Florida Blue Cross Shields Health Insurance MONEY "Team" Denied Doctors Prescription REVIEW

A medication my doctor had me prescribed on for over a year was turned down by Florida Blue's "clinical team" when I had to change health insurance companies once again like I had to the last 3 years with Healthcare.gov Obamacare. I originally had Cigna and Humana and I had the only horrific choice of choosing Florida Blue Select through the marketplace. I chose the $360 a month plan just for me because a prescribed medication my DR had me on was covered, or so I thought. When I went to my pharmacist they said that it had to be approved by FB. It took over a week and I had to call to find out it was not approved and that my doctor should choose a cheaper alternative. Needless to say I'm not happy and I"ll probably discontinue this awful plan. We have no options folks as it stands in this country.

When I consider purchasing an individual health insurance plan for myself or my family, do I have any financial obligations beyond the monthly premium and annual deductible? Answers: It depends on the plan, but some plans have the following cost-sharing elements that you should be aware of. Co-Payments: Some plans include a co-payment, which is typically a specific flat fee you pay for each medical service, such as $30 for an office visit. After the co-payment is made, the insurance company typically pays the remainder of the covered medical charges. Deductibles: Some plans include a deductible, which typically refers to the amount of money you must pay each year before your health insurance plan starts to pay for covered medical expenses. Coinsurance: Some plans include coinsurance. Coinsurance is a cost sharing requirement that makes you responsible for paying a certain percentage of any costs. The insurance company pays the remaining percentage of the covered medical expenses after your insurance deductible is met. Out-of-pocket limit: Some plans include an out-of-pocket limit. Typically, the out-of-pocket limit is the maximum amount you will pay out of your own pocket for covered medical expenses in a given year. The out-of-pocket limit typically includes deductibles and coinsurance. But, out-of-pocket limits don't typically apply to co-payments. Lifetime maximum: Most plans include a lifetime maximum. Typically the lifetime maximum is the amount your insurance plan will pay for covered medical expenses in the course of your lifetime. Exclusions & Limitations: Most health insurance carriers disclose exclusions & limitations of their plans. It is always a good idea to know what benefits are limited and which services are excluded on your plan. You will be obligated to pay for 100% of services that are excluded on your policy. Beginning September 23, 2010, the Patient Protection and Affordable Care Act (health care reform) begins to phase out annual dollar limits. Starting on September 23, 2012, annual limits on health insurance plans must be at least $2 million. By 2014 no new health insurance plan will be permitted to have an annual dollar limit on most covered benefits. Some health insurance plans purchased before March 23, 2010 have what is called "grandfathered status." Health Insurance Plans with Grandfathered status are exempt from several changes required by health care reform including this phase out of annual limits on health coverage. If you purchased your health insurance policy after March 23, 2010 and you're due for a routine preventive care screening like a mammogram or colonoscopy, you may be able to receive that preventive care screening without making a co-payment. You can talk to your insurer or your licensed eHealthInsurance agent if you need help determining whether or not you qualify for a screening without a co-payment. There are five important changes that occurred with individual and family health insurance policies on September 23, 2010. Those changes are: 1. Added protection from rate increases: Insurance companies will need to publically disclose any rate increases and provide justification before raising your monthly premiums. 2. Added protection from having insurance canceled: An insurance company cannot cancel your policy except in cases of intentional misrepresentations or fraud. 3. Coverage for preventive care: Certain recommended preventive services, immunizations, and screenings will be covered with no cost sharing requirement. 4. No lifetime maximums on health coverage: No lifetime limits on the dollar value of those health benefits deemed to be essential by the Department of Health and Human Services. 5. No pre-existing condition exclusions for children: If you have children under the age of 19 with pre-existing medical conditions, their application for health insurance cannot be declined due to a pre-existing medical condition. In some states a child may need to wait for the state's open-enrollment period before their application will be approved.

Anthem Blue Cross Blue Shield Vs. Aetna: Who is Better?

