Frequent Ejaculation May Protect Against Prostate Cancer

Good news men – in case you didn’t know already know, orgasms not only feel fantastic but are also linked to a decreased risk of prostate cancer. It appears that men who ejaculate a lot over the course of a month are less likely to be diagnosed with prostate cancer. This finding was first uncovered back in 2004 when researchers at the National Cancer Institute examined the association between ejaculation frequency – which includes sexual intercourse, nocturnal emission, and masturbation – and the risk of prostate cancer. The study first appearing in the Journal of the American Medical Association, tracked over 29,000 men in the U.S. aged 46 to 81 years using data from the Health Professionals Follow-up Study from 1992 to 2000. All men were cancer free when the study began. Each man filled out questionnaires every 2 years over the course of the study with the participants reporting on the average number of ejaculations they had each month during the ages of 20 to 29 years, 40 to 49 years, and during the past year. The results showed men who ejaculated 21 or more times a month had a 33% lower risk of prostate cancer compared with men who reported four to seven ejaculations a month throughout their lifetimes. An Australian study of 2,338 men had the same results in that men who averaged four to seven ejaculations a week were 36% less likely to be diagnosed with prostate cancer before the age of 70 than men who ejaculated less than 2.3 times a week on average. But another study found that men who had sex with 30 or more women were two to three times more likely to develop prostate cancer than men with only one partner. This is considered good news for all men even though there was no agreement as to what accounted for the protection. One reason given was that the prostate accumulates carcinogens or other harmful substances which are eliminated during ejaculation. Men need to remember that sexual activity can have some negative consequences, such as acquiring a sexually transmitted infection. Whatever the reason may be, men can enjoy knowing that sex is a very pleasurable method of possibly protecting himself against prostate cancer. High levels of male sex hormones, or androgens, may increase a man's risk of prostate cancer. But different studies of this question, done in different ways, have reached different conclusions. To look at the question in a new way, a team of researchers at England's University of Nottingham looked at whether men with more intense sex drives were at higher risk of prostate cancer. Polyxeni Dimitropoulou, PhD; Rosalind Eeles, PhD, FRCP; and Kenneth R. Muir, PhD, obtained detailed sexual histories from 840 men. About half the men got prostate cancer by age 60, and about half did not have cancer. The findings were surprising. Sexual intercourse did not affect prostate cancer risk. But frequent masturbation did -- in different ways, at different times of life. "Frequent masturbation during men's 20s and 30s increased their risk of prostate cancer," Dimitropoulou tells WebMD. "But men in their 50s who masturbated frequently had decreased risk." Of course, masturbation frequency is relative. For men in their 20s, "frequent masturbation" was two to seven times per week. Compared to same-age men who reported masturbating less than once per month, 20-something frequent masturbators had a 79% higher risk of prostate cancer by age 60. For men in their 50s, "frequent masturbation" was one or more times per week. Compared to same-age men who reported never masturbating, 50-something frequent masturbators had a 70% lower risk of prostate cancer. What's going on? The study wasn't designed to answer that question. But Dimitropoulou and colleagues have some theories. They suggest that young men genetically predisposed to have hormone-sensitive prostate cancer will be at higher risk if their bodies naturally produce high levels of male hormones -- the same hormones that give them an intense sex drive. So it's not masturbation itself that's increasing prostate cancer risk in young men. More masturbation may just mean more sex drive -- and more androgens bathing prostate tissues. That's not the case for older men. Dimitropoulou suggests that in older men, masturbation itself may actually be helpful, ridding the prostate gland of fluids that may contain cancer-causing substances. "In mature age, it may be more important that toxins get flushed out of the system," she says. "And because the masturbation frequency was not as high in the men's 50s as it was in their 20s, even low levels of masturbation in the 50s has a protective effect." These are just theories, Dimitropoulou warns. More research is needed to determine the exact role of sex hormones and sexual activity in prostate-cancer risk at different stages of life. Meanwhile, Dimitropoulou, now at England's University of Cambridge, advises moderation for both younger and older men. "It is kind