Patients with early stage of Knee Arthritis come to me at the age of 35 to 40 years, they are mainly youngsters who are gone to the gym or to a trek and they come with knee pain. There are another group of youngsters who come with rheumatoid arthritis, a little bit younger may be 30 to 35 years. These patients often come with pain on the inner aspect of the knee, swelling on the knee, a little bit of stiffness. So these are the main symptoms of Knee Arthritis. The main treatment for this early Arthritis is, it can be depicted by the mnemonic RICE meaning rest, ice, compression and elevation. Rest means stop the patient from doing those activities for 2 to 3 weeks Which gives them comfort. Ice the part or knee for around 10 to 15 minutes duration, 3 to 5 times a day with the help of gel packs, give some sort of compression as in crape compression and some amount of elevation this along with some amount of anti inflammatory medication and Physiotherapy should help the patient get back to his normal activities. One should know that this early Arthritis is quite common among youngsters. It is seen between 35 to 40 years because that's the age when the degeneration of the knee starts or setting it. So after a trek, a hill climb or a marathon these sort of pains are quite common there is nothing to get worried about this phenomenon.

Causes of knee pain may be related to: •Sports •Trauma •Inflammation or arthritis •Osteoarthritis Cartilage is a tough and flexible connective tissue that gives shape, support and provides a cushion between bones in the joints. Unlike other connective tissue, cartilages do not have its own blood supply, instead, it is composed primarily of water, chondrocytes, collagen, and proteoglycans, which produce and maintain a structural matrix giving cartilage tissue its form and function. Proteoglycans are composed of numerous glycosaminoglycans attached to a core protein. The predominant glycosaminoglycans in cartilage are chondroitin sulfate and keratin sulfate. Within the cartilage matrix, there are numerous prototype and monomers attached by link protein to a molecule of hyaluronic acid. This is all woven with collagen to form an elastic and compressible structure. Normal articular cartilage also known as hyaline cartilage provides a smooth crystal clear gliding surface to aid in the motion of the joint. The structure of hyaline cartilage is divided into four layers; superficial, middle, deep and calcified layer with the calcified layer representing a transition from articular cartilage to bone. This cartilage is designed to achieve and maintain proper function over the majority of a person’s life. If the cartilage is subjected to excessive weight, overuse, improper alignment or injury, it begins to wear away leaving bone to rub on bone. Articular cartilage does not have the ability to heal itself, however, there are several treatment options available to aid in the repair of injured cartilage. The knee is the most common joint in the body with cartilage injury issues. The knee joint is composed of femur, tibia, and patella. There are two types of cartilage in the knee joint, the articular cartilage and the meniscus, which is a shock absorbing cartilage. There are also important ligaments within the knee joint, the anterior and the posterior cruciate ligaments. The goal of treating cartilage injury is to reduce pain and improve the function of the affected area. Anti-inflammatory medications are commonly used to combat-injured cartilage but they can potentially cause cardiovascular or gastrointestinal toxicity. Damaged cartilage can also be treated with a series of injections. Steroid injections such as cortisone are powerful anti-inflammatory drugs used in the treatment of osteoarthritis. Hyaluronic acid is a substance produced naturally in the body and is present within cartilage when isolated. Hyaluronic acid is a thick viscous solution that can be added to the natural hyaluronic acid of the knee joint. This solution uses purified Hyaluronic acid derived from either rooster combs or genetically engineered cells. Glucosamine is another option for treatment that can be injected directly into the joint. Glucosamine is a modified sugar that is formed by the human body and is used to form larger molecules involved in the formation and repair of cartilage. Other methods include gene therapy, platelet-rich plasma, growth factors, stem cells, drilling and debridement of cartilage may help with injured cartilage. Occasionally the cartilage is replaced, regrowth and implanted. When the cartilage is damaged, the choice is often simple as the patient will need a total knee replacement. Many different types of designs and materials are currently used in total knee replacement surgery. Nearly all consists of three components, a patellar component, which consists of durable plastic, a tibial component, made of a durable plastic cushion often held within a metal platform and the femoral component, which is usually made of highly polished metal. Total knee replacement is the most predictable option to treat severe arthritis of the knee joint. When the arthritis is minimal, almost all treatment methods will work, however, when the extent of arthritis is moderate, there are many different treatment options presented, but none of them are predictable. It is logical to pursue and recognize the efficacy of platelets and stem cells in cartilage regeneration, simply because it is better for the patient to be able to regenerate their own cartilages and for it to be replaced with an artificial joint.

