Causes of knee pain may be related to: •Sports •Trauma •Inflammation or arthritis •Osteoarthritis Cartilage is a tough and flexible connective tissue that gives shape, support and provides a cushion between bones in the joints. Unlike other connective tissue, cartilages do not have its own blood supply, instead, it is composed primarily of water, chondrocytes, collagen, and proteoglycans, which produce and maintain a structural matrix giving cartilage tissue its form and function. Proteoglycans are composed of numerous glycosaminoglycans attached to a core protein. The predominant glycosaminoglycans in cartilage are chondroitin sulfate and keratin sulfate. Within the cartilage matrix, there are numerous prototype and monomers attached by link protein to a molecule of hyaluronic acid. This is all woven with collagen to form an elastic and compressible structure. Normal articular cartilage also known as hyaline cartilage provides a smooth crystal clear gliding surface to aid in the motion of the joint. The structure of hyaline cartilage is divided into four layers; superficial, middle, deep and calcified layer with the calcified layer representing a transition from articular cartilage to bone. This cartilage is designed to achieve and maintain proper function over the majority of a person’s life. If the cartilage is subjected to excessive weight, overuse, improper alignment or injury, it begins to wear away leaving bone to rub on bone. Articular cartilage does not have the ability to heal itself, however, there are several treatment options available to aid in the repair of injured cartilage. The knee is the most common joint in the body with cartilage injury issues. The knee joint is composed of femur, tibia, and patella. There are two types of cartilage in the knee joint, the articular cartilage and the meniscus, which is a shock absorbing cartilage. There are also important ligaments within the knee joint, the anterior and the posterior cruciate ligaments. The goal of treating cartilage injury is to reduce pain and improve the function of the affected area. Anti-inflammatory medications are commonly used to combat-injured cartilage but they can potentially cause cardiovascular or gastrointestinal toxicity. Damaged cartilage can also be treated with a series of injections. Steroid injections such as cortisone are powerful anti-inflammatory drugs used in the treatment of osteoarthritis. Hyaluronic acid is a substance produced naturally in the body and is present within cartilage when isolated. Hyaluronic acid is a thick viscous solution that can be added to the natural hyaluronic acid of the knee joint. This solution uses purified Hyaluronic acid derived from either rooster combs or genetically engineered cells. Glucosamine is another option for treatment that can be injected directly into the joint. Glucosamine is a modified sugar that is formed by the human body and is used to form larger molecules involved in the formation and repair of cartilage. Other methods include gene therapy, platelet-rich plasma, growth factors, stem cells, drilling and debridement of cartilage may help with injured cartilage. Occasionally the cartilage is replaced, regrowth and implanted. When the cartilage is damaged, the choice is often simple as the patient will need a total knee replacement. Many different types of designs and materials are currently used in total knee replacement surgery. Nearly all consists of three components, a patellar component, which consists of durable plastic, a tibial component, made of a durable plastic cushion often held within a metal platform and the femoral component, which is usually made of highly polished metal. Total knee replacement is the most predictable option to treat severe arthritis of the knee joint. When the arthritis is minimal, almost all treatment methods will work, however, when the extent of arthritis is moderate, there are many different treatment options presented, but none of them are predictable. It is logical to pursue and recognize the efficacy of platelets and stem cells in cartilage regeneration, simply because it is better for the patient to be able to regenerate their own cartilages and for it to be replaced with an artificial joint.

This bursa allows the kneecap to slide freely underneath the skin as we bend and straighten the knee. This bursitis causes pain, swelling, tenderness and a lump in the area on top of the kneecap. It may be very difficult to kneel down and put the knee on the floor due to tenderness and swelling. Type of knee bursitis: 1-Suprapatellar 2-Prepatellar (most common/popeye knee) 3-Infrapatellar 4-Pes anserine Causes: Trauma •Direct injury or fall onto the knee can damage the bursa with development of sudden large swelling. Occupational kneeling •Bursitis is chronic and develops slowly as seen in carpet layers, tilers, wrestlers, etc. Infection •Inspect the bursa for any breaks in the skin leading to infection. •Red, hot, painful and swollen bursa is a sign of possible infection. Inflammation Treatment of patellar bursitis includes: •Anti-inflammatory medication •Antibiotics if infection is suspected or confirmed •Ice therapy •Aspiration: send the fluid for culture and crystals. •Surgery: debridement & excision of the bursa. •Protective covering should be placed around the knee while avoiding activities that aggravate the condition.

