Avoiding malpractice is more than avoiding a lawsuit, it is avoiding the litigation process altogether. Proper documentation is arguably the most important element in avoiding malpractice and litigation for nurses. There are many reasons why nurses spend much of their time charting. However, when it comes to malpractice and the litigation process, documentation may be the only evidence a nurse has to defend his or her position. By the time a lawsuit is filed, chances are the nurses will not remember the particular patient involved, especially if they only cared for the patient for one or two shifts. Even if the nurses do remember the patient, it is their documentation that will be reviewed, presented and relied on, not their memory. All nurses have been taught, "If it's not written, it's not done!" But, what exactly does that mean? Whether you are a student nurse or have been in nursing for thirty years, this question may be difficult to answer. Over the years, documentation has changed a great deal. But, the rationale behind why documentation is important remains the same. Whether you are documenting with a narrative style, using flow sheets or charting by exception, the purpose of documentation is to memorialize what occurred while you took care of your patient and to capture relevant information about the patient's condition and medical history. The more accurately your documentation depicts what actually happened during the time you took care of your patient, the more likely you are to avoid becoming involved in litigation for malpractice. Regardless of the type or style of charting a nurse uses, it is important for documentation to be legible, logical, and complete. Not only to prove what occurred, but to also show what did not occur. For example, if a patient is admitted to a facility with both arms in tact, and claims to have left the facility with a broken arm, the first thing the patient's attorneys and their experts are going to do is review the medical records. They will try to determine from the records when and how this patient's arm could have been broken.

Impress your colleagues by already knowing how by following these steps. Step 1: Bring a notebook Bring a notebook to the meeting with you. If you will be writing minutes often, you may want to buy a good notebook to keep your minutes in rather than a cheap one. Tip If you have a laptop, you should use it to save time later. Step 2: Send around a sign in sheet Create a simple sign-in sheet that has columns for name and contact information and pass it around. You will be able to refer to this record in order to document attendance. Tip Find out who was supposed to be there and make sure you include the people who did not attend in the minutes under regrets. Step 3: Begin note-taking Begin note-taking by noting the date of the meeting and the time the meeting was called to order. Step 4: Record old business Record old business, which is anything that needed a follow up from the last meeting. Note anything that was done or not done and by whom. Step 5: Record new business Record new business. For example, when someone has an issue to address they will make a motion. Note exactly what they said along with who seconded the motion and whether or not the motion passed. Step 6: Finish notes Write down any action items or things specific participants agreed to do, but don't transcribe word for word debates or discussions. End the notes with the time the meeting adjourned and the date of the next meeting. Step 7: Type and distribute Type up the minutes as soon as possible after the meeting so it's fresh in your mind. Once you have typed them, distribute them to members for review and approval at the next meeting. Did You Know? During one day, there is an average of 17 million meetings in America.

