And it's important to test for this because if a woman has gestational diabetes or if she had previously diagnosed type 1 or type 2 diabetes, if uncontrolled, it can cause complications like preeclampsia, blood sugar problems for the baby after delivery, stillbirth, and an abnormally large baby, which has complications associated with that too. 5% to 18% of women develop gestational diabetes, and it all goes back to insulin resistance. Insulin is the key that unlocks your body's cells so that sugar can go inside and make the cells work. And your body naturally becomes a little more insulin-resistant during pregnancy so that there's enough sugar circulating in your blood so the baby gets what it needs. Normally, mothers can compensate for this by producing more insulin, but mothers who have gestational diabetes aren't able to make more insulin, and so blood sugar levels rise. So testing, like I said, for this is done are 28 weeks, and what will happen is the doctor will have you drink a very sugary drink. It contains 50 grams of glucose. And then they'll test your blood sugar an hour later and see how your body responded. If you get a value of 130 to 140, then you may have gestational diabetes, and the doctor will want to do a follow-up test. If the value you get on your first test is 200 or higher, then you just have gestational diabetes, but they may still want to do follow-up testing. And in both cases, it's doing a 3 hour glucose tolerance test. This is done when you're fasting, and then you drink double the amount of sugar. It's a 100 gram glucose sugar drink. And then they test your blood sugar every hour for 3 hours and see what it is. And then, based on those values, they determine if you truly have gestational diabetes or not. If the diagnosis is made, then there's a couple of different ways to manage it. And it's super important to keep your blood sugars under control so that you don't have the complications I discussed before. The management plan is going to be dependent on how severe your case is. And for a lot of women, it just involves some diet changes and exercise. For other women, it involves taking oral medication. And for about 15% of women with gestational diabetes, it has to be controlled with insulin. And you may start out just needing to make a few diet changes, and then as the pregnancy progresses, so do your body's needs, and so do your baby's needs, and so that may change the plan as well. So you could start out being diet-controlled, and then you may end up with oral medication or with insulin. So this speaks of the importance of consistent monitoring. A diabetic educator will meet with you and talk to you about your treatment plan and also tell you how often you should be monitoring your blood sugars. They'll take a peek at your values each week and, based on that, determine if additional changes in your treatment plan are necessary. Generally the high risk patients are those who had GDM in previous pregnancies, elderly pregnant women, PCOS, obese and people with strong family history. Usually in a planned pregnancy, pre pregnancy control few steps can be taken to avoid GDM in future that usually maintaining an ideal BMI by regular exercise, diet and losing weight. During pregnancy usually the GDM develops in second or third trimester though it can also start from the beginning itself. The fine tuning of diet and exercise is required to avoid GDM. It can be ideally done by maintaining ideal weight during pregnancy that is no weight gain about 6 months of pregnancy or total weight gain till 9 months about 4-5 kg is enough. Also high risk patient should go for regular exercises after consulting the doctor, 30 minutes walk per day on most of the days of a week, antenatal exercises as advised by the doctor, they can go for a swim which is good exercise during the pregnancy. Apart from that the diet should be controlled from the very beginning, no extra sugar, white sugar should be completely avoided, rice, high fiber diet should be taken from the beginning as it absorbs extra sugar from the body, it should be low carbohydrate and low fat diet. With this tuning probably the Gestational Diabetes can be put at hold, also regular checkups, good follow up with doctor, sugars should be checked at the first visit at about 6 months and almost every month at high risk case.