Knee Arthritis Stretches & Exercises - Ask Doctor Jo

These knee arthritis stretches and exercises are simple to do and should provide some relief from knee arthritis pain. They are also great for keeping your knees nice and fit. Read Doctor Jo's full blog post about this at http://www.askdoctorjo.com/content/knee-arthritis-stretches-exercises The first stretch is for your calf muscles. Start off with your legs out in front of you. Keep the leg you want to stretch out in front of you. Take a stretch strap, dog leash, belt, or towel and wrap it around the ball of your foot. Relax your foot, and pull the strap towards you stretching your calf muscle. You should feel the stretch under your leg. Hold the stretch for 30 seconds, and do three of them. The next stretch is for the hamstrings. Keep your leg that you want to stretch out in front of you. Try to keep your back straight and just bend at your hips. Keep your leg straight and pull your toes towards you to get an extra stretch. Hold this stretch for 30 seconds, and repeat three times. The last stretch is for your quad muscles. This time you will lie on your stomach to stretch. Wrap the strap or dog leash around your ankle, and then lie on your stomach. Bring the strap up and around your shoulder so you are comfortable. Then pull the strap towards you bringing the lower leg towards your bottom. Hold this stretch for 30 seconds doing three of them. Now we will move on to the exercises. The first exercise is going to be a quad set. Put your leg straight out in front of you again. You can use a towel roll underneath your knee as a target. Activate your quad muscle by squeezing it and pull your toes toward you at the same time. This will push your knee into the ground. Try to hold this for 5-10 seconds and do 10 of them. Next you will do a short arc quad (SAQ). For this, you will use a foam roll or a rolled up beach towel. You want something big so you have a good bend in your knee. Prop your leg over the roll with your knee on the roll. Try to keep your knee on the roll the whole time, and then lift your foot in the air until your leg is straight. If you pull your toes towards you at the end, this will help activate all the muscles in the leg. Try not to go fast because then you are only using momentum. Go slow and controlled with your movements. Start of with 10 of these. If you get to 20-25 and it is easy, then you can add ankle weights. For the next exercise, you will sit in a chair. This one is called a long arc quad (LAQ). This is similar to the SAQ where you want to go slow and controlled, and then bring your leg up straight out in front of you. Slowly come back down. Start of with 10 of these. If you get to 20-25 and it is easy, then you can add ankle weights. Now you are going to stand up for the last exercises. For the hamstring exercise standing up, you want to keep the top part of your leg as straight as possible in line with your body. It might try to come forward if your muscles are tight, but try to keep it down. Slowly bring your foot back behind you, like you are trying to kick your bottom. Make sure you are going slow and controlled with these. Start of with 10 of these. If you get to 20-25 and it is easy, then you can add ankle weights. The last one is for calf strengthening. With your feet about shoulder width apart, and holding onto something for balance if you need to, come up on your toes bringing your heels off the ground, and slowly come back down. If that becomes easy, you can do this one foot at a time. Start off with ten and work your way up to 20-25. DR Nabil Ebraheim UTMC Toledo. The cartilage of the knee is complex and it is made of elastic comprehensive structure. The normal articular cartilage is called hyaline cartilage; it provides a smooth, gliding surface to help the motion of the joint. There is about 2cc of the synovial fluid inside the knee that helps in the motion and lubrication of the joint. Between the hyaline cartilage which is called the articular cartilage you can see the meniscus, the lateral and the medial, the meniscus is a shock absorbing cartilage or cushion between the articular cartilages. The hyaline cartilage has 4 layers: 1- The superficial layer. 2- The middle layer. 3- The deep layer. 4- The calcified layer After the calcified layer you find the bone. These cartilage cells are supposed to live forever. Good cartilage cells are sterile; they can’t make more cartilage if these cartilage cells are destroyed. If the cartilage is subjected to excessive wear, trauma, injury, overuse, excessive weight or improper alignment, then the cartilage will wear away leaving the bone to rub against bone. The cartilage doesn’t have the ability to heal itself by hyaline cartilage, but sometimes it can heal itself by inferior type of cartilage called fibrocartilage, especially if the area that needs to be repaired is small. What is the treatment of the arthritis of the knee? 1- Medication: people respond differently to medications, the doctor select the type, dosage and duration of the treatment, the medication has to be safe and effective. Ask the doctor about the side effect of the medication. There are several medications for example: NSAIDS. 2- Losing Weight: it’s advisable to lose about 5% of the body weight, especially if the BMI was above 25. 3- Low Impact Physical Therapy: such activities like swimming or cycling, adds less stress on the knees, life style modification to protect the knee will slow the progress of the arthritis, there is a strong evidence that physical therapy will help the patient. The physiotherapy will decrease the pain, will improve the function, increase the strength, the range of motion; physiotherapy should be individualized with program that meets the patient’s needs, the life style, and expectations. 4- The physician will also use intra-articular injection of steroids; viscosupplementation, or hyaluronic acid, injection will relieve the patient’s pain and disability. 