The jelly-like substance in the back of the eye is the vitreous, which is responsible for providing cushion to the eye as well as an unobstructed path for light to travel to the retina. The lens is the front part of the eye and its responsibility is focusing light onto the retina. The optic nerve is the nerve that connects the eye to the brain. The most common eye problem caused by diabetes is diabetic retinopathy. This occurs when the tiny blood vessels within the retina become damaged because of high levels of blood glucose and high blood pressure. Diabetic retinopathy can take a long time to develop and your eyesight may not be affected during its early stages. Retina problems caused by diabetes may get progressively worse. This will cause new blood vessels in the retina to develop. These are weak and will tend to leak blood into the vitreous. This will then prevent light from reaching the retina. Over time, this will cause scar tissue to develop and may separate the retina from the eye. This is an urgent situation and must be taken care of immediately to prevent permanent blindness. Unfortunately, people with diabetes that suffer damage to the retina may not experience signs or symptoms. Sometimes signs will be present and those include blurry vision, double vision, blank spots, floaters, pain or pressure, and problems with peripheral vision. There are a couple of treatment options available for diabetics with problems of the retina. Laser treatment can be performed which will close off blood vessels that are leaking. Sciatica or sciatic neuralgia is a common condition in which one of the spinal nerve roots of the sciatic nerve is compressed resulting in lower back, b. and leg pain. Sciatic nerve is a large nerve derived from 5 spinal nerve roots: L4, L5, S1, S2 and S3. It runs from the lumbar spine through the b. down the leg and the foot on the posterior aspect. There is one sciatic nerve on each side of the body. Typically, only one side of the body is affected. A typical sciatica pain is described as a sharp shooting pain in the lower back, down the b., thigh and leg on one side of the body. There may also be numbness, burning and tingling sensations. The pain can get worse with sitting, moving, sneezing, or coughing. The patterns of pain depend on which nerve root is compressed, and follow the dermatome distribution. The most common cause of sciatica is a herniated spinal disc. The spinal disc is a soft elastic cushion that sits in between the vertebrae of the spine. With age, the discs become rigid and may crack; the gel-like center of the disc may protrude out and become a herniation outside the normal boundaries of the disc. Disc herniation presses on the nerve root as it exits the spine. In majority of the cases the condition resolves by itself after a few weeks of rest and conservative treatment. Pain relief, nonsteroidal anti-inflammatory drugs and muscle relaxants may be prescribed. Stretching exercises and physical therapy may be recommended. Surgery may be needed if the pain doesn’t go away after 3 months or more of conservative treatments. The herniated disc may be removed in a procedure called discectomy. Or, in another procedure called laminotomy, part of the bone of the vertebrae may be cut to make room for the nerve.

Epidural Injections for Back Pain and Sciatica - Cortisone Steroid Shots for Herniated Discs
Epidural Injections for Back Pain and Sciatica http://PainFreeInstitute.net If you suffer from sciatica due to a herniated disc you may be introduced to epidural steroid injections (ESI) by your doctor. Epidural injections have become one of the most common procedures for low back pain associated with herniated discs and I often get asked if this is something that’s worth having done, how long it will last, and what are its side effects. The best care is given when a patient is treated at a multi-disciplinary clinic where they can be co-managed by a chiropractor, physical therapist, medical doctor, etc. This gives the doctors a multi-dimensional approach to see what benefits the patient best. A conservative approach should come first when treating herniated discs and sciatica because the vast majority of people will see great improvement within 6 weeks or less with a rehabilitation program that’s specific to the cause of their pain. http://painfreeinstitute.net/do-epidural-injections-help-sciatica-and-herniated-discs/ When a disc herniates, it can put pressure on spinal nerves and cause sharp pain that travels down the leg(s), numbness, and tingling. Epidural injections are used because of their powerful anti-inflammatory properties believed to remove pressure off nerves. During epidural cortisone injections, the needle is inserted past the skin into an area called the epidural space. The doctor will be aided by the use of a video fluoroscopy machine which is a “live” x-ray, that