Anal cancer: Causes Causes In anal cancer, a tumor is created by the abnormal and uncontrolled growth of cells in the anus. The anus is the area at the very end of the gastrointestinal tract. The anal canal connects the rectum to the outside of the body. It is surrounded by a muscle known as the sphincter. The sphincter controls bowel movements by contracting and relaxing. The anus is the part where the anal canal opens to the outside. The anal canal is lined with squamous cells. These flat cells look like fish scales under the microscope. Most anal cancers develop from these squamous cells. Such cancers are known as squamous cell carcinomas. The point at which the anal canal meets the rectum is called the transitional zone. The transitional zone has squamous cells and glandular cells. These produce mucus which helps the stool, or feces, pass through the anus smoothly. Most anal cancers are squamous cell carcinomas, but adenocarcinoma can also develop from the glandular cells in the anus. Risk factors Multiple risk factors have been studied that are linked to anal cancer. They include any or a combination of the following: Human papilloma virus (HPV): Some types of HPV are closely linked to anal cancer. Around 79 percent of people with anal cancer have HPV 16 or 18, and 8 percent have other types of HPV. Multiple sexual partners: This activity increases the risk of contracting HPV, which, in turn, increases the risk of anal cancer, which is a known risk factor. Receptive anal intercourse: Men and women who receive anal intercourse have a higher risk of developing anal cancer. Men who are HIV-positive and who have sex with men are up to 90 times more likely to develop anal cancer, compared with the general population. Other cancers: Women who have had vaginal or cervical cancer, and men who have had penile cancer are at higher risk of developing anal cancer. This is also linked to HPV infection. Age: Anal cancer, like most cancers, are more likely to be detected at an older age. A weakened immune system: People with HIV or AIDS and those who are taking immunosuppressant medications after a transplant are at greater risk. Smoking: Smokers have a significantly higher risk of anal and other cancers than non-smokers. Benign anal lesions: Irritable bowel disease (IBD), hemorrhoids, fistulae, or cicatrices have been linked to anal cancer. Inflammation resulting from benign anal lesions may increase the risk. Chemotherapy and radiotherapy Most patients will probably need chemotherapy, radiation therapy, or both. Radiation therapy may be combined with chemotherapy to destroy anal cancer cells. Treatments may be given together or one after the other. This approach increases the chance of retaining an intact anal sphincter. Survival and remission rates are good. Chemotherapy uses cytotoxic drugs that prevent the cancer cells from dividing. They are given orally or by injection. Radiotherapy uses high-energy rays that destroy the cancer cells. Radiation can be delivered internally or externally. Radiotherapy and chemotherapy have adverse effects, and combining them may make the side effects more acute. Side effects may include: diarrhea or constipation soreness and blistering around the target area, which is the anus a higher susceptibility to infections during treatment fatigue loss of appetite nausea or vomiting mouth ulcers or sore mouth loss of hair narrowing and dryness of the vagina a low white blood cell count, increasing the risk of infection anemia, due to a low red blood cell count a low platelet count, raising the risk of bruising or bleeding dry skin rashes muscle and nerve problems excessive coughing and sometimes breathing difficulties fertility problems

Anal cancer: Symptoms and Signs Anal cancer occurs in the anus, located at the end of the gastrointestinal tract. It is different from and less common than colorectal cancer, which is cancer of the colon or rectum. Anal cancer is rare, but the number of new cases is rising. According to the American Cancer Society, in 2017, there are likely to be about 8,200 new cases, of which 5,250 will affect women and 2,950 will affect men. Around 1,100 people are expected to die from anal cancer, including 650 women and 450 men. Various risk factors are linked to anal cancer, but infection with two types of the human papilloma virus (HPV) appear to underlie 79 percent of cases. Anal cancer is rare before the age of 35 years. The average age of diagnosis is in the early 60s. Men have a 1-in-500 chance of getting anal cancer, and the risk is slightly higher in women. Symptoms and signs Common symptoms of anal cancer may include rectal bleeding noticeable if there is blood on feces or toilet paper pain in the anal area lumps around the anus, which may be mistaken for piles, or hemorrhoids mucus or jelly-like discharge from the anus anal itching changes in bowel movements, including diarrhea, constipation, or thinning of stools fecal incontinence, or problems controlling bowel movements bloating women may experience lower back pain as the tumor presses on the vagina women may experience vaginal dryness Outlook Cancer is diagnosed according to its stage. Treatment options and outlook depend on the stage at which it is diagnosed. A common way of staging cancer is from 0 to 4. In stage 0, the cancer cells are only in the top layer of the anal tissue. This is also known as Bowen disease. At stage 4, the cancer has spread throughout the body. The chance of surviving another 5 years or more after diagnosis depends on the stage and on the type of cancer. The American Cancer Society gives the following 5-year survival rates: It is important to remember that these figures are based on average rates from the past. As medicine progresses, the expectation is that treatments are increasingly effective, especially if the cancer is diagnosed at an early stage. The outcome also depends on a person's age and health condition apart from the cancer. Treatment Treatment for anal cancer will depend on various factors, including how big the tumor is, whether or not it has spread, where it is, and the general health of the patient. Surgery, chemotherapy, and radiation therapy are the main options. Surgery The type of surgery depends on the size and position of the tumor. Resection The surgeon removes a small tumor and some surrounding tissue. This can only be done if the anal sphincter is not affected. After this procedure, the person will still be able to pass a bowel movement. Abdominoperineal resection The anus, rectum and a section of the bowel are surgically removed, and a colostomy will be established. In a colostomy, the end of the bowel is brought out to the surface of the abdomen. A bag is placed over the stoma, or the opening. The bag collects the stools outside the body. A person with a colostomy can lead a normal life, play sports, and be sexually active.