I will also show you what it may mean about your general health and what you can do about it Let’s get straight into it No 1. Sudden Hair loss on the feet and toes What this might mean is you have Serious circulation problems You may think not having hair on your toes is a good thing especially during sandal season, but having hair on your toes is actually a good thing. Get it checked out Sudden baldness can be a sign that your feet aren’t getting enough blood flow to sustain hair growth. Expect your doctor to check for a pulse in your feet, which is an indication that your heart may not be able to pump enough blood to your feet. 2. You feel Frequent foot cramping What this may mean is your suffering from either Dehydration or nutritional deficiencies Randomly occurring cramps are extremely common in the feet so don’t get too worried. They can be as serious as circulation and nerve issues, or as harmless as a nutritional deficiency. If you’re exercising, be sure to drink plenty of water, since dehydration often leads to muscle cramping. You might also try upping your intake of potassium, magnesium, and calcium (with your doctor’s go-ahead, of course), since their deficiencies make cramps more common. Or you may like to try For relief, soaking your feet in a warm foot bath and stretching your toes toward your nose, not pointing down. Your podiatrist will probably tell you to ease up on walking at first, rethink your footwear, and adopt a good stretching routine. 8. Flaky, itchy, or peeling skin What this may mean: a Fungal infection Even if you’re never donned an athletic jersey in your life, you could still be walking around with athlete’s foot—which is basically a fungal infection. Psoriasis: Too Many Skin Cells In psoriasis, new cells build up in the top layer of your skin. They grow faster than your body can remove, or shed, them. Blood vessels below become swollen. This causes thick, red patches, or plaques. What they look and feel like depends on what type of psoriasis you have. Doctors aren't sure what causes it. They think problems with genes and the immune system play a role. Plaque Psoriasis This is the most common type. Patches of skin are red, raised and have silvery-white flakes, called scales. They usually show up on your scalp, elbows, knees, and lower back. They may crack and bleed and they feel sore and itchy. The more you scratch, the thicker they can get. A patch can be as wide as 4 inches; sometimes more. You can get it at any age, but it's more common in adults. Scalp Psoriasis About half the people with psoriasis have this type. It looks like dandruff, but it's not the same thing. Dandruff flakes are yellow and greasy. Scalp psoriasis is powdery and silver or white. Sometimes, the skin on the scalp is just a little bit crusty or flaky. This type can cover the whole head. It can also appear on your forehead, the back of your neck, and around your ears. Guttate Psoriasis Kids and young adults are more likely to get this type. Small red dots with raised edges appear suddenly, usually on the middle of your body. Other common sites are the arms, legs, scalp, ears, and face. Things that trigger this kind of psoriasis include strep throat, the flu, a cold, and other upper respiratory infections. About 1 in 10 people with psoriasis has it. Inverse Psoriasis These patches are shiny, bright red, and very sore. The area around them is usually smooth and doesn’t have silver scales. They appear only where skin touches skin, places called folds. Common areas are the armpits, groin, genitals, buttocks, under the breasts, and behind the knee. Rubbing and sweating can make the condition worse. Many people who have it also have another type of psoriasis. Pustular Psoriasis This rare type might be a reaction to an infection, stress, medications, or contact with certain chemicals. It causes red, swollen patches of skin with pus-filled bumps (called pustules). When these dry out, they turn yellow-brown and scaly. It usually shows up on the palms of your hands or the bottoms of your feet. The blisters may break open, leaving skin cracked and painful. Pustular Psoriasis: Emergency Symptoms This type can sometimes be life-threatening. Go to the hospital immediately if the bumps spread quickly all over your body. Other emergency symptoms are severe itching, rapid pulse, fever, muscle weakness, and chills. Doctors call this sudden form the von Zumbusch variant. Erythrodermic, or Exfoliative This rare form causes large areas of skin to turn bright red, like a bad sunburn, then fall off your body. Symptoms include extremely itchy and painful skin, rapid heartbeat and feeling very cold or hot. It's life-threatening, so go to the hospital. Causes include medications like corticosteroids, or untreated plaque psoriasis. It also affects people with von Zumbusch pustular psoriasis. Psoriatic Nail Disease About half of people with psoriasis also have skin cells build up under their nails, which become thick. They often split or crack. In severe cases, they could crumble or fall off. There may be brownish red or yellow spots underneath. Sometimes, the surface has tiny dents in it, like pin pricks. When they lift off the skin underneath (the nail bed), it's called onycholysis. Psoriatic Arthritis About one-third of people with psoriasis have joint pain, stiffness, and swelling. When both these problems show up, it's called psoriatic arthritis or psoriatic disease. The symptoms don't have to happen at the same time. Dry, red skin patches with silvery scales usually come first, but not always. Nail crumbling and color changes are common in people with this condition.

