Congestive heart failure (congestive heart failure CHF) pathophysiology, treatments, and symptoms nursing lecture on heart failure for nursing students. This congestive heart failure NCLEX review with give you mnemonics on how to remember the causes of heart failure and the different types of heart failure, such as left-sided or right-side heart failure. I also discuss the pathophysiology of both left ventricular systolic and diastolic dysfunction along with right-sided dysfunction. Signs and symptoms of heart failure affecting the left-side are pulmonary in nature with the patient experience, dyspnea, crackles, orthopnea, and paroxysmal nocturnal dyspnea. Whereas, the signs and symptoms of right-sided heart failure are peripheral in nature with the patient experiencing swelling of the liver, hands, feet, abdomen nocturia, and jugular venous distention. Tests used to diagnose heart failure include a blood test called BNP, echo-cardiogram, heart catheterization, chest x-ray, or nuclear stress test. Left-sided heart failure is the most common type of heart failure and can lead to right-sided heart failure. Patients with left ventricular systolic dysfunction have low ejection fractions (EFs) compared to patients with left ventricular diastolic dysfunction who may have a normal ejection fraction. Coronary artery disease (also called coronary heart disease) NCLEX lecture video on the nursing, anatomy, pathophysiology, treatment, and signs and symptoms of coronary artery disease. CAD is the build up of fatty plaques due to atherosclerosis that causes the arteries of the heart to become narrow. This leads to restrictive blood supply to the heart muscle which leads to chest pain (stable or unstable angina) to myocardial infraction (MI). The main coronary arteries that supply the heart with blood include the left and right coronary artery. The left coronary artery branches off into the left circumflex artery and left anterior descending artery. The right coronary artery branches off into the right marginal artery and posterior descending artery. If ischemia has been present for a long time collateral circulation can develop as well. This is where tiny blood vessels re-route the blood supply to the heart to pass the blockage. Signs and symptoms of heart disease include: chest pain (especially with activity and is relieved with rest which is known as stable angina), shortness of breath, and feeling very tired. Coronary artery disease is diagnosed in various ways: lipid profile which measures the cholesterol levels, EKG, stress test, and heart catheterization. CAD is treated with medications (discussed in part 2 of this series), heart catheterization (most commonly with stent placement) or coronary artery bypass graft surgery. Myocardial infarction (heart attack or MI) ischemia lecture on the pathophysiology, ECG, nursing role, complications, signs and symptoms. This video on myocardial infarction will help students prepare for the NCLEX exam. Myocardial infarction is when blood supplied to the heart muscle is limited which causes injury to the heart tissue. Injury from an MI causes complications such as pericarditis, cardiac rupture, cardiogenic shock, arrhythmias, ventricular aneurysm, heart failure, and depression. Signs of symptoms of an MI include: chest pain that radiates, is heavy/intense, and is not relieved by nitroglycerin or rest, nausea/vomiting, cold sweat, anxiety, increased heart rate or blood pressure, irregular heart rate. Tools used to diagnose a myocardial infarction include: cardiac markers (troponins, myoglobin, CK or CK-MB levels), EKG, echocardiogram, stress test with myocardial perfusion imaging, or heart catheterization. As the nurse it is important you know the basics about how to read an EKG during a possible myocardial infarction. You must know what areas of the leads reflect which heart wall and what type of EKG changes you are looking for: ST-segment depression or elevation, T-wave inversion or hyperacute, or pathological Q-wave. Infective endocarditis and non-infective endocarditis lecture on the nursing management, pathophysiology, treatment, signs and symptoms. In this video, I will discuss in detail the different types of endocarditis (infective endocarditis with the sub-types acute and subacute and non-infective). In addition, I detail the pathogenesis of how a vegetation grows on the heart valve and the complications of endocarditis, such as embolism, heart failure, valve regurgitation/stenosis. Signs and symptoms of IE include petechiae, anorexia, high fever, heart failure, cardiac murmur, Osler's nodes, Janeway lesions, Roth Spots, and splenomegaly. Infective endocarditis can be diagnosed with a TEE (transesophageal echocardiogram) which allows the physician to see the backside of the heart and the heart valves. Patients