But a 2015 study by Chloe Eldridge found that “Current evidence shows hypoglycaemia is considerably prevalent amongst people with type 2 diabetes, particularly for those on insulin, yet still fairly common for other treatment regimens. This highlights the subsequent need for educational interventions and individualisation of therapies to reduce the risk of hypoglycaemia.” First let’s define what we mean by hypoglycemia. Hypoglycemia happens when the amount of blood glucose (sugar in the blood) drops to a level that's too low to sustain normal functioning; in most people, this is defined as a blood-sugar level below 70 mg/dl. Left untreated, hypoglycemia can lead to a coma and even death. It is very important that those with diabetes monitor sugar levels and treat hypoglycemia when it is present and right away. It happens more often than we think. So what does it feel like to have hypoglycemia? Let’s look at the most common risks of hypoglycemia, the symptoms and then what you need to do to treat the condition. You are at risk of having a low blood sugar reaction if you: Skip or delay a meal or snack. This is why it is so important for you to make sure you eat. I know the temptation to lose weight and other factors may cause you to not eat regularly, but it is so important that you eat 3 meals a day and eat your snacks in between meals. This is to maintain a steady, even blood sugar level. Take too much insulin or eat too few carbohydrates. We have all made a mistake in our dosage of insulin. You take it when half asleep or just in a hurry. It happens. Don’t try to pretend you didn’t make this mistake. It is fine! Just correct it. Also, in many of my videos I have stated that you must eat carbs. That is one reason a very low carb diet is not good for diabetes. It will really mess up your blood glucose regulating system. Trust me, I know from experience. Exercise. When you exercise, you are burning off carbs and sugars. This is great, but know that with your exercise comes a possibility for your blood sugar to be low. Get into the habit of checking your blood sugar before, during and after you exercise and be prepared. Have a carb snack ready to get that sugar back up. Drink alcohol, especially without eating carbohydrates. I have done extensive research on this topic. I have written about it and I have included the information in several of our videos. If you are going to drink alcohol, you must do so with food. Drink with a meal, or at the very least with snacks. Alcohol can bring down blood glucose levels drastically and the symptoms of hypoglycemia are often mistaken for drunkenness. Even if you are not intoxicated. 15/15 Rule If after checking your blood sugar and it is less than 70 milligrams per deciliter (mg/dl), you will need to follow the 15/15 rule. Eat 15 grams of then rest for 15 minutes and then recheck your blood sugar If your blood sugar is still less than 100 mg/dl, take another 15 grams of carbohydrate and retest your blood sugar in another 15 minutes. Repeat if necessary. Important: If you have frequent low blood sugars speak to your doctor. You may need changes in your medication and/or meal plan. Quick Carbohydrate Guide for Treating Low Blood Sugars If your blood sugar is less than 70 mg/dl, you need 15 grams of a quickly absorbed carbohydrate, like the ones listed below. Foods that contain protein or fat — such as chocolate, candy bars, ice cream, cookies, crackers and bread — don't raise blood sugar quickly enough. If you have type 1 diabetes and you do not take care of low blood glucose, you may pass out. If you do, a drug called glucagon should be injected into your skin, like you do with insulin. This can be done by a family member or friend who has been taught how to do it. Since glucagon may cause you to vomit, you should be placed on your side when the injection is given. If no one knows how to give the injection, you should be taken to a hospital. You need a prescription for a glucagon kit. You should awaken about 10 minutes after the glucagon is injected. If you do not, you should be taken at once to a hospital.

