I'm the director of the Celiac Disease Center at Columbia University. Celiac disease is actually a disease of Caucasians. The genes that are involved in celiac disease are northern European genes. Now, they've been spread all around the world, but if you look at which ethnic groups have celiac disease, it's much less common in black people and Asian except South Asian. In the study that we did, the ethnic group with the greatest prevalence of celiac disease in this country was individuals from Northern India. It's common but there is an ethnic predilection for it to be in Caucasians. So to have celiac disease, you’ve got to be ingesting gluten, and 99% of us are doing that and you got to have these particular northern European genes, and 40% of us have those genes yet only 1% have celiac disease so that suggests that there are environmental factors and other genes, about 30 other genes have been described that can be factors in the development of celiac disease. But we think environmental factors are very important. There are a whole number of risk factors like antibiotics, PPI or the proton pump acid-reducing drugs, being born by cesarean section increases the risk. We need to do a lot more research to define what the risk factors are and then the mechanism of that so that we can prevent this increase in celiac disease that has been observed over the last 50 years worldwide. Perhaps you have seen the words gluten free written on a food label and wondered what it meant and is it healthy. Gluten is a protein that is used to help foods keep their shape, almost like glue. Most people eat it without any problems, but there is a small minority of people that have issues with it and this is called Celiac Disease. When I mean a small minority of people I'm talking about less than 1%. Their immune system thinks it's as evil as Dark Vader from Star Wars and it starts to harm the small intestines. This damages a person's ability to absorb nutrients and brings about a wide range of symptoms. Apart from people that have Celiac Disease there is also a class of people that have sensitivity or allergy to it. This just means their body doesn't like it, but it's not as extreme as Celiac Disease. There is also one more class of people associated with gluten and can you guess what it is? Yep, the last group is a huge majority of people that can eat gluten without any problems. The million dollar question is how do I know if I'm sensitive to gluten and I have an allergic reaction to it? The first area that might have some trouble will be digestion. If you constantly struggle with constipation, diarrhea and possibly stomach aches then gluten might just be the problem. Next when it comes to your mental health do you have trouble concentrating? How about problems with memory, brain fog and headaches? Another sign is if you feel irritable, anxious, depressed more often then you feel you should? Maybe everyone feels irritable and depressed more than they should, but it is a symptom of a gluten allergy. Lastly, there could be some skin problems if someone has trouble with gluten. This includes wanting to itch the skin a lot, which could be very annoying. Another sign would be hives and visible red rashes on the skin. If you do experience these symptoms I recommend giving a gluten free life a test drive. Eliminate gluten from your life for a little while and see what happens. If you notice many problems disappearing then you might want to continue going gluten free. If nothing happens then there is no reason not to consume this protein and you can enjoy all the food that comes with it. To sum everything up the leading gluten allergy symptoms involve digestion problems. This includes constipation, diarrhea and stomach pains. A person could notice trouble with concentration, memory, irritability, anxiety, brain fog and headaches. Lastly a person's skin might itch a lot and there could be a rash as well as hives. Food sensitivities are very common in our society, although most people don’t know if they have any. One of the best ways to discover your food sensitivities is to do a food elimination diet. You do this by removing a food from your diet for a minimum of six weeks (120 days is best), then reintroduce the food and see how you feel. Some of the most common food sensitivities are: gluten, wheat, dairy, corn, soy, and chocolate, but it could be any food. Food sensitivities have been associated with some diseases. A study on multiple sclerosis, published in Neurology 2001 states, “The authors describe 10 patients with gluten sensitivity and abnormal MRI. All experienced episodic headache, six had unsteadiness, and four had gait ataxia. MRI abnormalities varied from confluent areas of high signal throughout the white matter to foci of high signal scattered in both hemispheres. Symptomatic response to gluten free diet was seen in nine patients.” This study shows that 9 out of ten patients diagnosed with abnormal brain MRI’s diagnosed with M.S. showed dramatic improvement with a 30 day gluten free diet. Lancet 1999 stated "High levels of circulating antigliaden antibodies (gliaden is a protein sub fraction of gluten) were found in 57% of patients with neurological dysfunction and early stage dementia. Celiac disease nursing NCLEX lecture on the symptoms, pathophysiology, nursing interventions, and diet (gluten-free) foods. Celiac disease is an autoimmune GI disorder where when gluten is ingested (which is found in wheat barley, grains, and rye products) it causes damage to the small intestine, specifically the intestinal villi. There are several proteins in wheat and the main protein that causes problems in Celiac Disease is called GLUTEN. Gluten itself is constructed of a group of proteins called gliadin and glutenin. Gliadin is the problem with Celiac Disease which causes an immune response. The amino acids which