DVA skin cancer management  There are many types of skin cancers including BCCs or basal cell cancers, SCC or squamous cell cancers as well as melanomas. Todays review is on Bowen disease or Intraepidermal Squamous Cell Carcinoma. This is a common non-invasive cancer of the skin affecting the epidermal layers of skin. IECs most frequently occur on sun-exposed areas and present as red, scaly and persistent patches. IECs may arise de-novo, or from lesions of solar keratosis. If left untreated, Bowen disease may progress to an invasive Squamous Cell Carcinoma. This is one of the most common skin cancers I see in DVA or Department of Defence patients in Australia, and is due to the high UV radiation we have in cities like Brisbane.  Clinical Features Bowen disease most frequently presents as a persistent red scaly patch that slow grows over time. The most commonly affect areas include the legs, arms, and face- areas of sun exposure. Bowen disease or IECs can also affect the lips, oral mucosa, or even genital areas. As Bowen Disease is asymptomatic there is often a delay in the diagnosis, as it may mimic conditions such as dermatitis, psoriasis, tinea, and sun-spots.  Associations Most cases of Bowen disease arise from sun-damaged skin, including areas of actinic keratosis. The most common associations include- UV radiation. This plays a major contributing factor in the development of IECs. Immunosuppression. Patients on certain immunosuppressive medications are more likely to develop IECs.  Arsenic exposure. This was a common cause of Bowen Disease in the past, as arsenic was frequently used in medicines during the 60s and 70s including Bells Asthma Mixture, sheep dip, and as pesticides. Arsenic induced IECs-Bowen disease is a rare event today, as occupational safety has increased. A time lag of 10-20 years is seen between exposure and cancer.  Human Papilloma virus infections. Other associations include genetic diseases, radiotherapy and skin lesions such as porokeratosis, and solar keratosis. Investigations Bowen’s Disease can be suspected clinically, however in the majority of cases, a biopsy should be performed to confirm the diagnosis.  Treatment  There are multiple ways to manage Bowens/IECs. Treatment modalities will depend on the location and size of the cancer, the number of lesions, the degree of functional impairment.  Each modality has both advantages and disadvantages.  Cryotherapy or liquid nitrogen  Liquid nitrogen can be an excellent method of treating IECs. Most cases will require a double –freeze cycle of cryotherapy. This treatment is not recommended for IECs below the knee, as poor healing may occur.  Creams  Imiquimod, or 5-fluorouracil are effective in the treatment of Bowen disease. Clinical studies have shown cure rates approaching 80-90%. Treatment appears to be most successful when these creams are applied for 6-16 weeks.   Curettage and Electrocautery When small patches of IECs are present they may be treated via curettage and cautery. A local anaesthetic is given prior to the procedure to prevent pain and discomfort.  Photodynamic therapy  Photodynamic therapy or PDT can be an excellent non-surgical method of treating skin cancers such as Bowen’s/ IEC’s. PDT uses  a photosensitive cream called 5-aminolevulinic acid or methyl aminolevulinate, that is applied to the skin cancer. The cancerous cells of Bowens/ IEC take up this chemical over a period of 3 hours. A low-level laser light then activates the chemical, and destroys the skin cancer cells. Patients will require two treatments spaced a week or two apart. DVA or the Department of Veteran Affairs in Brisbane, covers PDT for both sunspots on the face and also IECs on all areas. PDT can also be used for multiple or large IECs. The benefit of PDT is that it selectively targets neoplastic cells without affecting the surrounding epidermis. This results in improved cosmetic outcomes, and faster healing times.  Surgery  Surgery is the ideal method of treating IECs in certain areas including recurrent IEC or ones that dwell in hair follicles. Surgery has the highest cure rate for Bowens/ IECs. For the majority of patients, especially DVA patients in Australia, non surgical methods, including the use of photodynamic therapy, is the future. Dr Davin Lim, Dermatologist. Photodynamic Therapy – PDT Photodynamic therapy is a treatment for acne and skin conditions such as sun spots, actinic keratosis, solar keratosis and some forms of skin cancer called IEC or Bowen’s disease. This treatment can also treat superficial BCCs or basal cell cancers. How is Photodynamic Therapy / PDT performed? Photodynamic therapy is a non-surgical and well tolerated treatment that combines the use of a special photosensitizing agent (ALA). This chemical is activated with a specific wavelength and intensity of light. It