What are the causes of hypoglycaemia in those with diabetes? Hypoglycaemia is one of the most important and dangerous complication for those who are on treatment for diabetes. The most common cause is the drug itself. Drugs like sulfonylureas which is known as glimipride, gliclacide, gliplizide, glibenclamide, all these drugs stimulate insulin continuously so they are likely to cause sugar levels to go down. Second drugs are insulin. So the reasons to cause sugar levels to go down, number 1 is reduced food intake. It is very common as the patient take the medicine and forget to eat normally especially in those who are taking insulin and sulfonylureas. The other drugs like metformin or PP4 inhibitors and to some extent glitazones usually don’t contribute to low glucose directly. To some extent glp-1 analogues can bring down sugar levels which are taken in the form of injection. Also there are drugs like ripaglinide which are short acting drugs which can also bring down the sugar levels. So drugs are the most common cause and second is reduced food intake or a mismatch between the drug and the food can cause the sugar levels to go down. What are the other causes? One is any dysfunction of the liver. Liver is the store house of glycogen and important source of immediate release of glucose. Any liver disease like hepatitis or alcoholic liver disease or excessive alcohol intake can itself bring down the sugar levels drastically and cause hypoglycaemia. Second reason is that the liver can get damaged because of cancer or cirrhosis which can occur in those with excessive fat deposit which is known as non-alcoholic fatty liver disease where the liver is destroyed or damaged and therefore the sugar levels come down. Third cause which is the most common is renal dysfunction. Most commonly kidney damage will cause the insulin levels to be retained in the body because it is not utilised and metabolised. So whenever we see a patient with low glucose levels we should check for the liver function and the kidney function. There are other low causes for the low glucose especially those who are taking some antibiotics like gatifloxacin sometimes hydroxychloroquine, anti malarial drugs. We also see in patients who have septicaemia and a severe infection to get their blood sugar down. One of the other important and life threatening cause is adrenal dysfunction which can be in the form of hypopituitarism i.e. the pituitary is not functioning well and the adrenal gland which is situated above the kidney is not functioning. Primary or secondary adrenal insufficiency can cause hypoglycaemia. Another rare cause for hypothyroidism is severe hypothyroidism where the body’s appetite and metabolism is very low. In any individual we need to find out the cause before we actually go for appropriate treatment. In an emergency we need to give some glucose but we need to find out the cause which can be a imbalance in food intake, liver damage, kidney damage, severe infection, adrenal insufficiency either primary or secondary and sometimes severe hypothyroidism.

Depending on what you had for dinner or what snacks you had during the evening. What time you took your medicine can play a factor as well. But if you have consistently high blood glucose readings every morning, it could be one of three reasons that we are going to discuss in this video. DAWN PHENOMENON Researchers feel the most common reason for high blood glucose levels in the morning is the dawn phenomenon. The glucose is going up from sources other than digested food. Some of it is produced by the liver from stored starch and fatty acids. Livers that produce too much glucose are one of the main ways diabetes causes high blood glucose levels. Other organs also produce small amounts of glucose. This is called “gluconeogenesis” for those of you who like the technical stuff. Organs produce glucose to keep blood glucose from going too low at night or other times of not eating. From about 2 AM to 8 AM, most people’s bodies produce hormones, including cortisol, glucagon, and epinephrine. All these hormones increase insulin resistance and tell the liver to make more glucose. The idea is to get you enough glucose to get out of bed and start the day. Everyone has a dawn phenomenon. Otherwise they’d be too weak to get breakfast. But in people without diabetes, insulin levels also increase to handle the extra glucose. People with diabetes can’t increase insulin levels that much, so their early morning blood glucose levels can rise dramatically. Experts disagree on how many people have a dawn phenomenon. Estimates range from 3% to 50% of Type 2s and from 25% to 50% of Type 1s. Is dawn phenomenon a serious problem? It can be serious. According to the American Diabetes Association, “Some people with dawn phenomenon find that their glucose continues to rise until they eat in the morning. For others, levels will settle down a few hours after waking, regardless of whether or not they eat.” According to columnist Wil Dubois, the higher your A1C, the more likely you are to have a significant dawn phenomenon. It could be that spending a number of hours each morning out of control is having a significant effect on your overall control. Some people have high glucose levels in the morning because their medicines wear off overnight. This could be true of medicines like insulin, and metformin. If you are taking any long-acting medicine, consider asking your doctor about changing meds, doses, or times THE SOMOGYI EFFECT In some cases, medicine can be too strong. If your glucose goes too low in the night, you could have a rebound high in the morning. This is called the Somogyi effect. If you are waking up high and are suffering pounding headaches, or find your sheets sweat-soaked, the odds are you are having lows in your sleep…You need to visit with your doctor about taking less meds. According to Dubois, the new insulins are much less likely to cause a Somogyi reaction. But because of cost, people are going back to NPH insulin. NPH is cheaper, shorter-acting, and more likely to cause a low, leading to a rebound high in the