Current medication management for neuropathic pain includes select neuromodulating agents such as anticonvulsants, serotonin norepinephrine reuptake inhibitors, tricyclic antidepressants, and certain opioids. Gabapentin remains among the most commonly used anticonvulsants for neuropathic pain. The established therapeutic dosing for gabapentin in neuropathic pain trials is 1800-3600 mg/day in 3 divided doses in patients with normal renal function. This means the minimum effective dose is 600 mg 3 times a day. Renal adjustments are recommended in patients with CrCl below 60 mL/min. For patients on dialysis, gabapentin can often be 3 times weekly following dialysis. Several cross-sectional studies have reported gabapentin being used in subtherapeutic doses among most patients. In a retrospective analysis of 939 patients with post-herpetic neuralgia, the mean daily dose of gabapentin was 826 mg. In another 2-year retrospective study of 151 veterans with various neuropathic pain syndromes, the median daily dose for gabapentin was 900 mg. In both studies, the most prevalent gabapentin dosing was half the therapeutic dosing. What Every Patient Should Know: Patients should be aware of the therapeutic dosing for neuropathic pain to establish realistic expectations and improve compliance and likelihood of remaining on therapy. The conversation may be as follows: “Gabapentin may reduce nerve pain at 600 mg 3 times a day but patients usually start on a low dose to make sure they tolerate it and is then increased slowly to give the body a chance to get used to it. Gabapentin Abuse: Despite its therapeutic role in neuropathic pain, gabapentin produces psychoactive effects and has an abuse liability. Gabapentin abuse typically involves taking higher doses in a single administration. In conclusion, pharmacists as medication experts are well-poised to educate prescribers and patients on therapeutic dosing of gabapentin to optimize its rational and appropriate use for treating neuropathic pain. Neuropathic pain can be diverse in nature, encompassing a wide range of pain types, including post-herpetic neuralgia, painful diabetic peripheral neuropathy, and painful cancer-related neuropathies. Gabapentin has been shown to be beneficial in treating several types of neuropathic pain; however, the mechanism of action by which gabapentin exerts its analgesic effect is still unknown. It is suggested that gabapentin may block the calcium channel alpha delta receptor in the brain. This protein-modulated receptor is involved in excitatory synapse formation. Therefore, the therapeutic effects of gabapentin may be attributed to prevention of new synapse formations. Gabapentin was shown to offer substantial improvement in neuropathic pain with side effects that were similar to those on placebo. Even with sufficient data supporting the use of gabapentin in the treatment of various neuropathic pain conditions, gabapentin only has Food and Drug Administration approval for PHN. Dosing recommendations for off-label use of gabapentin can be somewhat ambiguous, if a recommendation exists at all. Therefore, several studies further investigate dosing regimens specific to other neuropathic pain syndromes. Gabapentin Dosing Considerations: Three gabapentin products are FDA approved to treat PHN. The different formulations cannot be interchanged and each has its own dosing schedule. For immediate-release gabapentin, dosing may be initiated with 300 mg on day 1, doubled on day 2, and tripled on day 3 . The dose can then be titrated up as needed for pain relief to a maximum dose of 1,800 mg daily. Clinical studies referenced in the package insert state that efficacy for a range of doses from 1,800 mg/day to 3,600 mg/day were observed; however, there was no additional benefit seen with doses greater than 1,800 mg/d. Gralise is an extended-release gabapentin formulation that also is FDA approved for PHN with a titration schedule that begins with 300 mg on day 1; 600 mg on day 2; 900 mg on days 3 to 6; 1,200 mg on days 7 to 10; 1,500 mg on days 11 to 14; and 1,800 mg on day 15 and thereafter. The third gabapentin formulation for PHN treatment is another extended-release product, Horizant. The starting dose is 600 mg in the morning for 3 days, increased to 600 mg twice daily on day 4 and thereafter. A daily dose of Horizant greater than 1,200 mg provided no additional benefit at the expense of side effects. It’s used to treat everything from pain to mood disorders and can be just what the doctor ordered for many. But now, there is mounting research uncovering misuse of and even a black market for this popular prescription. We’re talking about gabapentin. It’s approved for some types of seizures and nerve pain. Doctors also use it off-label to treat everything from insomnia to migraines to anxiety. Sixty-four million prescriptions of it were sold in the U.S. in 2016, up more than 60% from four years earlier. The thing is, experts say there is growing evidence it’s being abused, too. It’s even being sold on the streets under the name “Johnnys.” Dr. Joseph Insler, an addiction psychiatrist, says he’s seeing it all too often. “Sometimes, I've even experienced patients ask me for their “Johnnys” and then they'll, maybe, catch themselves and say, ‘No, no, no. I mean gabapentin',” he says. Why is this prescription being misused? “I think that some individuals may say that they use it to get high, and others may say they use it and get a drowsy effect. So, we're talking about the euphoria versus sedation, “ says Rachel Vickers