If this applies to you, then you might be wondering how Medicare Supplemental coverage works while traveling. According to CMS as of January 2017 more than 515,000 Medicare providers actively receiving payments) and that large network is a real benefit to you while traveling. I’ll share with you all the details on how Medicare Supplemental coverage works while traveling in the United States, abroad, and how Medicare Supplement plan coverage differs from Medicare Advantage plans. And, make sure you read to the end of this post where I share tips on how to use your Part D drug plan while traveling. The great news about Medicare Supplement insurance is that it can travel with you across all 50 states and U.S. territories. So, all you have to remember is that a provider who accepts Medicare also accepts your Medicare Supplement plan...regardless of who your Medicare Supplement insurance company is. It’s not about the name of the Medicare Supplement insurance carrier, it’s about seeing providers in Medicare’s network. As long as the provider or facility accepts Medicare, your Medicare Supplement insurance company will pay the required costs associated with the Outline of Benefits for your Medigap plan. But can you use Medicare out of the country? Medicare is very limited in what it covers for foreign travel. And, in most cases, Medicare won’t pay for health care or supplies you get outside the United States and U.S. territories. However, there are 3 situations in which Medicare will cover health care services in a hospital setting located in Canada or Mexico. See the link at the top of this post that takes you to CMS’s four page publication that goes over those rules in detail. Medigap plans C, D, E, F, G, H, I, J, M, and N pay 80% of the billed charges for certain medically necessary emergency care needs outside the U.S. after you meet a $250 deductible for the year. Keep in mind, these Medigap policies cover foreign travel emergency care if it begins during the first 60 days of your trip, and has a lifetime limit of $50k. Medicare Supplement Plans and Medigap plans are the same type of health coverage. But, Medicare supplement plans are not the same as Medicare advantage plans. They don't work the same while traveling. One reason is because MAPDs don’t use Medicare's vast network. Medicare Advantage Plans are HMOs and PPOs and have service area networks. Variables such as how long you travel, where you travel, and the kind of care you need will impact whether or not your care will be covered while traveling or living somewhere part time. And, as for foreign travel, some Medicare Advantage Plans cover emergency care outside the U.S. but, again, you have to check the plan benefits to see what costs and rules apply. Which is completely different than Medicare Supplement plans, where you don’t have to constantly check plan benefits or your provider network. As long as the provider accepts Medicare (in the U.S. and territories), you’re covered… and your Medigap plan F, Medigap Plan G, or Medigap Plan N will pay accordingly. This offers greater peace of mind and ease of use while traveling compared to MAPD. And, let me share with you an example of how Medicare's network really came in handy for your father in law. He was visiting us before traveling out of the country and the day he was leaving for his trip, he noticed a spot on his skin that was bothering him. He wanted to have it checked before his trip. However, he was at our house and nowhere near his dermatologist. But, since he had Medicare and A Medigap plan, he was able to visit a dermatologist near my house with no network issues. Now, if he had a Medicare Advantage Plan, he would have been out of the plan's service area, and wouldn't have in network coverage. And...since it wasn't an emergency, it definitely wouldn't have been covered once he left the country. Lastly, if you have a Part D plan or are planning to get one, make sure you choose a plan that has national coverage so you can get prescriptions filled while traveling. If you use Medicare’s Plan Finder Tool, keep an eye out for the little N that indicates National Coverage. And, if you choose a plan that only provides coverage at one retail location (like the AARP Walgreens plan), then keep that in mind if you know you’ll be traveling.

