In this lesson, you will learn about how your blood glucose level is regulated (or controlled) by two important hormones – insulin and glucagon, via a negative feedback system. When you consume a meal that is high in carbohydrates, such as rice, pasta, and bread, this will cause your blood glucose level to increase. Carbohydrates are essentially long chains of repeating glucose monomer units, much like beads on a necklace. During digestion, this is broken apart into glucose, which absorbed into our bloodstream. This increased blood glucose level causes a gland known as the pancreas to secrete a hormone called insulin. Remember that a gland secretes hormones which act on specific target organs. In this case, the target organ is your liver, which is stimulated to convert glucose to glycogen. Glycogen is basically long, multi-branched chains of glucose monomers, stored in liver and muscle cells. Insulin also causes your body cells to uptake (or take in) glucose. So this decreases your blood glucose level back to its optimal state. When this system is faulty, this leads to a medical condition known as diabetes – If you want to learn more about diabetes, this will be addressed in another video. The same response also occurs when you consume foods and drinks high in sugar such as sweets, cakes, and fizzy drinks. When your blood glucose level drops, such as when you are hungry, the pancreas secretes a hormone called glucagon. Like insulin, the target organ for glucagon is also the liver, though it stimulates the opposite process – the breakdown of glycogen into glucose. This increases your blood glucose level back to its optimal state. So to review, insulin and glucagon are two hormones released by the pancreas, which act on the liver to regulate our blood glucose level. What are the causes of hypoglycaemia in those with diabetes? Hypoglycaemia is one of the most important and dangerous complication for those who are on treatment for diabetes. The most common cause is the drug itself. Drugs like sulfonylureas which is known as glimipride, gliclacide, gliplizide, glibenclamide, all these drugs stimulate insulin continuously so they are likely to cause sugar levels to go down. Second drugs are insulin. So the reasons to cause sugar levels to go down, number 1 is reduced food intake. It is very common as the patient take the medicine and forget to eat normally especially in those who are taking insulin and sulfonylureas. The other drugs like metformin or PP4 inhibitors and to some extent glitazones usually don’t contribute to low glucose directly. To some extent glp-1 analogues can bring down sugar levels which are taken in the form of injection. Also there are drugs like ripaglinide which are short acting drugs which can also bring down the sugar levels. So drugs are the most common cause and second is reduced food intake or a mismatch between the drug and the food can cause the sugar levels to go down. What are the other causes? One is any dysfunction of the liver. Liver is the store house of glycogen and important source of immediate release of glucose. Any liver disease like hepatitis or alcoholic liver disease or excessive alcohol intake can itself bring down the sugar levels drastically and cause hypoglycaemia. Second reason is that the liver can get damaged because of cancer or cirrhosis which can occur in those with excessive fat deposit which is known as non-alcoholic fatty liver disease where the liver is destroyed or damaged and therefore the sugar levels come down. Third cause which is the most common is renal dysfunction. Most commonly kidney damage will cause the insulin levels to be retained in the body because it is not utilised and metabolised. So whenever we see a patient with low glucose levels we should check for the liver function and the kidney function. There are other low causes for the low glucose especially those who are taking some antibiotics like gatifloxacin sometimes hydroxychloroquine, anti malarial drugs. We also see in patients who have septicaemia and a severe infection to get their blood sugar down. One of the other important and life threatening cause is adrenal dysfunction which can be in the form of hypopituitarism i.e. the pituitary is not functioning well and the adrenal gland which is situated above the kidney is not functioning. Primary or secondary adrenal insufficiency can cause hypoglycaemia. Another rare cause for hypothyroidism is severe hypothyroidism where the body’s appetite and metabolism is very low. In any individual we need to find out the cause before we actually go for appropriate treatment. In an emergency we need to give some glucose but we need to find out the cause which can be a imbalance in food intake, liver damage, kidney damage, severe infection, adrenal insufficiency either primary or secondary and sometimes severe hypothyroidism.