is a two-part treatment. First, the drug is administered to the skin via cream. An incubation period (wait time) of 90-180 minutes is need for absorption of the cream. This is followed by exposure under a specific light source for several minutes. In the United States the most popular is blue light combined with Levulan. In Australia, we use Metvix and Aktalite or Healite using the red light spectrum. What areas can be covered with photodynamic therapy? PDT is known as a ‘field treatment’, and is applied to the affected area and surrounding tissue. Virtually any area can be treated with PDT, including the face, scalp, hands and chest. PDT in the blue and red light spectrum can also treat acne, zits, pimples and blackheads. Is PDT painful? Patients often experience slight burning and stinging during the procedure. Patient comfort levels are helped with the administration of panadol one hour before illumination. A cool spray of nitrogen during the procedure will also decrease the stinging. If necessary an anaesthetic cream such as EMLA can be used to alleviate any discomfort during the procedure. As a guideline, 95% of patients do not require an anaesthetic. For large areas such as the face or scalp, we recommend an over the counter pain killer such as Panedine to be taken after the procedure. Levuan ALA is less painful than Metvix PDT as blue light is more superficial. What can I expect after photodynamic therapy treatment? Due to the photosensitivity of the drug used, patients may feel sunburned and occasionally swollen after the treatment. This usually subsides within 1-3 days, depending on the extent of PDT and the severity of the treated condition. Peeling and redness usually occurs for 3-6 days after treatment. Patients will need to avoid sun exposure for 48 hours after the treatment. How long does photodynamic therapy take and how much time off work will I need? Preparation of the area for PDT typically takes 10 to 15 minutes. This may involve degreasing the skin or in severe cases a laser assisted micropeel. The cream is the placed on the target area for 1-3 hours. This ‘wait time’ allows adequate absorption of the ALA cream to the target areas. The second part of the treatment (activation of the cream with a specific light source) takes several minutes to complete.) How much time off work will depends on the site and extent of treatment. As a guide the downtime for full face PDT is approximately 3-5 days, which compares favorably with Efudix treatment (downtime of 3-4 weeks). How is PDT different form other treatments for skin cancer? With PDT there is: Less potential for scarring Less down time and quicker recovery (typically days, not weeks/months) High cure rates. How does PDT compare to other methods such as freezing and Efudex therapy? Sunspots or solar keratosis is a common pre-cancerous lesion seen in the Brisbane. Efudex needs to be used twice a day for up to 4 weeks. PDT in contrast to Efudex, is a one off treatment for Solar keratosis. Treatment is over within hours, and downtime shortened to 6-7 days. DVA or the Department of Veteran Affairs and PDT. IF you hold a White Card for skin or a Gold Card, PDT or photodynamic therapy is covered! Patients are not out of pocket, as this treatment is subsidized for the treatment of solar keratosis –sun damage on the face, as well as pre-cancer and skin cancer anywhere on the body. Dr Davin Lim works closely with DVA for legal reports on sun damage to help you obtain this treatment, if you are a suitable candidate. Though PDT is an excellent treatment, I do feel that Fraxel fractional laser resurfacing has now taken over as the best treatment for sun spots. This is technology for you- lets move with the times. This week I decided to try a chemical peel. It had been a few years (pre-cancer) since I had a peel. Illinois Cosmetic & Plastic Surgery does dermaplaning before the peel, so you don't have the peeling of the skin afterwards. THIS really appealed to me! Key Points *Consist of small pink areas of skin which are rough or scaly *Caused by chronic accumulative sun exposure *Considered a pre-cancerous condition Actinic keratosis, also known as solar keratosis, is a precancerous condition related to damage from chronic sun exposure. Affected persons develop small pink or red lesions which are rough or scaly, usually appearing on parts of the body which are not typically covered by clothing. In some cases, squamous cell carcinoma develops within these lesions, which is the rationale for their preventative removal. Any part of the body with consistent sun exposure, most commonly the face, scalp, ears, neck, arms and top of hands, may be affected . Actinic keratoses are usually 1 centimeter or less in diameter, generally slightly scaly (with occasional instances of lesions which become thick with scale, as