allows him/her to see where the needle is being inserted. For the most part, the effects of epidural injections are short-lived. Some people feel relief for a couple of months and others only feel relief for a few days. The results vary from person to person but for the vast majority of people these injections are NOT effective for permanent pain relief. The upside is that ESI’s can give patients some relief so that they can get started on a comprehensive physical therapy program and resume everyday activities with less pain. Epidural injections are minimally invasive procedures, however, there are risks involved and this is why it’s important to research and look for a medical professional with solid training in this procedure. If your doctor recommends this procedure, it is wise to ask him/her for an informed consent form stating any complications that may occur from this procedure. This is the same form you will be signing before you agree to the injection. One side effect I’ve seen is an increase in pain. Other complications may include nerve injury, or an infection but those are rare. When treating herniated discs and sciatica, your doctor should start with a conservative approach. These treatments should include chiropractic, physical therapy, massage therapy, acupuncture, etc. For the majority of people these treatments will provide significant relief. If your symptoms are still not improving after giving these treatments a trial, then epidural injections and pain relievers should be used. Although ESI’s are not a cure for herniated discs, they are less risky than undergoing back surgery. Opioids such as Percocet and Lortab, can become highly addictive and when abused, can cause death. In 2014, more 28,000 people died due to an opioid overdose. When you compare conservative treatments and ESI’s to surgery and opioids, the benefits of going the conservative route outweigh the risks. The bottom line is that you as the patient should be involved in making an informed decision about your health. You should ask your doctor questions and understand the treatment plan and options presented to you. Then go home and research some more. Not everyone will need or benefit from epidural injections, but they may be helpful to reduce pain and inflammation so you can get back on your feet and start your way to recovery. Use a standing workstation Standing workstations are gaining popularity, and for good reason. Mobility and standing throughout your day are crucial to preventing and treating a pinched nerve. If you can’t get a standing workstation, be sure to get up and take a walk each hour. Roller balls for tight muscles and an hourly stretching program are a good idea if you use a keyboard frequently. (Wrist braces or supports aren’t recommended as an early treatment strategy.) 3. Rest No matter where you have a pinched nerve, the best thing is usually to rest as long as possible. Avoid the activity that is causing you pain, such as tennis, golf, or texting. Rest until the symptoms have completely resolved. When you do start moving that part of your body again, pay attention to how it feels. Stop the activity if your pain returns. 4. Splint If you have carpal tunnel, which is a pinched nerve in the wrist, a splint can help you rest and protect your wrist. This can be especially helpful overnight so that you don’t curl your wrist in a bad position while you’re sleeping. 5. Stretch Gentle stretches can help relieve the pressure on your nerve and improve your symptoms. Don’t go too deeply into a stretch. If you begin to feel pain or discomfort, ease up on the stretch. Remember that small movements can have a big impact. Anything that makes you feel better you should do, as long as it isn’t increasing your numbness or pain. Try to spend time in the positions that help alleviate the symptoms. Relieving the pressure on the nerve will help decrease irritation and pain. – Erin Hassett 6. Apply heat You can use heat to relax the muscles that might be tight around a pinched nerve. Heat also increases blood flow, which can help the healing process. You can find heating pads in various sizes at a drugstore. Hold heat directly onto the pinched nerve for 10–15 minutes at a time. 7. Use ice Ice reduces swelling and inflammation. Wrap a towel around an ice pack and hold it directly onto the pinched nerve for 10–15 minutes. 8. Elevate your legs For a pinched nerve in your lower back, try elevating your legs with a 90-degree bend in both your hips and knees. You can place a wedge or ottoman on your bed and rest your legs on top while lying on your back. This position helps relieve compression on the disc and nerve in your back. Take an over-the-counter pain reliever If you want to try a pain reliever, you can take over-the-counter nonsteroidal anti-inflammatory drugs, like ibuprofen (Advil, Motrin IB) or aspirin (Bufferin).
