http://www.fitnesseducationseminars.com/icei/ http://www.iihfe.com If you work with the older population like we do in our clinic, than you recognize that one of the most common issues our clients present with is osteoarthritis of the knees and hips. Many of these individuals will have had and/or are going to require surgery. Did you know that there are currently over 7 million Americans living with knee and hip replacements? And this is not counting the millions upon millions of Americans having orthopedic surgeries of the knees and hips each year. Even more disturbing is that the average age of individuals having hip and/or knee replacement surgery is around 51 years of age. I am quickly approaching the BIG 5-0 and that number nearly made me choke when I first heard it. The unspoken issue with these statistics is that early in my practice, it used to be that the individuals having knee or hip surgery and joint replacements were generally older (60+ years of age), overweight, and inactive. However, the landscape of individuals having knee and hip orthopedic surgeries and/or replacements has completely changed. Now I’m treating young individuals that are already demonstrating degenerative changes in their knees and hips. For example, I am currently treating a 35-year old active male who will likely need a hip replacement secondary to osteoarthritic changes. I have another active, healthy male runner (50 years of age) who has already had one knee replacement and will likely require a second knee replacement secondary to degenerative changes. I have been treating increasing numbers of active teen girls (some as young as 13 and 14) requiring knee and hip surgeries for non-contact sports-related injuries. These girls are already demonstrating osteoarthritic changes (bone spurs and decreased cartilage) in their knees and hips. Please note, I am not suggesting all surgery can be avoided because some individuals will absolutely require surgery to give them back their quality of life. However, what I am illustrating is that as individuals become more active and ‘fit’, there is an increasingly alarming tendency towards excessive and early wear and tear upon their joints. Despite what MD’s and orthopedic surgeons may tell us, being active and fit should not correspond to advanced wear and tear upon our joints. When the activities an individual is participating in are performed with proper form – in other words optimal alignment and control – and are progressed in a safe and appropriate manner, there should be normal amounts of wear and tear upon the individual’s joints. This means that barring significant trauma or hereditary joint conditions such as rheumatoid or psoriatic arthritis and/or hip dysplasia, an individual’s knee and hip joints should remain healthy easily into their 70’s and 80’s. So why do so many of our clients and even my young patients experience osteoarthritis or degenerative joint disease of the knees and hips? It really comes down to one thing… It is their posture and movement habits. You see, when an individual performs activities with non-optimal alignment and control and then performs them at increasingly higher levels and/or increased frequency, they will advance the degenerative changes of their joints. We are seeing more young individuals competing at earlier ages, all year round in the same sport or activity, and participating in organized strength training and conditioning programs, not resting enough, and oftentimes without the proper nutritional support to build and repair cartilage. And many of these individuals are performing these activities with non-optimal alignment and control which when taken in combination with the above listed factors, is a recipe for disaster for their joints. So, is there anything we can do about this issue because after all this condition is frequently associated with heredity or genetics? I love the following statement attributed to Dr. Francis Collins: “All illnesses have some hereditary contribution. It’s been said that genetics loads the gun and environment pulls the trigger.” The great news is that as a health and fitness professional, you are in the best position to help individuals experiencing knee and hip osteoarthritis change their environment!!! They tend to be underdeveloped, which can be a big problem since they support your arches, keep your toes properly aligned, and even contribute to balance. -Rolling foot massage works the muscles in the heels and balls of your feet. It’s also a nice relaxing massage for your arches. -If your feet are killing you after a long day, use one thumb to put pressure on the spot between your big toe and pointer toe and use the other thumb to press the point between your ring toe and pinky toe. It is a disease of articular cartilage, the gristle that caps the ends of long bones .Cartilage consists of a mixture of proteins, sugars, and collagen. Interspersed in this matrix of substances are chondrocytes, cartilage cells. The purpose of the chondrocytes is to manufacture new healthy matrix and keep it healthy. In the picture to your right you can see that osteoarthritic cartilage has cracks in it. These are called "fissures." Osteoarthritis starts when chondrocytes begin to manufacture destructive enzymes. In addition, there is a complex interplay of