Smith, Ph.D. Dr Vickers Smith wrote her dissertation on gabapentin abuse when studying drug abusers in Appalachia. “We found a nearly 3000 percent increase from 2008 to about 2014 in individuals reporting gabapentin abuse for the purposes of getting high,” she says. We wanted to see for ourselves, so we searched online. We easily found people talking about taking “Johnnys”, or gabapentin, to get high. There were also threads of people trying to spread awareness about the abuse. Experts say gabapentin is typically misused by substance abusers who mix it with other drugs. But, Dr. Insler says it’s also possible for people with legitimate prescriptions to misuse, too. He says, “If somebody’s taking excessively high doses or needing early refills” or if their mood changes, family members or clinicians should see these signs as red flags. A growing body of research shows the problem of abuse, especially among substance abusers is clear. But some experts also worry about the number of prescriptions being handed out. A recent New England Journal of Medicine letter warned that “clinicians who are desperate for alternatives to opioids” are “increasingly prescribing gabapentin” and that “evidence suggests that some patients misuse, abuse or divert gabapentin.” http://www.nejm.org/doi/full/10.1056/NEJMp1704633?rss=mostEmailed&#t=article Dr Rachel Vickers Smith says, “ I think that's why it's really important to get out the message,” about abuse, in general. There was little information on potential abuse although the National Institute on Drug Abuse pointed us to existing research and the DEA says it is beginning to receive calls. Dr Vickers Smith says, “ I don't think that gabapentin abuse is on the CDC, DEA’s, NIDA’s radar, in part because we had this opioid epidemic.” All experts we talked to stress that this is NOT the next opioid epidemic, but they believe gabapentin abuse is something to watch closely and believe more research is needed. We contacted two of the manufacturers of gabapentin, including Pfizer, which told us, “Gabapentin is an important treatment option for their approved indications”.

No evidence that proves Gabapentin helps people with bipolar disorder. If there is a clinical trial that was ever done that proves the efficacy of using Gabapentin to treat bipolar disorder I have never been able to locate it. There IS evidence however that Neurontin (which gabapentin is the generic form ) induces suicidal ideation and increases the risk of suivide. Is this a drug that should be given to bipolar patients? A drug that artificially induces depression by tamping down the synapses so that the neurons are no longer receiving electrical signals from the synapses? When the central nervous system is suppressed any function of the CNS has the possibility of becoming affected. Which is why so many bizarre side effects are occurring from these drugs. Which physicians in turn are telling their patients cannot possibly be related or a side effect of gabapentin or Neurontin or Lyrica. But they ARE related. Although more than likely it will be ascribed to another NEW & SEPARATE disorder. When in reality-because your central nervous system ceases to work properly on this drug. I almost died from respiratory failure on gabapentin. Because the respiratory system is controlled my the central nervous system. As far as Gabapentin & Lyrica being used to treat pain, the protocols for treating pain with Lyrica and Gabapentin were developed by a physician who later went to prison for his completely falsified "research". Yet those protocols have never been rescinded and are still being used today. I have included the links to this info. Take these drugs at your own risk. And be warned that WHEN you begin to experience side affects (and you will at some point) your physician will most likely dismiss the link to Gabapentin & Lyrica AND they will not tell you of Gabapentin withdrawals. Over a year after being off of Gabapentin I still suffer from dystonia, Hypertonia, as well as several other neuro movement disorders that may be permanent. I was also advised by my pharmacy to just stop taking it. Which could have killed me. In addition to making my movement disorders induced by these drugs as possibly being permanent. You are not getting the full story on these drugs. Hopefully someday this drug will be exposed for the reality of it being the big pharmaceutical hoax that it is. Snake oil. But until then, be proactive. Do your research. And if you decide to stop taking it -a slow taper off is necessary or you can easily be left with a wide variety of side effects or movement disorders that will cause you to suffer years later. Gabapentin/Neurontin, is a medication used primarily for seizure disorders and pain associated with the nerves. It can be used for nerve pain related to the herpes virus or shingles. It is also commonly used by diabetics who have suffered nerve pain. I find that this medication continues to work wonder for me, even after daily use for 3+ years. It helps prevent my migraines (at least it seems to) and once I have one, it helps the dull the pain. It also has help with occasional back pain problems. It has been a life saver for my insomnia. I am able to cope better with my day to day task because this medication helps me cope with my Clinical Depression, of which has haunted me since the passing of my father. There is no such thing as a miracle pill or a miracle cure for anything, but this medication has been a huge help for me over the years. Thanks for reading/watching my video.

