Thyroid cancer is three times more common in women than in men. There are four major types of thyroid cancer: papillary, follicular, medullary (MTC), and anaplastic. The cause of thyroid cancer is unknown, but certain risk factors have been identified and include a family history of goiter, exposure to high levels of radiation, and certain hereditary syndromes. The National Cancer Institute recommends that anyone who received radiation to the head or neck in childhood be examined by a doctor every one to two years to detect potential thyroid cancer. The most common signs and symptoms of thyroid cancer include a lump, or thyroid nodule, that can be felt in the neck, trouble swallowing, throat or neck pain, swollen lymph nodes in the neck, cough, and vocal changes. If a lump in the thyroid is found, the only certain way to tell whether it is cancerous is by needle or surgery biopsy and examining the thyroid tissue obtained. A CEA blood test, physical exam, X-rays, CT scans, PET scans, ultrasounds, and MRIs may also be used to help establish a definitive diagnosis and determine staging. Surgery is the most common form of treatment for thyroid cancer that has not spread to other areas of the body. Radiation therapy, chemotherapy, and radioactive iodine treatment are also medical treatment options in addition to surgery. The survival rate and prognosis of thyroid cancer depends upon a few factors, including the individual's age, the size of the tumor, and whether there is metastasis or spread of the tumor. It is not possible to prevent most cases of thyroid cancer. Thyroid cancer is the third most common solid cancer tumor in children and the most common endocrine malignancy.

Causes of thyroid cancer that you were not aware of The exact cause of thyroid cancer is not known. Similar to how other cancer forms develop, changes in the DNA of cells is responsible for the development of thyroid cancer. These alterations could either be genetically inherited or occur over a period of time. But, it is well established that cancer is not contagious. 1 Thyroid cancer risk factors Exposure to radiation increases one's chances of getting thyroid cancer. A mere dental X-ray, however, does not increase the chances of developing thyroid cancer, but a radiation therapy to the head, neck or chest surely does. There has been significant amount of research to understand DNA changes that become cancerous. Most cancers are not inherited, which means that DNA gets damaged by the exposure to something in the environment such as radiation or smoke during the course of one's life. The cancer may also occur when unexpected changes take place inside a cell without a specific external cause. The following are the prominent risk factors for thyroid cancer: 2 Benign thyroid disease Thyroid conditions i.e., either underactive or overactive thyroid are not related to the risk of thyroid cancer in any way, however, non-cancerous conditions such as nodules (adenomas), goitre (an enlarged thyroid) and thyroiditis (inflammation of the thyroid) increase thyroid cancer risk. 3 Radiation and low iodine levels Thyroid cancer is more likely to develop in individuals who have had radiotherapy treatments at a tender age, though the cancer usually develops years later. Survivors of atomic explosions or accidents are also at an increased risk of developing thyroid cancer. Individuals with low levels of iodine in their body are at more risk of thyroid cancer after any exposure to radiation. 4 Family history of thyroid cancer Individuals with a family history of thyroid cancer are at a higher risk of developing thyroid cancer. 5 Bowel condition FAP Familial adenomatous polyposis (FAP), a condition caused by an inherited gene, increases the chances of developing thyroid cancer. 6 Breast condition history Women with non-cancerous breast conditions are at increased risk of thyroid cancer. 7 Cancer history Individuals who have been treated for breast cancer, oesophageal cancer, testicular cancer or non Hodgkin's lymphoma have high risk of developing thyroid cancer. These are the possible risk factors for thyroid cancer suggested by research reports over the years. Well-differentiated thyroid cancer is one of the most common endocrine cancers, and is often treated using radioactive iodine. However, the risk of subsequent cancer development from radioactive iodine treatment has not been previously determined. Remco Molenaar, MD, PhD, from the University of Amsterdam, Amsterdam, Netherlands, discusses the use of this radioisotope, and explains the results of a recent study which suggested that this treatment could increase the risk of patients developing acute myeloid leukemia. This interview was recorded at the European Society for Medical Oncology (ESMO) 2017 conference in Madrid, Spain.

Read the full open access article: http://www.bmj.com/content/354/bmj.i3839 Differentiated thyroid cancer has been increasing in incidence for several decades and is expected to be the fourth leading cancer diagnosis by the year 2030. Most of this rise in incidence is explained by diagnoses of small, low risk cancers. These low risk cancers are associated with an excellent long term survival; disease specific survival at 10 years for patients diagnosed with low risk thyroid cancer is between 96% to 100%. Yet, despite this increase in low risk cancers, our previous work found that the use of imaging tests after primary treatment of thyroid cancer is rising. However, despite no evidence for survival benefit, there are costs, potential radiation exposure, and potential downstream treatment risks associated with imaging after primary treatment. Therefore, the rising use of imaging after primary treatment necessitates further inquiry into the effects on patient outcomes, including treatment for recurrence and disease specific survival. We set out to determine whether the use of imaging tests after primary treatment of differentiated thyroid cancer is associated with more treatment for recurrence and fewer deaths from the disease. We found that the marked rise in use of imaging tests after primary treatment of differentiated thyroid cancer has been associated with an increased treatment for recurrence. However, with the exception of radioiodine scans in presumed iodine avid disease, this association has shown no clear improvement in disease specific survival. These findings emphasize the importance of curbing unnecessary imaging and tailoring imaging after primary treatment to patient risk.

