Needed medications can be life-savers, and should never be discouraged. But, very often, a medication with significant side effects can be substituted with a medication which gives the same therapeutic effect, without the unwanted side effects. A single pill can have hundreds of listed "possible" side effects, these can be very hard to keep up with. But, when you take 2 or 3 pills daily, the possible side effects don't just double or triple, they multiply. One medication may not cause depression alone, but when combined with another pill, it might lead to significant depression. This video goes through the most common medications which can risk depression, or worsen existing depression. Headlines in Health: PPI's and Kidney Disease

PPI Kidney Injury G:welcome and introduction of Dr. Mark Vaughan of the Auburn Medical Group YouTube Channel M: thanks Dr. Carlson. It's an honor to be on your YouTube channel. I look forward to the conversation we will share and the opportunity it presents for you, our respective subscribers, to learn about The facts behind the sensational headlines. G: there has been quite a bit of media coverage of an article in the February edition of the Journal of the American medical Association internal medicine. It is a study which suggests that a common drug to both of our practices can cause injury to patients’ kidneys. The class of drug is called proton pump inhibitors. Most of you will recognize the names Prilosec and Nexium as popular name brands from this class. Their function is to turn off the acid pump in the stomach to decrease injury from acid inside the digestive tract. These medicines are usually prescribed for patients who have either short-term or long-term overproduction of acid resulting in symptoms ranging from heartburn to stomach ulcers. Now that they are available over-the-counter without a prescription they are used by many patients without any consultation with a physician. M: The article reports on a small study done on data collected during the atherosclerosis risk in communities study which studied over 10 thousand subjects. The results were then replicated by analyzing records of over 240 thousand individuals. They found that patients who used proton pump inhibitor drugs had a significantly higher incidence of chronic kidney disease on a blood test compared to people who did not use these drugs. They also found that there was a significant increase in kidney disease when compared to people who used H2 antagonist drugs. The cutoff they used to delineate between people with and without chronic kidney disease was based on a lab value found on a blood test. There was no requirement that the patients have any symptoms or other noticeable effect on their health to be labeled as having kidney disease. It is expected that the vast majority of these patients will never suffer any significant ill effects apart from this number found on a lab test. G: but that does not give us license to take these drugs willy-nilly. They should still only be used when there is significant benefit over alternative treatments. The simplest is through diet. Many of the problems which respond to this type of medication can be better treated by reducing intake Of fatty foods, alcohol, spicy foods, and acidic foods. Symptoms can also be decreased by eating smaller quantities during meals and not lying down within two hours of eating. If symptoms only occur occasionally, say less than three times per week, occasional use of an over-the-counter antacid can resolve the symptoms. More persistent symptoms may be treated by a doctor using an over the counter H2 antagonist like Zantac. M: without physician supervision, these medicines really should only be taken for a short period, not exceeding two weeks. I will often use these medicines for a 30 day period followed by a recheck. Patients continuing to have symptoms after this treatment are referred to a gastroenterologist for endoscopy. The gastroenterologist will check for serious disorders which can even include cancer of the digestive tract. Most patients do not have a serious disorder and many end up doing best continuing on these medicines long-term. G: we hope this gives you a better understanding of the concerns surrounding the use of proton pump inhibitors and kidney disease. If you are taking these medications for longer than a couple weeks without the supervision of a physician we urge you to make an appointment to discuss it with your doctor. [I] don't need two hundred note cards I can just do it from memory, and that's where I want you to get to so we'll start with chapter one gastrointestinal And we'll talk about thirteen medications and while I talk about the thirteen medications also [talked] about grouping them and grouping them When you're first learning these top 200 drugs when you group them Students are good at grouping them as okay these are four analgesics and these are Maybe for some kind of stomach upset and then these are for pain But [this] is another level where we're connecting all 200 drugs in one Order so let's start with two drugs that you're probably very familiar with two antacids And then from the antacids we're going to go to the H2 blockers and These are better known as histamine Two Receptor Antagonists So there is a histamine [1] that's the end of histamine you think of when you think of Claritin and things like that [the] Receptor just means that the drug is going to affect the receptor and an Antagonist means it blocks it so histamine [two] must release gastric acid if we're using a drug to block it and then proton pump inhibitors So within each of these categories, I'm going to alphabetize them Because it makes it easier [to] remember Although magnesium is above calcium on the periodic table. We're going to put calcium carbonate first So calcium carbonate is Tums But I also want you to know that it's something called Children's Pepto This is important because regular Pepto has a salicylate component. That's a lot like aspirin and that can be very dangerous in children, especially if a child has Some kind of Chickenpox or fever and things like that and get Reye's syndrome spelled Reye apostrophe "s" There's Magnesium hydroxide And that's Milk of Magnesia So these antacids they work very quickly And because they work quickly what we're going to do for the way that we're going to remember them is These work in a couple minutes. Maybe five minutes These work in about 30 minutes the H2 blockers, and then these work in about a day But antacids are probably the first thing you'll reach for because they work so quickly then if it persists you might go to an H2 blocker and We have two that we're going to use famotidine ranitidine I'm not going to put the brand names yet because I want [you] to notice that there's a -tidine ending t-i-d-i-n-e The first thing I want you to know is that the [een] ine is not the ending a lot of the videos on YouTube show that as the ending just because [20%] of all drugs end in -ine So you don't want to classify it by that. The -tidine has been set by a couple of [organizations] the World Health Organization the United States Adopted Names Council and they're the ones that make this into a group so that it's a -tidine makes it a cimetidine like H2 blocker and cimetidine was the H2 blocker that came out first and So let's put the brand names in there Pepcid Which combines peptic and acid Zantac You can kind of see the word antagonist in there for acid okay, and then to proton pump inhibitors And we're going to see similar ending or that they're going to have similar endings And then I'm going to introduce something new as well there so esomeprazole There's (brand) Nexium (generic) omeprazole (brand) Prilosec, okay so the -prazole ending is what lets us know it's a proton pump inhibitor, but you'll notice we have omeprazole and omeprazole What's the es- what's this doing if you look in and Wiki is a good place to look because it has good pictures of molecules this omeprazole is actually an R- plus S- Where this omeprazole is just an S- what does that mean? There's a left-handed Right or there's a right-handed rectus and left-handed sinister omeprazole, so it's a mixed there's two sides to the molecule and only the S- does anything...