Since Pfizer had to pay the US Government a one billion dollar criminal fine for falsifying the clinical trials of Gabapentin, then why is this drug STILL being prescribed? Why is this drug even STILL on the market ? The FDA knows the data was falsified but just pay the one billion dollar fine and move along, Pfizer? No ACCURATE & PROVEN clinical trials are now REQUIRED for drugs that are prescribed if you pay the fine? For what is currently one of the most prescribed drugs in America? A drug that is also given to children? I have researched this drug extensively I went off of it. Many of the most common side effects are not dangerous and can be treated by having your doctor reduce your prescription or prescribe another type of medication to minimize them. Serious Side Effects of Gabapentin: Gabapentin can cause serious issues as well, some of which are direct effects of the more common side effects associated with normal use of the drug. According to the US Food and Drug Administration, the “sleepiness and dizziness” often caused by normal gabapentin use can “increase the occurrence of accidental injury.” As a patient taking gabapentin, one must remember this possible side effect and be very careful during the times where the drug is active in one’s system. Some adult users may also experience mood changes, signs of dementia, symptoms of anxiety or depression, feelings of agitation or hostility, restlessness, hyperactivity, and self-harming or suicidal thoughts. If these occur, inform your doctor immediately. Your Medication Pregabalin Pregabalin is also known by the brand name: Lyrica. Pregabalin comes in 25, 50, 75, 100, 150, 200, 225 and 300 mg capsules and 20 mg/mL oral liquid. Pregabalin is most commonly used to help control certain kinds of seizures, painful nerve disease, and fibromyalgia. No matter what your doctor has you taking it for, pregabalin is usually taken by mouth with or without food most commonly 2 to 3 times a day. If upset stomach occurs take pregabalin with food. If you miss a dose, take the missed dose as soon as you think about it. If it is close to time for your next dose, skip the missed dose and resume your normal dosing schedule. Do not take 2 doses at the same time or extra doses. Pregabalin should not be discontinued abruptly because of the possibility of increasing seizure frequency. Therapy should be withdrawn gradually. Over the long journey of suffering from lower back pain, I've been through more than a handful of medications and I have found some helpful and others not so helpful. In this video, I explain several of the medications that I took and the one's that I found most helpful. The biggest thing with a disc bulge injury is that the disc that is protruding places pressure on the nerve roots. Ultimately, this results in a sharp pain and inflammation will eventually build up around the injured area. In order to treat the symptoms of pain and inflammation, their are several medications that may be prescribed to an individual. In my case, I was prescribed several different medications including Ibuprofen, Lyrica, Nucynta, Tylenol 3, and novo-moloxicam. The medications I found to be most effective and helpful for treating my symptoms of pain and inflammation were Ibuprofen and Tylenol 3. Also, I found Lyrica to be somewhat helpful at times as well. As for the other medications, I didn't' find them as helpful but that could of been for a number of reasons such as the dosage not being high enough. Additionally, in this video, I explain some of the side effects I experienced with taking several of these drugs including Lyrica. Gabapentin can also cause serious depressive symptoms when a person withdraws from the drug. If the individual seems extremely happy or content one day and very depressed the next, there is a strong chance they may be abusing the drug. Can the Recreational Use of Gabapentin Lead to Addiction? In addition, your loved one might be obtaining the drug illegally, which would mean they might be stealing or falsifying prescriptions. Some of the women took a vitamin D supplement and some didn't. 50 weeks later the women that took a supplement received less pain and morning fatigue. Also consuming more omega 3 fats such as with a supplement does have a direct impact on the nervous system. In a study published in the 2005 issue of Epilepsy Behavior suggest that omega 3 supplements could reduce the frequency of seizures. Lastly warm baths, exercise, keeping blood sugar under control and messages might help as well. There really are tons of more options. In this video, once again I re-emphasise how when I was taking a higher dose of Lyrica and started experiencing strange bladder sensations, the Dr's just confirmed to me that it was definitely the MS! When clearly it wasn't and I proved this by tapering off the drug, and the symptoms went away. I wonder how many people this is happening to, right now? I chat about stopping my the Lyrica completely, and the “withdrawal” I started to experience in the form of enhanced numbness in my feet/legs, pain, burning also.
