Intubation Difficult Airway
A Case of Huge Oral Tumor in Maxillofacial Surgery The article is contributed by Anesthesia Department of Peking University International Hospital. The case is performed by Resident Dr. Zhang Ying, Chief Physician Dr. Liu Kunpeng, and Director of Anesthesia Department Dr. Yao Lan. Medical history: Female patient, 56 years old, was found with left lower gingival mass 3 months ago. The gingival mass was about thumb nail size with local swelling and pain. After anti-inflammatory treatment, the swelling was slightly relieved, but the growth of the tumor accelerated. The pathology was admitted in other hospital and was diagnosed as "low-differentiated sarcoma." Due to problems such as coagulation disorder in patients, she received treatment in a large-scale tertiary hospital and was given 800 ml of frozen plasma. The tumor grew rapidly to be full of oral cavity with a goose egg size, local necrosis and eating difficulty. She came to our hospital for surgical treatment. The patient’s weight was significantly reduced since the illness. In 2002, the patient used to receive tongue cancer surgery in Peking University Stomatological Hospital and gave up 60Gy after therapy. Preoperative Evaluation When the patient is assessed in the anesthesia clinic before surgery, the anesthesiologist warns the patient to be a difficult airway in advance, and it is extremely difficult to predict tracheal intubation, which is a serious challenge. Patients can not receive trachea intubation from oral cavity because: (1) the tumor in the mouth is huge, occupying the entire mouth, the laryngoscope and other common intubation tools can not enter, and the tumor blocks line of sight; (2) the tumor is easily bleeding by touching, coughing or even airway obstruction can be caused by blood flowing into trachea; (3) Oral tracheal intubation interferes surgical operation. Nasal tracheal intubation was selected during anesthesia induction, including: avoiding respiratory tract occlusion after the administration of the anesthetic-inducing drug; nasal intubation under awake conditions not only ensured patient safety, but also did not interfere the operation. Before operation, the patient was interviewed with an electronic flexible laryngoscope (insight iS3-F). After the anesthesiologist explained to the patient and her family with mutual communication, the patient was subjected to exploratory observation of the nasal route under the anesthesia of lidocaine in the ward treatment room. This observation is mainly to understand: Is the tracheal intubation approach smooth? Is there a tumor occlusion occupancy? Can the patient cooperate well? It also enables patients to prepare physically and psychologically for the next day's related operations. Surgical Procedure and Outcome On the day of surgery, in addition to routine preparation of supplies and medicines, difficult airway rescue equipment and medicines are also prepared. After the patient was admitted to the room, she was given sedation and oxygen inhalation. After communicating with the patient, the eczema was contracted by the nasal cavity to contract the nasal blood vessels. The perivascular puncture was sprayed with tetracaine for tracheal surface anesthesia, and the vacuum suction device and the rescue device were placed with easy access. After the surface anesthesia was effective, the tracheal cannula was placed on the electronic flexible laryngoscope (iS3-F), and the flexible scope was operated to slowly and gently penetrate the patient's nasal cavity and pharyngeal cavity, and gradually entered slowly until the patient was disgusted and the glottis was observed from the display. The patient was instructed to inhale slowly and the operator continued to advance flexible scope after the glottis opened, then inserted a tracheal tube along the flexible scope, connected the ventilator and gave an intravenous anesthetic. After successful nasal intubation, the operation started. The operation lasted for 7 hours. The tracheotomy was performed, tracheal tube was removed, and the portable monitor and transport ventilator were connected to the ICU ward for further monitoring. On the 26th day after surgery, the patient's vital signs were stable, the mental state was good, and the procedure was discharged. Experience Summary Pre-assessment of the patient's airway condition before surgery is critical. Before the tracheal intubation, adequate preparation of relevant instruments, anesthetic drugs and rescue drugs must be prepared; preoperative communication with patients, family members and surgeons is reasonable, and patients are well coordinated. Perfect surface anesthesia is an important part of the successful awake tracheal intubation; adequate preparation before surgery and flexible adjustment according to the patient's condition during surgery are also an important part of the anesthesia plan. We have been working hard so that patients can get good therapeutic progress.

Marlin died the day after this from a bleeding spleen mass
Marlin (white chest, barking dog) was playing in this video and the next night he was very lethargic and I took him to the emergency vet where they found a large mass on his spleen. He bled to death from it before they could remove it. There were really no symptoms. These spleen tumors are known as the "silent killer" because often by the time you notice something is really wrong it is too late. He was playing and eating the day before. He got in a day of playing in the river a week before he died too. The only good side is it was quick and we had a lot of quality time together (hikes and playtime) just before he died. If you dog seems lethargic, has weakness in the hind legs and is drinking more, it could be a spleen tumor. We thought it was arthritis and a reaction to starting Rimadyl. Anmol's greedy wife Mona forces him to meet Vikrant in India and inform him about Guru Gulab and collect the prize money. Anmol comes to India with his father-in- law and the moment they step into India they are picked up by Choto Chhatri, who takes them to a local goon, Eaida Anna. Anna strikes a deal with Vikrant that he'll kill Guru and acquire the prize money offered by him. Vikrant gives him the contract. So Anmol returns, not with prize money but with Anna and Chota Chhatri, and others have this wrong notion that Anna is just interested in the petty contract money. His interest lies in the diamonds. Squamous cell carcinomas can develop on the skin and inside the mouths of dogs. Location of squamous cell carcinomas is strongly associated with survival times in the dog. Growths on the skin are often found early by owners and respond very well to surgery alone with little likelihood, less than 20%, of spread to other locations. 7. Mammary Carcinoma. Tumors of the mammary glands are the most common tumor seen in unspayed female dogs. They can affect any of the mammary gland and may behave locally and respond well to surgery or more aggressively with ulceration or open wounds of the mammary gland and rapid tumor growth. 8. Apocrine Gland Carcinoma (Anal Sac Cancer) Apocrine gland carcinomas are also known as carcinomas of the anal sac in the dog. Approximately 50% of these tumors are diagnosed by your veterinarian during a routine rectal exam and show no apparent clinical signs. They can also be diagnosed if your dog exhibits increases in blood levels of calcium because the tumor secretes a hormone-like substance that causes hypercalcemia (elevated calcium). Dogs that have surgery alone for apocrine gland carcinoma usually survive 1 year while additional treatments can increase this to 18 months. 9. Transitional Cell Carcinoma. Transitional cell carcinoma is the most common tumor of the lower urinary system (bladder and urethra) in the dog. This tumor is considered locally invasive and is moderately to highly likely to metastasize to another area. Much like the tumors of the anal gland, surgical options are dependent on the location, and often, these tumors cannot be completely removed. In such cases, chemotherapy or even anti-inflammatory drugs may be recommended. 10. Soft Tissue Sarcoma. Soft tissue sarcomas are a group of several different types of tumors that share similar characteristics. These tumors are made of connective tissue and are located either within the skin or in tissues just below the skin. Owners often find these tumors when casually petting or grooming their dogs. Tumors that are included in this group are fibrosarcomas, hemangiopericytomas, liposarcomas, leiomyosarcomas, and nerve sheath tumors.
