How To Treat Low Blood Sugar (Diabetes Type 2)

But a 2015 study by Chloe Eldridge found that “Current evidence shows hypoglycaemia is considerably prevalent amongst people with type 2 diabetes, particularly for those on insulin, yet still fairly common for other treatment regimens. This highlights the subsequent need for educational interventions and individualisation of therapies to reduce the risk of hypoglycaemia.” First let’s define what we mean by hypoglycemia. Hypoglycemia happens when the amount of blood glucose (sugar in the blood) drops to a level that's too low to sustain normal functioning; in most people, this is defined as a blood-sugar level below 70 mg/dl. Left untreated, hypoglycemia can lead to a coma and even death. It is very important that those with diabetes monitor sugar levels and treat hypoglycemia when it is present and right away. It happens more often than we think. So what does it feel like to have hypoglycemia? Let’s look at the most common risks of hypoglycemia, the symptoms and then what you need to do to treat the condition. You are at risk of having a low blood sugar reaction if you: Skip or delay a meal or snack. This is why it is so important for you to make sure you eat. I know the temptation to lose weight and other factors may cause you to not eat regularly, but it is so important that you eat 3 meals a day and eat your snacks in between meals. This is to maintain a steady, even blood sugar level. Take too much insulin or eat too few carbohydrates. We have all made a mistake in our dosage of insulin. You take it when half asleep or just in a hurry. It happens. Don’t try to pretend you didn’t make this mistake. It is fine! Just correct it. Also, in many of my videos I have stated that you must eat carbs. That is one reason a very low carb diet is not good for diabetes. It will really mess up your blood glucose regulating system. Trust me, I know from experience. Exercise. When you exercise, you are burning off carbs and sugars. This is great, but know that with your exercise comes a possibility for your blood sugar to be low. Get into the habit of checking your blood sugar before, during and after you exercise and be prepared. Have a carb snack ready to get that sugar back up. Drink alcohol, especially without eating carbohydrates. I have done extensive research on this topic. I have written about it and I have included the information in several of our videos. If you are going to drink alcohol, you must do so with food. Drink with a meal, or at the very least with snacks. Alcohol can bring down blood glucose levels drastically and the symptoms of hypoglycemia are often mistaken for drunkenness. Even if you are not intoxicated. 15/15 Rule If after checking your blood sugar and it is less than 70 milligrams per deciliter (mg/dl), you will need to follow the 15/15 rule. Eat 15 grams of then rest for 15 minutes and then recheck your blood sugar If your blood sugar is still less than 100 mg/dl, take another 15 grams of carbohydrate and retest your blood sugar in another 15 minutes. Repeat if necessary. Important: If you have frequent low blood sugars speak to your doctor. You may need changes in your medication and/or meal plan. Quick Carbohydrate Guide for Treating Low Blood Sugars If your blood sugar is less than 70 mg/dl, you need 15 grams of a quickly absorbed carbohydrate, like the ones listed below. Each of the following servings provides 15 grams of carbohydrate. Candies 5 small gum drops 12 gummy bears 6 large jelly beans 5 Life Savers 15 Skittles 1 Tablespoon honey, jam or jelly 1 Tablespoon sugar in water 4 Starburst Beverages 1/2 cup apple juice 1/2 cup orange or grapefruit juice 1/2 cup pineapple juice 1/2 cup regular soda (not diet) 1/3 cup grape juice 1/3 cup cranberry juice 1/3 cup prune juice 1 cup fat free milk Fruits 1/2 banana 1 small apple 1 small orange 1/2 cup applesauce 2 tablespoons of raisins 15 grapes Other 3 to 4 glucose tablets 1 tube glucose gel The foods I have given you are easily absorbed and will raise blood sugar levels quickly. Foods that contain protein or fat — such as chocolate, candy bars, ice cream, cookies, crackers and bread — don't raise blood sugar quickly enough. If you have type 1 diabetes and you do not take care of low blood glucose, you may pass out. If you do, a drug called glucagon should be injected into your skin, like you do with insulin. This can be done by a family member or friend who has been taught how to do it. Since glucagon may cause you to vomit, you should be placed on your side when the injection is given. If no one knows how to give the injection, you should be taken to a hospital. You need a prescription for a glucagon kit. You should awaken about 10 minutes after the glucagon is injected. If you do not, you should be taken at once to a hospital.

