How To Take Metformin Without Side Effects | When Is The Best Time To Take Metformin | How To Build Up Metformin | Metformin 500mg Side Effects Constipation Diarrhea Hey guys! This weeks video is about metformin and the importance of slowly building up your dose to the prescribed dose to help reduce it's side effects when you're first started on it. Metformin is a medicine used to treat type 2 diabetes and sometimes polycystic ovary syndrome (PCOS). Type 2 diabetes is an illness where the body doesn't make enough insulin, or the insulin that it makes doesn't work properly. This can cause high blood sugar levels (hyperglycemia). PCOS is a condition that affects how the ovaries work. KEY FACTS: • Metformin works by reducing the amount of sugar your liver releases into your blood. It also makes your body respond better to insulin. Insulin is the hormone that controls the level of sugar in your blood. • It's best to take metformin with a meal to reduce the side effects. • The most common side effects are feeling sick, vomiting, diarrhoea, stomach ache and going off your food. • Metformin does not cause weight gain (unlike some other diabetes medicines). • Metformin may also be called by the brand names Bolamyn, Diagemet, Glucient, Glucophage, and Metabet. HOW AND WHEN TO TAKE: It's best to take metformin tablets with a meal to reduce the side effects. Swallow your metformin tablets whole with a glass of water. Do not chew them. Metformin tablets come in different strengths. Your doctor will tell you how many tablets to take a day. DIFFERENT TYPES OF METFORMIN: Metformin comes as 2 different types of tablet - standard-release tablets and slow-release tablets. Standard-release tablets release metformin into your body quickly. You may need to take them several times a day depending on your dose. Slow-release tablets dissolve slowly so you don't have to take them as often. Your doctor or pharmacist will explain what type of metformin tablets you are on and how to take them. Metformin is also available as a liquid for children and people who find it difficult to swallow tablets. Liquid metformin is called by the brand name Riomet. WILL MY DOSE GO UP OR DOWN: Your doctor will check your blood sugar levels regularly and may change your dose of metformin if necessary. When you first start taking metformin standard-release tablets you will be advised to increase the dose slowly. This reduces the chances of getting side effects. (See video for explanation on how to build up dose slowly) If you find you can't tolerate the side effects of standard-release metformin, your doctor may suggest switching to slow-release tablets. HOW TO COPE WITH SIDE EFFECTS: • Feeling sick - take metformin with food to reduce the chances of feeling sick - it may also help to slowly increase your dose over several weeks. • Diarrhoea or vomiting - have small but frequent sips of water. It may also help to take oral rehydration solutions which you can buy from a pharmacy or supermarket to prevent dehydration. Don't take any other medicines to treat diarrhoea or vomiting without speaking to a pharmacist or doctor. • Metallic taste in the mouth - if you find that metformin is giving you a metallic taste in the mouth, try chewing sugar-free gum. If you find you are suffering from side effects talk to your doctor about switching to a slow-release tablet. This weeks video is my most complete guide to using a metered dose inhaler with a spacer device. Make sure to watch it all and I promise you will be an inhaler and spacer device pro at the end of it. An estimated 90% of patients use their metered dose inhaler incorrectly. Inadequate inhaler technique lowers drug deposition to the lungs, wastes medication and may lead to poor disease control, reduced quality of life, increased emergency hospital admissions and higher treatment costs. Please spread the word to friends and family - Like, Share & Tag a Friend. So they can also master their inhaler technique when using a spacer device and manage their respiratory condition better. HOW TO USE A SPACER WITH INHALER 1) Remove the inhaler and spacer cap. Carefully examine the products for damage, missing parts, or foreign objects. Remove any foreign objects prior to use. The products should be replaced immediately if there are any damaged or missing parts. 2) Firmly fit the two halves of the spacer together (If necessary). 3) Stand or sit upright when using your inhaler and spacer. 4) If you are starting a new inhaler or have not used the inhaler for more than one week you must prime your inhaler. Please read the information leaflet of your inhaler for instructions. 5) Hold the inhaler upright, insert into the spacer and shake 4 or 5 times. 6) Breathe out gently. 