who are at risk for infective endocarditis include patients with a history if IV drug use, rheumatic heart disease, mitral valve prolapse, or other valve defects. Pathogens can enter into the blood stream through invasive procedures (dental work, implanted devices, central line placement) and infect the valve. Pneumonia symptoms, pathophysiology (pathology) presentation for nursing NCLEX review. In this respiratory nursing NCLEX review, I will discuss the pneumonia pathophysiology, signs and symptoms, types of pneumonia, and how it is diagnosed. This respiratory disorder affects the lower respiratory system and is due to an infection caused by either bacteria, virus, or fungi that causes inflammation of the alevoli sacs. The lung parenchyma are affected which are the alevolar ducts, bronchioles, and aveoli. These three units play an important role in gas exchange in the respiratory system. Patients with pneumonia can experience respiratory acidosis with a PCO2 greater than 45, blood pH less than 7.35, PO2 less than 90, and if the body is trying to compensate the HCO3 will be greater than 26. Types of pneumonia include: community-acquired and hospital-acquired. Pneumonia is diagnosed with a chest x-ray and sputum culture along with abnormal pneumonia lung sounds like coarse crackles, rhonchi (type of wheezing), or bronchial breath sounds in the peripheral lung fields which represent lung consolidation. Pneumonia symptoms include productive cough, pleuritic pain, neuro changes (especially in elderly patients), elevated labs like PCO2 and WBCs, unusual breath sounds, mild to high fever, decreased oxygen saturation, nausea/vomiting, increased respiratory rate and heart rate, and activity intolerance with shortness of breath. This is an NCLEX hotspot practice question example about a nursing assessment for the drug Digoxin (Lanoxin). For this particular NCLEX practice question, you will have to identify what area on the diagram you will assess prior to administering Digoxin. Before a nurse administers Lanoxin, it is important to assess the apical pulse for 1 full minute. The apical pulse must be 60 beats per minute or greater. Therefore, to answer this question correctly you must identify on the diagram where the apical pulse is located on an adult patient. The apical pulse is located at the apex of the heart and is found at the 5th5th intercostal space midclavicular line. This location is also the point of maximum impulse. This video is part of a weekly NCLEX review series where I will be going over NCLEX-style questions with you. I will be helping you analyze and breakdown each question, and walk you through how to select the correct option. NCLEX questions require critical thinking and you must know how to use your nursing knowledge to gather the facts and analyze what the question is asking. Stroke nursing (CVA) cerebrovascular accident lecture on ischemic and hemorrhagic strokes along with nursing care, tPA, symptoms, treatments, and pathophysiology. A stroke occurs when blood rich in oxygen can NOT reach brain cells (due to either a blockage or bleeding) and the cells begin to die. Types of stroke: Ischemic (most common) is due to a blood clot within the blood vessel or stenosis of an artery feeding the brain tissue. This limits the blood that can reach the brain cells. This can happen due to: -Embolism: where a clot has left a part of the body (example the clot has left the heart due to a heart valve problem or atrial fibrillation) and has travelled to the brain, which stops blood flow. -Thrombosis: a clot forms within the artery wall within the neck or brain. This is seen in patients with hyperlipidemia and atherosclerosis Hemorrhagic: there is bleeding in the brain due to a break in a blood vessel. There will be limited blood flow to the brain cells and this can cause excessive swelling in the brain from the leakage of blood in the brain tissue. A ruptured brain aneurysm or uncontrolled hypertension can cause this. In addition, a patient can experience a TIA (transient ischemic attack) also called a mini-stroke. This is where signs and symptoms of a stroke last only a few minutes to hours and resolve. It is a warning signs an impending stroke may occur. Stroke symptoms include: facial drooping, arm numbness or weakness, slurred speech or difficulty talking etc. Treatment for stroke (specifically an ischemic type) is tPA (tissue plasminogen activator). This medication works to dissolve the clot that has caused th decreased blood flow. However, tPA must be administered within 3 hours of the onset of stroke symptoms. Nursing care for stroke includes: monitoring vitals (especially blood pressure, respiratory status), watching and educating the patient with neglect syndrome (this most likely occurs with patients who've had a right brain injury), diet