There are two classes of hypoglycemic symptoms - those due to lack of sugar in the brain and those due to the adrenalin response of your body. When blood sugar levels get too low, the body will signal for the adrenal glands to kick in. This signals the liver for more sugar. Symptoms from the adrenalin response: One of the first signs you might notice is an insatiable hunger. You’ve eaten but nothing you eat can seem to fill you up. If you have this feeling and you just can’t get enough to eat, check your blood sugar immediately. If it is lower than 70, get 15 grams of fast acting carbs in your body. This can come from a variety of sources: 4 ounces fruit juice 6 ounces regular soda (not diet) 4 or 5 saltine crackers 1 tablespoon of jelly or honey 8 pieces of hard candy Inability to rest at night. This could be due to nocturnal hypoglycemia. Signs of Nocturnal Hypoglycemia are: Restlessness. Making unusual noises. Attempting to get out of bed or accidentally rolling out of bed. Sleepwalking. Nightmares. Sweating. You may wake up with a headache in the morning if your blood sugar was low during the night. Sometimes a snack before bed can reduce the frequency and severity of these sleep disturbances. Some people have a strong feeling of anxiety. This is caused by that rush of adrenalin. This overload of adrenalin can cause some very anxious moments and can also make you feel panicked. You may have uncontrollable sweating even if you aren’t hot. This can be one of the very first signs of hypoglycemia. The central nervous system kicks in after the adrenalin has been pushed from the adrenal glands and this causes the autonomic nervous system to issue a “sweat order” to the skin. Shakes and tremors. Because of the adrenalin rush, the central nervous system starts to malfunction. As a result, it releases chemicals that encourage glucose production and also produce the tremors. Symptoms from lack of sugar in the brain: Fatigue can actually fall in both categories. There is a fatigue that can set in after an adrenalin rush, but also a fatigue from the lack of sugar in the brain. Either can be very serious. Emotional instability. Mood swings and sudden emotional episodes not typical of your normal behavior are among the neurological symptoms of hypoglycemia. These could include irrational outbursts, random crying for no reason which may become uncontrollable, extreme anger without cause, and a strong desire to be left alone. Mild mood changes that may not be as severe, such as general irritability or becoming easily annoyed, can also be a signal that your blood sugar may be dropping. Dizziness and light-headedness. If you experience these common symptoms of hypoglycemia, heed them and treat the hypoglycemia quickly. Dropping blood sugar levels can result in fainting. If you feel dizzy or light headed, sit or lie down immediately to avoid injuring yourself. Because the brain is especially sensitive to a drop in sugar, you may experience a sense of confusion and an inability to concentrate on one thing at a time. Your mind seems to wander all over the place. If your vision suddenly becomes blurry or you see double, a drop in blood sugar may be to blame. This is also a result of the brain’s lack of sugar affecting the optic nerve. Your sugar-starved brain may cause you to slur your words. To others this may resemble someone who has been drinking. But you may not even notice. These lists are not exhaustive, but are the more common symptoms of hypoglycemia. According to WebMD, Some people have no symptoms of low blood sugar. Think about it - your meds are supposed to lower blood sugars and when you fix the true problem and things normalize and you still are taking this sugar lowering drug - it can push it below normal. The thing to do is get with your doctor and have them adjust your meds because chances are you don’t need as much. If you are off the medication and your blood glucose (sugar) levels are too low (60s) BUT you feel fine, you may in fact be fine because when you do ketogenic diet and IF your body is no longer running on glucose, it's running on ketones which do not involve so much glucose. However, if you feel weird if your sugars are low, it could be adrenals (cortisol or adrenaline). In this case, go at this more gradual and not too fast so your body can really adapt over time. Hey guys, I recently have a question from someone who was a diabetic, they have a high blood sugars, and they did ketogenic diet and intermittent fasting and then ended up having low blood sugars, hypoglycemia. They are a bit concerned with this complication and side effect from doing ketogenic diet and intermittent fasting. No.1, the purpose of the medication