Similarly, during pregnancy, the female body naturally produces excess estrogen which can also result in the formation of fibroids. 2) Symptoms of Uterine Fibroids : Excessive bleeding is the classic symptom of fibroid tumors. Heavy bleeding is observed during all the 4-5 days of menstruation. It is also seen that the periods last longer than usual. Anemia can develop due to excess blood loss. Sometimes, bleeding or spotting is also observed between two menstrual cycles. * Abdominal pain * Swelling of the abdomen * Backache * Pain in hips In some cases, infertility can be one of the signs or complications of fibroid tumor. Presence of fibroid does not directly affect pregnancy but can cause problems in it. If the fibroid is located in the uterus, it can occupy the space meant for the baby. On the other hand, fibroids developed in other parts of the female reproductive system can even cause an obstacle in the path of the fertilized egg reaching its proper place. Most of the time, fibroid tumors are small in size and disappear at menopause. However, they need to be treated if they start causing problems. 3) Types of Uterine Fibroids : There are three main types known: * Submucous Fibroids : These grow just under the endometrium or the lining of the uterus usually causing menstrual problems, which include pain as they develop and move about the pelvic region. * Intramural Fibroids : This is a round-shaped fibroid which usually grows in the wall of the uterus. As it grows, it results in the enlargement of the uterus. * Subserosal Fibroids : This occurs on the outer region of the uterine wall and generally has no symptoms. It is only when they grow and interfere with other organs, that they create problems. In case the fibroid tumor has a pedicle or stalk, which is attached to the uterus wall, it is known as a pedunculated fibroid tumor. The pedicle allows the tumor to move about in the uterus, abdominal cavity, or even into the vagina. 4) Treatments of Uterine Fibroids : The location and size of the fibroid tumor and the age of the woman play an important role in deciding the treatment method to be adopted for fibroid tumors. If the fibroid develops during the time of menopause (between 43-45 years of age), there is hardly any necessity to treat it as it disappears after menopause. On the other hand, fibroids that are very small in size do not usually need emergency treatment. They can be treated with medications. On the other hand, certain fibroid tumors are large in size and are located at complicated places. Therefore, they need to be treated with the help of surgical procedures. * Myomectomy is the surgical procedure that is used for the removal of tumors. Although there are chances of blood loss, this is an effective technique. It is effective in treating multiple fibroids. If a woman develops a fibroid at an early age, there are higher chances of tumor regrowth. However, multiple Myomectomy can lead to problems and complications. * Another surgery to treat fibroid tumors is hysterectomy. It is the complete removal of the uterus. Hysterectomy is undertaken only when the tumor creates problems with other organs due to its large size. Hysterectomy is also considered as an effective treatment for fibroid tumors. Big Ovarian Cyst - 8cm - Ooforectomy - Ligasure
Present video demonstrates a ooforectomy indicated by the presence of a large ovarian cyst on the left ovary. Patient had 62 years and was complaing of pain in the lower left abdomen for about one year. Ultrasound showed a large cyst on the left ovary. Laparoscopy was indicated and it was performed an ooforectomy of the left ovary. During the surgery, it was noted that the cyst was blocked by mesoconlon of the colon sigmoid and was inlaid by the peritoneum. All mobilization of the ovary was made using ligasure device. No important bleeding was observed. The movie showed how the cyst was emptied using external puncture to reduce it's size and allowing their removal through a small incision. The robotic laparoscopy da vinci si system can be used in the management of early stage uterine cancer, benign gynecologic tumors or conditions, and in the management of early stage ovarian and cervical cancer. This allows the patient to avoid the need for a classical abdominal wall inscision. Laparoscopy may be performed using an 8 mm inscision in the belly button using the da vinci robotic system. This allows the surgery to be completed in under 90 minutes, and patients may often be discharged the same day ( outpatient ). Though not all patients are candidates, most may be considered for this surgical approach and evaluated preoperatively for this newer technology. When utilized, the robotic da vinci laparoscopy system is 'docked' to the patient prior to initiation of surgery, and is depicted here, requiring approximately 5 minutes to setup the system for use.

