Diabetes refers to a group of conditions characterized by a high level of blood glucose, commonly referred to as blood sugar. Too much sugar in the blood can cause serious, sometimes life-threatening health problems. There are two types of chronic diabetic conditions: type 1 diabetes and type 2 diabetes. Pregnant women may acquire a transient form of the disease called “gestational diabetes” which usually resolves after the birth of baby. Pre-diabetes is when the blood sugar level is at the borderline: higher than normal, but lower than in diabetics. Prediabetes may or may not progress to diabetes. During food digestion, carbohydrates - or carb - break down into glucose which is carried by the bloodstream to various organs of the body. Here, it is either consumed as an energy source - in muscles for example - or is stored for later use in the liver. Insulin is a hormone produced by beta cells of the pancreas and is necessary for glucose intake by target cells. In other words, when insulin is deficient, muscle or liver cells are unable to use or store glucose, and as a result, glucose accumulates in the blood. In healthy people, beta cells of the pancreas produce insulin; insulin binds to its receptor on target cells and induces glucose intake. In type 1 diabetes, beta cells of the pancreas are destroyed by the immune system by mistake. The reason why this happens is unclear, but genetic factors are believed to play a major role. Insulin production is reduced; less insulin binds to its receptor on target cells; less glucose is taken into the cells, more glucose stays in the blood. Type 1 is characterized by early onset, symptoms commonly start suddenly and before the age of 20. Type 1 diabetes is normally managed with insulin injection. Type 1 diabetics are therefore “insulin dependent”. In type 2 diabetes, the pancreas produces enough insulin but something goes wrong either with receptor binding or insulin signaling inside the target cells. The cells are not responsive to insulin and therefore cannot import glucose; glucose stays in the blood. In other words, type 2 diabetics are “insulin resistant”. Here again, genetic factors predispose susceptibility to the disease, but it is believed that lifestyle plays a very important role in type 2. Typically, obesity, inactive lifestyle, and unhealthy diet are associated with higher risk of type 2 diabetes. Type 2 is characterized by adult onset; symptoms usually appear gradually and start after the age of 30. Type 2 diabetes accounts for about 80 to 90% of all diabetics. Management focuses on weight loss and includes a low-carb diet.

Acute complications are dangerously elevated blood sugar or abnormally low blood sugar due to diabetes medications. Chronic complications are disease of the blood vessels (both small and large) that can damage the feet, eyes, kidneys, nerves, and heart. TREATMENT OF DIABETES Diabetes treatment depends on the type and severity of it. Type 1 diabetes is treated with insulin, exercise and a healthy diabetic diet. Type 2 diabetes is first treated with weight reduction than a diabetic diet along with exercise. If diabetes is not controlled by these measures, oral medications are used. If oral medications are still insufficient then insulin and other injectable medications are considered. While some of these factors are under personal control, such as diet and obesity, other factors are not, such as increasing age, female gender, and genetics. A lack of sleep has been linked to type 2 diabetes. This is believed to act through its effect on metabolism. The nutritional status of a mother during fetal development may also play a role, with one proposed mechanism being that of altered DNA methylation. The intestinal bacteriæ Prevotella copri and Bacteroides vulgatus have been connected with type 2 diabetes. Lifestyle Lifestyle factors are important to the development of type 2 diabetes, including obesity and being overweight (defined by a body mass index of greater than 25), lack of physical activity, poor diet, stress, and urbanization. Excess body fat is associated with 30% of cases in those of Chinese and Japanese descent, 60 to 80% of cases in those of European and African descent, and 100% of cases in Pima Indians and Pacific Islanders. Among those who are not obese, a high waist–hip ratio is often present. Smoking appears to increase the risk of type 2 diabetes mellitus.Dietary factors also influence the risk of developing type 2 diabetes. Consumption of sugar-sweetened drinks in excess is associated with an increased risk. The type of fats in the diet are important, with saturated fats and trans fatty acids increasing the risk, and polyunsaturated and monounsaturated fat decreasing the risk. Eating a lot of white rice appears to play a role in increasing risk. A lack of exercise is believed to cause 7% of cases. Persistent organic pollutants may play a role. Genetics Most cases of diabetes involve many genes, with each being a small contributor to an increased probability of becoming a type 2 diabetic. If one identical twin has diabetes, the chance of the other developing diabetes within his lifetime is greater than 90%, while the rate for nonidentical siblings is 25 to 50%. As of 2011, more than 36 genes had been found that contribute to the risk of type 2 diabetes. All of these genes together still only account for 10% of the total heritable component of the