Rubbing alcohol, or Isopropyl Alcohol, can be found in most home medicine cabinets. It can be used to fight infections, clean your skin and pores of toxins, disinfect surfaces and clean objects. Here are the top 6 uses of rubbing alcohol: 1. Prevents Swimmer’s Ear Swimmer's ear happens when water gets trapped deep in the ear canal. It is a common condition for swimmers and it can be quite annoying, as trapped water can cause infection and lead to swimmer’s ear. You can prevent an infection from occurring with rubbing alcohol. Here’s how to prevent swimmer’s ear: Mix equal parts of rubbing alcohol and white vinegar. Fill an ear dropper with the solution. Lie down with the affected ear facing upward. Put 2 to 4 drops into the ear and wait 10 minutes. Tilt your ear in the opposite direction to allow drainage of any excess liquid. Repeat as needed. 2. Cleans Minor Cuts and Abrasions Rubbing alcohol can also be used to clean minor cuts or scrapes because of the natural germ-fighting characteristics. It can disinfect the area and kill any bacteria to prevent infection. Rubbing alcohol can also help to stop bleeding in the cuts by tightening your capillaries, which are what are losing blood in your body. The next time you have a minor cut or scrape, try: Apply some rubbing alcohol to the affected area using a cotton ball. Leave it on for several minutes. As rubbing alcohol can dry your skin, apply some moisturizing lotion or simply aloe vera gel on the area after you are done. Do this several times a day for several days. 3. Prevents Acne and Pimples Rubbing alcohol can even help to treat and prevent acne and pimples by killing the germs, soothing your skin, removing any dirt and unclogging your pores. Be sure to begin this method as soon as you notice the first signs of a breakout occurring: Soak a cotton swab in rubbing alcohol (70% strength). Dab it on the affected area gently. Leave it on for 10 minutes. Rinse the affected area with lukewarm water. You can also follow up with a light moisturizer or just aloe vera gel. Do this twice daily for a couple of days only. 4. Treats Toenail Fungus Because of its ability to fight germs and infections, rubbing alcohol is effective in fighting toenail fungus. Rubbing alcohol can help keep your skin dry, so it does also prevent fungus from growing in the first place, as fungi tend to grow and spread in warm, moist environments. Dr. Ebraheim’s educational animated video describes knee pain examination of the meniscus. There is two types of cartilage between the femur and tibia: • Articular cartilage • Meniscus cartilage And there is a meniscus in between. The meniscus is a fibrocartilage structure that carries type I collagen, it is triangular shaped in cross section. There are two menisci: • Lateral meniscus:more mobile that the medial meniscus - Circular - Covers about 70% of the lateral tibial plateau • Medial meniscus: its tears occur about three times more than that of the lateral meniscus - C-shaped - Covers about 50% of the medial tibial plateau Function: • Shock absorption • Load sharing • Joint stability Meniscal tear causes: • Twisting, jumping, or changing directions during sports activities such as skiing or football. • Degenerative tears of the medial meniscus may occur in older patients. • Degenerative tears may occur in association with arthritis. Symptoms of the meniscal tear: - Pain on the medial or lateral side of the knee Mechanical symptoms: - Locking - Clicking - Swelling Examination: Joint line tenderness is the most sensitive exam; posterior knee pain may also be present when the knee is bent. Look for the knee joint effusion, effusion may be difficult to find and occur several hours to appear after the injury. With ACL tears, the hemorrhage and swelling is greater and develops rapidly. Mcmurray’s test is used to diagnose meniscal tear, a painful pop or click is obtained as the knee is brought from flexion to extension with either internal or external rotation of the knee. To test the medial meniscus, flex the knee and place the hand on the medial aspect of the knee. The knee is then extended in order to test the medial meniscus. Feel a pop or click with knee extension, the patient will experience pain. A positive test is indicated by pain, clicking, or popping within the joint and may signal a tear of the medial meniscus. To diagnose the lateral meniscus tear, do the Mcmurray’s test with internal rotation of the knee. Sometimes the knee is locked with lack of full extension due to a bucket handle tear of the meniscus. Differential diagnosis: • The clinical diagnosis accuracy of meniscal tear is about 70%. • MRI is usually done to confirm the diagnosis or identify other problems in the knee. • The differential diagnosis can be intra-articular problems or extra-articular problems. • Intra-articular problems: - Medial synovial plica irritation. - Osteochondritis lesions (OCD). - Patellofemoral pain - Loose bodies • Extra-articular problems: - Collateral ligament injury, especially the medial collateral ligament - Pes anserine bursitis - Lumbar disc herniation - Stress fracture - Iliotibial band syndrome - Slipped capital femoral epiphysis (SCFE). Blood supply: The meniscus receives blood supply from the genicular vessels and its capsular attachment. The peripheral 1/3 area is the most vascularized area and tears within this area usually heal. Where is the tear located? Treatment of meniscal