Ageing immune system may explain age-related cancer risk increase

Eight million people die of cancer across the world each year. One in two men and one in three women in the UK will be diagnosed with cancer in their lifetime, and for most cancers the chance of developing the disease rises dramatically with age. The key to cancer prevention may lie in the immune system rather than genetic mutations, the current focus of most anti-cancer efforts across the world, according to a major new study carried out at the University of Dundee. For decades, it has been known that mutations arising either as a result of genetic predisposition, or lifestyle or environmental factors cause cancer. The traditional view is that the way cancer risk increases with age could be understood and quantified if multiple (typically five to six) mutations in one cell are required to initiate cancer. The Dundee team, which also features researchers from Heriot Watt University, the University of Edinburgh and the Institut Curie in France, have shown that a declining immune system with age may actually be a stronger reason for the increasing risk of developing cancer than multiple mutations. Following the hypothesis that an ageing immune system may result in higher rates of cancer, just as it leads to older people being more prone to other diseases, they looked at data on 2 million cases of cancer over the 18-70 age range. They then developed a mathematical equation for how they would expect cancer incidence to rise in relation to a declining immune system and compared it to the age profiles for 100 different cancers. Question: Do you think the enzyme Telomerase could be used to reverse the aging process in our lifetime? (Submitted by Paul Cellura) Michio Kaku: Paul, Telomerase hit the headlines; however, I think we have to put it into perspective. It is not the fountain of youth; however, it is a significant breakthrough. We have to put it into a much larger perspective. First of all, we know that DNA is sort of like a shoelace. It has plastic tips at the end. Every time a cell reproduces, the tips get shorter and shorter and shorter until finally they fray. And you know that your shoelace, without the plastic tips will simply fall apart. That's what happens inside a cell. A cell, for example, your skin cell, will divide about 60 times, that's called a Hayflick Limit. Then the cell goes into senescence and eventually dies. So in some sense, every cell has a biological clock. It is doomed to die after about 60 reproductions. However, Telomerase can eliminate some of the contraction of the chromosomes and the chromosomes can maintain their length. So at first you may say, "ah-ha! We can now defeat the biological clock." But not so fast, first of all, cancer cells also use Telomerase. Cancer cells are immortal. Cancer cells are immortal and that's precisely why they kill you. Why are cancer cells so dangerous? Because they are immortal. They grow and they grow and they grow until they take over huge chunks of your body, meaning that your bodily functions cannot be performed and you die. So we have to make sure that when you hit ordinary cells with Telomerase that you don't also trigger cancer in the process. Now, also you have to realize that genes are also very essential for the aging process. It turns out that we know what aging is. Aging is the buildup of error. That's all aging is. The build up of genetic and cellular error. And cells begin to age; they begin to get sluggish because genetic mistakes start to build up. Now cells; however, have a repair mechanism. They can repair damage to their cells; otherwise we would all basically rot very soon after birth. However, even the repair mechanisms eventually get gummed up and then the cell really starts to get old as a consequence. So then the question is, can you accelerate cell repair? That is another branch of gerontology which is being looked at using genes and using chemicals to accelerate the repair mechanisms. For example, if I take any organism on the planet Earth from yeast cells to spiders, insects, rabbits, dogs, and even monkeys now. And I reduce their caloric intake by 30%, they live 30% longer. In fact the only organism which has not yet been deliberately tested by scientists are homo sapiens. All the other species obey this basic rule. You starve them to death, they live longer. This is independent of Telomerase. This is a function of the wear and tear that we have on the cells. And this is the only known way of actually deliberately extending the lifespan of any organisms almost at will. Now, what we want is a genetic way of mimicking this mechanism without having to starve yourself because how many people do you know would be willing to starve themselves in order to live 30% longer? Not too many. So then the question is, are there genes that control this process. And the answer is apparently, yes. There's something called the Sirtuin genes, Sir2 being the most prominent of them. They in turn stimulate certain enzymes, among them Resveratrol, which is found in red wine, for example. So this does not mean that drinking red wine or taking Telomerase is the fountain of youth. I don't think that anyone has the fountain of youth yet. What I am saying is, we are now finding pieces of the fountain of youth, tantalizing clues that mean that perhaps in the coming decades, we might be able to actually unravel the aging process. We don't have it yet. Don't go out to the drug store and stock up on these kinds of chemicals and enzymes thinking you're going to live forever. However it is conceivable that in the coming decades we'll come very close to finding it.