Moving with Medicare  (What you need to know)

How is your move going to impact your Medicare coverage, and what do you need to do to make sure your coverage is seamless? What you need to do depends on what type of Medicare plan you have. If you have a Medicare supplement like a Plan F or G or N or any of the ten different Medigap plans, you do NEED to do anything more than provide the insurance company with your change of address. Your Medigap plan is good with any doctor or hospital that accepts Medicare from coast-to-coast so there is no requirement to change plans and moving does not create a special enrollment period for Medicare supplements. However, it is in your best interest to at least re-shop the plan when you move. Every state has a different prices for their supplement plans. If you are moving to a state with generally lower premiums this may be an opportunity to save money. If you are moving to a state with generally higher premiums you may still want to shop because your insurance company will have the right to increase your premium to be consistent with their premiums for your age group in the new state. But, just because your insurance company may have been the best value for you at your current residence does not mean it will be the best value for you when you move. So, with a Medicare supplement plan there is no requirement to do anything when you move to another state, but it is in your best interest to re-shop your plan and it cost nothing to shop. If you have a Medicare Part D prescription drug plan or a Medicare Advantage plan (which are usually HMOs or PPOs) you will be required to take action before you move to another state. Unlike the Medicare supplement, your Part D prescription drug plan or your Medicare Advantage plan are only local coverage. They are designed and priced just for your county or surrounding area. If you move outside of their local service are you create a Special Enrollment Period. Here is what you do, about 30-days prior to moving you contact your Medicare Part D insurance company or your Medicare Advantage insurance company and you inform them when you are moving and where you are moving to. If you are moving out of the local area they service, they will send you what we call a Disenrollment Letter. The Disenrollment Letter is your Golden Ticket to have a Special Enrollment period. With the Medicare Part D plan, you can re-shop your coverage to find which plan is best for you at your new zip code. You want to get into this new Part D plan within 63-days of when your existing coverage ends. Your window to change plans is between 60-calendar days before the end of your current insurance coverage and 63-days after your current insurance coverage is terminated to take action. So plan ahead. If you have a Medicare Advantage everything is identical to what I just talked about with your Medicare Part D, with one twist. You want to make the change in your coverage seamless, but there can be up to a 63-day gap between when your current coverage ends and when your new Medicare Advantage plan begins. Keep in mind that Medicare Advantage plans vary greatly from state to state. Do not assume that the Medicare Advantage plan you have will be the same in another state. It won’t. You need to look carefully at your options and make an informed decision. But here is the twist; this Special Election Period also gives you the opportunity to change from a Medicare Advantage plan to a Medicare Supplement. You can only get certain Medicare supplement plans, but you can do so without medical questions and on a guarantee issue basis. Before January 01, 2020 you will be able to select from Medicare supplement plans A, B, C, F, F-HD or K. After January 01 2020 Plan F and C may not be available for guarantee issue. Nothing is set in stone at this time, but Plans F & C may be replaced by Plan G and a new Plan G-HD. If you are watching this video in 2020 or after, check with us for which plans are available to you. So that’s it. If you are moving and have Medicare, re-shop your Medicare supplement and use a Special Enrollment period to purchase a new Medicare Part D plan, a Medicare Advantage or convert your coverage from a Medicare Advantage plan to a guarantee issue Medicare Supplement. Do not assume that your best plan or best value one state is the same in another. Your Medicare supplement BENEFITS are the same everywhere, but prices change significantly.