make up Gliadin can not be broken down into single amino acids but stay as peptides. This causes an immune response. Celiac Disease tends to be genetic and can occur in both children and adults. Celiac Disease is different from a wheat allergy or gluten sensitivity in that it causes similar signs and symptoms, but there isn’t the same extensive damage to the small intestine as in Celiac Disease. There is no cure for Celiac Disease except following a gluten-free diet. Signs and symptoms of Celiac Disease include: Mouth ulcers, Anemia, Lactose intolerance (can't break down lactose), Nausea/vomiting, Osteo (bone) changes (thinning, fractures), Unexplained slow growth, delay puberty (children) and weight loss, Rashes (very itchy rash called dermatitis herpetiformis on the elbows, backside, hairline, and knees), Irregular periods, irritable (depression) (loss of nutrients), Stools: greasy and odorous, Hair loss, Enamel changes to the teeth (yellow or brown spot and deformity), Diarrhea It is important the patient avoids food with gluten. What is Gluten?? This is the main question that a person who is new to this term asks and wants to know about, so today in this video you will get to know all about Gluten, diseases due to it, their symptoms diagnosis and much more. . . Gluten is a family of proteins found in grains like wheat, spelt, rye and barley. Gliadin and glutenin are the two main gluten proteins. . What is gluten? Gluten is a family of proteins found in grains like wheat, rye, spelt and barley. Of the gluten-containing grains, wheat is by far the most commonly consumed. The two main proteins in gluten are glutenin and gliadin. Gliadin is responsible for most of the negative health effects. When flour is mixed with water, the gluten proteins form a sticky network that has a glue-like consistency. This glue-like property makes the dough elastic, and gives bread the ability to rise when baked. It also provides a chewy, satisfying texture. Interestingly, the name glu-ten is derived from this glue-like property of wet dough. Bottom Line: Gluten is a family of proteins found in grains like wheat, spelt, rye and barley. Gliadin and glutenin are the two main gluten proteins. Why is gluten bad for some people? Most people tolerate gluten just fine. However, it can cause problems for people with certain health conditions. This includes celiac disease, gluten sensitivity, wheat allergy and some other diseases. Celiac disease Celiac disease, also spelled as coeliac disease, is the most severe form of gluten intolerance. It affects about about 0.7-1% of the population. It is an autoimmune disorder, and involves the body treating gluten as a foreign invader. The immune system attacks the gluten, as well as the lining of the gut. This damages the gut wall, and may cause nutrient deficiencies, anemia, severe digestive issues and an increased risk of many diseases. The most common symptoms of celiac disease are digestive discomfort, tissue damage in the small intestines, bloating, diarrhea, constipation, headache, tiredness, skin rashes, depression, weight loss and foul-smelling feces. However, some people with celiac disease do not have digestive symptoms, but may have other symptoms like tiredness or anemia. For this reason, celiac disease can be very difficult to diagnose. In fact, up to 80% of people have celiac disease don't know that they have it. Bottom Line: Celiac disease is an autoimmune disorder that makes the body attack gluten in the digestive system. This can cause severe digestive disorders and other health problems. Non-celiac gluten sensitivity There are many people who do not test positive for celiac disease, but still react negatively to gluten. This condition is called non-celiac gluten sensitivity. It is currently not known how many people have this condition, but it has been estimated to be in the range of 0.5-13%. Symptoms of gluten sensitivity include diarrhea, stomach pain, tiredness, bloating and depression. There is no clear definition of non-celiac gluten sensitivity, but the diagnosis is made when a patient reacts negatively to gluten, but celiac disease and allergies have been ruled out. However, some experts believe this isn't a real condition. They think the adverse effects are imaginary or caused by substances other than gluten. One study looked at almost 400 people with self-diagnosed gluten intolerance, and investigated whether they improved on a gluten-free diet (20). The results showed that only 26 people had celiac disease, while 2 had a wheat allergy. Only 27 of the remaining 364 people were diagnosed as gluten sensitive. That means that of the 400 who thought they were gluten intolerant, only 55 people (14.5%) actually had an issue with gluten. Therefore, many people who think they're gluten intolerant actually have other causes for their symptoms. Bottom Line: Many people react negatively to gluten but do not have celiac disease. This condition, known as non-celiac gluten sensitivity, is controversial. Irritable bowel syndrome, wheat allergy and others Irritable bowel syndrome (IBS) is a common digestive disorder that causes symptoms like abdominal pain, cramping, bloating, gas and diarrhea. It's a chronic condition, but many people are able to manage their symptoms with diet, lifestyle changes and stress management. Interestingly, studies have shown that some individuals with IBS may benefit from a gluten-free diet. For about 1% of the population, a wheat allergy may be causing digestive issues after consuming gluten. Furthermore, studies have shown that a gluten-free diet may benefit some individuals with schizophrenia, autism and a disease called gluten ataxia. Bottom Line: Gluten may be problematic for people with irritable bowel syndrome and wheat allergy. People with schizophrenia, autism and gluten ataxia may also benefit from a gluten-free diet. . . .