with warts), and with a pink, red or brown color variation. Actinic keratoses most commonly affect fair-skinned persons, those who have trouble tanning (in other words, burn easily), those with light-colored (blue, green, hazel) eyes, and those with red or blond hair. Any person with excessive sun exposure, especially during their youth, is at greater risk. In addition, persons with weakened immune systems due to HIV, chemotherapy or organ transplant have a greater risk of developing actinic keratoses and skin cancers. Differential Diagnosis (Other conditions with similar appearance) Squamous cell carcinoma Bowen's disease (squamous cell carcinoma in situ, a very thin noninvasive type of squamous cell carcinoma) Basal cell carcinoma Seborrheic keratosis Wart Benign Lichenoid keratosis Diagnosis Key Points *Diagnosis based on appearance and location *Skin biopsy may be performed to rule out malignancy Actinic keratosis is generally diagnosed based on appearance of the affected area. Health care professionals may biopsy the affected area to exclude malignancy. *Prevention is the most important factor in combating the disorder *All occurrences should be treated, as the disorder has the potential to become malignant *Several treatments exist to destroy the pre-cancerous growths. Most commonly cryosurgery (destruction by freezing with liquid nitrogen), 5-fluorouracil topically, Imiquimod cream topically, Photodynamic therapy, and chemical peel are all options to treat AK's. *Self-examination is key in the timely recognitiont of actinic keratoses Prevention of actinic keratosis by moderating direct sun exposure is recommended. Prevention of sun exposure has been shown to reduce the occurrence of new lesions. Covering the skin with appropriate clothing during times of sun exposure, using sunscreen with a high Sun protection factor ("SPF") of 30 or more, seeking shade during prolonged outdoor activities (or planning outdoor activities during the morning or evening hours), and even wearing clothing with an UPF rating can assist in prevention. If an affected area begins to change quickly or bleed, attention from a medical professional is recommended. Several options are available to treat or destroy the affected tissue, including the use of creams (fluorouracil, diclofenac, Imiquimod), cryosurgery, curettage (scraping), dermabrasion, chemical peeling, or laser treatments. Photodynamic therapy has also been shown to help in affected persons.

A chemical peel is a simple procedure done in the office for a variety of skin concerns. After washing the face, a controlled amount of a chemical is carefully applied to the skin. Patients will experience a stinging, burning, or itching sensation. Often times a fan is used to help with this. After a short time the solution is neutralized or diluted with water, although some preparations do not require this step. After a few days, the skin begins to peel, removing the dead skin cells on the surface. Usually by one week, the new skin that re-grows is refreshed and rejuvenated, and you see improvements in terms of color, texture, and tone. During this period a list of aftercare instructions are provided to prevent unwanted effects such as scars and blisters. As we age, dead skin cells accumulate on the surface of the skin. When light hits such skin, it does not reflect, giving a dull appearance. Exfoliation with scrubs, at home solutions, and chemical peels removes this layer and provides a glow to the skin. It works great for a variety of skin conditions including acne, discoloration, enlarged pores, scars, and even some pre-cancerous lesions. When performed in a series at monthly intervals and combined with skin care products such as anti-oxidants, exfoliants, retinoids, bleaching agents, and sunscreens beautiful results can be obtained. How to heal faster from fractional laser resurfacing

This video shows my technique of accelerated healing following a Fraxel 1927 fractional laser treatment. Fractional lasers have reduced the down time compared to co2 and erbium laser, however this little secret will reduce your recovery time even further. Skin recovery following Fraxel is dependent on several factors including 1. Fraxel wavelength 2. Power and passes 3. Individual immune factors 4. The aim of fractional laser resurfacing 5. Location of fractional resurfacing 6. Methods of exfoliation and this technique Firstly- let us consider the wavelength of fractional lasers. Fractional lasers come in many wavelengths, including Scition HALO, Profractional , Pixel, Portrait, Starlux platforms, as well as Fraxel laser. Fraxel has 2 types of wavelengths- the 1927, the 1550 ( Fraxel Restore) and the most powerful of them all the CO2 or Fraxel Re:Pair. In order of recovery, Fraxel 1927 and 1550 is the quickest, ranging from 3 days to 7 days. This is because this form for laser skin rejuvenation is called non-ablative fractional laser resurfacing. CO2 Fraxel laser takes the longest to recover, namely 4- 9 days, with redness persisting for up to a few months if higher fractional densities are set. Wavelengths such as 1550 and CO2 laser are ideally suited for the treatment of acne scars, and deep wrinkles. 