For more than 125 million Americans, basic dental care is out of reach. But there’s a solution that can help: dental therapy. A dental therapist is similar to a nurse practitioner and works under the supervision of the dentist. They can travel all over the state to help people who may have trouble accessing dental care, such as children or seniors. “I see rampant tooth decay every day, but I also know that I’m reaching those who are most in need,” says Christy Jo Fogarty, a dental therapist practicing in Minnesota. But for more than 125 million Americans, basic dental care is out of reach. Either because their dentist office is too far from where they live, doesn’t accept their insurance, or isn’t open at the right time. Or because they just can’t afford it. Left untreated, dental problems can cause serious health issues, like heart disease and respiratory infections. It’s a problem that costs all of us. In 2014, $1.9 billion of taxpayer money was wasted on preventable ER visits, even though hospitals can do little to treat dental problems. So how do we solve this problem? To find out, we called up a dental therapist. "My name is Christy Jo Fogatry. I’m a dental therapist with Children’s Dental Services in Minneapolis. A dental therapist is much like a nurse practitioner or physician’s assistant. We work under the supervision of a dentist. We can do just about any kind of fillings, stainless steel crowns. We see children, we see pregnant women, we see veterans, we see seniors, any people who have trouble accessing dental care. I travel, really, all over the state. Unfortunately, I do see rampant tooth every day, but I also know that I’m reaching those most in need." With more than 63 million people in the U.S. living in areas with dentist shortages, dental therapists like Christy Jo are making a big difference. "In my job every day I am fortunate that I’m able to treat those most in need, those children who are most likely to fall through the cracks of the dental system that we currently have and, quite often, I’m paid in lots and lots of hugs." More people in Minnesota are getting the care they need. One private practice increased its Medicaid patient load 16 times over since bringing dental therapists on board. And in Alaska where dental therapists also practice, 40,000 people have care that didn’t before. More and more states are authorizing or considering dental therapists because they can expand care to people who need it the most. It's time to put dental care in reach for more Americans.

(USA Today) While government-managed health care programs such as Medicaid pay for emergency room visits for adult enrollees seeking temporary relief from toothaches, tooth abscesses and other dental emergencies, coverage of outpatient dentist office treatment for those problems varies greatly from state to state, according to a 2011 federal Medicaid report. Federally mandated preventive and other dental care usually ends when Medicaid recipients enter early adulthood. That leaves states to determine whether they will provide dental benefits to adult Medicaid patients and if so, what type and how much. Ten states — Alabama, Arizona, California, Colorado, Delaware, Missouri, Texas, Utah, Virginia and Washington — offer no Medicaid dental benefits to adults, the report shows. The remaining offer a hodgepodge of services that leave many of those adults with inadequate and limited access to dental care, according to the American Dental Association. U of MN Clerical Contract Negotiations Update: Tentative Agreement Reached

The final round of negotiations for the 2015-2017 contract between AFSCME 3800/3801 and the University of Minnesota happened on December 10, 2015. The bargaining team reports on the tentative agreement. Tentative Agreement Reached: After six months in bargaining, the University of MN came to the table today ready to address our top priorities and our need for #RaisesAndRespect. This afternoon, our clerical bargaining committee reached a tentative agreement with the U of MN, and we are recommending approval. Highlights of the agreement include: A $15 minimum wage for all clerical positions, effective June 29, 2015 Six weeks of paid parental leave for birth mothers 1.5% general increase effective June 29, 2015 1.5% general increase for those within the salary range and a 2% general increase for those at the range maximum, effective June 13, 2016 Memorandum of Understanding to address workplace bullying No insurance cost shifts or increases in copays for 2016 or 2017. No premium increases for health insurance and minimal premium increases in dental insurance for 2016. This agreement was reached due to the dedicated and ongoing actions of our members; our unified campaign with Teamsters 320, AFSCME Locals 3937, 3801, and 3260; the solidarity from faculty, students, and non-unionized staff at the University; and our community and union allies around the state. Thank you! We are proud of this agreement, and believe it begins to close the gap between the haves and have nots at the University, but it is just a start. We rededicate ourselves to the ongoing struggle to make this a University where all workers, not just those at the top, are able to make real economic progress. Dentists step up for families without dental coverage

