You mentioned that you have ovarian cysts, and you're wondering if this is going to affect your ability to become pregnant. And the cause behind the ovarian cysts is going to dictate whether you're going to have an issue getting pregnant. And they can be associated with a monthly cycle, because you're body actually releases what's called a follicle, and sometimes the follicle grows but does't release an egg, and instead turns into a cyst. Or if you get pregnant, then sometimes the egg is released and the cyst can stay on the ovary for a long time. So just normal a menstrual cycle can cause ovarian cysts, and so can pregnancy. But there are other causes. There's dermoid cysts that actually can contain fat, hair, and teeth, which all sounds really weird, and sometimes doctors feel like those need to be taken out, but they're not necessarily harmful to your health. There's ovarian cysts caused by polycystic ovarian syndrome (or PCOS) and this actually can cause infertility issues. And it's a metabolic syndrome, it affects a lot of different things in your body. And so again it would just be best for you to talk to your doctor, and they can decide what the cause is behind the ovarian cysts, and based on that, give you the best guidance and direction, and tell you whether or not it's going to affect your ability to become pregnant. But if a follicle breaks open, the fluid inside the follicle can form cyst on the ovaries. The most common types of ovarian cysts is called the functional cyst. The cysts are described as functional cysts. The cysts are described as functional because they develop during a menstrual cycle. These are of two types. The follicular cysts and the corpus luteum cysts. These usually go away on their own in one two three months. Thee cysts are formed when an egg doesn’t get released as expected. So the follicle keeps growing. Corpus luteum cysts go away on their own. These cysts are forced on their own, break open and an egg is released in the process called as ovulation. Fertility medicines used to promote ovulation such as clomiphene can increase the chances of developing these types of cysts. Ovarian cysts can be related to endometriosis or formed from the outer surface of the ovary called as cyst adenoma or non-ovarian tissue called as dermoid cyst.

Trolice, M.D., a board-certified reproductive endocrinology and infertility specialist and founder of Fertility CARE: The IVF Center describes the different types of cysts and why most of them are not a cause for worry. “Ovarian cysts cause more stress and confusion than probably anything else that we work with,” says Dr. Trolice. “Any woman in their reproductive years must have cysts on her ovaries. Cysts come in various sizes, shapes, and appearances. But all reproductive women need to have follicular cysts that represent microscopic eggs.” “When we do an ultrasound, we count the number of small cysts on the ovary. They represent microscopic eggs, and we want to see them in good numbers. The less antral follicles we see, the greater the ovarian aging (called an antral follicle count (AFC). When the follicles are less than 10 mm, there is nothing to worry about.” “What about a growing follicular cyst?” asks Dr. Trolice. “That’s what we do when we stimulate you with clomiphene citrate or letrozole or Genotropin injections. We are pushing those little antral follicles to get larger to grow mature eggs.” “What about if we see a cyst that is neither of those?” Dr. Trolice continues. “Those cysts can be either simple or complex. The simple ones look just like a little circle; it’s black inside with clear fluid and is nothing to worry about, as long as you’re not having any pain. Sometimes you can have cysts on the side of your ovary, called a para ovarian cyst or a cyst on the side of the tube called a paratubal cyst - these are also no cause for concern.” “There is a hemorrhagic cyst which occurs every time a woman ovulates. When you release the egg from the follicular cyst that‘s growing, the egg gets released from the surge of the pituitary hormone (HCG trigger), and that cyst left behind in the ovary is making progesterone and other hormones critical for implantation. We’ve got to have that cyst (known as a corpus luteum), which develops from the time of ovulation until your next period. When you get your period, sometimes that cyst can hang around causing some discomfort, minor bleeding, or hemorrhaging requiring surgery. When you look at the ultrasound of a hemorrhagic cyst, it may look scary or very simple. But the best thing to do is wait for six to eight weeks on average to see if it goes away.” “The cysts we get a little more concerned about are complex cysts,” says Dr. Trolice. “They appear solid or mixed simple and solid. Complex cysts are not common, but if they hang around and do not resolve, we’ll have you see your gynecologist for evaluation. It doesn’t mean that you have cancer, but we need to examine it if it doesn’t go away.” “Two cysts that we see a lot of are endometriosis cysts and dermoid cysts. Endometriosis cysts sometimes implant in the ovary and have a ground-glass appearance, which is very symmetrical smooth but not simple. Endometriosis cysts are often associated with pain and potentially represent significant endometriosis inside. If you don’t have any pain, we leave those cysts alone as well.” “A dermoid cyst is an embryological cyst that has many different cell lines in it. It could have hair inside, teeth, neurological tissue, anything. Size is important. Trying to Conceive - TTC - with Clomid | Infertility TV with Dr. Randy Morris MD

