If you are experiencing symptoms that you believe are associated with gluten consumption, and go to the doctor for a blood test, the doctor will look for gliadin, endomysium, and transglutaminase. One thing the doctor does not tell you is that there are different types of gliadin: alpha, beta, omega and gamma, and the test only looks for alpha gliadin. Glutenin is the other half of gluten. People can be equally as reactive to glutenin as they are to gliadin, but they are not being tested for it. Wheat germ agglutinin (WGA) is a lectin, which binds on to carbohydrates. They are found in high concentrations in whole wheat and sprouted wheat. When you eat WGA, it can cause your red blood cells clump together and it can pass through the blood-brain barrier, attach to your myelin sheath, and reduce nerve growth factor. Deamidated gliadin is the result of the food industry processing gluten to make it water soluble. Your T-cells are very reactive to deamidated gliadin. When you digest gliadin, you make gluteomorphin. The consumption of gluten may trigger antibodies to gluteomorphin and/or prodynorphin which can cause neurological issues. I'm the director of the Celiac Disease Center at Columbia University. Celiac disease is actually a disease of Caucasians. The genes that are involved in celiac disease are northern European genes. Now, they've been spread all around the world, but if you look at which ethnic groups have celiac disease, it's much less common in black people and Asian except South Asian. In the study that we did, the ethnic group with the greatest prevalence of celiac disease in this country was individuals from Northern India. It's common but there is an ethnic predilection for it to be in Caucasians. So to have celiac disease, you’ve got to be ingesting gluten, and 99% of us are doing that and you got to have these particular northern European genes, and 40% of us have those genes yet only 1% have celiac disease so that suggests that there are environmental factors and other genes, about 30 other genes have been described that can be factors in the development of celiac disease. But we think environmental factors are very important. There are a whole number of risk factors like antibiotics, PPI or the proton pump acid-reducing drugs, being born by cesarean section increases the risk. We need to do a lot more research to define what the risk factors are and then the mechanism of that so that we can prevent this increase in celiac disease that has been observed over the last 50 years worldwide. Perhaps you have seen the words gluten free written on a food label and wondered what it meant and is it healthy. Gluten is a protein that is used to help foods keep their shape, almost like glue. Most people eat it without any problems, but there is a small minority of people that have issues with it and this is called Celiac Disease. When I mean a small minority of people I'm talking about less than 1%. Their immune system thinks it's as evil as Dark Vader from Star Wars and it starts to harm the small intestines. This damages a person's ability to absorb nutrients and brings about a wide range of symptoms. Apart from people that have Celiac Disease there is also a class of people that have sensitivity or allergy to it. This just means their body doesn't like it, but it's not as extreme as Celiac Disease. There is also one more class of people associated with gluten and can you guess what it is? Yep, the last group is a huge majority of people that can eat gluten without any problems. The million dollar question is how do I know if I'm sensitive to gluten and I have an allergic reaction to it? The first area that might have some trouble will be digestion. If you constantly struggle with constipation, diarrhea and possibly stomach aches then gluten might just be the problem. Next when it comes to your mental health do you have trouble concentrating? How about problems with memory, brain fog and headaches? Another sign is if you feel irritable, anxious, depressed more often then you feel you should? Maybe everyone feels irritable and depressed more than they should, but it is a symptom of a gluten allergy. Lastly, there could be some skin problems if someone has trouble with gluten. This includes wanting to itch the skin a lot, which could be very annoying. Another sign would be hives and visible red rashes on the skin. If you do experience these symptoms I recommend giving a gluten free life a test drive. Eliminate gluten from your life for a little while and see what happens. If you notice many problems disappearing then you might want to continue going gluten free. If nothing happens then there is no reason not to consume this protein and you can enjoy all the food that comes with it. To sum everything up the leading gluten allergy symptoms involve digestion problems. This includes constipation, diarrhea and stomach pains. A person could notice trouble with concentration, memory, irritability, anxiety, brain fog and headaches. Lastly a person's skin might itch a lot and there could be a rash as well as hives. For more than 30 years, she has devoted herself to understanding how environmental and genetic factors affect the risk of developing allergies and asthma, and the molecular mechanisms underlying the diseases. As one of the nation’s foremost experts in adult and pediatric allergy and asthma, Dr. Nadeau's research is laying the groundwork for