How to Deal with Gestational Diabetes during Pregnancy While all else may be going just fine, some expecting mothers start developing high blood sugar levels between the 24th and 28th weeks of pregnancy. This is known as gestational diabetes mellitus (GDM). Similar to other types of diabetes, gestational diabetes affects how your cells use sugar (glucose). Up to 9.2 percent of pregnant women suffer from this problem, according to a 2014 analysis by the Centers for Disease Control and Prevention. In fact, it is recommended that all women get tested for gestational diabetes at the beginning of the third trimester. The exact cause of GDM is unknown, but the hormonal changes during pregnancy play a key role. During pregnancy, the body produces larger amounts of some hormones like human placental lactogen, estrogen and hormones that increase insulin resistance. These hormones affect your placenta and also interfere with the action of insulin, the hormone that regulates your blood sugar. Women over the age of 25 who suffer from high blood pressure, have a family history of diabetes and were overweight before becoming pregnant are at a higher risk of developing GDM. Also, women who have previously given birth to a baby weighing more than 9 pounds or had an unexplained miscarriage or stillbirth are at a higher risk. Some of the mild symptoms of gestational diabetes may include fatigue, blurred vision, excessive thirst and excessive need to urinate. Developing GDM during pregnancy doesn’t mean that you will have diabetes after delivery. In most cases, the blood sugar levels of pregnant women with gestational diabetes tend to return to normal within a few hour or days post delivery. However, it is recommended that women who had gestational diabetes during pregnancy get tested for diabetes every 2 to 3 years as they are at an increased risk for developing Type 2 diabetes. As gestational diabetes occurs in late pregnancy after the baby’s body has been formed, it does not cause the kinds of birth defects that can be seen in babies born to mothers who had diabetes before pregnancy. However, untreated or poorly controlled gestational diabetes can lead to macrosomia, or a “fat” baby. Babies with macrosomia can have very low blood glucose levels at birth or suffer from breathing problems. Such babies are at risk for obesity and Type 2 diabetes in the future. Gestational diabetes can be controlled and treated with lifestyle and dietary changes and medicines, in some instances. Here are the top 5 ways to deal with gestational diabetes during pregnancy. 1. Monitor Blood Sugar Level Regularly In order to reduce the risk of complications from gestational diabetes, it is important to keep a close eye on your blood sugar level. 2. Eat a Healthy Breakfast It’s very important to eat a healthy breakfast. Skipping breakfast is a big NO for pregnant women. 3. Eat the Right Carbohydrates Carbohydrates are an important part of any kind of diabetes diet, including GDM. Carbohydrates provide the fuel for your body and for your growing baby. 4. Eat High-Fiber Foods To keep your blood sugar level normal, it is important to increase your fiber intake. Fiber stimulates the activity of insulin receptors and inhibits the release of excess insulin into the bloodstream, thus helping to balance the blood sugar level. 5. Developing GDM during pregnancy doesn’t mean that you will have diabetes after delivery. In most cases, the blood sugar levels of pregnant women with gestational diabetes tend to return to normal within a few hour or days post delivery. However, it is recommended that women who had gestational diabetes during pregnancy get tested for diabetes every 2 to 3 years as they are at an increased risk for developing Type 2 diabetes. As gestational diabetes occurs in late pregnancy after the baby’s body has been formed, it does not cause the kinds of birth defects that can be seen in babies born to mothers who had diabetes before pregnancy. However, untreated or poorly controlled gestational diabetes can lead to macrosomia, or a “fat” baby. Babies with macrosomia can have very low blood glucose levels at birth or suffer from breathing problems. Such babies are at risk for obesity and Type 2 diabetes in the future. It's hard to say. The chances of your baby being large are reduced by you controlling your blood sugars well during the pregnancy. So if your doctor has recommended a certain diet make sure you follow that and if your doctor has recommended medication to control your blood sugars, make sure you're taking that as prescribed and checking your blood sugars as frequently as your doctor has suggested as well. It's likely that in your third trimester you'll go to see some specialists that will watch you and the baby for signs of complications from diabetes. Usually your doctor will have you go to a specialist called a perinatologist to make sure that your baby is growing well and that the pregnancy overall is going well. They'll probably also have you talk with diabetic educators that will have you keep a log of your blood sugars and have you come in and discuss any complications or issues you're having with them. Your doctor is also likely to have you get frequent non-stress tests where you come in and they put your baby on the monitor, meaning they watch your baby's heart rate and your contraction pattern, for a short period of time to make sure your baby has enough oxygen and is doing well. Most women with GD are told by their care providers they will need to be induced before their due date, destroying their hopes for a natural and intervention free birth. Having GD can have a big impact on mothers’ and babies’ health during pregnancy, but does it really need to change how we give birth? First let’s answer the question, “What Is Gestational Diabetes?” Gestational diabetes is a condition that only occurs during pregnancy. Certain pregnancy hormones interfere with your body’s ability to use insulin. Insulin converts blood sugar into usable energy, and if this doesn’t happen, blood sugar levels rise. Insulin resistance can cause high blood glucose levels and can eventually lead to gestational diabetes. Some women will have symptoms of GD, while others will have none. Many women can manage the condition without treatment other than dietary and exercise changes. Around 20% of women will need insulin injections. So for many women, well managed diabetes means they can still have healthy babies and normal births. GD and Induction Of Labor. Many health care providers routinely recommend that women with gestational diabetes be induced around 38-39 weeks. The most common reasons