1927 Fraxel is best for pigmentation, fine wrinkles and sun damage known as solar keratosis. High density 1927 Fraxel Fractional laser resurfacing still takes 7 days to recover, but with this method, make up can be worn as early as 4-5 days. Secondly- power and passes. Power can be tailored according to the skin type of the patient. For example- darker skin patients, such as Asian skin types will require less power and passes to treat their acne scars- this is purely based upon the safety profile. Conversely they may require more Fractional Laser treatments to achieve the end goal. The less power and the less density (ie. Less closely the laser beams are fractionated) the quicker the healing times. Thirdly- individual factors such as smoking, and general health can play a role in skin recovery following Fraxel laser, or any other skin laser resurfacing procedure. Make sure you are in top health before skin laser treatments. Fourth- and probably the most important point is the aim of fractional Fraxel skin resurfacing itself! Conditions that lie deep in your skin will take the longest to heal- namely acne scars, and collagen remodelling. Using the Fraxel CO2 for these conditions may entail a recovery time of up to 9 days or longer. Superficial pigmentation such as freckles, sun spots, liver spots and age spots can be treated with the Fraxel 1927 (or Pixel – Portrait fractional lasers), and will only take 3-7 days to heal. Acne scars can also be remodelled by using the Fraxel 1550 non ablative fractional resurfacing system- healing times are 3-7 days, however in my opinion CO2 laser is much better, and INFINI Microneedling Insulated Radiofrequency is even better still (see my other reviews). Fifth- location, location, location. Fraxel on the face heals up the quickest, whilst Fraxel performed for sun damage on the neck and chest can take an extra week to recover. Fractional laser resurfacing on the hands can take up to 2 weeks to completely recover. In most cases, you will require an extra laser session or two on extra facial areas to achieve the desired rejuvenation outcome. The Fraxel family of fractional laser resurfacing represents the most well known, scientifically proven, and safest laser platforms. With these wavelengths Fraxel laser can treat skin conditions including acne scarring, traumatic scars, skin rejuvenation, wrinkles, sun damage, stretch marks, melasma or hormonal pigmentation, fine, medium and coarse wrinkle, as well as provide general skin rejuvenation- all with predictable down times. Efudex Treatment Fluorouracil 5%

Efudex is fluorouracil topical cream 5%. It is used to treat areas of actinic keratosis and superficial basal cell carcinomas or skin cancers. I had to use the cream on my face twice a day for three weeks. The side effects were the unsightly red coloration, an occasional mild burning, and itching. towards the end of using the cream the itching became more and more annoying. video taken with an iPhone 4s

This is a three week elapsed time video about what to expect when you treat skin or your face with Fluorouracil. This is not a fun treatment, but it works. You'll have lots of redness, cracked and painful skin. You'll find that people will stare, or they will look away. That part was funny to observe. But at the end of the treatment and the short healing period, you're going to have much better skin. Take this treatment seriously. It is a topical form of chemotherapy and it works in the cell's DNA and RNA. Read up on the drug before starting. As you'll see in the video, and hopefully your own experience, your skin can heal very fast. I am very happy with the results and hope I never have to do this again. All of my scaly areas or places that were always flaking are gone. The results were amazing. Looking back on it now - I should have done my whole face at once and gotten it all over with sooner. Like I said, though the results are great, this is not a walk in the park. When I said it made me physically ill, I mean i felt fatigued, tired and brain dead. Like I said, this is serious medicine. On shaving - yes, you can shave, but just take it easy. No one will be touching that skin anyway, so don't worry about that close shave for a week or so. I used a GoPro 3 for this video. Sorry for the fish eye effect. Don't rate the quality of the video, but rather whether or not the video helped you understand or prepare for treatment. Good luck and hang in there. It does pass quickly.