7) (Mouthpiece Spacer - Adults) Place the mouthpiece between your teeth without biting and form a good seal around it with your lips. 7) (Masked Spacer - Adults & Children) Apply mask to face and ensure an effective seal. 8) (Mouthpiece Spacer - Single Breath Technique - Adults) Press the inhaler at the beginning of a slow inhalation. Breathe in slowly and deeply through the mouth until a full breath has been taken. Hold breath for 5-10 seconds or however much comfortable. 8) (Mouthpiece Spacer - Tidal Breathing Technique - Adults) Press the inhaler at the beginning of a slow inhalation. Breathe in slowly and deeply through the mouth until a full breath has been taken. Keep lips tight on the mouthpiece breathing normally 2-3 times through the chamber. 8) (Masked Spacer - Tidal Breathing Technique - Adults & Children) Press the inhaler at the beginning of a slow inhalation. Breathe in slowly and deeply through the mask until a full breath has been taken. Keep mask tight on the face breathing normally 5-6 times through the chamber. 9) If you have done the SINGLE BREATHING TECHNIQUE remove mouthpiece from you mouth and hold your breath for 10 seconds or as long as is comfortable. 9) If you have done the TIDAL BREATHING TECHNIQUE, holding your breath is not necessary simply remove spacer. 10) If you have been advised to take another dose repeat the process again after 30-60 seconds. 11) Remove inhaler from spacer and replace the caps straight away to keep out dust. What is a Trace Mineral? Most of us are familiar with the minerals our bodies need at high levels, such as calcium, magnesium, potassium and sodium. These minerals, because they are needed by our bodies in significant amounts, are called macrominerals. A number of other minerals, required in much smaller levels by our bodies, but no less important to good overall health, are called microminerals, or more commonly trace minerals.14 Trace minerals are usually found at extraordinarily small levels in our bodies, in the parts per million range.1 In spite of the relatively low concentrations of these minerals, scientists are discovering that they play an increasingly vital role in our health, including the health of our bones and joints.1-19 The Benefits of Trace Minerals Do we need trace minerals?....Yes! It is a certainty that a large number of trace minerals are directly linked to good bone health. Deficiencies of certain trace minerals, such as copper and manganese, for example, have been connected to lower bone density and weaker bones.5 While everyone is aware of the benefit calcium has on bone health, studies show that supplementing with calcium and trace minerals together increases bone density in post-menopausal women more than calcium alone.5 Calcium alone is not enough! A two year, double-blind, placebo controlled trial suggests that bone loss in post-menopausal women can be STOPPED when trace mineral intake is increased!6 The women in the study who took calcium alone only slowed their rates of bone loss. While the mechanisms of how trace minerals function in our bodies may not be entirely known, it is becoming clear that they are necessary for preserving bone mass and for general health as well! Graves’ disease - it may be one of the most common diseases you’ve never heard of. For young women in particular, it can be a grave concern. “Graves’ disease is one of the most common diseases in females. So the ratio from male to female is like 8 females to 1 male. And usually it affects young - around the age of 20,” says Dr. Heitham Ajlouni, who is an endocrinologist for Lee Memorial Health System. An autoimmune disease the body attacks itself. In Graves’ the thyroid gland over-produces hormones that impact metabolism. “You will start losing weight regardless of your appetite. The other thing you have is fast heartbeat. You have changes of your hair and skin. You will have heat intolerance, excess of sweating. Graves’ disease affects the eye, so their eyeball will protrude,” says Dr. Ajlouni. Patients are quick to realize something’s wrong. If it’s not treated, the disease can have serious consequences. “One of the most important thing it affects - the heart. When you have palpitations you are prone to something called arrhythmias. And this is life-threatening. If you are not treated, it will affect your bones. They are more prone to osteoporosis,” says Dr. Ajlouni. There are two fundamental ways to treat Graves’ disease. One is taking medications to regulate hormones. It requires frequent checkups to manage dosing. The other option is to remove or disable the thyroid. By surgery, or what is more common today, taking radioactive iodine. “We are giving a dose of radioactive iodine that will shut down your thyroid and you will end up having hypothyroid, under-active thyroid. And you will be taking levothyroxine for the rest of your life. It doesn’t have a lot of side effects. It’s easy to control, less frequent visits to your doctors, less frequent blood tests, and if you want to conceive, it’s easier to control while pregnant,” says Dr. Ajlouni. A Graves’ diagnosis means a lifetime of medical management, but in managing hormones, women can find balance in their lives. The