When there is an increase in bone size, ligaments stretch and thus create the maximum pull, thus directing ends of joint bones in the center, whereby they are tightly adjacent to each other in a joint capsule. In addition to tight ligaments in the centering of the articular components actively involved meniscus, which is a crescent-shaped, thus it prevents the slightest deviation from the center of the articular bones. Thus, here achieves a perfect balance and perfect biomechanics of the knee. It should be noted that the articular surfaces of bones is brittle hyaline cartilage liening and meniscus has a solid fibro-fibrous nature. With age, bone growth stops, more organic components of bone are replaced by inorganic ones, bone significantly loses moisture and begins to thin and dry and shrink in volume. With this turn of events, ligaments abruptly lose function and tensions, the joint loses its centricity, which leads to conflict between the meniscus and articular cartilage. It is a disease of articular cartilage, the gristle that caps the ends of long bones .Cartilage consists of a mixture of proteins, sugars, and collagen. Interspersed in this matrix of substances are chondrocytes, cartilage cells. The purpose of the chondrocytes is to manufacture new healthy matrix and keep it healthy. In the picture to your right you can see that osteoarthritic cartilage has cracks in it. These are called "fissures." Osteoarthritis starts when chondrocytes begin to manufacture destructive enzymes. In addition, there is a complex interplay of events that leads to bone spur formation and inflammation of the synovium (the lining of the joint), which causes further joint destruction. There are multiple factors that predispose to OA of the shoulder. These include: rheumatoid arthritis,previous surgery, instability, rotator cuff disease, and trauma. Trauma can come from recreational activities such as weightlifting... or from occupation. There are two endpoints that effective arthritis treatment must meet. The first is adequate control of symptoms. The second is possible slowing down or reversal of cartilage loss. So... what can be done? Unfortunately, many treatments have been tried but they all seem to fall short. The usual approaches to OA consist of non-steroidal anti-inflammatory drugs, analgesics, physical therapy, glucocorticoid injections, and exercises. Results are generally disappointing Most patients end up with a shoulder arthroplasty (shoulder replacement). While this is OK for many people, it is not a good option for the younger, healthy active person who wishes to continue to be active. Recent advances from standard to reverse procedures may be better. However, the most exciting news may be the use of mesenchymal stem cells combined with growth factors, administered after fenestration and scarification of the exposed bony. Osteoarthritis Corrective Exercise Strategy for the Knee

http://www.fitnesseducationseminars.com/icei/ http://www.iihfe.com If you work with the older population like we do in our clinic, than you recognize that one of the most common issues our clients present with is osteoarthritis of the knees and hips. Many of these individuals will have had and/or are going to require surgery. Did you know that there are currently over 7 million Americans living with knee and hip replacements? And this is not counting the millions upon millions of Americans having orthopedic surgeries of the knees and hips each year. Even more disturbing is that the average age of individuals having hip and/or knee replacement surgery is around 51 years of age. I am quickly approaching the BIG 5-0 and that number nearly made me choke when I first heard it. The unspoken issue with these statistics is that early in my practice, it used to be that the individuals having knee or hip surgery and joint replacements were generally older (60+ years of age), overweight, and inactive. However, the landscape of individuals having knee and hip orthopedic surgeries and/or replacements has completely changed. Now I’m treating young individuals that are already demonstrating degenerative changes in their knees and hips. For example, I am currently treating a 35-year old active male who will likely need a hip replacement secondary to osteoarthritic changes. I have another active, healthy male runner (50 years of age) who has already had one knee replacement and will likely require a second knee replacement secondary to degenerative changes. I have been treating increasing numbers of active teen girls (some as young as 13 and 14) requiring knee and hip surgeries for non-contact sports-related injuries. These girls are already demonstrating osteoarthritic changes (bone spurs and decreased cartilage) in their knees and hips. Please note, I am not suggesting all surgery can be avoided because some individuals will absolutely require surgery to give them back their quality of life. However, what I am illustrating is that as individuals become more active and ‘fit’, there is an increasingly alarming tendency towards excessive and early wear and tear upon their joints. Despite what MD’s and orthopedic surgeons may tell us, being active and fit should not correspond to advanced wear and tear upon our joints. When the activities an individual is participating in are performed with proper form – in other words optimal alignment and control – and are progressed in a safe and appropriate manner, there should be normal amounts of wear and tear upon the individual’s joints. This means that barring significant trauma or hereditary joint conditions such as rheumatoid or psoriatic arthritis and/or hip dysplasia, an individual’s knee and hip joints should remain healthy easily into their 70’s and 80’s. So why do so many of our clients and even my young patients experience osteoarthritis or degenerative joint disease of the knees and hips? It really comes down to one thing… It is their posture and movement habits. You see, when an individual performs activities with non-optimal alignment and control and then performs them at increasingly higher levels and/or increased frequency, they will advance the degenerative changes of their joints. We are seeing more young individuals competing at earlier ages, all year round in the same sport or activity, and participating in organized strength training and conditioning programs, not resting enough, and oftentimes without the proper nutritional support to build and repair cartilage. And many of these individuals are performing these activities with non-optimal alignment and control which when taken in combination with the above listed factors, is a recipe for disaster for their joints. So, is there anything we can do about this issue because after all this condition is frequently associated with heredity or genetics? I love the following statement attributed to Dr. Francis Collins: “All illnesses have some hereditary contribution. It’s been said that genetics loads the gun and environment pulls the trigger.” The great news is that as a health and fitness professional, you are in the best position to help individuals experiencing knee and hip osteoarthritis change their environment!!! When some people complain about a pain in the neck, they really mean it. "Neck arthritis is very common presentation for primary care physicians and for spine surgeons," says Dr. Jon Kimball, an orthopedic surgeon on Lee Memorial Health System's medical staff. Fifty million Americans have some form of arthritis; it's frequently related to years of wear and tear. Not quite as common as in the hip or knee, neck arthritis tends to sneak up on people. "Arthritis can present any number of ways in the neck. Stiffness is one of the more common ones but many people play it off as maybe that they slept wrong. After the problems become more constant or more bothersome and then they seek care and then they find out that they have arthritis and most likely have had it for many years," says Dr. Kimball. Stiff neck is a prime symptom but not the only symptom. "Other symptoms are radiating pain into the arms or legs, balance changes, handwriting or coordination changes, or it can be things that you might not necessarily associate with neck arthritis such as headaches and muscle spasms," says Dr. Kimball. Because the neck holds some of our body's most prime real estate, people are very cautious about how they treat it. The earlier they see a doctor the more likely they'll be able to hold their head up high. "With our understanding of the process of neck arthritis, we can do interventions, whether its surgery, whether its injections, very safely and hopefully improve their quality of life," says Dr. Kimball. The key is to use your head and seek a diagnosis if you have frequent neck stiffness that may include radiating pain into the arms.