a variety of potential future therapies to prevent and cure allergies and asthma. She leads a diverse team of specialists—in areas from immunology to chemical engineering—a team that was among the first to show that high dimensional immunophenotyping of T cells involved in allergy could be used in therapies for patients. Her research has also shown that blocking antibodies in conjunction with allergen immunotherapy produced desensitization even in patients with up to five food allergens. She has overseen more than 40 clinical trials and enrolled more than 2,000 patients in allergy studies. Her work has changed the lives of many who, for the first time in their lives, are able to control life-threatening food allergies. Dr. Nadeau received her MD and PhD from Harvard Medical School. She completed a residency in pediatrics at Boston Children's Hospital and a clinical fellowship in asthma and immunology at Stanford and at University of California, San Francisco. Her research and scientific breakthroughs have been covered in Lancet, JAMA, Nature Medicine, US News and World Report, New York Times Magazine and Discover Magazine, as well as other national and international media. Dr. Nadeau has served as a White House Medical Consultant, a member of the Scientific Advisory Board of the EPA, NHLBI DSMB, NIH Study Sections, FARE Scientific Board, and a member of the American Lung Association Medical Board. This protein made up of the peptides gliadin and glutenin is found in grains such as wheat, semolina, rye and barley. It's known for giving bread its airy and fluffy substance and dough its sticky texture. But recently it's become notorious for sending dieters, restaurateurs, and the medical community into a tailspin. Why? Because every time you eat a Cronut or a turkey sandwich, your body either decides to break the gluten down and absorb it, or produce antibodies to attack it. The latter is known as celiac disease, and to a lesser extent, non-celiac gluten sensitivity. Avoiding gluten completely is, as of now, the only known treatment to help heal these conditions. But what about the rest of us who haven't been diagnosed? What really happens to the body when you give up gluten? I think we need to understand the immune system here and help you understand exactly what is happening inside your body when you have an allergic response to something versus an intolerance that might trigger some inflammation later on down the line. This is all very important stuff. Whether you're doing a ketogenic diet, whether you're fasting, or whether you're just trying to live a healthy lifestyle, understanding how the immune system works when it comes down to our response to food is going to make your life a lot easier, but it's also some pretty fun information in the first place. Also make sure you turn on notifications. All right. Let's get into the fun stuff. The first thing that I really want to talk about is a food allergy and what happens inside your body. Okay? A food allergy is a reaction that begins with a food shortly after eating it. You've probably heard of a food allergy before and you know what ends up happening. You might get them in the way of hives, you might get them in the way of maybe some rashes, or maybe you end up having to run to the bathroom or you end up getting some bloating. You know if you have a reaction to a food. You've probably experienced it before. Now that's the simplicity of it, but let's talk a little bit more about what's actually happening. You see, at the very, very root of it, when you have an allergy, it's just a simple substance that is harmless that your body is responding to as if it was a harmful foreign invader. Basically, your body sees a food come in and it thinks that it's bad, so it triggers a reaction, but there's a pretty interesting process that occurs when it comes down to that. You see, that response that actually happens with that food is coming from something known as IgE. It's a specific kind of protein, and we're going to talk about these all in a second. They're called immunoglobulins. Now in this particular case, this IgE protein attaches itself to the foreign invader, in this case, the harmless food that is labeled as a foreign invader. The IgE protein attaches itself to the foreign invader, and then from there it triggers multiple chemical responses, one of which is usually histamine. Now if you've ever taken an antihistamine before, this is what you're fighting. You're not fighting the IgE response. You're fighting the histamine response. So here's what's interesting. If you have allergies to multiple different foods, a lot of times you might find the reactions are similar amongst all those foods. Let's say, for example, you're allergic to shellfish and you're allergic to peanuts. You might find that if you eat shellfish, you end up with your throat closing up. Well, you might find that if you eat peanuts, you end up having the same kind of reaction, and that's simply because your reaction is to the histamine. It's not really to the food. There's a deeper problem going on where your body tends to recognize the shellfish or the peanuts as a foreign invader, triggering this already built-in mechanism that's causing your throat to close up. What I wanna do is I wanna talk about gluten intolerance, gluten sensitivity, and true celiac disease and talk about how they're all interrelated. A lot of people seem to