Dr Randy Morris MD-The BOARD CERTIFIED fertility expert with weekly TTC tips on InfertilityTV If you are trying to conceive, the fertility medication known as clomiphene citrate or Clomid may be an option for you. Clomid is used in two circumstances. First, Clomid is used for women who don’t ovulate to try to induce ovulation. But clomid is also used for women who already ovulate to try to further improve their chances for getting pregnant. The protocol for treating women with Clomid varies from doctor to doctor. I am going to tell you about the highly effective methods we use at IVF1. First, it is important to perform a baseline ultrasound of the ovaries and some baseline blood tests. Women who get their periods can do this on the 2nd or 3rd of menstrual bleeding. Women who don’t get periods can do the baseline at any time. As long as everything looks okay, a woman may then start to take the Clomid pills. The starting dose is usually 1 to 2 pills a day for five days. Women who don’t ovulate may use a higher dose up to 5 pills a day or use them for a longer period of time such as 7 or 10 days. After the last pill is taken, it is time to start monitoring for ovulation. With clomid, monitoring can be done by using a home ovulation predictor kit which allows a woman to test her urine every morning. A positive result means ovulation will follow in 12-24 hours. Another popular method for ovulation monitoring is to have it done at the doctor’s office. Eggs mature inside of a cyst known as a follicle. As the egg matures, the follicle grows larger. This can be seen and measured on ultrasound. The cells that make up the follicle produce estrogen that can also be measured using blood tests. After the last pill of clomid is taken, follicle ultrasounds and estrogen blood tests can be performed every few days. At times, the eggs will reach maturity without the body triggering ovulation. To get around this problem, doctors will instruct the patient to take an injection of a medication called hCG. Thirty six hours after the hCG is given ovulation will occur. About a week after ovulation was predicted, another blood test called progesterone can be drawn to confirm ovulation. A week after that a pregnancy test can be performed and then -- fingers crossed. Dr Randy Morris MD-The BOARD CERTIFIED fertility expert with weekly TTC tips on InfertilityTV Endometriosis is when the tissue which grows inside your uterus, starts to grow outside of the uterus in your abdomen. Endometriosis can cause pain and infertility or both. Diagnosing endometriosis can be tricky. Not all women with painful periods have endometriosis. Women with more severe forms of endometriosis will usually have endometriosis cysts in their ovaries that can been on ultrasound. Women with a less severe stage, however, cannot be diagnosed with ultrasound. For these women, surgery to look inside the abdomen is the only way to prove whether endometriosis is present or not. If you have endometriosis, it can be more difficult to conceive. The more severe the endometriosis, the more difficult it may be. There are several options for trying to conceive with endometriosis. Surgery to vaporize endometriosis is an effective option to improve fertility. Word of caution, not every woman will get pregnant simply by vaporizing her endometriosis. In fact, for women with mildest forms of endometriosis, you would have to do surgery on 11 women to get one extra pregnancy. Women with most severe forms of endometriosis, who have endometriosis cysts in their ovary, should not have surgery if their ovarian reserve is poor as it can further lower the number of viable eggs they have remaining. The use of fertility pills such as Clomid or injections such as Follistim with either intercourse or inseminations are commonly to used to treat infertility but their effectiveness in women with endometriosis remains questionable. Finally, IVF has become a go to option for women with endometriosis. Its effectiveness is well proven compared to other treatments but questions remain over whether pregnancy rates are as good as they are compared to couples with other types of fertility problems.