Sugar levels for any normal adult should be - Upon waking up (before breakfast 80-120, before meals 80-120, 2 hrs after meals 160 or less and at bedtime 100 to 140. Talk about maintaining sugar chart when diabetic and the levels which should be optimal for the adult. Get more high-quality shut-eye Poor or limited sleep affects body chemistry, and getting more slumber helps with blood sugar control. Healthy volunteers who didn’t get enough sleep for several nights in a row had higher blood levels of fatty acids, which reduced the ability of insulin to regulate blood sugar, the researchers found. 6. Make friends with some cactus The paddle-shaped nopal cactus (also known as “prickly pear”) might reduce blood sugar. The fruit and stem of the nopal plant may slow carbohydrate absorption and lower post-meal blood glucose readings, according to some preliminary studies. Greens Go beyond your regular salad and try kale, spinach, and chard. They’re healthy, delicious, and low-carb, Powers says. Roast kale leaves in the oven with olive oil for quick, crunchy chips. You can also mix greens in with roasted veggies to add texture and a different flavor, or serve them with a little protein, like salmon. 3. Flavorful, Low-calorie Drinks Plain water is always good, but water infused with fruits and vegetables is more interesting. Cut up a lemon or cucumber and put it in your water, or make ice cubes with some flavoring in them. If you’re not a hot tea drinker, try cold tea with lemon or a cinnamon stick. “Not only are these beverages low-carb, they can also help fill you up so you don’t crave other foods,” Powers says. 4. They know your situation much better and can give you a plan to attack that high blood sugar. Watch for signs of high blood sugar You know the feeling: extreme thirst, sluggishness, nausea, blurred vision, a downright sick feeling. And your family or friends may tell you that extreme irritability is a major sign you need to check your blood sugar to see if it is high. The best thing to do is to catch it before it gets really high, or it will be harder to bring down quickly, causing havoc on your blood sugar readings for days. If you do not take insulin as a part of your treatment plan, these tips will show you how to lower your blood sugar fast. If you take insulin, you will first want to give the appropriate amount of insulin to correct the blood sugar. But exercise can also help lower blood sugar when it is excessively high by getting your heart pumping and the blood flowing, which uses up the glucose in your bloodstream faster. It will also get your endorphins going, which will help your body start to feel better, too. Try to keep your heart rate up for at least 10 to 15 minutes. Check your blood sugar intermittently to prevent it from rebounding too low in your attempt to stabilize it. (Note: If your blood sugars have been so high that you have high ketones in your blood—a condition called DKA or diabetic ketoacidosis—do not exercise as it may drive your blood sugar even higher.) Eat a protein-packed snack. Protein acts as a blood sugar stabilizer and can slow the absorption rate of glucose. One of the symptoms of high blood sugar is increased hunger, so this can help satisfy that craving while helping lower the blood sugar at the same time. Don't grab a snack that has a lot of sugar along with the protein, or you will be defeating the purpose of it. Good sources of protein include a tablespoon of no-sugar-added peanut butter or an ounce of cheese. But don’t overdo it. Our body gets glucose from some of the foods we eat. After the digestive process, the liver releases glucose into the bloodstream, sending it to cells to be used as energy. As glucose is released into the bloodstream, blood sugar levels rise. This triggers the pancreas to release the hormone insulin, which tells the cells to open up and let the glucose in, which lowers blood sugar levels. Excess glucose is stored as glycogen in the muscles and liver. This process happens many times throughout the day after we eat. A normal fasting (no food for 8 hours) blood sugar level is between 70 and 99mg/dl, and a normal level two hours after eating is less than 140mg/dl. So, why is this important? Well, there are 3 main reasons we need to be aware of our blood sugar level. The 1st reason is that high blood sugar levels can lead to type 2 diabetes. Type 2 diabetes is when the body doesn’t use insulin properly (insulin resistance), or when the pancreas stops producing enough insulin. Over time, High blood sugar levels can put a lot of stress on the pancreas, possibly causing it to become permanently damaged. 95% of all diabetes cases in the US are type 2. Researchers don't fully understand why some people develop type 2 diabetes and others don't. However, there are some factors that increase the risk. -being overweight – the more fatty tissue you have, the more resistant your cells become to insulin. -fat distribution – more fat storage in the abdomen increases the risk -inactivity - Physical activity helps you control your weight, uses up glucose as energy and makes your cells more sensitive to insulin. The 2nd reason we need to monitor our blood sugar levels is that High levels can also lead to atherosclerosis, which is hardening and narrowing of the arteries. This can block the arteries, putting blood flow at risk, which can lead to other problems. The 3rd reason is low blood sugar levels, this is rare in non-diabetics. This can be caused by certain medications, excessive alcohol consumption, excessive exercise, and from meals high in refined carbs.

The cause of reduced post prandial blood glucose levels can be divided into three categories. First one in normal individuals, second one in those with pre diabetes, i.e. early stage of diabetes and third in those with diabetes. Normal individual without any problem can have low blood glucose compared to what it was in the fasting state because their body metabolism is such that they are able to produce more insulin in order to bring down sugar levels to less than what it was in the fasting state. This is a normal physiological variation and nothing needs to be done for that. In pre diabetes group the post prandial blood glucose will be low because of what is known as hyper insulin state or insulin hypertrophy where the body id trying to restore the function because of some amount of insulin resistance. Because of that the blood glucose levels initially go up drastically and come down because the insulin is stimulated at a high level and therefore we have post prandial low glucose. This post prandial blood glucose can be symptomatic and it is also known as reactive hypoglycaemia where the sugar levels are high in the beginning, then it becomes very high and comes down drastically. The case for this is simple sugars, foods rich in high glycaemia known as high GI foods or high glycaemic index foods which includes white rice, potatoes, simple sugars, dates and fruit juices which is low in fibre. Anything with low fibre can stimulate insulin at a higher rate and bring it down after 2 to 3 hours which is known as reactive hypoglycaemia. The third situation is diabetes. In diabetes when you take medicine and don’t eat food properly your sugar levels will come down after food. It may be high before, it will down after eating. If the food content is low or they are on insulin or the insulin dose delivered is very high. Fourth group which is very rare is those who have underwent gastric surgeries where some part of intestine is cut and also those who undergo bariatric surgery for weight loss. In them there is a n hyperstimulation of insulin after eating and therefore the sugar levels will come down. This is usually seen in people who have undergone pancreatic surgery or GI surgery or bypass of the intestine or stomach.

While most Americans are barely getting around 5-10. 4. Eat Smaller Meals, More Often I’m sure you’ve heard about this when it comes to weight loss. Watch What You Drink You should never drink your calories -- no sodas, juices, alcohol, “shakes” or anything similar. They all digest quickly and spike up your blood sugar. 6. Manage Your Stress Levels You can’t have health without also discussing stress management. What’s interesting is that stress increases blood sugar levels AND decreases insulin sensitivity. (15,16) And this is because of a change in hormones such as glucagon, cortisol, testosterone, adrenal hormones and so forth. (17,18) 7. Better Quality Sleep You can’t manage your stress if you don’t get enough, quality sleep. Again, this has to do with a negative changes in your hormones when you don’t sleep well. (20,21) 8. Lose Fat Losing unwanted fat and decreasing your belly fat has numerous health benefits.