of logical that a moderate level of masturbatory activity has to be maintained," she says. "Not too much, and not none at all." In an article in the journal European Urology, Jennifer R. Often the concern that many men have when they’re informed that their PSA levels are elevated is whether they are at risk for developing prostate cancer. A PSA test is designed to measure the level of a protein in your blood called prostatic specific antigen. While prostate cancer will of course increase the levels of PSA in your blood stream, this increase isn’t always an indication of prostate cancer. There are number of things that can elevate your PSA levels that are not related to prostate cancer. In a majority of cases an elevated PSA may be an indicator of another disorder entirely that is affecting the prostate gland. Let’s take a look at some of the most common factors that can cause an increase in PSA levels. Causes for the increase in PSA Prostatitis: Prostatitis is the inflammation of the prostate gland due to a bacterial infection. The condition is treated with antibiotics and affects mostly males below the age of 50. Benign prostatic hyperplasia (BPH): Benign prostatic hyperplasia is an enlargement of the prostate gland which can cause difficulty with urination or increased frequency of urination. The condition itself is non-cancerous and primarily affects men over the age of 50. A digital rectal exam (DRE) is also conducted to properly diagnose BPH or other prostate related issues. Age: Age is one factor we simply have no control over. As men age PSA levels will gradually rise. For example, men with a PSA of 2.5 by the age of 40 is considered normal. A man who reaches the age of 60 will have a normal level of 4.5, what’s ironic however is that a younger man with a PSA score of 4.5 could potentially be at greater risk for prostate cancer. Sex (Ejaculation): Ejaculation during sexual intercourse can slightly raise PSA levels. They will generally return to normal within 48 hours. This slight increase however isn’t enough to make a significant difference unless your PSA level is borderline above normal. Before having a PSA test done ask your doctor if you should avoid sexual activity. Above all else it’s important that you don’t panic and closely examine all the factors relevant to you that may be causing an increase in your PSA. Get all the information you can and speak with your doctor before considering a biopsy to accurately determine if you do in fact have prostate cancer.

Sadly, many men don't find out they have prostate cancer until the cancer has spread, making it more difficult to treat. This is because in the early stages, prostate cancer often doesn't have many symptoms. 1. Difficulty urinating This may take the form of feeling like you have to go and then nothing comes out, stopping midstream, or having to go more often. Another sign is difficulty stopping, which often takes the form of extended dribbling, or the feeling that you still have to go even when you're done. 2. Pain while urinating Again, the problem may be a prostate tumor pressing on the urethra. However, pain while urinating is a classic symptom of infection in the urinary tract. It can also signal an infection of the prostate, known as prostatitis. 3. Blood in the urine This sign is less common, and it's a reason to go straight to the doctor. It doesn't have to be very much blood; all you might notice is a pinkish tint or smear. Blood in the urine is often caused by infections in the bladder or prostate, by kidney stones, and by BPH. 4. Difficulty getting or keeping an erection Yes, this one's really hard to talk about. But if it's not happening when you want it to, or things get stuck "half-mast," it's time for a checkup. Erectile problems are often caused by atherosclerosis (fatty buildup along the arteries), which can reduce blood flow to the penis, but they can also be due to a prostate tumor that interferes with erections or ejaculation. 5. Blood in the semen This sign, like blood in the urine , is often not described very clearly. It's not a large amount of blood, more like just enough to make semen pinkish or streaked, according to men with prostate cancer. One way to distinguish this type of pain from run-of-the-mill sciatica and lower back pain is that it may feel deeper and more like a dull ache. 8. Having to pee in the middle of the night This is one of the sly symptoms that men diagnosed with prostate cancer say they remember, now that they look back. If you find yourself waking more than once a night urgently needing to go, a trip to the doctor is in order. Bear in mind, though, that this symptom is far more likely to be caused by BPH or an overactive bladder than by prostate cancer. 