TITLE: Laparoscopic Modified Radical Hysterectomy and Staging for Uterine Papillary Serous Carcinoma with Cervical Involvement - Objective: To illustrate a case demonstration of laparoscopic modified radical hysterectomy in a patient with uterine papillary serous carcinoma involving the endocervical canal. Methods: This 47 year-old, para-3 woman with no family history of cancer and 3 prior cesarean deliveries presented with postmenopausal bleeding. Endometrial biopsy revealed a mixed poorly differentiated papillary serous and endometrioid carcinoma. On initial examination, a cervical lesion was noted and endocervical curettage was positive for a poorly differentiated adenocarcinoma with squamoid features. Due to the endocervical involvement, modified radical hysterectomy was recommended. All options for management were reviewed with the patient, and she consented to laparoscopic surgical staging. Results: She underwent laparoscopic modified radical hysterectomy, bilateral salpingo-oophorectomy, bilateral pelvic and paraaortic lymphadenectomy, and omentectomy without complication. Her total operative time was 330 minutes with a total blood loss of 100cc. She was discharged on postoperative day 3 and had postoperative urinary retention that was treated conservatively until she could void appropriately on postoperative day 14. Final pathology revealed a stage IIIC uterine papillary serous carcinoma. She underwent postoperative chemotherapy with Taxol, Adriamycin, and Cisplatin. She is currently with no evidence of disease at 10 months follow-up. Conclusions: Laparoscopic modified radical hysterectomy with staging as described is a feasible treatment approach and alternative to laparotomy for patients with advanced uterine papillary serous carcinoma. Woman with Giant Ovarian Cyst weighing as 10 BABIES thought to be World’s Largest removed whole
A woman with a cyst weighing the same as ten newborn babies has finally had the five stone tumour removed to save her life. The unnamed 24-year-old, from Mexico, noticed she was gaining weight despite being on a diet. But scans later revealed she was suffering from an ovarian cyst which grew ferociously over 11 months. By the time of surgery it had a density of five stone and put her at risk of heart failure as it crushed her internal organs, meaning she had problems walking, breathing and eating. Last year, Dr. Erik Hanson Viana at Mexico's General Hospital performed pioneering surgery to remove the cyst - believed to be the biggest-ever removed whole. Now, more than six months on, the patient has regained mobility and returned to a normal life. Dr. Hanson, from Mexico City, said: 'This is the biggest cyst I have operated on, it was so large that it took up 95% of the patient's abdomen. It's really rare to operate on a cyst this big, it measured half a metre in diameter and the circumference was 157 cm. 'From all the research I've found, this is the largest ovarian cyst ever to be removed whole without draining it first or spilling it. 'The largest in history weighed 320lb back in 1902, and took several days to drain, I can't even imagine one that large.' He added: 'The patient was quite overweight, so after starting on a diet, she noticed her face, arms and legs were getting thinner but her stomach was getting bigger. 'When I met her, she could just about walk a couple of steps before getting agitated and feeling like she was struggling to breathe because the cyst was crushing her lungs. 'Some experts say a giant cyst is anything bigger than 10cm, others that it has to weigh over 26lb (12kg) or that it has to be higher than the umbilical of the patient - this cyst was much bigger than all of them. 'If we hadn't operated it would have gotten to the point where she wouldn't have been able to walk, would be severely malnourished and unable to eat. It's hard to say how long she would have lived, but it would have been a really bad quality of life. 'Two days after surgery she walked out of hospital. Before she was used to carrying 73 lb (33kg) inside her because of the cyst, so movement was like she was levitating or walking on the moon. 'When we saw her six months after surgery to see how the scar was healing and monitor for other problems, she was fully healed. She was standing completely straight. She now walks like a normal person without needing a walking stick or other mobility aids and her abdomen is now flat.' The patient was referred to Mexico General Hospital as a last resort due to the concerning size of the cyst and complicated surgery needed to remove it. Dr. Hanson said: 'Since the cyst had become so large, it had started to compress her intestines and was preventing her from eating. She would be full after eating a tiny bit of food and because of the cyst's size, it was harder for food to pass from the stomach. 'The cyst was also compressing the two main vessels in the body since is a vain and an artery, which could have led to serious heart problems.' Due to its size, Dr. Erik Hanson and Dr. Abel Jalife removed the tumour whole fearing that draining the cyst could put the patient's life at risk. Up to 20% of giant ovarian cysts are malignant, meaning that if the cyst was pierced it risked leaking tumorous cells into the patient's body. 'Draining beforehand can be dangerous, if any of the fluid spilled it risked contaminating the patient's abdominal wall and spreading tumour cells,' he added. 