PLEASE HELP! She is between 7-11 years old. She had the sore nose for about 2 months and we just found out that it was tumour. The most experienced veterinarian here told us that she will need leathal injection sooner or later. We are looking for advice from a veterinarian who had treated these cases in the past. Can it be treated with surgery? What exact chemotherapy will she need? Anything else? Facts; she is "not to be caged crazy" cannot be hold for topical treatment. The medication should be given by mixing it with the food. The food can be fully controlled and she doesn't eat anywhere else. Neither, other cats come and eat her food. PLEASE HELP! Intubation Difficult Airway
A Case of Huge Oral Tumor in Maxillofacial Surgery The article is contributed by Anesthesia Department of Peking University International Hospital. The case is performed by Resident Dr. Zhang Ying, Chief Physician Dr. Liu Kunpeng, and Director of Anesthesia Department Dr. Yao Lan. Medical history: Female patient, 56 years old, was found with left lower gingival mass 3 months ago. The gingival mass was about thumb nail size with local swelling and pain. After anti-inflammatory treatment, the swelling was slightly relieved, but the growth of the tumor accelerated. The pathology was admitted in other hospital and was diagnosed as "low-differentiated sarcoma." Due to problems such as coagulation disorder in patients, she received treatment in a large-scale tertiary hospital and was given 800 ml of frozen plasma. The tumor grew rapidly to be full of oral cavity with a goose egg size, local necrosis and eating difficulty. She came to our hospital for surgical treatment. The patient’s weight was significantly reduced since the illness. In 2002, the patient used to receive tongue cancer surgery in Peking University Stomatological Hospital and gave up 60Gy after therapy. Preoperative Evaluation When the patient is assessed in the anesthesia clinic before surgery, the anesthesiologist warns the patient to be a difficult airway in advance, and it is extremely difficult to predict tracheal intubation, which is a serious challenge. Patients can not receive trachea intubation from oral cavity because: (1) the tumor in the mouth is huge, occupying the entire mouth, the laryngoscope and other common intubation tools can not enter, and the tumor blocks line of sight; (2) the tumor is easily bleeding by touching, coughing or even airway obstruction can be caused by blood flowing into trachea; (3) Oral tracheal intubation interferes surgical operation. Nasal tracheal intubation was selected during anesthesia induction, including: avoiding respiratory tract occlusion after the administration of the anesthetic-inducing drug; nasal intubation under awake conditions not only ensured patient safety, but also did not interfere the operation. Before operation, the patient was interviewed with an electronic flexible laryngoscope (insight iS3-F). After the anesthesiologist explained to the patient and her family with mutual communication, the patient was subjected to exploratory observation of the nasal route under the anesthesia of lidocaine in the ward treatment room. This observation is mainly to understand: Is the tracheal intubation approach smooth? Is there a tumor occlusion occupancy? Can the patient cooperate well? It also enables patients to prepare physically and psychologically for the next day's related operations. Surgical Procedure and Outcome On the day of surgery, in addition to routine preparation of supplies and medicines, difficult airway rescue equipment and medicines are also prepared. After the patient was admitted to the room, she was given sedation and oxygen inhalation. After communicating with the patient, the eczema was contracted by the nasal cavity to contract the nasal blood vessels. The perivascular puncture was sprayed with tetracaine for tracheal surface anesthesia, and the vacuum suction device and the rescue device were placed with easy access. After the surface anesthesia was effective, the tracheal cannula was placed on the electronic flexible laryngoscope (iS3-F), and the flexible scope was operated to slowly and gently penetrate the patient's nasal cavity and pharyngeal cavity, and gradually entered slowly until the patient was disgusted and the glottis was observed from the display. The patient was instructed to inhale slowly and the operator continued to advance flexible scope after the glottis opened, then inserted a tracheal tube along the flexible scope, connected the ventilator and gave an intravenous anesthetic. After successful nasal intubation, the operation started. The operation lasted for 7 hours. The tracheotomy was performed, tracheal tube was removed, and the portable monitor and transport ventilator were connected to the ICU ward for further monitoring. On the 26th day after surgery, the patient's vital signs were stable, the mental state was good, and the procedure was discharged. Experience Summary Pre-assessment of the patient's airway condition before surgery is critical. Before the tracheal intubation, adequate preparation of relevant instruments, anesthetic drugs and rescue drugs must be prepared; preoperative communication with patients, family members and surgeons is reasonable, and patients are well coordinated. Perfect surface anesthesia is an important part of the successful awake tracheal intubation; adequate preparation before surgery and flexible adjustment according to the patient's condition during surgery are also an important part of the anesthesia plan. We have been working hard so that patients can get good therapeutic progress.