Radioactive iodine is a common treatment for thyroid cancer after the thyroid cancer surgery. We take advantage of the fact that the thyroid gland needs iodine to make thyroid hormone. In this video, Dr. Clayman discusses when to consider radioactive iodine in the treatment of thyroid cancer. Most of the common thyroid cancers will take up iodine just like normal thyroid cells. When you take a radioactive iodine pill, the radioactive iodine can go directly to thyroid tissue and thyroid cancer cells. The purpose of radioactive iodine is to kill any thyroid tissue and most importantly to kill any thyroid cancer cells that may still be in your neck. Radioactive iodine is not necessary for all thyroid cancers. Radioactive iodine is typically used for thyroid cancer that has grown outside of the thyroid gland, for example into the muscle on top of the thyroid gland. This observation, however, relies upon the expertise of the surgeon. Radioactive iodine is also indicated when the thyroid cancer has spread to lymph nodes or to distant sites. Your thyroid surgeon’s experience and observations will play a critical role in determining whether you should have radioactive iodine therapy. Your endocrinologist will manage the administration of the radioactive iodine. It will not make you sick, and will not make your hair fall out.

If lymph nodes are enlarged or show signs of cancer spread, they will be removed as well. In addition, recent studies have suggested that patients with micro-papillary carcinomas (very small thyroid cancers) may safely choose to be managed by close observation with ongoing ultrasounds rather than have immediate surgery.

What Are Signs And Symptoms Of Thyroid Cancer In Women | Thyroid Disease Problems Symptoms In Women The symptoms start slowly. Fatigue is the most common. There might be changes in hair, nails or skin, and other vague complaints that could be caused by aging, diet, stress or dozens of other factors. Women in the prime of their lives, busy with work and families, may not even notice. When a doctor finally diagnoses an underactive thyroid due to cancer, it often comes as a shock. Thyroid disorders are more common in women, probably due to the roles of hormones, which are different in females than in males. Thyroid nodules (growths), Russell says, affect up to 80 percent of women, but only 5 percent to 15 percent of those lumps and bumps are malignant. Better testing means thyroid tumors are on the rise, he notes, saying that it’s projected to become the third most common cancer. “Malignant” and “cancer” are scary words, but Russell says that most thyroid cancer is highly treatable, even when the cancer cells spread to nearby lymph nodes, which occurs frequently. “With thyroid cancer we talk about prognosis in terms of 20-year survival instead of five years, as we do with most other cancers. It’s usually a slow-moving disease. There’s a 98 to 99 percent survival rate at 20 years,” he says. “We treat it almost like a chronic condition where the patient gets treatment and visits her doctor regularly for follow-up. If a woman notices a lump in the base of her neck, or if a doctor notices a lesion on the thyroid on an X-ray or CT, the next diagnostic test is usually lab work, followed by an ultrasound, which Russell says provides a lot of information on the nodule. Doctors may recommend watchful observation of small nodules. Larger growths may be tested with fine-needle aspiration, in which the doctor collects a sample of cells from the nodule with a needle and looks at them under a microscope. Russell says that about 70 percent of nodule biopsies will show that the nodule is benign. Another 25 percent of biopsies are inconclusive, and the remaining 5 percent show that cancer is present. It’s a butterfly-shaped gland found in the center of the neck. While small, the thyroid gland plays an important role in the body. “It’s kind of like a thermostat for the body. It regulates how hot and cold you are, it regulates your metabolism, and it’s very, very common to have cancer in this gland,” said Dr. Anthony Anfuso, an otolaryngologist on the medical staff of Lee Health. Thyroid cancer can affect patients as young as their 20s and is more common in women than men. “Exposure to radiation therapy is a known risk of thyroid cancer, and it can run in families as well. But most often it’s a sporadic finding,” said Dr. Anfuso. There are times when patients develop a lump in their neck, but more often patient’s thyroid will be functioning normally, and they will have no symptoms. The cancer is simply discovered through an imaging procedure like a cat scan or an ultrasound. “The good news about thyroid cancer is survival rate is greater than 98 percent,” he said. Studies have found that in many cases thyroid cancer behaves in a non-aggressive way. “One interesting study did an autopsy on patients who died from a variety of reasons, and a third of them were found to have thyroid cancer that never caused any problems throughout their life,” said Dr. Anfuso. But if patients are diagnosed, doctors say it needs to be treated to prevent future health problems. “A lot of times surgery is all you need, so that’s great. You get the thyroid removed. You have to take medicine, which is thyroid hormone, for the rest of your life. But after that many times you’re cured,” he said. The quick procedure leaves patients with a small scar on their neck but allows them to quickly get back on their feet.