think that this gluten thing is just a fad, that people are going gluten-free because it's popular right now. But the reality is that there's some true science and some true physiology that proves that we're starting to have a bigger issue with gluten as a whole, and it all comes down to how our bodies respond to it in the first place and how wheat has changed over the years. But what I wanna do is I wanna help you understand the link between what is gluten intolerance, what is true celiac disease, and then what is a wheat allergy. Okay, they're all interrelated to some degree, but if you understand what's happening you might not be so hard on that friend that says he has an issue with gluten, even though he's not celiac. Let's break it down. First off, we have to understand why we're having so much of an issue with gluten these days. Is it the fact that it's just more so in the mainstream and we're hearing more about it? I'm sure to some degree that's true. I am sure there is some psychological effect that is causing more people to have a gluten reaction when they don't really have one. That's always gonna be the case. But the reality is that we have changed as humans. The over-consumption of wheat has led us to develop different processes in the body that cause us to have more reactions to wheat. When you consume one of the same foods for a long time, over and over, you do start to cause an issue within your body. But that's not even the issue I'm talking about today. The other issue is the hybridization of wheat. Now, say what you want about GMOs. Like them or not, I don't really care. The fact is this hybridization of wheat in particular has elicited a specific change in the proteins. You see, wheat now has new proteins in it. Approximately 5% of modern day wheat is entirely new proteins that our bodies don't even know how to assimilate or utilize we don't really have the enzymes to break those proteins down. That truly is causing an issue. We've hybridized wheat to make it a little bit more drought-resistant, make it bug-resistant, but also we end up doing specific things, like spraying Roundup on it, to dry it out faster so you can have more harvesting cycles. This causes some issues within our guts. But all that aside, let's talk about what's happening when you have a gluten intolerance. The first one I wanna talk about is non-celiac gluten intolerance. These are the people that whenever they consume bread or whenever they consume gluten starches, they just feel sick. They feel bloated. They feel really lethargic and they just don't feel good. Gluten-free foods are almost everywhere, and for a good reason – around 1% of the world population suffer from celiac disease and cannot eat gluten. What happens in the body when gluten is eaten, and why does it lead to symptoms like diarrhea, malnutrition, or even anemia? Watch this episode of Medicurio to find out! EXTRA INFORMATION ABOUT CELIAC DISEASE *Are oats safe for celiac patients to eat?* Most people with celiac disease can tolerate avenins, the gluten protein found in oats. However, they should still be cautious because oats may be contaminated with gluten proteins from wheat/barley/rye, as these grains are often processed in the same factory. Discuss with a dietician or physician when deciding if oats should be included in a gluten-free diet. *What kinds of food contain gluten?* The Celiac Disease Foundation has a list here: http://bit.ly/1DtfMdL *Why are antibodies against gluten and tissue transglutaminase produced?* B cells have unique B cell receptors (BCR). Whatever the BCR can bind to, the antibodies this B cell produces will bind to. When a protein binds to a particular B cell’s BCR, it gets internalized, broken down into small peptides, and presented on MHC class II proteins on the surface. If an activated helper T cell can bind to the B cell’s MHC class II protein using their TCR, they can activate the B cell. The B cell then starts producing antibodies against the protein that was internalized. For gluten peptides, the B cell internalizes these peptides and presents them to T cells, which will recognize them with their TCR since they were activated by dendritic cells earlier with the same peptide. The B cell then produces antibodies against the gluten peptides. For tissue transglutaminase (tTG), since tTG needs to bind to gluten peptides to modify them, they often exist as one protein complex. B cells that have BCRs that can bind to tTG will internalize a complex with both tTG and gluten peptides. When they break that complex down into smaller peptides, the B cell will present both fragments of tTG and gluten peptides on their MHC class II protein. Activated helper T cells will recognize gluten peptides and activate this B cell, but this B cell will produce antibodies that are against tTG since its BCR could bind to tTG. *What are the rates of HLA-DQ2 or 8 in the world? Wheat allergies occur very quickly and can result in anaphylactic shock, but damage to tissue is limited. Imagine a peanut allergy, but with gluten instead. Gluten ataxia is when the autoimmune damage is to the brain instead of the intestine, resulting in loss of motor coordination. Finally, non-celiac gluten sensitivity is a blanket term that is diagnosed by elimination – if a person’s symptoms get better after going on a gluten-free diet but tests have been negative for the previous three conditions, then they are diagnosed with NCGS. Symptoms include both intestinal and non-intestinal (headaches, depression, fatigue) symptoms, but the mechanism is unknown. *Why do some celiac disease patients develop rashes?