Saturday morning in Golden Valley, dentists were hard at work with drills, suction machines and all of the other necessary tools to clean and fix teeth. On this busy day, every one of the professionals at Bassett Creek Dental did the work strictly on a volunteer basis. "We have approximately 20-25 volunteers here in our office, we're gonna see between 35-40 kids over our half day, and really trying to provide as much care as we can give them in the one day that they're here," said Dr. Todd Tsuchiya, a dentist with Bassett Creek Dental. Bassett Creek Dental was one of 200 clinics across Minnesota taking part in an annual event called "Give Kids a Smile," where children whose families can't afford or access quality dental care, receive that care free of charge. "It is not the solution, but it definitely has an impact amongst the kids in our state," Dr. Tsuchiya said. The service also has an impact on parents such as Vanessa Padua-Evans of Minneapolis. "Presently, I'm a graduate student, and my husband's a chef, so financially, it helps," she said. She brought in her 5-year-old daughter, Margaux, for her first-ever visit to the dentist, and it turned out to be quite the memorable trip. "Going to the dentist for the first time and them saying, 'Oh, you have your permanent molars, let's seal that. You have two cavities, let's fix that.' She's like 'oh.' It can be overwhelming, yes," Padua-Evans said. For these dental professionals, it's all in a day's work, yet that work also includes an educational component about preventive care. "So we just spend a fair amount of time on oral hygiene instruction, tips in terms of diet, things that they do on a daily basis to help maintain themselves and improve their dental health in the future," Dr. Tsuchiya said. In the future, Padua-Evans hopes to have dental insurance, but if not, she'll look to come back here again next year. "I'm super grateful. I just think it's incredible," she said. Meanwhile, dental professionals say Minnesota ranks dead last in the United States in Medicaid funding rates for pediatric dental care. As a result, a coalition comprised of the Minnesota Dental Association and others are working with the state legislature to improve that reimbursement. Roughly 45 million Americans live in areas with a dentist shortage, making it difficult to access dental care regardless of income or insurance coverage. Additionally, only a small portion of dentists accept Medicaid, which means many low-income people face added barriers. Together, these problems compound to limit access to dental care. New Mexico Dental Health Care Professional Shortage Third Greatest in Nation