disease. The TCF7L2 allele, for example, increases the risk of developing diabetes by 1.5 times and is the greatest risk of the common genetic variants. Most of the genes linked to diabetes are involved in beta cell functions. There are a number of rare cases of diabetes that arise due to an abnormality in a single gene (known as monogenic forms of diabetes or "other specific types of diabetes"). These include maturity onset diabetes of the young (MODY), Donohue syndrome, and Rabson-Mendenhall syndrome, among others. Maturity onset diabetes of the young constitute 1 to 5% of all cases of diabetes in young people. Medical conditions There are a number of medications and other health problems that can predispose to diabetes. Some of the medications include: glucocorticoids, thiazides, beta blockers, atypical antipsychotics, and statins. Those who have previously had gestational diabetes are at a higher risk of developing type 2 diabetes. Other health problems that are associated include: acromegaly, Cushing's syndrome, hyperthyroidism, pheochromocytoma, and certain cancers such as glucagonomas. Gestational diabetes sometimes occurs in pregnant women, because certain hormones produced during pregnancy increase the body’s resistance to insulin. Gestational diabetes is usually temporary, and blood sugar levels return to normal following childbirth. The buildup of glucose in the bloodstream causes similar symptoms for people affected by any type of diabetes. These symptoms include increased thirst and urination as the body tries to expel excess glucose from the bloodstream, fatigue, hunger, and weight loss as the cells of the body are starved of their energy source, and vision problems, sores, and nerve pain as the buildup of glucose in the bloodstream damages small blood vessels. In type 1 diabetes, these symptoms often appear suddenly during childhood or adolescence, as some environmental factor like an illness or exposure to harsh weather triggers the immune system to attack the pancreas. For this reason, Type 1 diabetes was once called juvenile diabetes, but further research has shown that Type 1 diabetes can be triggered later in life, and Type 2 diabetes may appear early in life. In Type 2 diabetes, formerly known as adult onset diabetes, symptoms often gradually appear later in life, as the cells slowly build up resistance to insulin. Several factors, including being overweight, lack of exercise, high blood pressure, and advanced age contribute to the body’s insulin resistance. Because the underlying cause of Type 1 diabetes is a lack of insulin, treatment involves injecting insulin with a needle or pump. While those with Type 1 diabetes are urged to monitor their diet and exercise, Type 1 diabetes is irreversible and cannot be controlled with a healthy lifestyle alone. Type 2 diabetes may be controlled with changes to diet and lifestyle, oral medications, or insulin injections, depending on how far the disease has progressed. Another archaic term for Type 1 and Type 2 diabetes was insulin dependant and non-insulin dependant diabetes, respectively, but these terms are no longer used, as some Type 2 diabetics require insulin to regulate their blood sugar levels. An essential part of treatment for any type of diabetes is the tracking of blood sugar levels with a glucose monitor. If the bloodstream contains too much or too little glucose, life threatening complications can suddenly occur, often preceded by worrisome symptoms like confusion, dizziness, weakness, seizures, and hallucinations. While Type 1 and Type 2 diabetes share many of the same symptoms, their root cause, population affected, and treatment options are very different. Well, those of us with diabetes probably do know that there are 2 or maybe 3 depending how you want to classify it, types of diabetes. Some will call prediabetes a form of diabetes, and I guess that is true, kind of like pre-school is kind of like school. Then there are Type 1 and Type 2 diabetes. And don’t forget about gestational diabetes. We will talk briefly about that one at the end. In this video, I will attempt to give you the similarities and differences in each as well some possible causes. Diabetes affects approximately 30 million Americans today. That is nearly 10% of the population. Diabetes is a disorder of the endocrine system. With diabetes, blood sugar levels stay high because either the pancreas can no longer produce enough insulin or the cells of the body are resistant to insulin and the pancreas can’t keep up. Either way, the glucose or sugar level in the blood stream becomes too high and the body cannot function properly. Pre-Diabetes In general, people who have a fasting plasma blood glucose in the 100-125 mg/dl range are defined as having impaired fasting glucose or pre-diabetes. Like many diseases, early detection can be very good. Talk to your doctor about testing for pre-diabetes. You may be able to prevent type 2 diabetes Type 1 Diabetes This used to be called juvenile diabetes or insulin-dependent diabetes. Type 1 diabetes occurs because the insulin-producing cells of the pancreas are actually destroyed by the immune system. Type 1 diabetes is actually considered an autoimmune disease. Its onset has nothing to do with diet or lifestyle. There is nothing you can do to prevent Type 1, and—at present—nothing you can do to get rid of it. Type 1 diabetes most commonly starts in people under