* Medically, this rash is called dermatitis herpetiformis, and is described as an intensely itchy blistering that can occur anywhere on the body. While the mechanisms are not well understood, scientists believe that the tissue transglutaminase antibodies that are produced in the intestine eventually enter the bloodstream and end up binding to tissue transglutaminase found in the skin (epithelium). Antibodies stimulate the immune system, resulting in inflammation that causes itchiness and a burning sensation, which subsides after going on a gluten-free diet. *What is refractory celiac disease?* Refractory celiac disease describes the condition in which even after going on a gluten-free diet for over a year, the intestine does not recover and symptoms persist. It is not well understood what causes this phenomenon. If this is the case, immunosuppressants such as steroids should be used to suppress the immune system. However, most causes of “refractory” celiac disease are actually due to the patient still eating gluten without realizing it. This is why it is so important for people to know what contains gluten and what does not.

TechKnow investigates what's fuelling the spike in food-related allergies and whether science can cook up a cure. Every three minutes, a food allergy reaction sends someone to the emergency room in the US. It's an alarming statistic that worries parents and adults across the nation. In some extreme cases, mere nanograms of peanut protein can trigger a dangerous reaction and a mad dash to the hospital. The US Centre for Disease Control reported that from 1997 to 2011 the number of children with food allergies increased 50 percent. Over 15 million Americans suffer from food allergies, at a cost of $25bn each year. "A hundred years ago people did have food allergies, but it wasn't to the same epidemic proportions that we are suffering from now," says Dr Kari Nadeau, an expert in adult and paediatric allergies and head of a special allergy research centre at Stanford University. "People are very much living with this disabling worry of having an accidental exposure." Allergic reactions occur when the body's immune system overreacts to certain proteins and food. While any food can trigger an allergic reaction, about 90 percent are caused by eight sources: milk, eggs, peanuts, tree nuts, fish, shellfish, soy and wheat. When the immune system launches a protective response, cells release antibodies known as immunoglobulin E (IgE). "An IgE is the match that lights the fire behind the allergy, within six minutes you can have this very serious allergic attack," Nadeau explains. The marching order for decades has been avoidance and training children to become allergen inspectors before consuming certain foods. But, it's very difficult to track and vet every plated meal or crumb that a child with a food allergy might come across. Accidental ingestion happens and, for some, it can be fatal. So now, instead of avoidance, experts are looking at new ways to retrain the immune system and develop new treatments to desensitise food allergies. At Stanford, a treatment called oral immunotherapy was effectively used for desensitising patients - one allergen at a time. But doctors faced a new problem, as a third of people the who suffer from allergies are allergic to not just one, but multiple foods. "The minute you have a food allergy you also have a higher rate and a chance of having other food allergies too," says Nadeau. In 2009, the team at Stanford decided to tackle this problem head-on. The breakthrough came in a form of an IgE-suppressing asthma drug called Xolair. According to the national Institute of Health, about 15 percent of people with food allergies also have asthma. The link between the two are the symptoms that can trigger an attack on the lungs. Since the Federal Drug Administration's (FDA) approval in 2003, Xolair has proven effective in severe cases of asthma. "If it's working against IgE and we know that IgE is important in food allergy, what if we start giving it initially to people with food allergies and then we start giving them the food that they're allergic to? Could that protective cover help us increase the ability to get children and adults to food allergens to the same level that they'd like to eat them?" According to Nadeau out of 700 patients in the studies conducted since 2003, all who completed the trials have had positive results. This is not the only way to tackle food allergies: Using traditional Chinese medicine, Dr Xiu-min Li has created a therapy called food allergy herbal formula two, clinically known as Fahf- 2. Studies show that herbal compounds in Fahf-2 reduce IgE levels and combat symptoms of peanut-induced anaphylaxis. The treatment is currently in advanced clinical trials, being tested as a new botanical drug under the complementary alternative medicine arm of the FDA. "This approach is effective to prevent reactions ... we will continue to work hard to build up an even stronger tolerance," Li explains. Scientists still don't know why food allergies are on the rise, but whether it's high-tech western-style medicine or alternative eastern herbal treatments, the solutions seem to be coming into focus.