and swallowing techniques, hemianopsia, and how to communicate with a patient with aphasia (watch the video for more in depth nursing interventions for stroke). Chronic obstructive pulmonary disease (COPD) nursing NCLEX lecture on the pathophysiology, signs and symptoms, types (chronic bronchitis and emphysema), complications, and how it is diagnosed. COPD is a pulmonary disease that causes chronic obstruction of airflow from the lungs. The cause of the limited airflow is due to chronic inflammation and narrowing of the bronchioles which becomes deformed over time and produces excessive mucous (mainly in chronic bronchitis) and the loss of elasticity of the alevoli sacs (mainly found in emphysema). These problems lead to limited gas exchange such as respiratory acidosis due to extreme retention of carbon dioxide (CO2) and low availability of oxygen. COPD is a "catch all" term used to describe diseases that limit airflow which now includes diseases such as chronic bronchitis and emphysema. Patients with chronic bronchitis are sometimes referred to as "blue bloaters". This is because of the cyanosis and edema (hence the bloating) experienced with this disease. Patients with emphysema are sometimes referred to as "pink puffers". These patients do not experience cyanosis, although they have low oxygen levels but maintain a "pink" appearance due to the compensatory state of hyperventilation keeping the oxygen level sufficient enough. The hyperventilation leads to the increased usage of the accessory muscles which leads to a barrel chest look (increased anteroposterior diameter noted on inspection). Signs and symptoms of COPD include: chronic dry or productive cough, lack of energy, extreme dyspnea, abnormal blood gases, abnormal lung sounds (coarse crackles or wheezing), barrel chest, tripod position for breathing, poor nutrition, recurrent lung infection etc. Complications of COPD include: heart disease such as right-sided heart failure, pulmonary hypertension, pneumothorax, pneumonia, increase risk of lung cancer. Chronic obstructive pulmonary disease is diagnosed with spirometry. Don't forget to watch Part 2 which includes the nursing interventions and medication regime for COPD. Increased intracranial pressure (ICP) nursing, pathophysiology, NCLEX, treatment, and symptoms NCLEX review. Increased intracranial pressure occurs when the inside of the skull experiences increased pressure. This is a medical emergency! How is ICP created? Intracranial pressure is the pressure created by the cerebrospinal fluid and brain tissue/blood within the skull. It can be measured in the lateral ventricles with an external ventricular drain (ventriculostomy). What is a normal intracranial pressure (ICP)? 5-15 mmHg (greater than 20 mmHg…needs treatment) Pathophysiology of increased intracranial pressure: The human skull is very hard and is limited on how much it can expand when something inside the skull experiences a change that leads to increased pressure exerted within the skull. Inside the skull are three structures that can alter intracranial pressure: brain, cerebrospinal fluid (CSF), and blood. The Monro-Kellie hypothesis deals with how ICP is affected by CSF, brain's blood, and tissue that works to maintain cerebral perfusion pressure (CPP). In a nutshell, this hypothesis says that when the volume of one of these structures increases than the others will have to decrease their volume to compensate for the increased intracranial pressure. Intracranial pressure fluctuates and this depends on many factors like: the person’s body temperature, oxygenation status, especially CO2 and O2 levels, body position, arterial and venous pressure, anything that increases intra-abdominal or thoracic pressure (vomiting, bearing down). Cerebral perfusion pressure is important when dealing with intracranial pressure. A normal CPP is 60-100 mmHg. When CPP (pressure that pushes the blood to the brain) falls too low the brain is not perfused and brains tissue dies. How is CPP calculated? CPP= MAP - ICP (see video on how to calculate cerebral perfusion pressure) Symptoms of increased intracranial pressure: mental status changes (EARLIEST), cushing's triad, vomiting, headache, decorticate or decerebrate posturing, optic and oculomotor nerve damage, abnormal doll's eye, seizures etc. Nursing care for increased ICP includes monitoring and preventing further increases in intracranial pressure, administering drugs such as Mannitol, anticonvulsants, corticosteroids etc. See video for a mnemonic on the nursing interventions for increased ICP. Chronic renal failure, also called chronic kidney disease, nursing NCLEX review lecture on the pathophysiology, symptoms, stages, and causes. Chronic kidney disease is a significant decline in kidney function that happens over a long period of time