tears: • Nonoperative: - Especially for small nondisplaced tears and degenerative tears. - Physiotherapy - NSAIDS - With persistence of symptoms, cortisone injection may be given. - When the conservative treatment fails, then do surgery. • Surgery: - Excision of the tear (partial meniscectomy): you do excision if the tear is complex, degenerative, or if it is a radial tear that cannot be repaired. - Repair of the meniscal tear: usually done for a peripheral tear which is vascular and will heal. It is better to have the combination of repair of the meniscus at the same time you do ACL reconstruction (controversial). - Meniscal transplant: done in younger patients who had total meniscectomy (especially lateral meniscus). It takes about one year for the graft to heal. Re-tear of the transplanted meniscus is also common. If you do total meniscectomy, this will probably lead to future arthritis of the knee. Special situations: • In general, medial meniscal tears occur more often than tear of the lateral meniscus. - In older patients, the posterior horn of the medial meniscus is usually more often affected. • With ACL tears that are acute, then there will be more incidences of lateral meniscal tears. • With ACL tears that are chronic, then there will be more incidences of medial meniscal tears. • With tibial eminence fractures in children, the medial meniscus can become trapped in this injury. • Tibial plateau fractures: - If the injury is medial then it can affect the medial meniscus. - If the injury is lateral then it can affect the lateral meniscus. • If you have depression or displacement separation more than 5 mm, you can have meniscal injury. Discoid meniscus: the meniscus is usually larger than normal and occurs more in the lateral meniscus, the treatment is usually saucerization plus or minus repair of the lateral meniscus if symptomatic.

Today I am here to talk to you about massage for dog arthritis. I get questions all the time "Dr. Smith, how can I help my dog at home with arthritis?" What can I do to alleviate his pain?" Besides giving a medication that I instruct and tell people to use, and following the techniques I tell people to use, another thing you can do is massage your dog. Today Becki and I are going to teach you and show you some tips on the best way to alleviate your dog's pain through massage. All right, meet Stella. Stella is not the prototype that you will be performing the massage on, because Stella is a young dog. However, she is very laid back and she's a great dog to illustrate our point of how to massage your dog. The first place we are going to focus on is the stifle. The stifle is otherwise known as the knee in the dog. You can see Becki's hands, she's right in the area of the stifle. The stifle is a common place for arthritis because of cranial cruciate rupture or ACL tears in the dog. Becki is performing a variety of techniques. She is using her fingers to massage the muscles and joints, and now she is using her knuckles to apply pressure over a wider area. She can also use her palm, and Becki can also take that joint through it's full range of motion by flexing and extending the area. Now occasionally, kind of like Stella--she doesn't like that--and that's OK. So what we do in those situations is that we take the joint right to the point where it hurts and we hold it there. Hold it for 10 seconds, and let it sit there. What that does is breaks down a lot of the adhesions that form, either from a surgery, from arthritis, from trauma to the leg, or from any other reason that the leg is not utilized and used and extended and flexed through its full range of motion. So that is the stifle region. The next region we are going to go to is the hip. The hip is a unique joint in the dog. Becki, show us where the hip is. OK, there's the hip, right in that area. The hip is the place where the femur,which is the long bone of your thigh, attaches to the pelvis). This is kind of a socket joint, where there is a ball that sits in a socket, kind of like where a light bulb sits in a light fixture. And so there's a couple of different ways to massage this joint. As you'll see Becki doing right now, she is actually using her fingers to massage the whole area of the hip. See it going around and around and around. She's also using her palm to apply pressure evenly over the area. The hip is unique because it's a joint that moves in 360 degrees. She's now taking that hip through it's full range of motion, and every once in a while this will cause dogs... they'll resent this. Especially an older dog that has hip arthritis. But this is good because it keeps the muscle supple, it keeps the muscles going through their range of motion, and it helps to strengthen the muscles and ultimately alleviate pain. Now she can do that in that direction, or she can take it and do it in the other direction. She can also do it by flexing it and extending the stifle at the same time. Now this is a little bit more vigorous exercise and can cause more pain or objection, but it's a good exercise to keep that leg rolling and to keep that leg moving, and to hopefully eliminate pain through massage. The next area we are going to move on to is the back. Becki, can you show us where the back is? The back, if you feel down the center of your dog's back, you will feel some small, little protuberances or little points, and those are the dorsal spinus process of the