A little girl fighting for her life now faces another battle: health insurance. Alexis Shapiro, a 12-year-old girl from Texas, had a brain tumor removed. It resulted in a rare disorder that caused her to gain massive amounts of weight, as much as two pounds a week. Shapiro's body thinks it's starving, so no diet or exercise has helped. Now she has diabetes, and doctors say gastric bypass weight-loss surgery is the only thing that can help Alexis. The U.S. military, which provides her family's health insurance, says it won't pay for the operation because Alexis is too young. TRICARE and Humana Military, which provide family insurance for Alexis' father, Air Force veteran Ian Shapiro, denied the request citing rules that say gastric bypass surgery may be covered, but only if the patient is 18 or has achieved full bone growth. Officials said the family could appeal the decision.

Medical plans differ in how much you pay out of your paycheck versus how much you pay at the doctor and how much freedom you have in choosing a doctor. Different types of medical plans vary in their monthly costs as well as their requirements for choosing specific doctors. An HMO, or Health Maintenance Organization, has a lower monthly premiums, but also provides a limited amount of doctors to choose from. You must designate a Primary Care Physician and receive a referral to see any other doctors or specialists. A PPO, or Preferred Provider Organization, has a higher monthly premium, but allows you greater freedom to choose your medical care provider. PPOs don’t require a primary care physician, so you can go directly to a specialist. An HDHP, or High Deductible Health Plan, offers the least expensive monthly premium, but requires you to pay all medical expenses up to a certain deductible amount. Like a PPO, HDHPs do not require a primary care physician. HDHPs are often attached to a Health Savings Account that allows you to set aside pre-tax dollars for out-of-pocket health expenses, which helps to cover the cost of a high deductible.

http://www.jsdowney.com (Version 1.0) UNDERSTANDING HEALTH REIMBURSEMENT ACCOUNTS (HRA) A Health Reimbursement Account (HRA) allows an employer to set aside funds to pay for medical expenses on a tax-free basis for participating employees. The Employer opens and owns this type of account. An HRA is an employer-funded plan. The contribution limit is determined by the employer’s plan design. It’s possible for the unused funds to carry over to the next year if written in the employer’s plan design. Funds from this account reimburse employees for out-of-pocket qualified medical expenses. This has no tax advantage, can’t earn interest, won’t reduce health insurance premiums, and can’t be used for retirement income. However, it can reduce the out-of-pocket health care expenses of the account holder. H.R.A. Summary: Health Reimbursement Accounts (HRAs) are employer funded group health plans from which employees are reimbursed tax-free for qualified medical expenses up to a fixed dollar amount per year. Unused amounts may be rolled over to be used in subsequent years. The employer funds and owns the account. Health Reimbursement Accounts are sometimes called Health Reimbursement Arrangements. Money in an HRA can only be used to pay for eligible medical expenses incurred by employees and their dependents enrolled in an HRA. An employer may limit what expenses are eligible under an HRA plan.
