vitamin C. During the cycle itself if the cramps are very strong - get calcium orotate. Just after your cycle, you need more iron, which red meat will provide. Kale and spinach will help. If you have gone through menopause, then you need more vitamin E because the pituitary gland uses this vitamin for the production of sex hormones. raw sunflower seeds, almonds, leafy greens all provide a good amount of vitamin E. If you are low in estrogen, use boron (trace mineral) to increase estrogen. Basically, Polycystic Ovary is an ovulation, and the hyperinsulinemia. So, basically, most of the cycles in a polycystic ovary, there is no formation of the egg, there is no ovulation happening. So, now here the patient has already taken 3 cycles of oral contraceptive pills, so hormones have already been taken. So, we hope that the patient is having a good cycle or hormonal levels, and post hormone she is on metformin. The germ of metformin is into increase the chances of ovulation, decrease hyperinsulinemia, decrease hyperandrogenism. Basically, it increases the level of ovulation for PCOD patients. So, now this patient who is on pills for 3 months have got a regular cycle, following which patient is on metformin, so the chances of pregnancy coming up quite early is definitely there, but, no fix dates can be told 1, 2, 3 months that metformin can cause. But, Metformin definitely increases chances of ovulation and pregnancy.