vertebral bodies. On each side of those little points are two muscles, and lots of times those muscles will get tight, they'll get painful they'll get inflamed when a dog has back arthritis. So we can actually massage those two areas, and right now Becki is running down the length of those two muscles, putting pressure--about as much pressure you would put if you were trying to draw a line in the sand, let's say, with your fingers. However, each type of psoriasis has very distinct symptoms and characteristics and can appear on the skin in a variety of ways. Knowing which kind of psoriasis you have helps you and your doctor make a treatment plan. Here's how you can spot the 7 types of psoriasis. Plaque Psoriasis Plaque psoriasis is the most common type and causes raised, inflamed, red skin covered with silvery, white scales which may itch and burn. They can appear anywhere on your body but often pops up in these areas: elbows, knees, scalp and lower back. Guttate Psoriasis This type often starts in children or young adults and causes small, pink-red spots on your skin. They often appear on your trunk, upper arms, thighs, and scalp. Inverse Psoriasis Inverse Psoriasis is found in the armpits, in the groin, under the breasts, and in other skin folds around the genitals and the buttocks. This type of psoriasis appears as bright-red lesions that are smooth and shiny. Pustular Psoriasis Primarily seen in adults, pustular psoriasis is characterized by white blisters of noninfectious pus surrounded by red skin. It may either be localized to certain areas of the body, such as the hands and feet, or covering most of the body. Erythrodermic Psoriasis This type is the least common, but it's very serious and affects most of your body and causes widespread, fiery skin that appears burned. You might also have: severe itching, burning, or peeling a faster heart rate and changes in body temperature. Nail Psoriasis Up to half of those with psoriasis have nail changes and this is even more common in people who have psoriatic arthritis, which affects your joints. Common symptoms include: pitting of your nails, tender, painful nails, color changes (yellow-brown) and chalk-like material under your nails. Psoriatic Arthritis This is a condition where you have both psoriasis and arthritis (joint inflammation). Cinnamon Because cinnamon helps bring down swelling and has antioxidants, it is great for easing the pain from arthritis. You can also make cinnamon and honey into a paste and massage it gently on the painful area. Fish Oil Fish oil has omega-3 fatty acids that help stop swelling in the body and ease pain. Alfalfa Alfalfa can also help people with arthritis, because it is full of minerals and helps even out acid levels in the body. Symptoms vary on the type you have, but joint pain, tenderness and stiffness, inflammation in and around the joints, restricted movement of the joints, warm, red skin over the affected joint, and weakness and muscle wasting, are the usual signs. There are more than 100 types of arthritis and they all have risk factors, individual features, behaviours and circumstances that are associated with the condition, according to Arthritis Foundation. It states: “There are risk factors that are not modifiable. That means there is nothing you can do about them. Being female and having a family history of arthritis are two examples of factors that make people more likely - but not certain - to get some types of arthritis. ”But in contrast, some risk factors are considered to be modifiable. So how can you prevent rheumatoid arthritis? The nonprofit organisation advises that to prevent rheumatoid arthritis you should not smoke. Osteoarthritis, another comment type of arthritis, can be prevented by keeping a healthy weight. You can also reduce the risk of gout by eating a healthy diet, low in sugar, and alcohol. There’s no cure for arthritis, but the NHS lists prescribed medications under treatment, which includes painkillers, non-steroidal anti-inflammatory drugs, and corticosteroids. Some health experts recommend more natural remedies to ease arthritis pain, such simple changes to diet, as well as exercise. Soaking in warm water is also one of the oldest forms of alternative therapy.“Research shows our ancestors got it right. It makes you feel better. It makes the joints losers. It reduces pain and it seems to have a somewhat prolonged effect that goes beyond the period of immersion,” Bruce E. Becker, director of the National Quatics & Sports Medicine Institute at Washington State University, told Arthritis Foundation. Soaking in warm water works. It reduced the force of gravity that’s compressing the joint, offers 360-degree support for sore limbs, can decrease swelling and inflammation and increase circulation. But how long should you soak in a warm bath for?Dr Becker added that patients he’s studied seem to reach a maximum benefit after about 20 minutes. You should also make sure you drink water before and afterward to stay well hydrated. Arthritis Foundation recommends that the temperature of the water be between 33.3 degrees Celsius and 37.7 degrees Celsius. If you have cardiovascular problems you should beware of water that’s too hot as it can put stress on the heart. It adds: “Warm water is great for relaxing, but it is also good for moving.Adding this fish dinner to your diet could reduce signs of rheumatoid arthritis or osteoarthritis.