Michael Fossel - Defeating Aging

Michael B. Fossel, M.D., Ph.D. (born 1950, Greenwich, Connecticut) was a professor of clinical medicine at Michigan State University and is the author of several books on aging, who is best known for his views on telomerase therapy as a possible treatment for cellular senescence. Fossel has appeared on many major news programs to discuss aging and has appeared regularly on National Public Radio (NPR). He is also a respected lecturer, author, and the founder and former editor-in-chief of the Journal of Anti-Aging Medicine (now known as Rejuvenation Research). Prior to earning his M.D. at Stanford Medical School, Fossel earned a joint B.A. (cum laude) and M.A. in psychology at Wesleyan University and a Ph.D. in neurobiology at Stanford University. He is also a graduate of Phillips Exeter Academy. Prior to graduating from medical school in 1981, he was awarded a National Science Foundation fellowship and taught at Stanford University. In addition to his position at Michigan State University, Fossel has lectured at the National Institute for Health, the Smithsonian Institution, and at various other universities and institutes in various parts of the world. Fossel served on the board of directors for the American Aging Association and was their executive director. Fossel has written numerous articles on aging and ethics for the Journal of the American Medical Association and In Vivo, and his first book, entitled Reversing Human Aging was published in 1996. The book garnered favorable reviews from mainstream newspapers as well as Scientific American and was published in six languages. A magisterial academic textbook on by Fossel entitled Cells, Aging, and Human Disease was published in 2004 by Oxford University Press. Since his days as a teacher at Stanford University, Fossel has studied aging from a medical and scientific perspective with a particular emphasis on premature aging syndromes such as progeria, and since at least 1996 he has been a strong and vocal advocate of [telomerase therapy]] as a potential treatment of age-related diseases, disorders, and syndromes such as progeria, Alzheimer's disease, atherosclerosis, osteoporosis, cancer, and other conditions. However, he is careful to qualify his advocacy of telomerase therapy as being a potential treatment for these conditions rather than a "cure for old age" and a panacea for age-related medical conditions, albeit a potential treatment that could radically extend the maximum human life span and reverse the aging process in most people. Specifically, Fossel sees the potential of telomerase therapy as being the single most effective point of intervention in a wide variety of age-related medical conditions. Howard S. Hochster, an oncologist at Yale Cancer Center who specializes in gastrointestinal cancers, explains that screening for colon cancer is recommended to begin at age 50 due to the increase in cancer incidence at that age. However, adults under 50, including those who may be considered at high risk due to factors related to genetics or lifestyle, can also be diagnosed with the disease. "It's still kind of rare to have people in their 40s develop colorectal cancer," he says, but it does happen. Current guidelines state that colonoscopies are recommended for people age 50 and older every 10 years if they do not have polyps. If a screening exam has identified polyps, more frequent colonoscopies are recommended. For people who are at higher risk, they should be screened earlier, around age 40. A common high-risk factors includes having a first-degree relative with a history of colorectal cancer. Each year more than 160,000 men are diagnosed with prostate cancer. But when it comes to the understanding the disease, doctors say there are a lot of misconceptions. “One myth related to prostate cancer is that it’s an old man’s disease and it’s not,” said Dr. Constantine Mantz, a radiation oncologist on the medical staff of Lee Health. While a patient’s risk does increase as they age, studies show about one in five men will be diagnosed with prostate cancer before they reach age 60. “Prostate cancer sometimes will occur in younger patients because there’s a family history or a genetic predisposition to developing the disease,” said Dr. Mantz. A second myth is patients with BPH, benign prostatic hyperplasia, are at a higher risk for developing prostate cancer. “Although we certainly have patients with both conditions, there’s never been a compelling study that has associated one with the other,” he said. Many patients believe all prostate cancer is a slow growing---but doctors say that to is false. “About 50 percent of patients do have the slow-growing prostate cancer. About 35 percent will have an intermediate risk of prostate cancer, one that is likely to spread over a period of ten years. How we judge the risk of prostate cancer is a truly individualized process,” said Dr. Mantz. But the most recent myth is screening for prostate cancer has no impact on the patient’s lifespan or mortality. Doctors encourage yearly screening for men after the age of 60. “There are certain groups of patients who need to be identified, and they need to be treated, and you will never identify them unless you screen for prostate cancer using a simple PSA test,” he said.
