How is bladder cancer treated?
The treatments for bladder cancer depend on the stage of bladder cancer. Briefly, I break it down into two different stages. The first is what we call ""superficial bladder cancer""--a small growth in the lining of the bladder that hasn't grown into the muscle layer of the bladder. Usually this is treated--first we have to diagnose the problem, so we usually look into the bladder with a cystoscope, and we find out if there are bladder tumors .If there are bladder tumors, we either biopsy them or scrape them using a transurethral recession, done with a telescope through the urethra. We scrape the tumor to get staging--how deep it's gone into the muscle. It's also curative for those people who have superficial cancer. That one procedure, if it is confined, will be the treatment. Many patients, when they present, will have more than one tumor or a poorly differentiated tumor. Some of those will require some treatments like therapies in the bladder, similar to chemotherapy, but not really chemotherapy--it's called immunotherapy. We place a small catheter in the bladder and still fluid into the bladder that causes inflammatory changes. The body's response to that is to attack the lining of the bladder and wipe out all residual cancer cells. We usually do that once a week for about six weeks, and it is dependent on what type of cancer patients have. It doesn't make people sick, like traditional chemotherapy. It may make them have worsening urinary frequency, or some burning with urination. The other type of bladder tumors that grow into the muscle wall behave much more like what people consider a real cancer. These are aggressive tumors, they can spread to other areas of the body, and they cause rapid progression into the muscle or blocking the ureter tubes or spreading. Most times, we recommend as the gold therapy for treatment of bladder cancer is to take the bladder out. Sometimes patients can receive a combination of chemotherapy and radiation therapy in an attempt to preserve the bladder. This has a slightly higher incidence of recurrence, and it if does fail, we recommend taking the bladder out in addition. When you take the bladder out, you have to find a new way for urine to get to the outside of the body. We can either do that by making a small conduit that comes to the skin as a stoma, or we can create what's called a neobladder out of portions of the bowel, and hook it up to the urethra, so you can void almost normally.

After culturing them in the laboratory for a stipulated time, the organisms are weakened to a certain extent. When introduced in the body, they do not cause tuberculosis, rather BCG elicits the body's immune system to restore the natural defense mechanism of the body. While following this treatment, the doctor prescribes preparatory steps to every patient. In order to get optimal effectiveness, BCG should be given in an empty bladder. So, one should not drink anything for 4 hours before the treatment. In the session, a urinary catheter is used to deliver BCG directly into the bladder. Another consideration is to keep the medication in direct contact with the bladder walls for at least 2 hours. Hence, urination is not allowed for two hours after the treatment to get better results. After administering BCG, the patient should reduce the intake of fluids for the next few hours. He/she will be asked to lie down and change their position after every 15-20 minutes. This helps in washing the bladder walls with the medicinal solution. For the first 6 weeks, BCG is administered once per week. Following this, the treatment is done at an interval of 3-6 months for the next 2 years. During this duration, the patient is closely monitored by the doctor. The bladder is a hollow organ in the lower abdomen (pelvis). It collects and stores urine produced by the kidneys. •The bladder is connected to the kidneys by a tube from each kidney called a ureter. •When the bladder reaches its capacity of urine, the bladder wall contracts, although adults have voluntary control over the timing of this contraction. At the same time, a urinary control muscle (sphincter) in the urethra relaxes. The urine is then expelled from the bladder. •The urine flows through a narrow tube called the urethra and leaves the body. This process is called urination, or micturition. Cancer occurs when normal cells undergo a degenerative, dangerous, or what is called a malignant change or transformation causing them to grow abnormally and multiply without normal controls. A mass of cancerous cells is called a malignant tumor or cancer. The cancerous cells are capable of spreading to other areas of the body through the process of metastasis. A cancer can become destructive locally to the tissues adjacent to where it arises. Cancer cells can also metastasize. Metastasis means that cells spread through the tissue fluid's circulation called the lymphatic system or through the bloodstream where they can then stop off in other tissues or organs where they may grow as metastases or metastatic deposits and can become destructive in these new locations. The term cancer is further described by the tissue in which it has arisen. For example: bladder cancer is a different disease than lung cancer. If a bladder cancer cell metastasizes -- that is, spreads to the lungs through the bloodstream it is still called, and is treated as metastatic bladder cancer, not as lung cancer. Cells which transform in a less dangerous fashion may still multiply and form masses or tumors. These are called benign tumors. They do not metastasize. Of the different types of cells that form the bladder, the cells lining the inside of the bladder wall are those most likely to develop cancer. Any of three different cell types can