Competition is reducing costs in health care. That’s the argument in a recent Fortune article by Ezekiel J. Emanuel, Chair of the Department of Medical Ethics and Health Policy at the University of Pennsylvania. Exhibit A is the health insurance exchanges of the Affordable Care Act. An evaluation by Kaiser Health News of the 2015 premiums for plans in 34 states with federally operated exchanges shows that in counties with no new insurers in the market, premiums increased an average of seven percent. In those with at least one new insurer, premiums increased just one percent. Emanuel cites a previous example where increased market competition has resulted in decreased costs. Medicare Part D, which went into effect in 2006, established a competitive market for drug benefit plans. Today, there are over one thousand Part D plans, and the Congressional Budget Office reports that the program has cost $197 billion less than projected, 50 percent below original estimates. Competitive bidding for durable medical equipment, also included in the Part D law, has resulted in savings of 42 percent. This program will expand under the ACA in 2016. Part A and B are what makes up original Medicare. Part A is your hospital insurance and it helps cover you for inpatient care in the hospital, skilled nursing care, hospice care, and home health care… Part B is your medical insurance and it helps cover services from doctors, healthcare providers, outpatient care, home health care, durable medical equipment, and some preventive services… So basically part A pays for your room and board at a hospital and part B pays for your doctors and the equipment they use… Part C or Medicare Advantage is not part of original Medicare and is run by Medicare-approved private insurance companies. This plan replacing your Medicare parts A and B with a private insurance plan, and it must at least cover what Medicare parts A and B cover. Some plans even offer prescription drug coverage along with other benefits… Medicare Part D or Medicare prescription drug coverage helps cover the costs of prescription drugs. You can no longer appeal your benefits to Medicare. Medicare is out of the picture as far as your personal service in concerned. If you decide to appeal an insurance decision, you must appeal to the insurance company. 2. Medicare Advantage plans are typically HMOs or PPOs and on occasion a Private-Fee-for-Service plan. HMO stands for Health Maintenance Organization. PPO stands for Preferred Provider Organization. We will talk more about both of those in a minute. What is important to understand is that Medicare Advantage plans are not standardized, they are regulated to meet a certain minimum coverage. That is an important concept, so let me explain further. Medicare Part A & B is standardized. Medicare Supplement plans are standardized. Everyone in those plans receives the exact same benefits, they can see the exact same doctors and should have the exact same experience. With Medicare Advantage plans, each plan has different benefits, different co-pays, and deductibles, covers different medical services and has a different network of doctors and hospitals. Everyone’s experience is different and depends on which plan they have chosen. In additional; all the benefits, co-pays, deductibles and the doctors and hospitals in the network can and will change each and every year. It will be your responsibility to keep up with the changes via their mandatory Annual Notice of Change. It’s your responsibility to study this annual notice and determine how the changes may impact your healthcare. Medicare Advantage plans are regulated to meet a certain minimum standard. That minimum standard dictates that they must be actuarially equivalent to Medicare Parts A and Part B. That’s an interesting term “actuarially equivalent”. What does that mean? Maybe it is the annual enrollment period and you simply want to change your drug plan or switch from a Medicare advantage plan. Maybe you just are tired of running your drugs through the system only to find out the Medicare approved part D plans are all expensive. My advice is to get a broker to help you through this. Why is it that people always want to do things themselves? It doesn't cost you anything more to get a Medicare broker to walk you through this time period. Would you take Aviane or a ethinyl estradiol/levonorgestrel for your back pain; I don't think so. oyu need to get a list of Medicare Part D drug plans and compare the premiums. You also want to compare the best Medigap plan F and see if you can save money on your supplemental Medigap plan. By the end of this video Medicare will be made clear for you. You will understand how Medicare works. You will understand Medicare Part A & Medicare Part B and the choices you have to make once you sign up for Medicare. Once you sign up for Medicare you will have to choose from one of three options. We will go over each of those three options and pros and cons you should consider before you make a decision. Once you understand the Medicare basics of Part A and Medicare Part B, you will be faced with choosing one of three paths to take with your Medicare. This will be the most important Medicare decision you have to make because it