Next Step: http://hubs.ly/H01ndv70 Anthem and Aetna Policy and Plan Comparisons Now that we're done with the boring history lesson, let's start comparing these two health insurance heavy weights. It's possible Anthem is going for quantity over quality. They have 14 total plans (Inside and Out of the Marketplace) available for you to choose from in 2015. Bronze - 5 Silver - 7 Gold - 1 Platinum - 0 Catastrophic - 1 Aetna has the fewest number of plans of any health insurance company in Ohio with a grand total of five. Bronze - 2 Silver - 2 Gold - 1 Platinum - 0 Simple math will tell you Anthem is going to give you more choice. But having the most plans doesn’t make you the best. Just like having the least doesn’t make you the worst. It’s a matter if there’s one that works for you. Anthem and Aetna Doctor/Hospital Network Differences This is where both companies fall pretty far behind. There are other companies that haven’t let Obamacare get the best of them, like Anthem and Aetna. Those companies still offer a mostly unchanged network. Both Anthem and Aetna decided to significantly limit your choice of doctors and hospitals. Usually only leaving one major hospital network to choose from in your area. If they happened to pick your prefered hospital system, that might work. If not, than you might have some problems. You don't want to have to try and squint really hard to see a scenario where one of their plans make sense. The biggest risk you run with limited choice is not being able to see a top special if needed. As you'll see in the next section, if this could save you money, I might allow it. But, nine time out of 10 it's just not in the cards. Anthem and Aetna Pricing and Cost This is where things turn into a battle for last place. Both Anthem and Aetna have had little interest in offering competitive rates. There are companies like Medical Mutual and even InHealth Mutual that are cheaper. Also, don’t forget the network limitations both companies have. This has been the case for the last two years, with a few exceptions popping up here and there. Sometimes I would run a quote for a client and Anthem and Aetna would be in the ballpark, but rarely cheaper. If no major adjustments are made by either company in 2016 you can expect this trend to continue. The rate for 2016 should start to show up at the start of open enrollment on November 15.

With the changes of its current carriers, more than 95 percent of consumers will have at least two carriers to choose from and 82 percent of consumers will be able to choose from three or more carriers. Covered California also announced that quality providers such as Hill Physicians, UCSF Health and others are available in more plans for 2018

PPO Vs. HMO: What's the Difference and Which is Better?

This type of plan offers a higher level of reimbursements when you receive treatment from a “preferred” doctor or hospital. In less fancy insurance terms, you pay less. This is also known as your health insurance company’s network. These “preferred” providers offer their services to you at a cheaper rate if they’re in your insurance company’s network. You even have the ability to see non “prefered” doctors with this plan, they are just consider out-of-network doctors. PPO’s have dominated the health insurance landscape for the last 10-15 years. What Does HMO Mean? HMO stands for Health Maintenance Organization. This plan type works a little different from a PPO. Instead of being able to choose from a list of doctors at a wide array of facilities, you are strictly limited to doctors and facilities contracted with that health insurance company. The goal of this plan structure is to have a primary physician manage your care. The most famous example of an HMO structure is the company HealthSpan, formerly known as Kaiser Permanente. These plans generally are less expensive than a PPO plan, however that is in exchange for a limited selection of doctors and hospitals. Let’s Take a Closer Look So I’ll ask you again, how many doctors do you want to be able to see if something really bad happens? The answer to that question is the answer to your internal PPO vs. HMO debate. But if you want to go even deeper, here’s what you need to think about. When does a HMO plan Make Sense? This is a hard question to answer since it will always come down to personal preference. But, this is how I like to explain it to clients. If there is a HMO plan that has doctors close to your home that you like and trust and also has an adequate number of hospital facilities, that’s a decent first step. You also need to be able to save a considerable amount of money on your health insurance payment every month. Finding both of those together is the tricky part. I have yet to see a HMO plan that offers enough savings to consider recommending one to a client. When you only save a couple dollars every month, you have to ask yourself if it is really worth it to limit yourself that severely when it matters the most. I've never been able to answer yes to that question for a client.

