Your Medication Lisinopril Lisinopril is also known as the brand names: Prinivil, Zestril, and Qbrelis. Lisinopril is most commonly used for high blood pressure, but is also used for heart failure, following a recent heart attack, and for patients with kidney problems. No matter what your doctor has you taking it for, lisinopril is usually taken by mouth once daily and is safe to take either with meals or on an empty stomach. ... เป็นที่ทราบกันแล้วนะครับว่า ยาสำหรับรักษาโรคความดนโลหิตสูง นั้น สามารถลดความเสี่ยงต่อการเกิดโรคหัยวใจ และสมองถูกทำลาย เป็นอัมพาตได้ถึง 30 % อย่างไรก็ตาม ยาทุกชนิดที่เรารับประทานเข้าไป รวมถึงยาลดความดัน โลหิตสูง ต่างมีผลข้างเคียงได้เช่นกัน นั้นคือ ยาลดความดันโลหิตสูง ทำให้ความดันโลหิตลดต่ำลงได้ เรียก hypotension สำหรับท่านที่รับประทานยาลดความดันโลหิตสูงเป็นประจำ ท่านควร ทราบผลข้างเคียงจากการใช้ยาเอาไว้ด้วย ที่ท่านควรทราบได้แก่... ยากลุ่มแรก...ยาขับปัสสาวะ (Diuretics): ยาขับปัสสาวะ (hydrochlorothiazide (Microzide), chlorothiazide (Diuril), flurozamide (Lasix)… ยากลุ่มนี้จะออกฤทธิ์ด้วยการกระตุ้นไตให้ปล่อยน้ำ และเกลือแร่ออก จากร่างกาย ซึ่งยังผลให้ระดับความดันลดลง ยาขับปัสาวะจะทำให้ระดับของ sodium และ postassium ในกระแส เลือดลดต่ำลง (hyponatremia & hypokalemia) การที่มีระดับของ potassium ลดต่ำลง สามารถทำให้เกิดผลอัน ไม่พึงประสงค์ได้ นั้นคือ ทำให้กล้ามเนื้อปั้น (muscle cramps )และ อ่อนแรง (weakness) นอกจากนั้น ยาขับปัสสาวะ ยังทำให้คนที่เป็นโรคเก้ามีอาการของโรค เก้าปรากฏขึ้นอย่างเฉียบพลันได้ ยากลุ่มที่สองที่ใช้ในการรักษาความดันโลหิต (Beta Blockers and Calcium Channel Blockers) ยาทั้งสองชนิดต่างออกฤทธิ์โดยตรงต่อหัวใจ (heart) และเส้นเลือด (blood vessels) ยังลผให้ระดับความดันโลหิตลดลง ยากลุ่ม beta blocker ที่ถูกนำมาใช้ได้แก่ Atenolol (Tenormin), labetanlol (Trandate) และ bidoprolol (Zebeta) เป็นยาที่ถูกนำมา ใช้ในการรักษาโรคความดันโลหิตสูง ผลข้างเคียงที่เกิดจากการใช้ยาในกลุ่ม beta blockers คือ การทำให้ เกิดอาการเหนื่อยเพลีย, เกิดอารมณ์หดหู่ได้ และบางครั้งทำให้ความรู้สึก ทางเพศเสียไปได้ นอกเหนือจากนั้นยาในกลุ่ม Beta blockers ยังสามารถทำให้อัตรา การเต้นของหัวใจช้าลงไดเหมือนกับยาในกลุ่ม calcium channel Blockers เช่น verapamil (Isoptin) และ diltiazem (Cardizem) ดังนั้น ท่านไม่ควรใช้ยา Beta blockers ร่วมกับยากลุ่ม calcium Channel ที่มีชื่อว่า verapamil หรือ diltiazem เพราะมันสามารถ ทำให้ท่านเกิดอาการวิงเวียน (dizziness) และทำให้เป็นลม (fainting) ได้ ยากลุ่มที่สาม...(Angiotensin-Converting Enzyme Inhibitors) ยากลุ่มนี้ได้แก่ Angiotensin-converting enzyme inhibitors หรื ACEIs และ Angiotensin receptor blockers หรือ ARBs มันออก ฤทธ์ด้วยการยับยั้งการทำงานของฮอร์โมนชนิดหนึ่ง ชื่อ Renin ซึ่งเป็น ฮอร์โมนที่ทำให้ความดันโลหิตสูงขึ้น ยาในกลุ่มนี้ได้แก่ lisinopril (Zestril), enalapril (Vasotec),Valsartan (Diovan) และ olmesartan (Benicar) ข้อควรทราบ... การใช้ยา Angiotensin converting enzyme inhibitors สามาระทำให้เกิดผลข้างเคียง คือ ไอเรื้อรัง...เมื่อหยุดยา อาการจะหาย ในกรณีที่ท่านตั้งครรภ์ การใช้ยากลุ่มดังกล่าว สามารถทำให้เกิดมีอาการ บวม (angioedema) หรือมีอาการบวมอย่างรุนแรง...หากท่านทานยา ตัวนี้มาก่อน และท้องทีหลัง ต้องบอกให้แพทย์ได้รับทราบทันที...ต้องหยุดยาตัวนี้ทันที กลุ่มสุดท้าย (Alpha Agents) ยาในกลุ่มจะออกฤทธ์ลดระดับความดันโลหิต ด้วยการออกฤทธิ์โดยตรง ที่สมอง และเส้นเลือด (alpha receptors) ซึ่งมีบทบาทในการควบคุม ความดันโหลิต ยาที่ใช้ได้แก่.. clonidine (Catpres), soxazosin (Cardura), prazosin (Minipress) และ terazosin (Hytrin) การใช้ยาชื่อ Clonidine สามารถทำให้ท่านรู้สึกเหนือยเพลีย, ปากแห้ง และหากท่านหยุดยาตัวนี้ทันที สามารถทำให้ระดับความดันโหลิตเพิ่ม สูงขึ้นถึงขั้นอันตรายต่อชีวิตได้...ระวัง อย่าทำเป็นอันขาด ส่วนยาตัวอื่นๆ ( Doxazosin, prazosin และ terzosin) สามารถทำให้ เกิดอาการปวดศีรษะ (headache), ใจสั่น (palpitations) และเกิด อาการวิงเวียน (dizziness) โดยสรุป... ยาที่เรานำมาใช้ในการรักษา โรคความดันโลหิตลิต มีหลายชนิด เป็นต้น ว่า...Diuretics, beta –blockers & calcium channel blocers, Angiotnsin-converting enzyme inhibitors & Angiotensin receptor blockers และ alpha blockers.. ยาทุกชนิดต่างออกฤทธิ์แตกต่างกัน เพื่อลดระดับความดันโลหิต และใน ขณะเดียวกัน ก็มีผลข้างเคียงได้ หากเกิดขึ้นกับท่านผู้ใด ท่านต้องปรึก ษาแพทย์ และอย่าหยุดการรับประทานยาพละการ เพราะอันตรายจะเกิดขึ้นกับท่านได้