the age of 20, but may occur at any age. According to the Mayo Clinic, the most common symptoms of Type 1 are: Increased thirst Frequent urination Bedwetting in children who previously didn't wet the bed during the night Extreme hunger Unintended weight loss Irritability and other mood changes Fatigue and weakness Blurred vision In females, a vaginal yeast infection There is no cure for Type 1 and it cannot be reversed. To determine diabetes, a blood sample will be taken after an overnight fast. A fasting blood sugar level less than 100 is normal. A fasting blood sugar level from 100 to 125 is considered prediabetes. If it's 126 or higher on two separate tests, you have diabetes. If you're diagnosed with diabetes, the doctor may do other tests to distinguish between type 1 and type 2 diabetes — since the two conditions often require different treatments. Type 2 Diabetes According to Web MD, with type 2 diabetes, the body continues to produce insulin, although insulin production by the body may significantly decrease over time. The pancreas produces either not enough insulin, or the body is unable to recognize insulin and use it properly. When there isn't enough insulin or the insulin is not used as it should be, glucose can't get into the body's cells to be used as energy. This glucose then builds up in the blood. This condition was once known adult-onset or noninsulin-dependent diabetes. More common in adults, type 2 diabetes increasingly affects children as childhood obesity increases. There's no cure for type 2 diabetes, but you may be able to manage the condition by eating well, exercising and maintaining a healthy weight. If diet and exercise aren't enough to manage your blood sugar well, you also may need diabetes medications or insulin therapy. The symptoms for Type 2 are basically the same as for Type 1 with a few additions: Slow-healing sores or cuts Itching of the skin (usually in the vaginal or groin area) Recent weight gain Numbness or tingling of the hands and feet Impotence or erectile dysfunction A quick word about gestational diabetes, Hormone changes during pregnancy can affect insulin's ability to work properly. The condition, called gestational diabetes, occurs in about 4% of all pregnancies. GB is tested for during the pregnancy and is treated if present. Usually blood sugar levels return to normal about 6 weeks after giving birth. I hope this has shed some light on the different types of diabetes.

The defective responsiveness of body tissues to insulin is believed to involve the insulin receptor. However, the specific defects are not known. Diabetes mellitus cases due to a known defect are classified separately. Type 2 DM is the most common type of diabetes mellitus. In the early stage of type 2, the predominant abnormality is reduced insulin sensitivity. At this stage, high blood sugar can be reversed by a variety of measures and medications that improve insulin sensitivity or reduce the liver's glucose production. Type 2 DM is due primarily to lifestyle factors and genetics. A number of lifestyle factors are known to be important to the development of type 2 DM, including obesity (defined by a body mass index of greater than 30), lack of physical activity, poor diet, stress, and urbanization. Excess body fat is associated with 30% of cases in those of Chinese and Japanese descent, 60–80% of cases in those of European and African descent, and 100% of Pima Indians and Pacific Islanders. Even those who are not obese often have a high waist–hip ratio. Dietary factors also influence the risk of developing type 2 DM. Consumption of sugar-sweetened drinks in excess is associated with an increased risk. The type of fats in the diet is also important, with saturated fats and trans fatty acids increasing the risk and polyunsaturated and monounsaturated fat decreasing the risk. Eating lots of white rice also may increase the risk of diabetes. A lack of exercise is believed to cause 7% of cases. Gestational diabetes Gestational diabetes mellitus (GDM) resembles type 2 DM in several respects, involving a combination of relatively inadequate insulin secretion and responsiveness. It occurs in about 2–10% of all pregnancies and may improve or disappear after delivery. However, after pregnancy approximately 5–10% of women with gestational diabetes are found to have diabetes mellitus, most commonly type 2. Gestational diabetes is fully treatable, but requires careful medical supervision throughout the pregnancy. Management may include dietary changes, blood glucose monitoring, and in some cases, insulin may be required. Though it may be transient, untreated gestational diabetes can damage the health of the fetus or mother. Risks to the baby include macrosomia (high birth weight), congenital heart and central nervous system abnormalities, and skeletal muscle malformations. Increased levels of insulin in a fetus's blood may inhibit fetal surfactant production and cause respiratory distress syndrome. A high blood bilirubin level may result from red blood cell destruction. In severe cases, perinatal death may occur, most commonly as a result of poor placental perfusion due to vascular impairment. Labor induction may be indicated with decreased placental function. A Caesarean section may be performed if there is marked fetal distress or an increased risk of injury associated with macrosomia, such as shoulder dystocia. Well, those of us with diabetes probably do know that there are 2 or maybe 3 depending how you want to classify it, types of diabetes. Some will call prediabetes a form of diabetes, and I guess that is true, kind of like pre-school is kind of like school. I am not saying it is any less serious, it is just that pre means before and those with pre-diabetes are at a high risk to get diabetes, they just haven’t gotten it yet. Then there are Type 1 and Type 2 diabetes. And don’t forget about gestational diabetes. In this video, I will attempt to give you the similarities and differences in pre-diabetes and Type 2. Diabetes affects approximately 30 million Americans today. 