Swallowing, or deglutition, is the process by which food passes from the mouth, through the pharynx and into the esophagus. As simple as it might seem to healthy people, swallowing is actually a very complex action that requires an extremely precise coordination with breathing since both of these processes share the same entrance - the pharynx. Failure to coordinate would result in choking or pulmonary aspiration. Swallowing involves over twenty muscles of the mouth, throat and esophagus which are controlled by several cortical areas and by the swallowing centers in the brainstem. The brain communicates with the muscles through several cranial nerves. Swallowing consists of three phases: 1. Oral or buccal phase: this is the voluntary part of swallowing, the food is moistened with saliva and chewed, food bolus is formed and the tongue pushes it to the back of the throat (the pharynx). This process is under neural control of several areas of cerebral cortex including the motor cortex. 2. Pharyngeal phase starts with stimulation of tactile receptors in the oropharynx by the food bolus. The swallow reflex is initiated and is under involuntary neuromuscular control. The following actions are taken to ensure the passage of food or drink into the esophagus: - The tongue blocks the oral cavity to prevent going back to the mouth. - The soft palate blocks entry to the nasal cavity. - The vocal folds close to protect the airway to the lungs. The larynx is pulled up with the epiglottis flipping over covering the entry to the trachea. This is the most important step since entry of food or drink into the lungs may potentially be life threatening. - The upper esophageal sphincter opens to allow passage to the esophagus. 3. Esophageal phase: food bolus is propelled down the esophagus by peristalsis - a wave of muscular contraction that pushes the bolus ahead of it. The larynx moves down back to original position.

History of malunited facial fractures after RTA surgery elsewhere This young man had suffered injuries to his face with fractures from a motor bike accident. This happened in his hometown a few years ago. He had undergone emergent surgery, which resulted in a malunited nasal bone fracture. His nasal septum was also deviated to the left side. He has been suffering from breathing problems and snoring since then. He decided to get this corrected. A general dentist in his hometown referred him to our hospital for correction of his problem. Treatment plan explained to the patient The patient presented at our hospital for management of his multiple facial deformities. Dr. SM Balaji, Cranio-Maxillofacial surgeon examined the patient. He explained the treatment plan to the patient. This also involved harvesting a rib graft from the patient. The patient was in full agreement with the treatment plan. Deviated nasal septum set right to correct breathing problems Under general anesthesia, a rib graft was first harvested from the patient. A Valsalva test demonstrated patent thoracic cavity. The incision was then closed in layers. Attention was then turned to the nose. The deviated nasal septum was first refractured. A nasal speculum was then used to widen the nasal choanae. Intranasal incisions were then performed to avoid visible scarring. Hardware from previous surgery removed Attention was next turned to the zygomaticomaxillary bone. Maxillary vestibular incisions were then made in the buccal sulcus region. Old hardware from the previous surgery was exposed and removed. Sutures were then used to close the incisions. Augmentation rhinoplasty performed The rib graft was next crafted to fit into the bridge of the nose. Graft was then placed through the intranasal incision to fit into the bridge of the nose. This augmented the nasal bridge with precision. All incisions were then sutured. Nostril breadth reduction surgery performed Attention was next turned to the nostril reduction procedure for the patient. Crescent shaped incisions were then marked out at bilateral alar bases. The tissue was next incised and removed. The alar bases were then sutured closer to the midline. This resulted in narrowing of the nose. The patient expressed his utmost satisfaction to Dr SM Balaji before final discharge. He said that his nasal form was back to its original shape. How 3-D Printing Could Change Breast Cancer Treatment
http://www.dailyrxnews.com/ After breast cancer surgery, many women opt for breast reconstruction. One company is working to revolutionize that procedure. The company, TeVido Biodevices, is trying to develop 3-D bioprinted nipples. "The technology, if we are successful with this, will be utilized and could change the industry," said Dr. Scott Collins in an interview with dailyRx News. Dr. Collins, chief technology officer of product development for TeVido Biodevices in Austin, TX, said 3-D bioprinting creates 3-D human tissue for patients from their own cells. "There's a lot of hype out there, but there's also a lot of interest. So, conceivably a lot of these things will converge into products. It's just a question of how do we make that happen quicker?" Dr. Collins said. Before they print out hearts or kidneys, the researchers at TeVido are starting with something much smaller — but it could still have a big impact.