become cancerous. The resulting cancers are named after the cell types. •Urothelial carcinoma (transitional cell carcinoma): This is by far the most common type of bladder cancer in the United States. The so-called transitional cells are normal cells that form the innermost lining of the bladder wall, the urothelium. In transitional cell carcinoma, these normal lining cells undergo changes that lead to the uncontrolled cell growth characteristic of cancer. •Squamous cell carcinoma: These cancers are comprised of cells that typically form as a result of bladder inflammation or irritation that has taken place for many months or years. These cells grow in flat masses of interconnected cells. •Adenocarcinoma: These cancers form from cells that make up glands. Glands are specialized structures that produce and release fluids such as mucus. •In the United States, urothelial carcinomas account for more than 90% of all bladder cancers. Squamous cell carcinomas make up 3%-8%, and adenocarcinomas make up 1%-2%. •Only transitional cells normally line the rest of the urinary tract. The kidney's internal collecting system, the ureters (narrow tubes that carry urine from the kidneys to the bladder), the bladder, and the urethra are lined with these cells. Treating Early Stage Bladder Cancer
For George Furlan, blood in the urine turned into a worse case scenario. "Within 10 days he found some cancer and a tumor in my bladder," says George Furlan,. There was a silver lining to his bladder cancer. It was caught at an early stage when it's most treatable, says Furlan's doctor Alejandro Miranda-Sousa, urologist on Lee Memorial Health System's medical staff. "Correct. Usually the low- grade cancer, the scrapping or removal of the tumor is done with no cuts all. Sometimes medication is given within the bladder; a form of chemotherapy or immunotherapy could be given to treat and prevent recurrences," says Dr. Miranda-Sousa. Low- grade tumors, like Furlan's, can present like cauliflower with roots that grow into the bladder lining. In this instance, treatment included both the removal and chemotherapy. "He said to me you're doing fine but I have to cut one of the tumor and I left some of the roots which did not penetrate the wall yet. I want to take care of that by giving you my treatment for 6 weeks," says Furlan. Bladder cancer tends to recur; close to 50% of patients will battle it again. "The follow up is to check the bladder every 3 months for the first 2 years after the tumor has been discovered. Many tumors come back as low a grade. Those are much more simple to treat and have a high rate of curing," says Dr. Alejandro Mirando-Sousa. When the cancer is caught early, the outlook is good. If it's diagnosed in situ, meaning the layer of cells it where it began, patients have a 97% 5-year survival rate. If it's stage 1- the rate is 70%. Patients with stage IV bladder cancer are rarely cured, making it important to check out any symptoms. "Blood in the urine is most common, but obviously urgency, frequency, going to the bathroom very often at night, changes in the flow of urination. The most important thing I recommend to patients is periodically visit your doctor," says Dr. Miranda-Sousa. The vigilance paid off for Furlan. "Everything was fine, the tumor completely disappear," says Furlan. They did a biopsy and still found cancer, so I repeated again for six weeks, repeated the test at the end, and they still found cancer, only now the cancer they found was more serious. It couldn't be treated by BCG. I went to another neurologist. The first the he did was send me to a CAT scan. In the CAT scan, we saw in the ball at the bottom, it had a huge tumor. So he told me that the only thing that they could do would be to remove my bladder, and I would wear a bag on my side for the rest of my life. He also offered me chemotherapy, but I've had friends who've gone through chemotherapy and died. He offered me radiation, and I've had friends who had to have treatment for the burns from the radiation. And then he told me that the surgery, was only a 50/50 chance of coming out of it without cancer. I asked him how long I would have to live if I did nothing, and he said two years. After I thought about it for a few days, I called him back and said, I think I'll take the last alternative, because I'm more concerned with the quality of my life than the length of my life." Just by chance, a friend of mine that I've known for 35 years, who knew that I was being treated for cancer, and was just calling me to find out how it was going, so I told him what I just told he. He said, 'I'm going to make a call, and I'll call you right back.' That's all he said to me. So he called me back in maybe half an hour. He gave me the name of a doctor, and a telephone number. He said, 'I talked to this doctor, he's a friend of mine, I've known him almost as long as I've known you. I want you to call him, he's expecting your call tomorrow at 10:00. His name is Wolfstein.' So I called him, and I told him everything I just told you. And he said, four words to me. I'll never forget it. He said: "I can help you." He didn't say he was going to take out my bladder. He didn't say he was going to give me any horrible radiation or chemotherapy. He told me all about what you guys do here. He told me about the hyperthermia, he told me about the IMRT, he told me none of it would kill me. And that he felt that there was a very, very good chance that I would be out of here, cured, minimum three months, maximum, six. Bladder Cancer | John's Story