86 million have been diagnosed with pre-diabetes. Half of the adults in the US have either diabetes or prediabetes. Diabetes is a disorder of the endocrine system. With diabetes, blood sugar levels stay high because either the pancreas can no longer produce enough insulin or the cells of the body are resistant to insulin and the pancreas can’t keep up. Either way, the glucose or sugar level in the blood stream becomes too high and the body cannot function properly. Pre-Diabetes In the U.S., 86 million people have blood glucose levels that are above the normal range, but not high enough to be classified as diabetes. This is known as pre-diabetes. It is also known as impaired glucose tolerance. This type of diabetes is difficult to diagnose because most people with pre-diabetes usually have no symptoms. But pre-diabetes is almost always present before a person develops type 2 diabetes. Without symptomology however, it is hard to diagnose because most people don’t go see a doctor if they have no symptoms of a disease. Complications normally associated with diabetes, such as heart disease, can begin to develop even when a person has pre-diabetes. In general, people who have a fasting plasma blood glucose in the 100-125 mg/dl range are defined as having impaired fasting glucose or pre-diabetes. Like many diseases, early detection can be very good. Talk to your doctor about testing for pre-diabetes. You may be able to prevent type 2 diabetes. Type 2 Diabetes According to Web MD, with type 2 diabetes, the body continues to produce insulin, although insulin production by the body may significantly decrease over time. The pancreas produces either not enough insulin, or the body is unable to recognize insulin and use it properly. But a 2015 study by Chloe Eldridge found that “Current evidence shows hypoglycaemia is considerably prevalent amongst people with type 2 diabetes, particularly for those on insulin, yet still fairly common for other treatment regimens. This highlights the subsequent need for educational interventions and individualisation of therapies to reduce the risk of hypoglycaemia.” First let’s define what we mean by hypoglycemia. Hypoglycemia happens when the amount of blood glucose (sugar in the blood) drops to a level that's too low to sustain normal functioning; in most people, this is defined as a blood-sugar level below 70 mg/dl. Left untreated, hypoglycemia can lead to a coma and even death. It is very important that those with diabetes monitor sugar levels and treat hypoglycemia when it is present and right away. It happens more often than we think. So what does it feel like to have hypoglycemia? Let’s look at the most common risks of hypoglycemia, the symptoms and then what you need to do to treat the condition. You are at risk of having a low blood sugar reaction if you: Skip or delay a meal or snack. This is why it is so important for you to make sure you eat. I know the temptation to lose weight and other factors may cause you to not eat regularly, but it is so important that you eat 3 meals a day and eat your snacks in between meals. This is to maintain a steady, even blood sugar level. Take too much insulin or eat too few carbohydrates. We have all made a mistake in our dosage of insulin. You take it when half asleep or just in a hurry. It happens. Don’t try to pretend you didn’t make this mistake. It is fine! Just correct it. Also, in many of my videos I have stated that you must eat carbs. That is one reason a very low carb diet is not good for diabetes. It will really mess up your blood glucose regulating system. Trust me, I know from experience. Exercise. When you exercise, you are burning off carbs and sugars. This is great, but know that with your exercise comes a possibility for your blood sugar to be low. Get into the habit of checking your blood sugar before, during and after you exercise and be prepared. Have a carb snack ready to get that sugar back up. Drink alcohol, especially without eating carbohydrates. I have done extensive research on this topic. I have written about it and I have included the information in several of our videos. If you are going to drink alcohol, you must do so with food. Drink with a meal, or at the very least with snacks. Alcohol can bring down blood glucose levels drastically and the symptoms of hypoglycemia are often mistaken for drunkenness. Even if you are not intoxicated. 