Toronto: Elsevier. Many students try to make nursing diagnoses the hard way. They make the mistake of jumping to conclusions during the assessment phase. They try to skip cue clustering and just guess at a nursing diagnosis right away instead of doing a thorough assessment. Then end up comparing the defining characteristics that are listed in their handbook for each diagnosis to their patient’s assessment data. Could you imagine going through all of the nursing diagnoses for each patient … Wow! You would never get your assignment done! Plus, if you do that it is likely that you will miss important information and important diagnoses. You might also misdiagnose the patient because you will be tempted to just use the first diagnosis that sort of matches what is going on. Dangerous! During assessment if you think you know what the diagnosis might be don’t jump to conclusions - make sure you have asked enough questions and gathered enough data to support your inference. It is good to note your inference then ask more questions or collect more data to confirm or exclude your inference. Always assess your patient first. The easy way to make nursing diagnoses is to assess first, cluster cues, investigate any inferences then identify one or more nursing diagnoses. During your assessment identify any abnormal data. The abnormal data that you collect will often become the defining characteristics in the nursing diagnosis. When you construct a nursing diagnosis you essentially work backwards from the defining characteristics to the diagnostic label, but we will go over the components of nursing diagnoses in another video. Once you have identified clusters of cues that seem to fit together it is time to think critically about what the problem could be. Match your cue clusters to one or more nursing diagnoses by looking up the ones you think match the assessment data. Then, pick the best nursing diagnoses for your patient. In the real world it is impossible to find and treat all nursing diagnoses at once, especially when hospital stays are short. Focus on the most important areas for your patient when creating your diagnoses. Keep in mind that not all of the conclusions you draw from your assessment will lead to a nursing diagnosis. If you have identified information that just does not fit into a nursing diagnosis it is possible that it isn’t meant to. The data could be related to a collaborative problem or medical diagnosis instead. Don’t panic, you can still have nursing interventions without a nursing diagnosis. For example, you will still give your patient medication for a medical diagnoses that is not the main focus of nursing care. Also, remember that there are many types of nursing diagnoses, including health promotion diagnoses. We will discuss the types of nursing diagnoses in a subsequent video. This is one of a set of 5 films designed for interprofessional education. • Main Story (this film): Mr King's hospital admission after a stroke • Doctors and Nurses: Interviews with doctors and nurses who work in the stroke team • Therapists: Interviews with a Physiotherapist, an Occupational Therapist and a Speech and Language Therapist who work in the stroke team • Discharge and Social Care: Interviews with a social Worker and the other members of the stroke team who help the patient prepare for a safe discharge home • Team Work: Perceptions from all the stroke team members about team working in the care of people who have a stroke Background: This story follows the care pathway of a gentleman just turned fifty who has a sudden unexpected stroke and is admitted to hospital. The patient consented for his experiences to be used for student learning and to ensure confidentiality we have changed his name to Mr King. The patient and family in this film are actors but the health care team members were practitioners working on the stroke unit at the time. We acknowledge the support of University Hospitals of Leicester NHS Trust. This material was commissioned from the work of the Interprofessional Education Strategy, chaired by Dr Liz Anderson. It was funded by De Montfort University Faculty of Health & Life Sciences, University of Leicester Department of Medical & Social Care Education, University of Northampton School of Health. Directed and filmed by Cuckooland TV. Teaching Materials This film has been designed for interprofessional education. The learning content can be re-purposed in many different ways. In this guide we offer our insights for student learning. We use these learning materials to support placement learning when students are on clinical practice. The film can be viewed during reflective study time in study rooms adjacent to the clinical area. The adult learning approach requires small (n=2-5) interprofessional student groups and the film triggers debate and discussion. The Main Story The film begins with the ambulance crew preparing the patient for hospital admission. They phone ahead to the relevant hospital stroke admissions unit to report the planned arrival of Mr King. The patient is safely admitted to the stroke unit which in this Hospital is within an Emergency Medical Unit. Here the Stroke Consultant and his medical team confirm the diagnosis of probable stroke and then with the radiographer/radiologist assesses the type of stroke and then makes a relevant management plan. Following the initial assessment and early diagnostic tests Mr King is taken to the stroke ward. Here the day to day care of Mr King is managed by the nursing team who act as go betweens liaising with many of the specialist therapists, the medical team and the patient and family. The film portrays the progression of the patient from early problems with swallowing as assessed by the Speech and Language Therapist, to treatment from the Physiotherapist and Occupational Therapist (OT). With time the patient makes improvement in functional tasks and options for ongoing care and discharge are discussed at the weekly interprofessional team meetings with the patient and family present. A home visit is arranged to look at facilitating firstly some home leave, and later a safe discharge. The Social Worker is portrayed supporting the family towards this, and the OT is seen preparing the home with adaptations. Discharge some 10weeks following the admission is to the local Primary Health Care Team and the community physiotherapy team who will all continue to monitor Mr King's progress. Points for discussion • What local arrangements are in place where locally to you? In this story Mr King came in through a Rapid Ambulance Protocol Team, where paramedics who find patients to be FAST positive alert the stroke team directly. • Who constitutes your local stroke team? In this film there is a stroke physician and specialist stroke nurse. • What imaging facilities are available locally? Why is access to these important? In the UK initial imaging would be Computerised Tomography. Further imaging using magnetic resonance imaging (MRI) might be indicated. How might the results of the imaging influence management? • How might the efficiency of this initial assessment influence management?

Abstract: Internationally, the health workforce has undergone rapid transformation to help meet growing staffing demands and population requirements. Several tools have been developed to support workforce change processes. The Calderdale Framework (CF) is one such tool designed to facilitate competency-based training by engaging team members in a seven step process involving awareness raising, service and task analysis, competency identification, establishing support systems, training, and sustaining. This paper explores the utility of the CF as an appraisal tool to assess whether adherence to the tool influences outcomes. The CF was applied retrospectively to three complete evaluations of allied health assistant role introduction: a new podiatry assistant role (Australia), speech pathology assistant (Australia), and occupational therapy assistant practitioner role (UK). Adherence to the CF was associated with more effective and efficient use of the role, role flexibility and career development opportunities for assistants, and role sustainability. Services are less likely to succeed in their workforce change process if they fail to plan for and use a structured approach to change, assign targeted leadership, undertake staff engagement and consultation, and perform an initial service analysis. The CF provides a clear template for appraising the implementation of new roles and highlights the potential consequences of not adhering to particular steps in the implementation process. However, only approximately 30% of our new patients are asked for X-ray evaluations. These sophisticated tools allow us to instigate precise treatment regimes, tailored to each patient’s personal requirements. The EMG (electromyography) scanner is a highly effective diagnostic tool, specifically designed for the Chiropractic profession. It is a non-invasive, painless procedure, suitable for all patients from babies, children and women who are pregnant and senior patients. State-of-the-art technology allows the scanner to produce a computerised image of the spine, by assessing the voltage variations on the surface of the skin; these changes indicate differences in muscular tension or inflammation along the spine and help the chiropractor in evaluating the spinal imbalances he will then correct. It is a great visual tool for the patient to understand what the therapist will act upon to improve their health.