5- Other injection methods: - Gene therapy - PRP - Growth factor - Stem cells 6- Other treatment options: - Acupuncture - Massage - Glucosamine - Chondroitin sulfate - Valgus directing brace - Wedges in the foot - Arthroscopic debridement and lavage 7- Assistance devices: - Cane - Shock absorbing shoe - Shoe inserts - Knee sleeves - Support brace 8- Arthroscopy shouldn’t be done in arthritis unless there is loose body or meniscal tear causing mechanical symptoms of recurring lacking, catching, swelling and pain. There are some guide lines from the American Academy of Orthopaedic Surgeon AAOS: these guidelines are suggestive, and the treatment of the arthritis patient should be individualized and based on: - The doctor’s clinical judgment. - The patient’s clinical situation. - The evidence tested and published. It is a combination of all of these factors. If it works for you, it may not work for your neighbor. So basically arthritis can be minimal, and anything can help the arthritis ion this situation. But if the arthritis is moderate then the physician has many options for treatment and none of those options are predictable in the result. When the arthritis is severe, total knee is the most predictable option for treatment of arthritis. You have to reduce the pain and improve the function of the knee. Osteoarthritis is the most common form of arthritis and is associated with the aging process. Osteoarthritis is a chronic disease causing the deterioration of the cartilage within a joint. For most people, the cause of osteoarthritis is unknown, but metabolic, genetic, chemical, and mechanical factors play a role in its development. Symptoms of osteoarthritis include loss of flexibility, limited movement, and pain and swelling within the joint. The condition results from injury to the cartilage, which normally absorbs stress and covers the bones, so they can move smoothly. The cartilage of the affected joint is roughened and becomes worn down. As the disease progresses, the cartilage becomes completely worn down and the bone rubs on bone. Bony spurs usually develop around the margins of the joint. Part of the pain results from these bone spurs, which can restrict the joint’s movement as well. Osteoarthritis (OA) is a type of joint disease that results from breakdown of joint cartilage and underlying bone. The most common symptoms are joint pain and stiffness. Initially, symptoms may occur only following exercise, but over time may become constant. Other symptoms may include joint swelling, decreased range of motion, and when the back is affected weakness or numbness of the arms and legs. The most commonly involved joints are those near the ends of the fingers, at the base of the thumb, neck, lower back, knee, and hips. Joints on one side of the body are often more affected than those on the other. Usually the symptoms come on over years. It can affect work and normal daily activities. Unlike other types of arthritis, only the joints are typically affected. Causes include previous joint injury, abnormal joint or limb development, and inherited factors. Risk is greater in those who are overweight, have one leg of a different length, and have jobs that result in high levels of joint stress. Osteoarthritis is believed to be caused by mechanical stress on the joint and low grade inflammatory processes. It develops as cartilage is lost and the underlying bone becomes affected. As pain may make it difficult to exercise, muscle loss may occur. Diagnosis is typically based on signs and symptoms, with medical imaging and other tests occasionally used to either support or rule out other problems. In contrast to rheumatoid arthritis, which is primarily an inflammatory condition, in osteoarthritis, the joints do not typically become hot or red. Treatment includes exercise, efforts to decrease joint stress, support groups, and pain medications. Efforts to decrease joint stress include resting and the use of a cane. Weight loss may help in those who are overweight. Pain medications may include paracetamol (acetaminophen) as well as NSAIDs such as naproxen or ibuprofen. Long-term opioid use is generally discouraged due to lack of information on benefits as well as risks of addiction and other side effects. If pain interferes with normal life despite other treatments, joint replacement surgery may help. An artificial joint typically lasts 10 to 15 years. Osteoarthritis is the most common form of arthritis with disease of the knee and hip affecting about 3.8% of people as of 2010. Among those over 60 years old, about 10% of males and 18% of females are affected. It is the cause of about 2% of years lived with disability. In Australia, about 1.9 million people are affected, and in the United States, 30 to 52.5 million people are affected. It becomes more common in both sexes as people become older. The main symptom is pain, causing loss of ability and often stiffness. "Pain" is generally described as a sharp ache or a burning sensation in the associated muscles and tendons, and is typically made worse by prolonged activity and relieved by rest. Stiffness is most common in the morning, and typically lasts less than thirty minutes after beginning daily activities, but may return after periods of inactivity. Osteoarthritis can cause a crackling noise (called "crepitus") when the affected joint is moved or touched and people may experience muscle spasms and contractions in the tendons. Occasionally, the joints may also be filled with fluid. Some people report increased pain associated with cold temperature, high humidity, and/or a drop in barometric pressure, but studies have had mixed results. Osteoarthritis commonly affects the hands, feet, spine, and the large weight-bearing joints, such as the hips and knees, although in theory, any joint in the body can be affected. As osteoarthritis progresses, the affected joints appear larger, are stiff, painful and may swell, but usually feel better with gentle use but worse with excessive or prolonged use, thus distinguishing it from rheumatoid arthritis. Osteoarthritis is the most common cause of a joint effusion of the knee. Sometimes called degenerative joint disease or degenerative arthritis, osteoarthritis (OA) is the most common chronic condition of the joints.