15/15 Rule If after checking your blood sugar and it is less than 70 milligrams per deciliter (mg/dl), you will need to follow the 15/15 rule. Eat 15 grams of then rest for 15 minutes and then recheck your blood sugar If your blood sugar is still less than 100 mg/dl, take another 15 grams of carbohydrate and retest your blood sugar in another 15 minutes. Repeat if necessary. Important: If you have frequent low blood sugars speak to your doctor. You may need changes in your medication and/or meal plan. Quick Carbohydrate Guide for Treating Low Blood Sugars If your blood sugar is less than 70 mg/dl, you need 15 grams of a quickly absorbed carbohydrate, like the ones listed below. Each of the following servings provides 15 grams of carbohydrate. Candies 5 small gum drops 12 gummy bears 6 large jelly beans 5 Life Savers 15 Skittles 1 Tablespoon honey, jam or jelly 1 Tablespoon sugar in water 4 Starburst Beverages 1/2 cup apple juice 1/2 cup orange or grapefruit juice 1/2 cup pineapple juice 1/2 cup regular soda (not diet) 1/3 cup grape juice 1/3 cup cranberry juice 1/3 cup prune juice 1 cup fat free milk Fruits 1/2 banana 1 small apple 1 small orange 1/2 cup applesauce 2 tablespoons of raisins 15 grapes Other 3 to 4 glucose tablets 1 tube glucose gel The foods I have given you are easily absorbed and will raise blood sugar levels quickly. Foods that contain protein or fat — such as chocolate, candy bars, ice cream, cookies, crackers and bread — don't raise blood sugar quickly enough. If you have type 1 diabetes and you do not take care of low blood glucose, you may pass out. If you do, a drug called glucagon should be injected into your skin, like you do with insulin. This can be done by a family member or friend who has been taught how to do it. Since glucagon may cause you to vomit, you should be placed on your side when the injection is given. If no one knows how to give the injection, you should be taken to a hospital. You need a prescription for a glucagon kit. You should awaken about 10 minutes after the glucagon is injected. If you do not, you should be taken at once to a hospital.

These types of damage are the result of damage to small vessels, referred to as microvascular disease. • Diabetes also is an important factor in accelerating the hardening and narrowing of the arteries (atherosclerosis), leading to strokes, coronary heart disease, and other large blood vessel diseases. This is referred to as macrovascular disease. • From an economic perspective, the total annual cost of diabetes in 2012 was estimated to be 245 billion dollars in the United States. Globally, the statistics are staggering. • Diabetes is the 7th leading cause of death in the United States listed on death certificates in recent years. Blindness is 25 times more common among diabetic patients compared to nondiabetics. It is proposed that by the year 2010, diabetes will exceed both heart disease and cancer as the leading cause of death through its many complications. Diabetics have a high level of blood glucose. The blood sugar level is regulated by insulin, a hormone produced by the pancreas, which releases it in response to food consumption. Insulin causes the cells of the body to take in glucose from the blood. The glucose is used as fuel for cellular functions. Diagnostic standards for diabetes have been fasting plasma glucose levels greater than 140 mg/dL on two occasions and plasma glucose greater than 200 mg/dL following a 75-gram glucose load. More recently, the American Diabetes Association lowered the criteria for a diabetes diagnosis to fasting plasma glucose levels equal to or greater than 126 mg/dL. Fasting plasma levels outside the normal limit require additional tests, usually by repeating the fasting plasma glucose test and (if indicated) giving the patient an oral glucose tolerance test. The symptoms of diabetes include excessive urination, excessive thirst and hunger, sudden weight loss, blurred vision, delay in healing of wounds, dry and itchy skin, repeated infections, fatigue and headache. These symptoms, while suggestive of diabetes, may be due to other reasons also. There are two different types of diabetes. Type 1 Diabetes (juvenile diabetes or insulin-dependent diabetes): The cause of Type 1 diabetes is caused by pancreatic inability to produce insulin. It is responsible for 5-10% of cases of diabetes. The pancreatic Islet of Langerhans cells, which secrete the hormone, are destroyed by the body's own immune system, probably because it mistakes them for a virus. Viral infections are thought to be the trigger that sets off this auto-immune disease. It is more common in caucasians and runs in families. If untreated, death occurs within a few months of the onset of juvenile diabetes, as the cells of the body starve because they no longer receive the hormonal prompt to take in glucose. While most Type 1 diabetics are young (hence the term Juvenile Diabetes), the condition can develop at any age. Autoimmune diabetes can be definitely diagnosed by a blood test which shows the presence of anti-insulin/anti-islet-cell antibodies. Native peoples such as North American Indians whose traditional diets did not include refined starch until its recent introduction by Europeans have extremely high rates of diabetes, up to 5 times the rate of caucasians. Blacks and hispanics are also at higher risk. Though Type 2 diabetes is not fatal within a matter of months, it can lead to health complications over several years and cause severe disability and premature death. As with Type 1 diabetes, the condition is found primarily in one age group, in this case people over 40 (which is why it is often termed Adult Onset); however, with the rise in childhood and teenage obesity, it is appearing in children as well.