'But to remove it whole is also difficult because the cyst wall is less than one millimetre thin and needs to be prevented from drying out, otherwise it could easily break too.' 'There's no real consensus in the medical community about which is the best technique to operate on giant cysts.' The patient chose to take out the ovarian cyst completely rather than draining it first, despite knowing she would be left with a lengthy scar. 'She went for the open procedure knowing there would be a big scar from her chest to her pubic area, she wasn't very concerned about the aesthetics and was happy. 'She had bad stretchmarks on her abdomen and the skin on her abdomen was wrinkled, but now the abdominal wall has returned to the place it should have.' The woman has now returned to her normal life in a rural part of Mexico, living a day's travel away from Mexico General Hospital. Dr. Hanson said: 'A big part of what we do is the gratification you feel when you could change a person's life completely, and knowing they would never forget you, even if they forget to say thank you.' Endometrial cancers are the most common gynecologic cancers in the United States, The most common subtype, endometrioid adenocarcinoma, typically occurs within a few decades of menopause, is associated with excessive estrogen exposure, often develops in the setting of endometrial hyperplasia, and presents most often with vaginal bleeding. Endometrial carcinoma is the third most common cause of gynecologic cancer death (behind ovarian and cervical cancer). 12:50 – Lily - A few years ago I got a yeast infection, it seemed to pass but still often reoccurs and hasn’t been the same down there.. Around that time I started getting acne, only very mild. The acne progressed and me not knowing about natural health at the time went on antibiotics. After 2 years of being on them I realised being on them wasn’t living in alignment with who I am and my beliefs for natural living and health. I came off them around 6months ago and my skin is at an all time low. Every square inch of my face is covered in acne and scars. I got an iridology scanning and show Sulfur, my eyes have had yellow around the pupil since I was a kid. I also have a a lot of digestive issues. I had anxiety and mild depression before the acne came back but now it’s debilitating and I’m even considering antidepressants because I don’t know what else to do. I know what I need to do to heal myself but everytime I try and go on a detox i always fall off. It’s almost like subconsciously I don’t want to heal, and that I want to suffer. But what does that actually mean? Hysterectomy is the removal of the uterus - the organ that holds and protects the fetus during pregnancy. Hysterectomy often also involves the removal of other parts of the reproductive system, including the ovaries - where eggs are produced - the fallopian tubes which carry the eggs to the uterus and the cervix - or neck of the uterus. There are many different reasons why a doctor may recommend this kind of surgery. In many cases, disease or the growth of abnormal tissue will lead a doctor to recommend the removal of the uterus. In some cases, unusually heavy menstrual flow and the accompanying discomfort may make hysterectomy an important treatment option for patient and physician to consider. But no matter what the reason behind it, you should be aware that the removal of the uterus and other reproductive organs is a serious step and it can mean significant changes in your life. After having a hysterectomy, you will not be able to have children and if your ovaries are removed as part of the procedure, you may even need to take medication to replace hormones that your body once produced on its own. Patient Education Company Your doctor will decide whether to make a vertical... or horizontal incision. An incision is made cutting through the skin and muscle of the abdomen. Next, the surgeon will inspect the general condition of the abdominal organs. Once the ovaries are exposed the uterus can then be separated from the bladder. Next, the fallopian tubes are tied off and cut. All arteries and veins connected to the uterus are tied off and cut as well. Now the uterus can be pulled upward. This stretches the vagina allowing the surgeon to cut the uterus free at the cervix. The surgeon closes the top of the vagina with stitches, and provides added support by attaching the ligaments that once held the uterus in place. The incision is then closed... and a drainage tube may be left inserted at the site. Finally, a sterile bandage is applied. Patient Education Company As a normal effect of hysterectomy, you will no longer menstruate and will not be able to have children. If your ovaries were removed you may be prescribed hormone replacement drugs. Fortunately, Hysterectomy surgery only rarely leads to complications. One potential complication is a persistent residual neuralgia - or pain - around the scar. It can be either localized or general. It may develop soon after surgery - or even weeks or months later. Usually it will decrease in intensity with time. But in very rare situations, it can become permanent. A more serious complication comes from accidental damage to the bladder or urinary tract during surgery. Patient Education Once you return home, you will be responsible to keeping the dressing intact and clean. As with all surgery, you should be alert for signs of infection near the incision - increased swelling, redness, bleeding or other discharge.