In addition, there are fluid sacs in the knee capsule to keep the joint lubricated. These bursae can become irritated or inflamed if injured, resulting in bursitis. There are many potential causes for knee joint pain: Osteoarthritis – a chronic condition that occurs when the cartilage between bones breaks down Rheumatoid arthritis – an autoimmune disease that affects joints, causing swelling Bursitis – inflammation of fluid sacs either in front or directly behind the kneecap Immuno-inflammatory conditions Injury/over-use Infection Tendonitis – inflammation of tendons Gout – a form of arthritis in which uric acid accumulates and crystals of it settle in a joint Fortunately for our knees, there are natural remedies to reduce inflammation and ease the pain. The following recipe is a knee joint pain relieving rub. It contains effective anti-inflammatories and analgesics. Let’s look at what’s in it. 1. Cayenne. Cayenne has been used by Native Americans for millennia for pain relief. Capsaicin is a phytochemical in this spicy pepper that relieves pain through a variety of mechanisms. First, capsaicin blocks chemical pain transmitter “substance P” from reaching the brain. Second, when applied topically, it penetrates the skin to defunctionalize fibers that respond to pain stimuli. Through the skin, capsaicin stops nerves fibers from feeling pain and transmitting it. The capsaicin in cayenne is also effective for inflammatory skin conditions like psoriasis and eczema. Cayenne works to relieve inflammation when taken internally as well. 2. Apple Cider Vinegar. Supremely beneficial for human health in a wide range of applications, ACV is a potent anti-inflammatory that can relieve joint stiffness. The acetic acid in ACV penetrates the skin and can help to break up acid crystals that can form in the joint. Acetic acid has also been found to provide effective relief for tendonitis and other joint inflammation. (2) Additionally, unfiltered ACV contains bone-supporting minerals calcium, magnesium, potassium, and phosphorus. 3. Olive Oil. Anti-inflammatory and emollient, olive oil is a wonderful carrier oil. 4. Ginger. A potent anti-inflammatory, the phytochemical gingerol inhibits excessive immune response that results in inflammation. A 2014 study of the daily topical application of ginger on joints of people with osteoarthritis (OA) resulted in pain relief within a week and continued for the 24-week duration of the study. Natural Rub Recipe for Bone, Knee and Joint Pain You can apply this paste on any joint to reduce inflammation and relieve pain. You may want to experiment with the recipe and mix half the amount of ACV with the olive oil; sometimes the rub can be quite strong and the olive oil will lessen heat and make the paste easier to apply. All of these ingredients will also reduce inflammation if you eat them. Please note that cayenne can stain fabrics, so take care around adjacent clothing. The pain usually is not located in one specific part of the knee but instead throughout the knee. You might have difficulty walking, climbing the stairs, squatting or doing your desired sports or exercise. X-ray findings include generalized articular cartilage damage or breakdown, bone spurs, and joint space narrowing. While an MRI can show damage to the articular cartilage, x-rays are often more helpful for determining the extent of a patient’s arthritis and often make MRI unnecessary. I am considering this, as I am a young athlete (29) who would like to keep being active, and hopefully avoid or postpone a knee replacement down the road. What are your thoughts on stem cell therapy for knee injuries? As always, thanks for taking the time to answer reader questions. Your site and information have been a great help to me over the last few years as I have gone through multiple meniscus repairs and a partial meniscectomy. Thanks Dr. Geier. Stem cells are treatments in which tissue is harvested from your body. Cells are grown in a lab from that tissue. These stem cells are then reimplanted or injected into your injured area, such as the knee, with the hope that they turn into mature tissue and heal that area. In theory, in the setting of osteoarthritis, the stem cells would mature into articular cartilage and heal areas of damage. While these treatments have been tried for years in other parts of the world, they have only recently been attempted in the United States. The challenge with knowing if they work is that we don’t have many quality studies looking at their effectiveness, namely randomized controlled