Top 10 Fields To Invest Your Future In Predicting the future is a handy skill to have when it comes to enrolling in college, starting a fresh career, or investing in new skills. Here are, according to U.S. News, the Top 10 Fields To Invest Your Future In. 1. Data Crunching The era of big data is just getting started, with many firms eager to tap vast new databases to gather more info on their customers, their competitors, and even themselves. The challenge isn't just crunching numbers; it's making sense of them, and gaining useful insights that can be translated into a business edge. Marketing and market research are two growing fields where the use of data is exploding. 2. Counselling & Therapy There's now widespread recognition that mental health is as important as physical health, which is likely to increase demand for professionals in this field. The US Bureau of Labour Statistics (or the BLS) expects the need for marriage and family therapists, as one example, to grow 41 percent by 2020. 3. Scientific Research New technology will continue to generate breakthroughs in medicine, manufacturing, transportation, and many other fields, which means there will be strong demand for workers schooled in biology, chemistry, maths, and engineering. Some areas that show particular promise: biotechnology and biomedicine, nanotechnology, robotics, and 3D printing, which allows the manufacture of physical products from a digital data file. 4. Computer Engineering A lot of software development is done overseas these days, but the need for high-level computer experts able to tie systems together is still strong. In finance and investing, for instance, high-speed computing is increasingly a prime competitive advantage. And most big companies will need networks that are faster, more seamless, and more secure. 5. Veterinarians Pets are more popular than ever, and some of them get medical care that's practically fit for a human. The BLS expects the need for vets to rise 36 percent by 2020. 6. Environmental & Conservation Science Making better use of the planet's resources will be essential as population growth strains existing infrastructure. Green energy, despite some political controversy, still seems likely to boom. Developers need more efficient ways to heat and cool buildings. And dealing with global warming may require new technology not even on the drawing board yet. 7. Healthcare It's well-known that the aging of the baby boomers will require more caregivers in many specialties. Some healthcare jobs tend to be low-paying, with a lot of workers flocking to what are supposed to be "recession-proof" fields. And the need to lower overall healthcare costs could pinch some doctors, hospital workers, and diagnosticians. But demand should be strong for nurses, optometrists, audiologists, dentists, physical therapists, and some doctor specialists. 8. Management The boss earns a lot for good reason: His job isn't as easy as it might seem. Effective management in the future will require basic business knowledge plus the ability to oversee operations in many locations and countries, and some technical know-how. Anybody who can improve a unit's performance while lowering costs should rise quickly. The BLS and IBISWorld also expect growing demand for some support fields such as human relations, benefits administration, and event planning. 9. Finance The movement and management of money is technically complex, and integral to most companies. Plus, non-traditional investing firms such as hedge funds and private-equity firms are likely to grow as the traditional banking sector complies with new regulations and reins in risk-taking. That means there will be more need for finance experts. There may even be a shortage as students once interested in finance veer into other fields, turned off by the 2008 financial crisis and the vilification of banks. 10. Entrepreneurship It's often overlooked, but the need for innovators running their own businesses could be more important than ever in 2020. Forecasters expect strong growth in traditional businesses such as used-car dealers, hair and nail salons, pet grooming, and office services, which means anybody able to come up with better, cheaper ways to serve customers will reap a windfall. Technology start-ups will no doubt keep changing the way consumers work and live. And nobody really knows what the next iPad, Twitter, or Pinterest will be—except, perhaps, some entrepreneur who's dreaming about it right now. Some schools, particularly when applying to doctoral programs, will invite candidates to an on-campus interview. These interviews are generally frightening to candidates because there's not much in the way of guidance as to what to expect. 1. The first is to tell me more about yourself. There’s a good chance you’ll be asked this question. The most important thing that you want to do is to have a synthesized answer, no more than one or two minutes, that showcases your personal, academic, and professional accomplishments in a nutshell. In other words, a kind of preview. Don’t go on a rant, and don’t go on a tangent! 2. Second, you may be asked: why are you interested in this field? Again, go back to your personal statement and think about what you wrote in that statement and try to expand on it. Perhaps you can go deeper into some of the reason that you discussed or introduce addition reasons that were not discussed in the personal statement. in either case, try to make a compelling case as to why this particular field is what you were born to do. 3. Third, why are you interested in our school? This is almost certainly going to be asked in the interview process. Think about specific elements of the program that set it apart -- the faculty, the facilities, the curriculum, student activities, job placement record, and other reasons -- and make a compelling argument once again. 4. Fourth, what are you going to research? This is particularly relevant if you’re applying to a doctoral program, but for master’s applicants too there’s often a master’s thesis and this is something that you can mention. In answering this question, you don’t necessarily have to have a definitve answer that you’re going to be held accountable to, as much as you want to demonstrate that you’ve conducted some preliminary thinking on the subject and that you're a serious candidate for such research. 5. Fifth, think about your strengths and weaknesses; your interviewer may very well ask you to talk about them. For strengths, pick ones that are relevant to academia, for example critical thinking, quantitative abilities, qualitative abilities, research capabilities, and so forth. And with respect to weaknesses, try to pick weakness that are really weakness turning into strengths; so, for example, if you’re a soft-spoken individual you might say that your weakness is that you’re occasionally soft-spoken but that you’ve taken steps to be more assertive. 6. Sixth, you’ll probably be asked, in so many words, why they should accept you. Have a bullet-proof answer for this question. Talk about your personal, academic, and professional achievements, discuss your long-range plans and how you plan to take full advantage of the university’s resources to make them come true. You want to convince them that they’re not going to be wasting an admissions spot with you. 7. Seventh, you may be asked to discuss your career goals. If you’re applying to a doctoral program, it’s usually appropriate to discuss the probability of teaching, and if you’re applying to a master’s program you can also discuss the possibility of extending your studies in a doctoral program or professional applications. 8. Eight, you may be asked where else you’re applying. This is a very difficult question to navigate. If you answer and provide full details about all the place that you’re applying to, you may dilute your application to the university in question. On the other hand, if you’re too secretive you may alienate your interviewer. A possible middle of the road approach is to indicate that you’ve applied to a number of other universities, without being too specific, but reiterate how important it is for you to be accepted to the one that you’re talking about today. Reiterate your desire and how much you’d love to have a position in the program. 9. Ninth, you may be asked to discuss what you’ve read recently. Ideally, you should be able to cite scholarly publications, whether it’s journal articles or books published by academics that demonstrate that you’re connected with the community. 10. Tenth, you may be asked if you have any questions for the interviewer. Think about these questions carefully; you can prepare them ahead of time so you’re not nervous. Try to ask questions that either delve more deeply into the interviewer’s background and research interests or that explore aspects of the program that are not obviously stated on the web site or the marketing materials of the university.

Opening of Workshop for Strategic Planning program for Primary School Headteachers - Part 1

The passing of the new draft constitution will impact positively on the provision of quality education in the country, Vice President Kalonzo Musyoka has said. Mr. Musyoka said the new constitution dispensation will open up the country for investment in education, therefore making it the education hub of the region. The Vice President made the remarks today when he opened a three-day Workshop for Greater Mwingi District Primary School Headteachers Strategic Planning program at a Nakuru hotel. Mr. Musyoka said in the new draft constitution Kenyans are guaranteed shelter as a basic human right, noting that even developed countries don't provide. He urged the headteachers to be agents of change in their areas of jurisdiction, adding that the strategic plans they are deliberating on will be more meaningful in the new constitution. He appealed to headteachers to give sufficient motivation to students so that they can improve education, saying their image needs to be safeguarded in order for students to perform well. The Vice President added that in the new constitution education will be a basic human right and reminded them that corporal punishment had been outlawed in schools. Mr. Musyoka disclosed that he had invited all the clergy and leaders from South Eastern Region for a consultative meeting to iron out the contentious issues in the draft constitution on Friday in Machakos Golf Club.