that you are taking as a diabetic is to lower the blood sugars. If you are still taking the medications and you are doing ketogenic diet and IF you could end up with excessively low blood sugars because you don’t even need the medications, or you don’t need as much. Ketogenic diet and Intermittent Fasting decrease the need for diabetic medication, get with your doctor, adjust the dosage, reduce it or maybe you don’t need it at all. If your blood sugars is 100 or less, you don’t need the medication. Let’s say for example you are off with the medication and it is still too low but you feel fine, that is not a problem because when you do ketogenic diet and intermittent fasting you will actually have the new normal because you are not running your body on sugar anymore so your body will adapt and you can get by with a lot less 80, 70 or even 60 and feel fine because your body is not running in sugar anymore. But if you feel bad, if you feel jittery and if you feel your blood sugars are a problem, it can also be symptoms of the adrenal gland that is trying to compensate in bringing your sugars up but it can’t and then you’ll notice a symptoms of more of the spike of either cortisol or adrenaline and that could be why you are feeling that. If that’s the case then just basically go about it a little bit more gradual. Instead of going one-meal-a-day, do two meals, instead of doing 2 meals, do 3 meals no snacks adding a lot of greens, b vitamins, potassium supplements so you can actually correct this situation a lot better so you can go longer without meals. What are the causes of hypoglycaemia in those with diabetes? Hypoglycaemia is one of the most important and dangerous complication for those who are on treatment for diabetes. The most common cause is the drug itself. Drugs like sulfonylureas which is known as glimipride, gliclacide, gliplizide, glibenclamide, all these drugs stimulate insulin continuously so they are likely to cause sugar levels to go down. Second drugs are insulin. So the reasons to cause sugar levels to go down, number 1 is reduced food intake. It is very common as the patient take the medicine and forget to eat normally especially in those who are taking insulin and sulfonylureas. The other drugs like metformin or PP4 inhibitors and to some extent glitazones usually don’t contribute to low glucose directly. Some of it is produced by the liver from stored starch and fatty acids. Livers that produce too much glucose are one of the main ways diabetes causes high blood glucose levels. Other organs also produce small amounts of glucose. This is called “gluconeogenesis” for those of you who like the technical stuff. Organs produce glucose to keep blood glucose from going too low at night or other times of not eating. From about 2 AM to 8 AM, most people’s bodies produce hormones, including cortisol, glucagon, and epinephrine. All these hormones increase insulin resistance and tell the liver to make more glucose. The idea is to get you enough glucose to get out of bed and start the day. Everyone has a dawn phenomenon. Otherwise they’d be too weak to get breakfast. But in people without diabetes, insulin levels also increase to handle the extra glucose. People with diabetes can’t increase insulin levels that much, so their early morning blood glucose levels can rise dramatically. Experts disagree on how many people have a dawn phenomenon. Estimates range from 3% to 50% of Type 2s and from 25% to 50% of Type 1s. Is dawn phenomenon a serious problem? It can be serious. According to the American Diabetes Association, “Some people with dawn phenomenon find that their glucose continues to rise until they eat in the morning. For others, levels will settle down a few hours after waking, regardless of whether or not they eat.” According to columnist Wil Dubois, the higher your A1C, the more likely you are to have a significant dawn phenomenon. It could be that spending a number of hours each morning out of control is having a significant effect on your overall control. Some people have high glucose levels in the morning because their medicines wear off overnight. This could be true of medicines like insulin, and metformin. If you are taking any long-acting medicine, consider asking your doctor about changing meds, doses, or times THE SOMOGYI EFFECT In some cases, medicine can be too strong. If your glucose goes too low in the night, you could have a rebound high in the morning. This is called the Somogyi effect. If you are waking up high and are suffering pounding headaches, or find your sheets sweat-soaked, the odds are you are having lows in your sleep…You need to visit with your doctor about taking less meds. In this video I