During a visit to his primary care doctor, John Squire was told that there was blood in his urine and that he should see a urologist. John saw a urologist close to home and was told he had bladder cancer. After receiving this diagnosis, John and his girlfriend Jean were determined to find the best care and found Dr. Trinity Bivalacqua at the Johns Hopkins Brady Urological Institute. Dr. Bivalacqua listened to John and Jean's lifestyle and concerns and reassured them that there were options available including blue light cyctoscopy for detection and diagnosis as well as BCG treatments. Three months later when John returned, Dr. Bivalacqua had found the cancer had spread to the muscle tissues in the bladder and John would have to have surgery for the removal of his bladder. A multidisciplinary team, from the Brady Urological Institute and The Kimmel Cancer Center, removed John's bladder and provided a care plan. After all, it's far more common among men than women, and the majority of cases affect patients over age 65. But don't let those stats keep you from learning to spot the symptoms. "Many people mistakenly think bladder cancer is only a disease of older men," says Arjun Balar, MD, an oncologist at NYU Langone Medical Center. "But there are more than 18,000 women who are diagnosed with this cancer every year in the United States." And because women may not be on the lookout for early bladder cancer symptoms, the Bladder Cancer Advocacy Network reports that women are more likely to be diagnosed with bladder cancer at an advanced stage. Knowing the symptoms can help you get diagnosed sooner, which can improve your prognosis. Here are a few warning signs to watch for. 1. Blood in your urine. This is the most common early symptom of bladder cancer, and it's an easy one for women to overlook especially because it's typically painless and you can go weeks or even months between occurrences. "Many women ignore this symptom because they connect it with menstruation or menopause," Balar says (in fact, take a look at these 6 symptoms that seem like menopause but may be something scarier). "Blood in the urine doesn't always mean you have bladder cancer, but it's important to discuss with your doctor." 2. UTI-like symptoms. Bladder cancer can often be mistaken for a urinary tract infection, because many of the symptoms overlap. "Patients may experience increased frequency of urination, urgency to urinate, pain with urination, or urinary incontinence," says Susan Constantino, MD, an oncologist with UF Health Cancer Center at Orlando Health. If you've noticed any pee-related problems you have to go all the time, or you feel like you have to go but can't, or you have a hard time emptying your bladder or if antibiotics don't seem to be helping your UTI symptoms, talk with your doctor. 3. Unexplained pain. "Pain is often associated with more advanced bladder cancers," Constantino says. "The pain can be in the flank area, abdomen, or pelvis. Patients can also develop pain in their bones if the cancer has spread to their bones." If you're having aches and pains in those areas, tell your doctor especially if you've also had the aforementioned spotting or UTI symptoms. 4. Decreased appetite Appetite loss is a common cancer symptom, and bladder cancer is no exception. If the cancer has grown or spread, Balar says, "You might have weight loss or feel tired and weak." Of course, there are plenty of other things that can mess with your appetite, so don't automatically assume the worst—but do talk with your doctor about it if it persists. 5. Smoking. OK, so this isn't technically a symptom but according to the National Institutes of Health, about 50% of women diagnosed with bladder cancer are smokers. "Smoking is the biggest risk factor for bladder cancer," Constantino says. If you notice any of the above symptoms and you smoke, let your doctor know ASAP. Because it's easy to chalk up the symptoms to a stubborn UTI or normal spotting, Balar says, "Bladder cancer may be overlooked in women, and they are not diagnosed until their cancer has spread and it's harder to treat." So if you're worried, don't just write off your symptoms. How is Bladder Cancer treated What is TURBT?- Dr. Santosh Bethur
Bladder cancer depends on the stage of the bladder cancer. If the patient comes with a small tumour in the bladder, the bladder cancer can be treated by endoscopy. By endoscopy, we mean we pass a small endoscope, we pass a small metallic tube inside the bladder through the urethra or the penis in the male patients, we go inside the bladder and with the help of a specialised instrument, we remove the tumor, if it is a low stage tumour or if it is an early stage tumor. This removal of tumour is sufficient to treat the bladder cancer. This procedure is called as TURBT or Trans Urethral Resection of Bladder Tumor. Once we remove this tumor, we send it for pathological examination for the staging of this tumor. The pathological staging depends on the amount of cancer that has penetrated into the bladder wall. It varies from quite superficial to quite deep. If it is a very superficial tumor, then the treatment by just endoscopic resection, it is quite sufficient. If it is slightly advanced then there is something called as intravescicle BCG, which is basically similar to chemotherapy, but it is basically called as immunotherapy. What we do is the BCG, which is given for the children for a vaccination, similar thing is available for bladder cancer. It is a live BCG organism, which is put inside the bladder, which will stimulate the bladder immunity to fight against the bladder cancer. If it is further advanced, then the entire balder has to be removed. If it is too such advanced, like the cancer has already spread to the other organs, like the bones, lungs, then at that time, we can only do something called as palliative treatment, Palliative treatment means we can only do something for the symptoms of the disease we cannot cure the disease at that stage.
