http://www.stomponstep1.com/fat-soluble-vitamins-and-water-soluble-vitamins-steatorrhea/ Fat Soluble Vitamins are hydrophobic (fat loving) nutrients that are necessary in small amounts to sustain a healthy life. Excess fat soluble vitamins can be stored in the liver and fat (adipose tissue). Therefore, these vitamins do not need to be eaten every single day as stores of the vitamins can sustain a person for some time. This is why a deficiency may take months to develop as the stores of the vitamin can take a long time to be depleted. This ability to store vitamins in tissue can also lead to vitamin toxicity. This toxicity is a vitamin overdose caused by taking a vitamin in excess. Since fat soluble vitamins can be stored in the body they can accumulate to the point of causing illness. Fat soluble vitamins are usually absorbed as part of chylomicrons (fat lobules) in the small intestine after being digested by pancreatic enzymes. Therefore, if fat absorption is inhibited the digestion of fat soluble vitamins will also be limited. This malabsorption can result in a deficiency of fat soluble vitamins as they are being excreted in the feces rather than being absorbed. This condition of excess fat excretion in feces is called Steatorrhea. It results in bulky, foul-smelling bowel movements with an oily appearance that float. Common causes of Steatorrhea on the USMLE Step 1: • Cystic Fibrosis (Thick mucus = decreased ability to secrete pancreatic enzymes) • Pancreatitis or Pancreatic Cancer (decreased ability to secrete pancreatic enzymes) • Celiac Disease & Crohn's Disease (damages portions of small intestine responsible for fat absorption) • Liver disease, biliary cirrhosis and sometimes gallstones (= less bile for fat absorption) Water Soluble Vitamins are hydrophilic (water loving) nutrients that are necessary in small amounts to sustain a healthy life. Unlike fat soluble vitamins they are generally not stored in body tissues. If you take in excess water soluble vitamins, your kidneys excretes most of the excess in urine. This is why some people say the US has the most expensive urine in the world. We spend so much on expensive vitamin supplements and much of it just ends up in the toilet. B12 and Folate are exceptions as they are water soluble vitamins that are stored in the liver. Excreting excess in the urine and not storing much in the body means that water soluble vitamins need to be consumed daily and that deficiencies can become apparent quickly. Also toxicity of water soluble vitamins is much less common than toxicity for fat soluble vitamins, because excess water soluble vitamins don't easily build up in the body. Patients with renal damage may reach toxicity of water soluble vitamins at much lower doses as their excretion of excess water soluble vitamins in impaired * Usually on Step 1 they will give you the vitamin letter/number and the name in parenthesis. They wouldn't usually give you one without the other, so perfectly memorizing both is not necessary. I would just learn the letter/number as that is easier. In certain studies, Americans has been found to be the #1 population in vitamin D deficiency. Some of the main symptoms of vitamin D deficiency are a weakened immune system, candida or bacterial overgrowth, emotional ups and downs, hormone imbalance, trouble putting on muscle or losing weight, weakened bones or teeth, cancer, heart disease, or diabetes. Vitamin D supports having a healthy immune response, digestive health, hormone balance, and more. The biggest causes of vitamin D deficiency are not getting enough sunlight, certain chemicals from plastic bottles (bpa), and toxicity. Try to get outside everyday for about 20-30 minutes of natural sunlight. Certain foods that are sources of vitamin D are wild caught fish, raw fermented milk, mushrooms, and eggs. However, there are small amounts in the food you eat. For a good supplement, I would recommend a high quality vitamin D supplement. You don’t want to take vitamin D2, but you do want to take vitamin D3. For supplementation, take about 2000-5000 IUs a day. However, if you have a severe deficiency, then for 1-2 months, you may take 5000 IUs twice daily. Lastly, you want to take your vitamin D3 with some form of fat like coconut oil or avocado to help absorption in your body. 9 Thyroid supplements Every Hypothyroid Patient Should Consider Using