Dr. Schultz used to prescribe retinoids to his patients. That's because at the time, it was the best exfoliant available. However, it can be extremely irritating and has some other side effects. In this episode of DermTV, Dr. Schultz shares some DermTV viewers' stories which serve as examples of why he no longer prescribes retinoids and instead uses the gold standard of exfoliants: glycolic acid. Dr. Schultz used to prescribe retinoids to his patients. However, time and time again, I get questions about retinoids. Retinoid products are also effective exfoliating treatments for acne and are also helpful for facial rejuvenation. Retinoids include not just Retin-A, whose generic name is Tretinoin, but also other derivatives of tretinoin like Ta zarotene, and Adapalene. The reason that I've moved to glycolics is the side effects from retinoids and is really well underscored by a few questions and comments I've recently gotten from viewers on the topic. I'd like to share them with you. Here are a few. First. "Hey Dr. Schultz. My name is Eva and I'm from Africa. I have just started using Retin-A cream and my face got worse within the third day. I got rashes on my forehead. Trying as much as possible to keep away from sunlight although it's really hot on this side of the Sahara! So, should I stop using it? Need some serious help!" Another is... "Hi Dr. Schultz. I'm on Tretinoin 0.025% topical cream for acne and acne scarring. I've been using it for one week. It's making my face darker than the rest of my body. Also flaky and stingy. How long will these effects last? Will my skin return to its normal color? And last... "I used tretinoin under my eyes and on my face. My eyes have more wrinkles, and my cheeks are dark. How can I fix this? Please help!" Obviously, all of these viewers need to stop using their tretinoin and get help from whomever prescribed it for them. Again, I have no personal bias against retinoids but unfortunately patients often have difficulty with them. They can lead to discoloration, irritation, flaking and even soreness. And one other issue of importance with retinoids: They can have potential harmful effects to a forming fetus, so you can't use any retinoids if you're pregnant. All retinoids should only be used only under the direction of a physician. Of course, if you're using retinoids under proper medical supervision and you like the results they're providing then you should certainly continuing using them but to avoid the issues I just mentioned, I prefer glycolics. Key Points *Consist of small pink areas of skin which are rough or scaly *Caused by chronic accumulative sun exposure *Considered a pre-cancerous condition Actinic keratosis, also known as solar keratosis, is a precancerous condition related to damage from chronic sun exposure. Affected persons develop small pink or red lesions which are rough or scaly, usually appearing on parts of the body which are not typically covered by clothing. In some cases, squamous cell carcinoma develops within these lesions, which is the rationale for their preventative removal. Any part of the body with consistent sun exposure, most commonly the face, scalp, ears, neck, arms and top of hands, may be affected . Actinic keratoses are usually 1 centimeter or less in diameter, generally slightly scaly (with occasional instances of lesions which become thick with scale, as with warts), and with a pink, red or brown color variation. Actinic keratoses most commonly affect fair-skinned persons, those who have trouble tanning (in other words, burn easily), those with light-colored (blue, green, hazel) eyes, and those with red or blond hair. Any person with excessive sun exposure, especially during their youth, is at greater risk. In addition, persons with weakened immune systems due to HIV, chemotherapy or organ transplant have a greater risk of developing actinic keratoses and skin cancers. Differential Diagnosis (Other conditions with similar appearance) Squamous cell carcinoma Bowen's disease (squamous cell carcinoma in situ, a very thin noninvasive type of squamous cell carcinoma) Basal cell carcinoma Seborrheic keratosis Wart Benign Lichenoid keratosis Diagnosis Key Points *Diagnosis based on appearance and location *Skin biopsy may be performed to rule out malignancy Actinic keratosis is generally diagnosed based on appearance of the affected area. Health care professionals may biopsy the affected area