When we do a follicular ultrasound scan, preferably an endovaginal scan, we will be able to visualize the uterus, the lining of the uterus and the ovaries very clearly. And when we look at the ovaries, we will be able to see the follicles inside the ovaries and these follicles are those which had eggs inside them. It is a known fact that every follicle is supposed to have one egg within it and every month there is one follicle that grows big and in the midcycle this follicle grows to an appropriate size of about 20mm after which it ruptures and the egg comes out of the ovary and travels upto the fallopian tube and there it is alive for about 48 hours within which if there is sexual intercourse there is a chance of pregnancy to happen. But most of the times it is very difficult to understand if ovulation has really happened, but the ovulation induction kits are not really reliable and sometimes they may not show that there has been ovulation especially when the hormonal levels are not right enough. We see that very commonly in women who have Polycystic ovaries. In women with Polycystic ovaries, they might not ovulate in the midcycle because these cycles are irregular. They might have delayed cycles, so it is not that the ovulation should happen on dot 14 or 15th day of the cycle. In women with polycystic ovaries who have a cycle length of 45 days, their ovulation might even happen on 29th day or 30th day of the menstrual cycle. So using L8 kits from 14th day to 30th day would not be cost effective and also not worth the stress you take in doing these tests. So the best thing to do would be a follicular monitoring. When we do a follicular scan we will be able to see the follicles very clearly and we will be able to assess if the follicle is growing or not. If the follicle is growing, then we know that yes, follicle is growing and there is a good chance that ovulation also might happen and at the same time we get the added advantage of looking at the lining of the uterus and making sure your womb is also getting prepared for pregnancy to happen. If not, there are some hormonal supplements which can be given to make sure the womb lining also is good enough to support pregnancy. That’s the reason why a follicular tracking ultrasound scan would be best time to understand when the ovulation happens so as to make sure you are trying for pregnancy effectively.

Surgery to vaporize endometriosis is an effective option to improve fertility. Word of caution, not every woman will get pregnant simply by vaporizing her endometriosis. In fact, for women with mildest forms of endometriosis, you would have to do surgery on 11 women to get one extra pregnancy. Women with most severe forms of endometriosis, who have endometriosis cysts in their ovary, should not have surgery if their ovarian reserve is poor as it can further lower the number of viable eggs they have remaining. During a women’s menstrual cycle, the egg grows in a sac called as follicle. The sac is located inside the ovaries. This follicle or sac breaks open and releases an egg. But if a follicle breaks open, the fluid inside the follicle can form cyst on the ovaries. The most common types of ovarian cysts is called the functional cyst. The cysts are described as functional cysts. The cysts are described as functional because they develop during a menstrual cycle. These are of two types. The follicular cysts and the corpus luteum cysts. These usually go away on their own in one two three months. Thee cysts are formed when an egg doesn’t get released as expected. So the follicle keeps growing. Corpus luteum cysts go away on their own. These cysts are forced on their own, break open and an egg is released in the process called as ovulation. The follicle reseals and the follicle starts to build within it. They can enlarge, cause pain, bleed and twist the ovary. Fertility medicines used to promote ovulation such as clomiphene can increase the chances of developing these types of cysts. Ovarian cysts can be related to endometriosis or formed from the outer surface of the ovary called as cyst adenoma or non-ovarian tissue called as dermoid cyst.

Cysts come in various sizes, shapes, and appearances. But all reproductive women need to have follicular cysts that represent microscopic eggs.” “When we do an ultrasound, we count the number of small cysts on the ovary. They represent microscopic eggs, and we want to see them in good numbers. The less antral follicles we see, the greater the ovarian aging (called an antral follicle count (AFC). Dr Randy Morris MD-The BOARD CERTIFIED fertility expert with weekly TTC tips on InfertilityTV If you are trying to conceive, the fertility medication known as clomiphene citrate or Clomid may be an option for you. Clomid is used in two circumstances. First, Clomid is used for women who don’t ovulate to try to induce ovulation. But clomid is also used for women who already ovulate to try to further improve their chances for getting pregnant. The protocol for treating women with Clomid varies from doctor to doctor. I am going to tell you about the highly effective methods we use at IVF1. First, it is important to perform a baseline ultrasound of the ovaries and some baseline blood tests. Women who get their periods can do this on the 2nd or 3rd of menstrual bleeding. Women who don’t get periods can do the baseline at any time. As long as everything looks okay, a woman may then start to take the Clomid pills. The starting dose is usually 1 to 2 pills a day for five days. Women who don’t ovulate may use a higher dose up to 5 pills a day or use them for a longer period of time such as 7 or 10 days. After the last pill is taken, it is time to start monitoring for ovulation. With clomid, monitoring can be done by using a home ovulation predictor kit which allows a woman to test her urine every morning. A positive result means ovulation will follow in 12-24 hours. Another popular method for ovulation monitoring is to have it done at the doctor’s office. Eggs mature inside of a cyst known as a follicle. As the egg matures, the follicle grows larger. This can be seen and measured on ultrasound. The cells that make up the follicle produce estrogen that can also be measured using blood tests. After the last pill of clomid is taken, follicle ultrasounds and estrogen blood tests can be performed every few days. At times, the eggs will reach maturity without the body triggering ovulation. To get around this problem, doctors will instruct the patient to take an injection of a medication called hCG. Thirty six hours after the hCG is given ovulation will occur. About a week after ovulation was predicted, another blood test called progesterone can be drawn to confirm ovulation. A week after that a pregnancy test can be performed and then -- fingers crossed. Polycystic ovary syndrome is a problem in which a woman's hormones are out of balance. It can cause problems with your periods and make it difficult to get pregnant. PCOS also may cause unwanted changes in the way you look. If it isn't treated, over time it can lead to serious health problems, such as diabetes and heart disease. Most women with PCOS grow many small cysts camera.gif on their ovaries. That is why it is called polycystic ovary syndrome. The cysts are not harmful but lead to hormone imbalances. Early diagnosis and treatment can help control the symptoms and prevent long-term problems. What are the symptoms? Symptoms tend to be mild at first. You may have only a few symptoms or a lot of them. The most common symptoms are: • Acne. • Weight gain and trouble losing weight. • Extra hair on the face and body. Often women get thicker and darker facial hair and more hair on the chest, belly, and back. • Thinning hair on the scalp. • Irregular periods. Often women with PCOS have fewer than nine periods a year. Some women have no periods. Others have very heavy bleeding. • Fertility problems. Many women who have PCOS have trouble getting pregnant (infertility). • Depression. How is it treated? Regular exercise, healthy foods, and weight control are the key treatments for PCOS.