Thyroid supplements (if used correctly) can help boost your thyroid function, help with weight loss and help you feel better. The problem comes with knowing WHICH supplements to take and at WHAT dose. It takes over 13 nutrients to create thyroid hormone, convert T4 to T3 and sensitize the cellular receptors to T3. Many thyroid supplements cram all of these nutrients into a supplement and call it "good". This is the WRONG approach to take. Instead basing your supplementation off of the needs of your body is far superior. That means using lab work and other markers to find what nutrients you need in what doses. 9 Thyroid supplements that I find most hypothyroid patients are deficient in: 1. Vitamin B12 - Thyroid hormone is involved in the proper absorption of thyroid hormone. See the video for a complete list of symptoms. Repleting low vitamin B12 is best done through sublingual or intramsucular injections. 2. Adrenal support - Most thyroid patients also have issues with cortisol dysregulation. If you have advanced adrenal fatigue most patients do better on adrenal glandulars. For more mild disease consider using adrenal adaptogens. If your cortisol is elevated consider using phosphatidylserine. 3. Zinc - Low levels of zinc can cause issues with your skin and issue with hair growth. Adding zinc boosts immune function, T4 to T3 conversion and acts as an anti inflammatory. Make sure to supplement with zinc bound to picolinic acid. 4. Iron - This is what I consider a "goldilocks" nutrient in that you don't want TOO much or TOO little. Unfortunately most doctors look for iron deficiency anemia and don't necessarily jus treat iron deficiency unless anemia accompanies it. Use these optimal levels to supplement with iron. 5. Magnesium - Hypothyroidism alters magnesium and low levels of magnesium can lead to issues like depression, anxiety and other changes like muscle cramps or restless leg syndrome. Replace low levels with magnesium glycinate, magnesium citrate if you have constipation or magnsium threonate if you have neurological issues. 6. Selenium - This nutrient is involved in the thyroid conversion process. Not only that but supplementing has been shown to reduce TPO antibodies and boost the immune system. In addition using iodine with selenium can help reduce negative side effects. 7. Probiotics - Up to 20% of thyroid conversion happens in the gut and inflammation can reduce this number. Hypothyroidism sets up the gut for imbalances that lead to inflammation. Using probiotics can help improve your GI function. 8. Digestive enzymes - Hypothyroidism leads to low stomach acid which can predispose patients to malabsorption of certain nutrients. Using enzymes can help digest your food, increase nutrient absorption and if taken in between meals may promote the breakdown of immune complexes. 9. Iodine - If used correctly iodine can be helpful in boosting thyroid function. The thyroid hormone is made up of iodine moieties and low levels of iodine can reduce the amount of functioning thyroid hormone in the body. Therefore, deficiencies affect vision and cause Night Blindness. Vitamin A also acts as a growth factor that causes the differentiation of cells. Therefore, vitamin A can be used to treat certain types of AML as it causes differentiation of immature leukemic cells. This effect of differentiation of Vitamin A also means deficiencies can affect the immune response. Vitamin A supplements for deficient patients can be an effective treatment for disease like Measles. Deficiencies can result from fat malabsorption or inadequate intake of foods high in vitamin A (Liver, fruits, & vegetables). Vitamin A is also a teratogen that can cause numerous problems for an unborn fetus. This makes sense as prenatal development is a period where cell differentiate needs to be highly orchestrated. Too much vitamin A could interrupt this delicate process due to its effects on cell differentiation. Vitamin A should not be used as a treatment for women who may be pregnant. Vitamin A toxicity can result in a wide array of non-specific symptoms like nausea, irritability, headaches, visual problems, and changes to skin or hair. Most other vitamin deficiencies or toxicities have a more specific classic presentation. Therefore, I usually just rule out the other answer choices and select Vitamin A toxicity if nothing else fits. Vitamin D [1,25-(OH)2D3] is synthesized in the body as a result of sun exposure or ingested as part of cow's milk (breast milk is actually low in vitamin D). The inactive precursor to Vitamin D undergoes enzymatic processing in the liver and the kidney to become 1,25-(OH)2D3. This active form of Vitamin D primarily increases the absorption of calcium and phosphate from the diet. Therefore, deficiencies of Vitamin D lead to decreased bone mineralization. Vitamin D deficiency in children leads to Rickets and in adults it