N Engl J Med image of the week shows a color photo of a person’s lower extremities. We see a scaly erythematous rash with a bilateral distribution. The patient has weight loss and a pancreatic mass on CT. You are offered insulinoma, gastrinoma, glucagonoma, vipoma, and somatostatinoma. Only one of these neuroendocrine tumors is associated with a specific dermatitis. Subclinical thyroid disease during pregnancy may be associated with adverse outcomes, including a lower-than-normal IQ in offspring. It is unknown whether levothyroxine treatment of women who are identified as having subclinical hypothyroidism or hypothyroxinemia during pregnancy improves cognitive function in their children. We review the results of two parallel-randomized trials of these issues. Interleukin-31 may play a role in the pathobiologic mechanism of atopic dermatitis and pruritus. Investigators assessed the efficacy and safety of nemolizumab (CIM331), a humanized antibody against interleukin-31 receptor A, in the treatment of atopic dermatitis. In a phase 2, randomized, double-blind, placebo-controlled, 12-week trial, they assigned adults with moderate-to-severe atopic dermatitis that was inadequately controlled by topical treatments to receive subcutaneous nemolizumab (at a dose of 0.1 mg, 0.5 mg, or 2.0 mg per kilogram of body weight) or placebo every 4 weeks or an exploratory dose of 2.0 mg of nemolizumab per kilogram every 8 weeks. The dermatitis did not improve but there was less itching. Blinatumomab, a bispecific monoclonal antibody construct that enables CD3-positive T cells to recognize and eliminate CD19-positive acute lymphoblastic leukemia (ALL) blasts, was approved for use in patients with relapsed or refractory B-cell precursor ALL on the basis of single-group trials that showed efficacy and manageable toxic effects. We next learn that blinatumomab improves survival in adult ALL patients after relapse. A century ago, James Herrick identified the “peculiar elongated and sickle-shaped red blood cells” associated with the disorder. Herrick subsequently also showed that heart attack is caused by coronary artery occlusion. Hydroxyurea is the only treatment that improves outcomes in sickle-cell anemia, although not all patients respond. A gene therapy is needed. French investigators describe their first patient treated with lentiviral vector–mediated addition of an antisickling β-globin gene into autologous hematopoietic stem cells. Fifteen months after treatment, the level of therapeutic antisickling β-globin remained high (approximately 50% of β-like–globin chains) without recurrence of sickle crises and with correction of the biologic hallmarks of the disease. The N Engl J Med review is on the “informed consent” process. Here we learn that the erstwhile goal of protecting human subjects has largely changed to protect institutions and study sponsors. Thus, only lawyers can understand the pages and pages the patients have to sign. Another review is on patient-advocacy organizations. We learn that these patient rights groups are primarily industry sponsored and their boards of directors are often filled with former industry executives. Is this not a conflict of interests? The N Engl J Med patient of the week has a acute granulomatous liver disease. A transjugular liver-biopsy sample revealed a lymphohistiocytic infiltrate with small lobular and portal nonnecrotizing granulomas. Infection probably began with the inhalation of spores that originate in infected cattle, sheep, and goats. In the Lancet, investigators aimed to investigate whether oral methotrexate increases the efficacy of intra-articular corticosteroid therapy. It did. The efficacy of ceritinib in patients with untreated anaplastic lymphoma kinase (ALK)-rearranged non-small-cell lung cancer (NSCLC) is not known. We inspect a study of ceritinib that increased survival of these patients (barely P=0.0506). Bronchiolitis is the most common lung infection in infants and treatment focuses on management of respiratory distress and hypoxia. High-flow warm humidified oxygen is increasingly used, but has not been rigorously studied in randomized trials. We learn that oxygen via nasal cannula is just as good. The syndemics model of health focuses on the biosocial complex, which consists of interacting, co-present, or sequential diseases and the social and environmental factors that promote and enhance the negative effects of disease interaction. I had never heard of this word and could not find a German translation. I found out that the neologism “syndemic” refers to the aggregation of two or more diseases in a population in which there is some level of positive biological interaction that exacerbates the negative health effects of any or all of the diseases. We close with a 63-year-old man who presented to the department of dermatology and allergy in January, 2016, with a 32-year history of reddish-brown skin lesions, which had worsened over the preceding year.