An employer may limit what expenses are eligible under an HRA plan. H.R.A. definition from Healthcare.gov: Health Reimbursement Accounts (HRAs) are employer-funded group health plans from which employees are reimbursed tax-free for qualified medical expenses up to a fixed dollar amount per year. Unused amounts may be rolled over to be used in subsequent years. The employer funds and owns the account. Health Reimbursement Accounts are sometimes called Health Reimbursement Arrangements. F.S.A. - FLEXIBLE SPENDING ARRANGEMENT A Health Care Flexible Spending Arrangement (FSA) is an employer sponsored benefit that enables employees to set aside pre-tax dollars out of their paycheck to pay for eligible health care expenses. Monies put into the plan avoid both Federal Income Tax and FICA. 11/11/14 update - The Internal Revenue Service announced the following new benefit plan limits for 2015. In addition, the IRS just issued an important change to Section 125 Cafeteria Plans. We're notifying FSA clients about this change now. Flexible Spending Account (FSAs) Limits: Healthcare FSA: The annual maximum for Healthcare FSAs has increased from $2,500 to $2,550 for 2015. Dependent Care FSA: At this time, the IRS has not released information on contribution limit changes to these plans. Transit & Parking FSA: Contribution limits remain unchanged for 2015. The monthly limits are $250 for parking, $130 for transit and $20 for bicycle commuting. F.S.A. definition from Healthcare.gov: A Flexible Spending Account (also known as a Flexible Spending Arrangement) is a special account you put money into that you use to pay for certain out-of-pocket health care costs. You don’t have to pay taxes on this money. This means you’ll save an amount equal to the taxes you would have paid on the money you set aside. You can use funds in your FSA to pay for certain medical and dental expenses, including copayments and deductibles. FSAs are available only with job-based health plans. Employers may make contributions to your FSA. Ever wonder what makes an HMO plan different from a PPO plan? Or, what exactly is a Consumer Directed Health Plan (CDHP)? Here are some basic facts: When you sign up for an HMO plan, you select a Primary Care Physician (PCP) from a network of doctors. Your PCP is your first point of contact for most of your health care needs. HMO plans generally have lower premium costs than other types of plans. Like HMO, PPO plans feature a network of physicians and hospitals. With a PPO plan you do not have to choose a PCP. PPO premiums are usually higher than HMO plans; however, you can save money by receiving care from in-network providers. Consumer Directed Health Plans (CDHPs) often consist of a high-deductible plan, paired with a Health Savings Account. Premiums with CDHPs are often lower than other plans because you are responsible for a greater share of your health care costs. With an HSA, you can contribute to the account and use the money to cover a wide range of medical expenses. The provider networks on the carriers are different than their old provider networks. So, if you were on the Blue Cross PPO in prior years, the new Blue Cross EPO network or their new Blue Cross PPO network, whether it is On Exchange from Covered California, if you purchase a plan from there, or if it is Off Exchange, that you bought directly from Anthem Blue Cross, those provider networks are different and this is the same with basically all the carriers. Cigna uses a new network which is called Local Plus on their PPO Network, which is smaller than their old network. Blue Shield uses a new network. HealthNet uses a new network. They are all using new networks. So don't take for granted that your doctor is in the new provider network simply because they were in the old provider network. You need to check to make sure that your doctors are participating on your new insurance plan, if you have a new insurance plan, with the new network. And the networks can be different whether you purchase the insurance through Covered California, or On Exchange, or if you purchase the insurance directly from the insurance carrier, or Off Exchange. So, it's really an important point, and I just wanted to make sure that you are aware of it. When you talk to your health care provider, asking them, they may consider your plan. They may say, "Is it Obamacare?" Obamacare is basically all of the new health care, whether it's on or off the exchange and basically all the new reforms are, I guess, potentially considered Obamacare. So, be careful about definitions that are thrown at you by the providers. We had a lot of confusion with that, and just be wary of your health care providers. Wary in the sense that you want to make sure that they participate on your plan. Anyway, I just want to bring that up.