9. Leaking or dribbling urine Here's one almost no one talks about, but we need to. It's basically the male version of urinary incontinence, which typically occurs on a small but still noticeable scale. Longer bathroom trips while you wait for the dribbling to stop or leakage on the way to the bathroom are the telltale signs. Benign Prostatic Hyperplasia (BPH) and Prostate Cancer epidemiology, etiliogy, causes and symptoms, investigations, treatment and management. Handwritten full lecture for USMLE. ANATOMY of PROSTATE Central zone of prostate sorrounds ejaculatory duct. The Transitional zone of prostate encompases meddle part of urethra. Peripheral zone is placed outside. Prostate cancer is most common in peripheral zone (60-70%). The Central zone is the second most common site for prostate cancer (25%). The Transition zone only has an occurence rate of 5% for prostate cancer. Benign Prostate Hyperplasia (BPH) is in the transition zone and that is why it presents mostly with urinary symptoms. BENIGN PROSTATIC HYPERPLASIA (BPH) Prostate is heavily influenced by DHT and testosterone, however, DHT is ten times more sensitive. Testosterone is converted to DHT by 5alpha reductase. This causes hyperplasia of collagen cells and the smooth muscle. The collagen will cause a physical obstruction while the smooth muscle will cause mechanical obstruction. Eventually there is bladder enlargement which may forma false diverticulum (outpouching of the bladder). Clinical Signs and Symptoms Obstructive symptoms - Consists of the patients experiencing hesitancy, difficulty voiding, straining, decrease in the force of stream and dribbling of urine. Irritation symptoms - increase frequency or urgency as well as nocturia. These symptoms occur as the bladder gets larger and begins to overflow. These symptoms can produce an International Prostate Symptoms Score (IPSS) which determines severity of Benign prostatic Hyperplasia through symptoms. Diagnosis of Benign Prostatic Hyperplasia Digital Rectum Exam is done initially, however it doesn't correlate well with symptoms and it is very difficult to palpate. Urinalysis is also done to rule out prostatitis. PSA is organ specific but can be elevated in cancer and prostatitis. Ultrasound or CT to look for complications such as bladder distention. Transrectal ultrasound is used before surgery. Treatment for Benign Prostatic Hyperplasia (BPH) If the degree of benign prostatic hyperplasia (BPH) is minimal such as 0-7 on the IPSS score than watchful waiting because most spontaneously. So for these patients continue monitoring PSA and Digital Rectums Exams. If getting worse continue to medical treatment. Alpha 1 blocker allows relaxation of the smooth muscle that may be constricting the urethra. Finasteride is a 5 alpha reductase inhibitor to prevent formation of DHT from testosterone and help debulk the prostate. Commonly used drugs are terazosin and doxazosin but this can lead to hypotension. More specific is an Alpha A1 blocker such as tamsolusin which is more specific for the prostate and therefore hypotension is not present but can lead to retrograde ejaculation. Surgery - If medical therapy doesn't work for the Benign Prostatic Hyperplasia (BPH). Two procedures transurethral and prostatectomy. PROSTATE CANCER CLINICAL SIGNS AND SYMPTOMS of PROSTATE CANCER Can range from asymptomatic to completely fatal. Screening - Digital Rectal Exam is not helpful and PSA is the only method being used. Local Disease causes symptoms similar to Benign Prostatic Hyperplasia symptoms, but this suggests involvement of the transition zone. Metastasis disease include symtpoms related to osteoblastic bone growth, primarily the spine leading to neurological symptoms and back pain. Prostatic Cancer patients may also have patients related to lower extremities due to lymphatic obstruction INVESTIGATION for PROSTATE CANCER Digital Rectal Exam shows a single nodule and correlates well with severity of disease. However, it may be a cyst or a stone. PSA normally should be less than 2.5ng/dL (some say less than 4 ng/dL). May also be helpful to measure as PSA Velocity and Free PSA versus Bound PSA. This helps distinguish Benign Prostatic Hyperplasia from Prostate Cancer. Transrectal Ultrasound may also be used. Biopsy uses the Gleasson Score which looks at the tissue of prostate under microscope and given a score out of 5. Staging of prostate cancer utilizes the TNM score. TREATMENT AND MANAGEMENT of PROSTATE CANCER Start with active surveillance by monitoring PSA/DRE/Biopsies. Radiation - Has a lot of GIT complications and may lead to erectile Dysfunction. Brachytherapy is the new treatment. Radical Prostatectomy - May lead to neruovascular damage causing Erectile Dysfunction and damage to urethra my lead to incontinence. After treatment continue to monitor PSA to look for Metastasis or resurgency of cancer. TREATMENT OF METASTASIS OF PROSTATE CANCER Castration - Orchiectomy (Gold Standard), Leuprolide/Ghrelin (GnRH) but these have a temporary increase in androgens, and Anti-Androgens (Flutamide). Castration Resistant Prostate Cancer (CRPC) are tumors that are still growing despite castration. Then use chemotherapy (Doxitexal, Mitoxantrone, Dosatinib)