According to 2012 Bloomberg data, New Mexico has the third worst dental care professional shortage in the nation- following Mississippi and Louisiana. Almost 40 percent of New Mexicans live in a Dental Desert –an area where there is a shortage of dental health care professionals. Some advocates are pushing the state to allow dental therapists to perform basic procedures now done by dentists. While this could improve access to dental care some dentists say it would risk safety and the quality of care. Every morning after Maria Alvarez flosses and brushes her teeth- she dulls the pain of the two cavity ridden molars throbbing on each side of her mouth… with cooking cloves she soaks in water- she says it is an old Mexican remedy she learned in Chihuahua, before she moved to Anthony New Mexico. “It is a small herb that serves like an anesthetic. So if you have pain you place it on the molar that has a hole or and it stays there and then you bite the clove when you chew it and it starts too anesthize that place and it will take the pain away” Alvarez said. Alvarez hasn’t always had to resort to old Mexican remedies to deal with tooth pain that keeps her awake at night- and aches through her shifts at work. Alvarez, like many New Mexicans living near the border, used to cross into Juarez- with her two sons and husband to get dental work done. “It is better to go to Juarez. Because over here I don’t have enough money for the consultation“ Alavarez said. But the drug war that engulfed Juarez and much of the state of Chihuahua took the lives of a number of Alvarez’s family and friends- and she doesn’t ever want to go back or relive the suffering- she says the tales of her friends being attacked and threatened, even when they were just there for dental work- are still on her mind. “They went to the dentists and the violence entered the place where they and they assaulted some people, they were scared and threw themselves on the ground and I’m scared, that’s why I don’t go to Juarez” Alavaerez said. Since the Affordable Care Act went into effect Alavarez’s two sons been able to access better dental treatment. But as an adult Alvarez says her insurance doesn’t cover enough of the cost for her to go to a dentist in New Mexico. Health Action New Mexico policy director Pam Blackwell says close to 40% of New Mexicans are living in dental health care professional shortage areas or “dental deserts”. Where basic preventive care is less accessible and basic dental issues are more likely to turn into major health issues like heart diseases. “We have absolutely an oral health crisis" Blackwell said. Modeled on programs dental therapist programs in Minnesota and Alaska Health Action New Mexico is pushing policy to fill the gap. While New Mexico already has dental hygienists a dental therapists program would be license to perform other basic procedures. “They would be able to fill cavities they would be able to do basic extraction, they would be able to take radiographs - and also really diagnose within a dental therapists scope of practice, so it is really routine in basic care” Blackwell said. President elect of the New Mexico Dental Association Dentist Dr. Michael Law is concerned about some of the procedures dental therapists would be able to use on patients after 24 months of training without direct supervision. “A dentist doesn’t even have to be in the same area for them to be able to perform things like surgery it is just dangerous! If something were to go wrong with that surgery and they don’t know how to handle it, I am 200 miles away from them” Law said. A 2014 report by the Minnesota legislature on that state’s program found that “Dentist therapists appear to be practicing safely, and clinics reported high patient satisfaction with dental therapist services” But the study acknowledges the program and “the profession is still in its infancy". Law says it is too soon to draw c onclusions. The Minnesota’s program has been place for 4 years. He says a better way to improve access and affordability would be to raise the state Medicaid reimbursement- currently less than half of New Mexico dentists are taking patients on Medicaid. “The reason I do not is purely and simply because the reimbursement is so poor, we couldn’t function. I mean we couldn’t do what we do every day on that. Now if they were to raise it and reevaluate it and possibly be able to start accepting it and providing care for those patients” Law said. Blackwell says doctors put up the same opposition to licensing nurse practitioners and physician assistants to perform functions that were once only performed by medical doctors. “It is turf and unfortunately what happens in these turf arguments is that consumers are forgotten, patients are forgotten and those communities who are underserved are forgotten”

EDINA, MINNESOTA — Larnie Thomas was walking through a suburban neighborhood in Edina, Minnesota when he was stopped by a plainclothes cop for what appears to be a case of walking while black. Thomas was walking on the side of the street to avoid construction taking place on the sidewalk. As he was walking, a police squad car pulled up beside him. The police statement alleges Officer Tim Olson tried to get Thomas’ attention, but couldn’t because he was wearing headphones. The officer then drove further ahead and blocked the street with his car so Thomas would have to stop, but Thomas just walked around it. Officer Olson claims he followed Thomas on foot to tell him to get out of the lane of traffic, and when he didn’t listen, he was grabbed by the jacket. Video taken of the interaction that followed has gone viral online. The seven-minute clip shows Larnie Thomas removing his own shirt to escape the grip of Officer Olson. The white woman recording the video, Janet Rowles, witnessed the entire incident, and can be heard pleading with the cop to resolve this in another way. She says Larnie Thomas absolutely should not have been stopped. Once Olson called for backup, another squad car arrived minutes later, and Thomas was arrested. Olson later claimed Thomas smelled of alcohol, and a breathalyzer confirmed there was alcohol in his system. However, his blood-alcohol level has not been reported. Furthermore, the matter of how much alcohol was in his system is not exactly relevant; in Minnesota, public intoxication is not considered a crime. Thomas was cited for disorderly conduct and failure to obey a traffic signal. The biggest risk you run with limited choice is not being able to see a top special if needed. As you'll see in the next section, if this could save you money, I might allow it. But, nine time out of 10 it's just not in the cards. Anthem and Aetna Pricing and Cost This is where things turn into a battle for last place. Both Anthem and Aetna have had little interest in offering competitive rates. There are companies like Medical Mutual and even InHealth Mutual that are cheaper. Also, don’t forget the network limitations both companies have. This has been the case for the last two years, with a few exceptions popping up here and there. Sometimes I would run a quote for a client and Anthem and Aetna would be in the ballpark, but rarely cheaper. If no major adjustments are made by either company in 2016 you can expect this trend to continue. The rate for 2016 should start to show up at the start of open enrollment on November 15.