discuss what is blood sugar level, and what causes high and low blood sugar levels. We also look at insulin's role in blood sugar levels, and some of the risk factors for developing type 2 diabetes. Transcript (partial) In this video we are going to discuss what blood sugar or blood glucose is, and why it is important. Blood sugar level is the amount of glucose(sugar) present in the blood. Our body gets glucose from some of the foods we eat. After the digestive process, the liver releases glucose into the bloodstream, sending it to cells to be used as energy. As glucose is released into the bloodstream, blood sugar levels rise. This triggers the pancreas to release the hormone insulin, which tells the cells to open up and let the glucose in, which lowers blood sugar levels. Excess glucose is stored as glycogen in the muscles and liver. This process happens many times throughout the day after we eat. A normal fasting (no food for 8 hours) blood sugar level is between 70 and 99mg/dl, and a normal level two hours after eating is less than 140mg/dl. So, why is this important? Well, there are 3 main reasons we need to be aware of our blood sugar level. The 1st reason is that high blood sugar levels can lead to type 2 diabetes. Type 2 diabetes is when the body doesn’t use insulin properly (insulin resistance), or when the pancreas stops producing enough insulin. Over time, High blood sugar levels can put a lot of stress on the pancreas, possibly causing it to become permanently damaged. 95% of all diabetes cases in the US are type 2. Researchers don't fully understand why some people develop type 2 diabetes and others don't. However, there are some factors that increase the risk. -being overweight – the more fatty tissue you have, the more resistant your cells become to insulin. -fat distribution – more fat storage in the abdomen increases the risk -inactivity - Physical activity helps you control your weight, uses up glucose as energy and makes your cells more sensitive to insulin. The 2nd reason we need to monitor our blood sugar levels is that High levels can also lead to atherosclerosis, which is hardening and narrowing of the arteries. This can block the arteries, putting blood flow at risk, which can lead to other problems. The 3rd reason is low blood sugar levels, this is rare in non-diabetics. This can be caused by certain medications, excessive alcohol consumption, excessive exercise, and from meals high in refined carbs.

The cause of reduced post prandial blood glucose levels can be divided into three categories. First one in normal individuals, second one in those with pre diabetes, i.e. early stage of diabetes and third in those with diabetes. Normal individual without any problem can have low blood glucose compared to what it was in the fasting state because their body metabolism is such that they are able to produce more insulin in order to bring down sugar levels to less than what it was in the fasting state. This is a normal physiological variation and nothing needs to be done for that. In pre diabetes group the post prandial blood glucose will be low because of what is known as hyper insulin state or insulin hypertrophy where the body id trying to restore the function because of some amount of insulin resistance. Because of that the blood glucose levels initially go up drastically and come down because the insulin is stimulated at a high level and therefore we have post prandial low glucose. This post prandial blood glucose can be symptomatic and it is also known as reactive hypoglycaemia where the sugar levels are high in the beginning, then it becomes very high and comes down drastically. The case for this is simple sugars, foods rich in high glycaemia known as high GI foods or high glycaemic index foods which includes white rice, potatoes, simple sugars, dates and fruit juices which is low in fibre. Anything with low fibre can stimulate insulin at a higher rate and bring it down after 2 to 3 hours which is known as reactive hypoglycaemia. The third situation is diabetes. In diabetes when you take medicine and don’t eat food properly your sugar levels will come down after food. It may be high before, it will down after eating. If the food content is low or they are on insulin or the insulin dose delivered is very high. Fourth group which is very rare is those who have underwent gastric surgeries where some part of intestine is cut and also those who undergo bariatric surgery for weight loss. In them there is a n hyperstimulation of insulin after eating and therefore the sugar levels will come down. This is usually seen in people who have undergone pancreatic surgery or GI surgery or bypass of the intestine or stomach.