given for induction at this gestation are to prevent stillbirth, and to prevent babies growing too large for vaginal birth. However, the evidence related to induction for women with GD comes from the review of one trial, which looked at 200 women who had either GD, Type 1 or Type 2 diabetes. The World Health Organization states this evidence for induction before 41 weeks’ gestation is weak, and induction before 41 weeks should not be suggested if gestational diabetes is the only medical issue. The American Congress of Obstetricians and Gynecologists (ACOG) guidelines for gestational diabetes also do not recommend induction of labor before 39 weeks for women with well controlled GD. The evidence for induction of labor is neither strong, nor clear. A recent review looking at the evidence supporting indications for induction concluded there wasn’t strong enough evidence for induction of women with gestational diabetes. Why Be Induced? There are several reasons why care providers will recommend induction for women with GD. What is important to remember is that many of these complications are more relevant to those women whose condition is not well managed. Large Baby A large baby (macrosomia) is considered to be a problem if you have gestational diabetes. The extra sugar in your bloodstream crosses the placenta and triggers your baby to make more insulin. This can cause your baby to store more fat and tissue and be larger than average (over 9lb). It’s not possible to diagnose macrosmia before birth. Doing ultrasounds in the third trimester allows care providers to make an educated guess, but the estimated weight prediction can vary from the actual birth weight by up to 15%. In the US 10.4% of babies weigh more than 8 lb at birth. If you have GD, there is a 13.7% chance of having a baby over 9 lb, so the risk of having a large baby is increased, but not by a great deal. Keeping your GD well managed can reduce that risk of a large baby by up to 50%. If care providers believe a baby is already large for gestational dates, they might suggest early induction to avoid a c-section later. This is due to a fear of shoulder dystocia, which is when the baby becomes stuck in the pelvic outlet because his shoulders are too wide to pass through. Most care providers consider this a medical emergency, needing interventions — such as episiotomy, forceps or vacuum delivery, or emergency c-section. Even though shoulder dystocia can be prevented and managed by a care provider, it sometimes results in a nerve injury called brachial plexus injury. This injury can also occur in babies who don’t have shoulder dystocia, and even after elective c-section. Death from lack of oxygen due to shoulder dystocia is possible but it’s extremely rare. Stillbirth The risk of stillbirth is higher for women with Type 1 or Type 2 diabetes and this is often used as an indication for induction for women who have gestational diabetes. High blood sugar can cause blood vessel damage to the placenta, which means poor oxygen and nutrient supply to the baby. This can lead to health complications for the baby, or to stillbirth or death. However, these complications rarely occur in pregnancies where gestational diabetes has been diagnosed and is well managed...

We would like to keep it in the normal range where it is below 140 mg/dl even after two hours of diet. So by way of diet change, you have to minimize your intake of carbohydrates and at the same time, maintain a balanced nutrition for the growing fetus. So instead of cutting down on carbohydrates totally, I would advice you to take small, frequent meals or you can distribute the meal intake. In between three meals, you can take some fruits and still maintain the blood sugar in the same level.. nnBest advice will be to take more of proteins and vegetables and fruits. To minimize sugars, sweets, candies and pastries. Do not skip meals and go on diet during pregnancy.. nnA little bit of exercise and an active life is mandatory. Ellen Seely, MD, Director of Clinical Research in the Division of Endocrinology, Diabetes and Hypertension at Brigham and Women’s Hospital discusses the health risks associated with gestational diabetes mellitus (GDM) during and after pregnancy. Dr. Seely also describes Balance After Baby, a web-based program designed to educate women on eating healthfully and increasing their physical activity, with the goal of helping them lose their pregnancy weight within a year of giving birth. GDM is a type of diabetes that occurs only in pregnancy. It comes on in the second half of pregnancy and it goes away after delivery. In the US it’s estimated that five percent of all pregnancies are complicated by GDM. The rates of GDM are higher in Hispanic and non-white populations, ranging from ten to 20 percent of all pregnancies. Obesity is one of the main risk factors for GDM. In the United States, most health centers screen all women for gestational diabetes because obesity is becoming so common in the overall population. Gestational diabetes can impact the health of the mother and the fetus, as well as the newborn. Women with GDM have an increased risk of preeclampsia, which is a type of high blood pressure that develops during pregnancy. Preeclampsia is a serious condition that can lead to early delivery. Women who have gestational diabetes may also have larger babies, increasing their risk of cesarean section. While GDM goes away after pregnancy, the health risks persist. Research has found that 50 to 70 percent of women who have gestational diabetes may develop Type 2 diabetes later in life. Research has also found that babies born to mothers with GDM, have a higher birth weight and may become obese during adolescence, increasing their risk of developing diabetes and other conditions such as high blood pressure. The Pregnancy and Diabetes Program at Brigham and Women’s Hospital offers multidisciplinary care to women with GDM. The program includes a high risk obstetrician, two diabetes specialists, a nutritionist, and a nurse practitioner who is certified in diabetes education. The first approach to treatment is to control blood glucose levels with healthy eating and physical activity. Lifestyle changes, healthy eating and physical activity, are effective in controlling blood glucose levels about 75 percent of women with GDM. If the lifestyle changes don't work, insulin therapy is started to control a mother’s blood sugar for the rest of her pregnancy. She can do it at home every alternate days if possible so that if there are any variations in the blood sugar level, she can immediately consult a doctor and bring to down to normal.