morning. WANING INSULIN If you take insulin and have been experiencing high blood sugar in the morning, your insulin may simply be wearing off too soon. If this is the case, your doctor can adjust your dosage or change what time you are taking the insulin to prevent high glucose levels. Pinpointing the Cause for Effective Treatment If your blood sugar is fairly even when you go to bed and at 3 a.m. but is higher in the morning, you are probably experiencing dawn phenomenon. If your blood sugar is low at 3 a.m., but high in the morning, you probably suffer from the Somogyi effect. If your blood sugar is elevated at 3 a.m. and then higher still in the morning, you probably have waning insulin. Even if you’ve identified the reason behind your high morning number, never attempt to correct it on your own. Instead, talk with your doctor. Together, you can find a treatment plan that gets you back on track in the morning. How can this situation be corrected? Once you and your doctor determine how your blood sugar levels are behaving at night, he or she can advise you about the changes you need to make to better control them. -Changing the time you take the long-acting insulin in the evening so that its peak action occurs when your blood sugars start rising -Changing the type of insulin you take in the evening -Taking extra insulin overnight -Eating a lighter breakfast -Increasing your morning dose of insulin -Switching to an insulin pump, which can be programmed to release additional insulin in the morning

However, it is important to understand why it happens and what you can do about it… The dawn phenomenon Logically you’d think that your blood sugar reading should be at it’s lowest in the morning. After all, you’ve eaten nothing and done nothing but sleep. But regardless of whether you eat, glucose production continues anyway… The reason for this is your body’s cells need fuel for your heart to beat, your brain to work and your organs to keep functioning. When you don’t eat, or when you’re asleep, the body can break down stores of available glucose (glycogenolysis) or enter a process called gluconeogenesis — a process that can use non-carbohydrate stores such as amino acids to produce glucose. Various hormones such as glucagon, growth hormone and cortisol, are also involved in raising glucose levels. To wake you up every morning, your body naturally activates these hormones from around 3 am onwards, which explains why it’s called the dawn phenomenon. How do we manage hypoglycaemia, especially in those with diabetes? The most important form of treatment is acute treatment or immediate management. i.e. to grab anything which contains high sugar, glucose or fruit juice or anything which can release sugar immediately. But taking chocolate which contains coco and fat does not raise the sugar immediately. So that should not be the treatment of choice. You have to take something which can immediately give you energy in the form of glucose or candies or sugar itself in the form of sugar tablet. Sugar tabs are available which are called as hypo tabs or gluco tabs should be kept with the patient always and whenever they feel symptoms of low sugar like sweating, pounding heart, giddiness and shivering of limbs, they should immediately treat themselves. The second part of management is sub acute. That is to eat something that is rich in calories and contains some amount of protein and fat and the body gets enough glucose for at least half an hour to one hour. The most common reason why there is low glucose is because the drug effect is little more and going to act for a longer period of time we need to take enough food that is going to last in our body get converted to glucose over a period of one to two hours, so that we can go and have our normal food. If the sugar levels fall down just before eating then take little glucose and have your normal food and ensure that you reduce the dose of your medicine which you are likely to take before the meal. If you are on insulin reduce the dose upto 4 units. If you are taking the tablet make it half and immediately go and consult your doctor so that they can find out any other cause apart from the food and medicine. So the change in food and medicine are the two important things that you can do to ensure that the sugar levels are normal. But we need to find what is the cause in order to treat. It could be kidney problem, liver problem or some simple infection in the body which can be associated also with fever. Next is to find out if the other organs are damaged. Especially the adrenal gland, pituitary gland and the thyroid dysfunction mainly hypothyroidism. Then if you find that the liver and kidney are not involved and the food and drugs are managed in spite of that the sugar levels are falling then always think of the adrenal insufficiency and severe hypothyroidism and get the blood test done with your doctor so that approximately the glucose levels are managed and they are brought to normal. Hypoglycaemia if it occurs again and again can cause severe symptoms which can be life threatening leading to coma and sometimes fits especially in the night time and early morning headaches will be there. Sometimes somogyi phenomena can occur in diabetes where the sugar levels go down very low in the night and actually when you check in the morning it will be high. Whenever you see high glucose in the morning always confirm with your doctor whether you have somogyi phenomena. So if you can manage the diet, drugs and find out the cause then life will be better for diabetics without hypoglycaemia.

Our body gets glucose from some of the foods we eat. After the digestive process, the liver releases glucose into the bloodstream, sending it to cells to be used as energy. As glucose is released into the bloodstream, blood sugar levels rise. This triggers the pancreas to release the hormone insulin, which tells the cells to open up and let the glucose in, which lowers blood sugar levels. Excess glucose is stored as glycogen in the muscles and liver. This process happens many times throughout the day after we eat.