On-screen ethnically-diverse actors were shot against green-screen for the opening close-up in order to customize for various global markets. Once the main talent greets the viewer, the camera then dollies back, allowing him/her to blend into the congregating 3-D crowd via seamless digital compositing. The virtual camera then tilts up to reveal the crowd assembling to form the word, "more." This technique was later repurposed to produce 11 variations of the word "more" to facilitate each market and corresponding language.

Worries about the quality of Chinese exports to America have become a major feature of bilateral trade ties, with substandard Chinese food and toxin-laced toothpaste among product safety scares this past year. US Health and Human Services Secretary Mike Leavitt said a new strategy was needed because the United States imported 2 (t) trillion dollars worth of goods this year alone. He said the FDA's future strategy was to build in safety precautions at every step, not just at US borders. Chinese Vice Minister of Health and Commissioner of China's State Food and Drug Administration, Shao Mingli said that protecting public health was the government's "top responsibility." The FDA office in Beijing will be followed by two more in the Chinese cities of Shanghai and Guangzhou. Offices will also open in India next month and Latin America in January as the FDA tries to globalise its presence. Inside this video you’ll learn how to recognize the effects of reflux on your vocal cords and what steps to take to protect your voice! Hi, I’m Chuck Gilmore with Power To Sing. I have reflux. But it’s not the heartburn kind. That’s called GERD or Gastroesophageal Reflux Disease. It hurts. You know it when you have it. It burns in your chest and throat. You take Tums or Pepto Bismol for relief. It can fry your vocal cords with the acid from your stomach. That’s not the reflux I have. I have a silent kind of reflux. I didn’t even know I had it until I injured my vocal cords when singing sick. The ENT doctor scoped my cords and found nodules on the false cords and moderate reflux. He said the reflux was making it difficult for the nodules to heal because acid was irritating my injury. Yuck. My kind of reflux was silent to me because I didn’t feel heartburn. It’s called LPR which stands for Laryngopharyngeal Reflux. Here are the causes, symptoms and treatment options. Fifty percent of people with LPR do not have symptoms of heartburn or an upset stomach. When there are symptoms, they include: Mild hoarseness Sensation of a lump in the throat Need to clear the throat Sensation of mucous sticking in the throat and/or post-nasal drip Chronic cough Difficulty swallowing Sore throat Red, swollen, or irritated voice box 02:34 ==================== ==================== 03:35 “There are treatment options for people who have LPR. Most of the treatments can also be used as prevention measures. Follow a bland diet (low acid levels, low in fat, not spicy) Eat frequent, small meals Lose weight Avoid the use of alcohol, tobacco, and caffeine Do not eat food less than 2 hours before bedtime Elevate your head when you sleep. I use a couple of pillows. Avoid clearing of the throat Take medications, both prescribed and over-the-counter, as directed by a doctor. In very severe cases, surgery may be recommended as treatment I take medications prescribed by my doctor to treat my LPR. I think it helps. I’m not a doctor and I don’t diagnose or prescribe treatment. If you suspect you have acid reflux, go see an ENT doctor. I recommend one who specializes in the voice.