to exclude malignancy. *Prevention is the most important factor in combating the disorder *All occurrences should be treated, as the disorder has the potential to become malignant *Several treatments exist to destroy the pre-cancerous growths. Most commonly cryosurgery (destruction by freezing with liquid nitrogen), 5-fluorouracil topically, Imiquimod cream topically, Photodynamic therapy, and chemical peel are all options to treat AK's. *Self-examination is key in the timely recognitiont of actinic keratoses Prevention of actinic keratosis by moderating direct sun exposure is recommended. Prevention of sun exposure has been shown to reduce the occurrence of new lesions. Covering the skin with appropriate clothing during times of sun exposure, using sunscreen with a high Sun protection factor ("SPF") of 30 or more, seeking shade during prolonged outdoor activities (or planning outdoor activities during the morning or evening hours), and even wearing clothing with an UPF rating can assist in prevention. If an affected area begins to change quickly or bleed, attention from a medical professional is recommended. Several options are available to treat or destroy the affected tissue, including the use of creams (fluorouracil, diclofenac, Imiquimod), cryosurgery, curettage (scraping), dermabrasion, chemical peeling, or laser treatments. Photodynamic therapy has also been shown to help in affected persons.

Photodynamic Therapy(PDT) is the newest most exciting trend in laser light applications for photo aging, acne and precancerous actinic keratoses. It involves placing a liquid photosensitizer on the site to be treated and then irradiating that area with specialized wavelengths of light to achieve a desired result. What this means is we paint the skin with an invisible colored solution called ALA (alpha levulonic acid) that soaks into the skin. This chemical has a special affinity for pre-skin cancers and oil glands. The skin will metabolize it into a special invisible color called a porphyrin. The laser light sees this porphyrin color as purple and super heats it. The result is shrinkage of oil glands and destruction of precancerous cells. What the patient experiences is a mild tingling during the treatment. The next few days feel like a sunburn as the old cells are shed off. Many patients compare it to a mild to moderate chemical peel. What is most important is that after having the ALA applied that the patient AVOIDS DIRECT SUNLIGHT FOR 36 HOURS, this includes car windows. This is because sunlight contains the spectrum of light that can unpredictably activate the chemical and result in a much deeper peel than was expected. The positive effects (decrease in sun damage) of Fraxel is variable, and is dependent on genetic factors, previous sun exposure, and your future sun exposure. Most patients will have lasting results for 3-8 years. This procedure can be repeated if needed. Is Fraxel laser painful? Pain is minimal as I use a special aneasthetic cream before treatment. Fraxel takes approximately 20-40 minutes to perform, depending on the areas treated. How long will it take to recover from Fraxel laser? 4-7 days. Your skin will be swollen from day 2-3, and then peeling from day 3-6. Most patients are comfortable to perform daily duties from day 3. You must refrain from exercise for 5 days after Fraxel 1927 treatment. What are the advantages of Fraxel laser over other treatment options for solar keratosis and sunspots? Fraxel laser has an excellent clearance rate for solar keratosis and sunspots. In most cases patients will only require one 1927 Fraxel treatment. This is what I experienced 24 hours Post-Levulan Photodynamic Therapy on my sun damaged skin. During treatment, a clear thin liquid is applied to your skin in the affected areas. You will wait for the "incubation period" of about 90 minutes to 2 hours. In my case I had hands and arms done as well, which required 4 hours. The light therapy lasts for 16 minutes and can be quite uncomfortable. You must stay out of sun or bright light for 48 hours after the treatment. You will experience a sunburn effect, with some peeling after a couple of days. The Levulan only attacks cancer and pre-cancer cells. Remember to wear sunscreen for at least a week after the initial 48 hours, and try to incorporate that into your daily routine. I highly recommend products that are organic or at least contain no chemicals. Good Luck!