could set you on a course that will impact the quality of your medical care and can potentially be irreversible. To keep this video brief, I have a separate video that covers how, when and where to sign up for Medicare Parts A & Medicare Part B. I have it linked in the upper right of this video, you’ll see a white circle with a letter “i”. You can click on that at any time to go to the video on how to sign up for Medicare. With terminology out of the way, next in the Medicare Explained video is Medicare Part A & Medicare Part B. Medicare Part A is that portion of your health insurance that pays for your healthcare whenever you are an inpatient in a medical facility. As soon as you become an inpatient in a hospital, Medicare Part A takes over. The same when you become an inpatient in a skilled nursing facility or hospice. The one exception is that if you are not able to physically get to a medical facility for healthcare, Medicare Part A also pays for home healthcare. Think of Medicare Part A is inpatient insurance. Medicare Part B is the part of Medicare that pays for outpatient and physician services. This is the portion Medicare insurance you use when you see a doctor or have lab tests or physical therapy and so on. Medicare Part B also includes some wellness coverage. Medicare Part B is the part of Medicare people use most often because most of our medical care is via outpatient services. Even many surgeries and hospital services are now done as an outpatient and are thus covered under Medicare Part B. Your Original Medicare (Part A & B) is good anywhere in the country. You can see any doctor, go to any medical facility in any state or territory, as long as they accept Medicare. You do not need to get permission from a primary care physician. You have both freedom to go where you need and control to see who you choose. Also it is important to understand that Medicare Advantage Plans are local coverage. If you are a snowbird, and RVer or just travel the country that should rule MAPD as an option. Next in our Medicare Explained video are your three choices. You can do one of three things: # 1 Do nothing. Just keep Original Medicare (Your Part A & B). This is the only choice that makes understanding those co-pays and deductibles very important. This is not an advisable choice because Original Medicare was never intended to be stand-alone health insurance. You will find when going over the details of the copays and the limits of the coverage that there is no maximum out-of-pocket limit on your potential financial obligation. You are at substantial financial risk. Still, this is a choice some people make. # 2 You can trade in your Medicare Part A and Medicare Part B for a Medicare Advantage Plan. Medicare Advantage plans are also called Medicare Part C. #3 Your third choice is to keep you Original Medicare Parts A & B, but add a Medicare supplement plan that pays the co-pays and deductibles that come with just Original Medicare, so you don’t have to. The premium cost of all of these plans are different, the co pays are different, the mail order pharmacies, the drugs, the Lipitor generic, the simvastatin. It goes on and on, and the one general consensus is that Medicare Part D prescription drug plans are just darn confusing. Rocky mountain health plans in Colorado are easier to figure out than the drug plans for Part D. The first step is to get your Medicare A and B card. If you already have that taken care of and you have a Medicare supplement plan in place the battle is almost won. Maybe it is the annual enrollment period and you simply want to change your drug plan or switch from a Medicare advantage plan. Maybe you just are tired of running your drugs through the system only to find out the Medicare approved part D plans are all expensive. My advice is to get a broker to help you through this. Why is it that people always want to do things themselves? It doesn't cost you anything more to get a Medicare broker to walk you through this time period. If you want Medicare Part D info and you are tired of looking around then call us at Medigaplist. We can help. So many people have asked me what the best Medicare drug plan is and I always tell them it is different for everyone. The AARP drug plan from United Healthcare is the best Medicare drug plan in Colorado for certain people. Humana is the best Medicare Part D plan for someone else. It really depends on the drugs you take. If you take Lisinopril to treat high blood pressure aka hypertension, or you take another ACE inhibitor, the plan isn't going to be the samer Part D Medicare plan that someone taking Actos or Avandia. Omeone who is taking Valium or a Xanax obviously has a different condition than someone taking Altocor, lovastatin or Crestor. Would you take Aviane or a ethinyl estradiol/levonorgestrel for your back pain; I don't think so. oyu need to get a list of Medicare Part D drug plans and compare the premiums. You also want to compare the best Medigap plan F and see if you can save money on your supplemental Medigap plan. What Did I Learn? I learned that you can buy A LOT of things over the counter in Mexico. Xanax, Viagra, Valium, steroids, and much more. Now, some of those may be the generic brand of the drug, but