events that leads to bone spur formation and inflammation of the synovium (the lining of the joint), which causes further joint destruction. There are multiple factors that predispose to OA of the shoulder. These include: rheumatoid arthritis,previous surgery, instability, rotator cuff disease, and trauma. Trauma can come from recreational activities such as weightlifting... or from occupation. There are two endpoints that effective arthritis treatment must meet. The first is adequate control of symptoms. The second is possible slowing down or reversal of cartilage loss. So... what can be done? Unfortunately, many treatments have been tried but they all seem to fall short. The usual approaches to OA consist of non-steroidal anti-inflammatory drugs, analgesics, physical therapy, glucocorticoid injections, and exercises. Results are generally disappointing Most patients end up with a shoulder arthroplasty (shoulder replacement). While this is OK for many people, it is not a good option for the younger, healthy active person who wishes to continue to be active. Recent advances from standard to reverse procedures may be better. However, the most exciting news may be the use of mesenchymal stem cells combined with growth factors, administered after fenestration and scarification of the exposed bony. Unlike other connective tissue, cartilages do not have its own blood supply, instead, it is composed primarily of water, chondrocytes, collagen, and proteoglycans, which produce and maintain a structural matrix giving cartilage tissue its form and function. Proteoglycans are composed of numerous glycosaminoglycans attached to a core protein. The predominant glycosaminoglycans in cartilage are chondroitin sulfate and keratin sulfate. Within the cartilage matrix, there are numerous prototype and monomers attached by link protein to a molecule of hyaluronic acid. This is all woven with collagen to form an elastic and compressible structure. Normal articular cartilage also known as hyaline cartilage provides a smooth crystal clear gliding surface to aid in the motion of the joint. The structure of hyaline cartilage is divided into four layers; superficial, middle, deep and calcified layer with the calcified layer representing a transition from articular cartilage to bone. This cartilage is designed to achieve and maintain proper function over the majority of a person’s life. If the cartilage is subjected to excessive weight, overuse, improper alignment or injury, it begins to wear away leaving bone to rub on bone. Articular cartilage does not have the ability to heal itself, however, there are several treatment options available to aid in the repair of injured cartilage. The knee is the most common joint in the body with cartilage injury issues. The knee joint is composed of femur, tibia, and patella. There are two types of cartilage in the knee joint, the articular cartilage and the meniscus, which is a shock absorbing cartilage. There are also important ligaments within the knee joint, the anterior and the posterior cruciate ligaments. The goal of treating cartilage injury is to reduce pain and improve the function of the affected area. Anti-inflammatory medications are commonly used to combat-injured cartilage but they can potentially cause cardiovascular or gastrointestinal toxicity. Damaged cartilage can also be treated with a series of injections. Steroid injections such as cortisone are powerful anti-inflammatory drugs used in the treatment of osteoarthritis. Hyaluronic acid is a substance produced naturally in the body and is present within cartilage when isolated. Hyaluronic acid is a thick viscous solution that can be added to the natural hyaluronic acid of the knee joint. This solution uses purified Hyaluronic acid derived from either rooster combs or genetically engineered cells. Glucosamine is another option for treatment that can be injected directly into the joint. Glucosamine is a modified sugar that is formed by the human body and is used to form larger molecules involved in the formation and repair of cartilage. Other methods include gene therapy, platelet-rich plasma, growth factors, stem cells, drilling and debridement of cartilage may help with injured cartilage. Occasionally the cartilage is replaced, regrowth and implanted. When the cartilage is damaged, the choice is often simple as the patient will need a total knee replacement. Many different types of designs and materials are currently used in total knee replacement surgery. Nearly all consists of three components, a patellar component, which consists of durable plastic, a tibial component, made of a durable plastic cushion often held within a metal platform and the femoral component, which is usually made of highly polished metal. Total knee replacement is the most predictable option to treat severe arthritis of the knee joint. When the arthritis is minimal, almost all treatment methods will work, however, when the extent of arthritis is moderate, there are many different treatment options presented, but none of them are predictable. It is logical to pursue and recognize the efficacy of platelets and stem cells in cartilage regeneration, simply because it is better for the patient to be able to regenerate their own cartilages and for it to be replaced with an artificial joint.