that leads to the buildup of waste, water, and electrolyte imbalances in the body. CKD is irreversible. There are various stages of chronic kidney disease and each stage is based on the patient's glomerular filtration rate (GFR). Stage 1 is kidney disease with normal renal function (GFR greater than 90 mL/min) and proteinuria for 3 months or more. Each stage progresses with a declining glomerular filtration rate until the GFR is 15 mL/min or less and this is known as end-stage renal disease, which is Stage 5. In the severe stages of chronic renal failure, the GFR is dramatically decreased and this leads to the following signs and symptoms of chronic kidney disease: increased BUN and creatinine levels, hypervolemia, electrolyte imbalances (hyperkalemia, hyperphosphatemia, hypocalcemia, hypermagnesemia, oliguira or anuria, metabolic acidosis, and proteinuria/hematuria). In addition, the kidneys are no longer producing hormones, such as EPO (erythopoietin) or renin. This leads to anemia and more hypertension. Furthermore, the kidneys are failing to activate vitamin, which leads to more hypocalcemia. Treatment for chronic renal failure includes dialysis, medications, and kidney transplant. Coronary artery disease (also called coronary heart disease) NCLEX lecture video on the nursing interventions, treatment, and medications for CAD. CAD is the build up of fatty plaques due to atherosclerosis that causes the arteries of the heart to become narrow. This leads to restrictive blood supply to the heart muscle which can lead to chest pain (stable or unstable angina) to myocardial infraction (MI). Nursing interventions for coronary heart disease include education about modifying lifestyle such as smoking cessation, diet (low fat and calorie), and exercise. Medication management plays another role in coronary artery disease management. As the nurse, it is important to provide education to patients about being compliant with medications, how to take them, and possible side effects. Medications for CAD include nitrates (Nitroglycerin), antiplatelets (Aspirin or Plavix), statins (Lipitor), beta blockers (Metoprolol), or ACE inhibitors (Lisinopril). The main coronary arteries that supply the heart with blood include the left and right coronary artery. The left coronary artery branches off into the left circumflex artery and left anterior descending artery. The right coronary artery branches off into the right marginal artery and posterior descending artery. If ischemia has been present for a long time collateral circulation can develop as well. This is where tiny blood vessel re-route blood supply to the heart to pass the blockage. Signs and symptoms of heart disease include: chest pain (especially with activity and is relieved with rest which is known as stable angina), shortness of breath, feeling very tired. Coronary artery disease is diagnosed in various ways: lipid profile which measures the cholesterol levels, EKG, stress test, heart catheterization. Pancreatitis: acute pancreatitis and chronic pancreatitis nursing NCLEX review lecture on symptoms, treatment, pathophysiology, diet, and nursing interventions. Pancreatitis is inflammation of the pancreas that can lead to the digestion of the pancreas by its own enzymes and/or irreversible structural damage to the organ. There are two types of pancreatitis: acute pancreatitis and chronic pancreatitis. In acute pancreatitis, there is sudden inflammation of the pancreas due to something that has triggered the digestive enzymes to become activated inside the organ (high amylase and lipase levels will be found in the blood) and the pancreas starts to digest itself and swell. Two common causes of acute pancreatitis include: gallstones obstruction and heavy consumption of alcohol. Typically, acute pancreatitis is reversible if the patient is treated promptly. Chronic pancreatitis is chronic inflammation of the pancreas (can be caused by repeated episodes of acute pancreatitis but most common cause is years of alcohol abuse) that has led to irreversible damage to the structure of pancreas. There will be loss of the function of the endocrine and exocrine cells (acinar and islet of langerhans cells), damaged to the pancreatic duct etc. Symptoms of pancreatitis include: sudden, very painful mid-epigastric or left upper quadrant which can be felt in the back as well, fever, increased heart rate, decreased blood pressure, Cullen's sign, Grey-Turner's sign, hyperglycemia, dark-urine, jaundice, steatorrhea (oily/fatty stools). What is Cullen and Grey-Turner's Sign? Cullen's Sign is a bluish discoloration around the belly button, whereas Grey-Turner's Sign is bluish discoloration found on the flanks of the abdomen. They are due to retroperitoneal bleeding. Watch the video for an easy way to remember how to tell them apart.