the process to get them is still much easier than in the United States. Here are the prices that I was able to write down: Valium- 220 pesos, 12.54 USD Xanax- 540 pesos, 30.79 USD Cialus- 1500 pesos 85.53 USD Claritan D 10 pack- 163 pesos, 9.29 USD Sostenon- 420 pesos, 23.95 USD Deca (1 month supply) - 681 pesos, 38.83 USD So what can't you buy from the pharmacies? You can't buy any sleeping pills or more serious pain killers. It was indicated that those are regulated pretty heavily, but I believe the process to go to the doctor to get a prescription is pretty simple and straightforward. Also, you can't get any antibiotics over the counter. Those need a prescription as well. This is for two reasons – first; because the previously popular plans Medicare supplement Plan F and Plan C are being phased out as of January 2020. Second, Medicare supplement Plan G is usually a lower premium AND a better value. Most people shopping for a Medigap Plan today find Medicare supplement Plan G as their best value. Let’s take a quick look at the Medicare supplement plans so you can see why: . In the column below the Plan letter you can see the percentage of insurance coverage offered by that plan. For example; Medigap Plan F show 100% coverage in each category all the way down. That means that Medicare supplement plan F covers 100% of all the co-pays and deductibles that you would normally have to pay with Medicare Part A (inpatient hospital coverage) and Medicare Part B (outpatient coverage). It’s a great plan and would be an easy decision for many people entering Medicare, except that it is being phased out in January 2020 and Plan G usually offers equal benefits for less money. Take a look -- Medicare supplement Plan G has all the same benefits except this row right here: Medicare Part B deductible. That is a calendar year annual deductible. It resets every January 01. It can change every year. In 2017 it’s just $183. We expect it to increase to $250 over the next five to seven years. That’s it. All you pay is the annual Medicare Part B annual deductible and your premium. Other than that, Medicare and your Medigap Plan G pay all your Medicare related in-patient and outpatient expenses. The math here is simple, if Plan G is $183 less in premiums per year, then it would be exactly equal to Plan G. You pay the $183 deductible out of pocket, then you have all the same benefits as a Plan F. However, Medicare Supplement Plan G is usually $300 to $400 a year less in premiums than a Medigap Plan F. That means you can pay the $183 Medicare part B deductible out of pocket and have money left over. That makes it a better value. People considering a Medigap Plan G often compare the price and benefits to a Medigap Plan N. Plan N usually cost much less than Plan G, but there is a catch! In fact, Medicare supplement Plan N can cost 25% to 35% less than a Medigap Plan G. However, Plan N has less coverage and an asterisks you need to be aware of. Just like with Medigap Plan G, with Medigap Plan N you pay that Medicare Part B deductible out of pocket. The asterisks indicate that you will also pay a small co-pay of up to $20 when you see a doctor, $50 for an emergency room visit. Medicare supplement Plan G is for those who want peace-of-mind. Once you have a Plan G you don’t have to worry about co-pays or excess charges or anything. You can see any doctor, whether they charge excess charges or not. As we go over what you can expect with the cost of cancer treatment, we have divided your financial risk into two different categories; direct costs and indirect costs for cancer treatment. Direct costs are the bills you receive for medical services, chemotherapy and other medications. Indirect costs are expenses you incur to get better treatment or care, but are not directly in response to medications or specific services. For example; let’s say that you decide to have some of the top oncologists in the country treat your cancer and visit MD Anderson in Texas or Sloan-Kettering in New York for service. While Medicare may pay for your services, depending on the plan you have, you still need to pay for transportation to get to and from the clinic and for the cost of lodging and meals while you are there. These costs will often be in the thousands of dollars and are indirect costs for cancer treatment. It is also important to understand which drug treatments may be covered under Medicare Part B for outpatient services or your Advantage plan equivalent, and which are typically covered through the Part D or prescription drug portion of your insurance. This is important because it defines your level of financial risk or obligation, so this is where we will start.

The Medicare Part D program provides prescription drug coverage to almost 30 million enrollees, and will cost federal taxpayers almost $1 trillion in the next decade. The hearing will examine whether the private insurers that receive government subsidies to provide the Part D benefit are able to effectively obtain prescription drug discounts from drug manufacturers. Since the Part D program's inception in January 2006, observers have questioned whether the private insurers who run the program are effectively negotiating with drug manufacturers for low prices.