trials comparing these treatments to standard treatments like physical therapy, anti-inflammatory medications and cortisone injections. A tremendous amount of research is underway now to determine how well they work. There are thought to be few complications of the treatments. They are expensive, and there is no guarantee they work. Insurance generally does not pay for them, so people often pay hundreds or thousands of dollars out of pocket. For active people who have tried the other non-surgical treatment options and do not want to modify their lifestyles or undergo joint replacement surgery, these treatments offer some hope. The goal of this video is to reduce stress on the knee and maximize pain-free function. This program targets thigh muscles, hamstrings, hip muscles and calf muscles to improve support and reduce stress on the knee, and is broken down into 3 phases: STRETCHING to reduce tension and irritation in the knee STRENGTHENING to improve stability and function CARDIOVASCULAR EXERCISE to increase endurance Please remember the following as you engage in this knee arthritis conditioning program: These are easy exercises for knee arthritis that can be performed at your home. Perform this knee arthritis workout program 3-4 times per week Start with 5-10 minutes of low impact exercise such as walking, walking in place or stationary bike to get muscles warm and reduce chance of irritation or injury. Perform all exercises on both legs – this will help prevent imbalance between the rehab and non-rehab knee. Pain-relief modalities are essential throughout this program to help limit discomfort and allow you to progress with the exercises. Please remember the following as you engage in early strengthening exercises for knee arthritis: Let pain be your guide; if you find a certain exercise too painful to complete, then reduce the intensity of the exercise or abandon it altogether Start with no weight and progress to light dumbbells or ankle weights to increase resistance Perform 1-2 sets for 12-15 repetitions each exercise When performing leg extension, keep resistance light to avoid irritation The following early strengthening exercises, which are demonstrated in this video, should be conducted during your knee arthritis conditioning program: Half squats Standing hamstring curls Calf raises Leg extensions Straight leg raises Prone hip extensions Hip abduction Hip adduction Advanced Strengthening & Conditioning Exercises The goal of these exercises is to advance muscle strength, coordination and endurance for long-term pain relief. Please remember the following as you engage in advanced strengthening and conditioning exercises for knee arthritis: Continue EARLY STRENGTHENING exercises. Today I am going to share with you simple way you could treat your knee, in case you’ve got an ligament injury, tendón tear or even meniscus tear. This home treatment is also good as a pain remedy for those who suffer from arthritis. Recently I’ve come across old homemade treatment for knee injuries which helped me quickly to restore my right knee. I love sports. I am practicing Brazilian Jiu-Jitsu and two weeks ago I got serios injury during the training . It was an collateral ligament injury. I thought it is over. I had a great pain and my knee was not stable enougth to support the weight of my body. This treatment is very simple it is just a circular massage on knee with olive oil. After 3 weeks doing this massage I got back a a strong knee without pain. I would like to tell you Little bit more about Extra Virgen Olive Oil. What Olive Oil will do for your knee? 1) Firstly it has great anti-inflamatory power. This means it will diminish the knee pain quick. The component in charge with this is called Oleocanthal. 2) Secondly : Olive Oil will stimulate your blood circulation. What do we need from your knee? We want to restore our knee function. So for this reason it is in our interest to stimulate the blood circulation. This will restore quicker your knee ligaments, tendons and meniscus. 3) The third reason why olive oil is so good for your knee : Olive oil is very old remedy for a lot of injuries. Mayo Clinic and the Mayo Center for Regenerative Medicine is studying biologically based non-surgical treatments for osteoarthritis. One such treatment is the harvesting of the patient's own stem cells from their bone marrow. "In our procedure we draw cellular rich bone marrow from both sides of the pelvis. We then filter the resulting product and concentrate the stem cells and their corresponding growth factors.