leads to Osteomalacia. Vitamin K is responsible for the modification of numerous proteins required for coagulation. Therefore, deficiencies of Vitamin K lead to deficiencies of Coagulation Factors II, VII, IX & X as well as deficiencies in Protein C & S. This can lead to an increased PT, increased PTT, bruising (purpura, ecchymosis, perifollicular hemorrhages) and other problems related to clotting. Vitamin K is taken in through the diet & synthesized by normal gut flora. Both of these routes lead to an inactive form of Vitamin K that must be activated by liver Epoxide Reductase. Coumadin (AKA Warfarin) inhibits Epoxide Reductase to intentionally create a Vitamin K deficiency and inhibit clotting. Patients with liver failure may have difficulty activating their vitamin K also. Patients who are on wide spectrum antibiotics may have a deficiency if Vitamin K because antibiotics can kill off the normal gut flora which produce Vitamin K. Newborns also often have Vitamin K deficiency because they have not yet been colonized with the normal flora that create Vitamin K and breast milk is low in Vitamin K. This is referred to as Hemorrhagic disease of the Newborn and is why newborns are usually given a shot of Vitamin K soon after birth. Your body uses all of these for a range of metabolic processes, from digestion to proper nerve function. There are two types: water-soluble and fat-soluble. Water-soluble vitamins dissolve in water and then are easily absorbed by the gut. The body uses what it needs and then the kidneys flush out the excess. This is why you need a constant supply of these vitamins from your diet. Fat-soluble vitamins are retained in the body and therefore can be used later, and so don't need constant replenishing. This is because fat-soluble vitamins are easily stored in the liver and fatty tissues. If you eat a nutritious, well-balanced diet, you normally can get all the vitamins you need. Your body can also make two of them...Vitamins D and K. If someone isn't able to get enough of a certain vitamin from food, a dietary supplement or multivitamin might be recommended. Vegetarians, for example, often need to take extra B12. There are also certain groups with increased nutritional needs. For example, women with heavy periods need more iron. Women who are pregnant or are trying to get pregnant also benefit from taking folic acid and other prenatal vitamins. For a long time, vitamin deficiency diseases, like scurvy and rickets, were common. Then when nutritious food became more plentiful, research shifted to investigating whether large quantities of vitamins might be able to fend off certain diseases. But just because someone gets a definitive disease due to a serious lack of a particular vitamin doesn't mean that consuming more than the Recommended Daily Allowance can prevent a range of diseases. Remember when megadoses of Vitamin C were thought to be a cure-all for everything from colds to cancer? Then Vitamin E was in the spotlight because of a possible link to Alzheimer's and heart disease. Even the B's had their moment of fame. And now it's D. But follow-up studies haven't shown any conclusive preventive benefit to megadosing. And in some cases, taking too much can be harmful. But that shouldn't be so surprising. Our bodies are complex....and so it makes sense that the nutrients needed interact in really complicated ways. We also now understand that just because someone swallows a vitamin pill, that doesn't mean it's bioavailable, meaning it's in a form our body can easily use. Real foods have many components, and it's both the combination of various foods and of the components within them that helps our bodies absorb what we need in the quantities we need. Cut them in half and remove their yolks 13. Using a spoon, fill the egg whites with the prepared chicken mixture 14. A thiamine deficiency can cause weakness, chronic fatigue, heart complications, psychosis, and nerve damage. Thiamine can be found in many commonly eaten foods including yeasts, certain whole grains, beans,nuts, and meat. Additionally it is included in many vitamin B complex supplement products. Vitamin B1/Thiamine Deficiency Symptoms The clinical signs of a thiamine deficiency include: • Anorexia or rapid weight loss • Poor appetite • Colitis • Ongoing digestive problems such as diarrhea • Nerve damage • Nerve inflammation (neuritis) • Fatigue • Decrease in short-term memory • Confusion • Irritability • Muscle weakness • Mental changes such as apathy or depression • Cardiovascular effects such as an enlarged heart A thiamine deficiency is not very common in western, developed nations.