A rectal examination This may be uncomfortable, but it is not usually painful. The doctor may use a proctoscope, anoscope, or sigmoidoscope to examine the area in more detail. The examination will determine whether a biopsy is needed. A biopsy A small sample of tissue is taken from the anal area and sent to the lab for testing. Tissue will be examined under a microscope. If the biopsy reveals cancerous tissue, further tests will be done to find out how large the cancer is and whether it has spread. A CT, MRI, or ultrasound scan can help confirm the results. This may be a rectal ultrasound, where the instrument is inserted into the anus. Prevention Anal cancer is rare, but some recommendations can help reduce the risk further. These include: reducing the chance of being infected with HPV through vaccination use condoms when having sex limiting the number of sexual partners abstaining from anal intercourse not smoking Most cases of anal cancer can be diagnosed early. However, if symptoms start higher up in the anal canal, this might not be possible. People who have a higher risk should consider screening for anal cancer. This includes women who have had vulvar or cervical cancer, known HPV infection, anyone who is HIV positive, those with a history of anal warts, and recipients of an organ transplant. It is used for the early detection of anal cancer. Some types of human papillomavirus (HPV) can cause anal cancer. Other HPV types cause anogenital warts. Cigarette smokers, men who have sex with men, individuals with a history of immunosuppression (such as in HIV infection) and women with a history of cervical, vaginal and vulval cancer are at increased risk of getting anal cancer. Vaccination against HPV before initial sexual exposure can reduce the risk of anal cancer. There is no consensus on who should get an anal Pap smear. Some individuals recommend that all men and women who have anal sex should have an anal Pap smear performed regularly. Some recommend it for all men who have had sex with men, for all individuals with HIV and anal warts, or for all individuals with a history of anal warts. Cost-effectiveness studies have reported conflicting conclusions, due to incomplete understanding of the natural history of anal HPV infection and lack of clarity of the efficacy of interventions. An informal survey of local infectious-disease doctors in southeast Michigan suggested that few know where to send patients for an anal Pap smear. The procedure is very simple, and can be easily performed in any doctor's office with a little training. Typically, a small brush or Dacron-tipped rod is inserted into the anus. The cells collected by the brush or rod are smeared onto a glass slide, air-dried, and sealed with an adhesive. Many medical offices prefer to suspend the collected cells in a liquid medium; the suspension is then processed in a laboratory before being plated on a glass slide. The specimen is sent to a pathologist, indicating the source of the smear (anal canal). The liquid in which the cells are suspended can also be used for HPV typing. The procedure is easily performed in a doctor's office, using the same kit as for cervical cancer detection. It can be performed quickly, as a vaginal speculum or anoscope is not required. The sensitivity and specificity of anal Pap smears are lower than those of their cervical counterparts. This means that a single negative anal Pap smear does not prove that the person is cancer-free, and a positive result does not prove that the person has cancer. There are currently no agreed algorithms for the investigation of abnormal anal Pap smear results. However, referral for High Resolution Anoscopy (HRA) is often considered, if available. HRA allows microscopic visualization and biopsy of potentially abnormal areas. Treatment decisions are then made on the basis of the biopsy results. A person might be referred to a colorectal surgeon, an infectious disease doctor, or a physician trained in colposcopy, examination, or biopsy of this region. At a minimum, a digital rectal exam is performed. A proctoscopic exam might follow. Ultimately, a biopsy might be performed, with or without the aid of a colposcope, a dermatoscope, or high-resolution anoscopy.

Much of the information about HPV virus (human papillomavirus) centers on women, since having the virus increases their risk of getting cervical cancer. But HPV virus in men can cause health problems, too. So it's important for men to understand how to reduce the risks of HPV infection. It can increase a man's risk of getting genital cancers, although these cancers are not common. HPV can also cause genital warts in men, just as in women. More than half of men who are sexually active in the United States will have HPV at some time in their life. Often, a man will clear the virus on his own, with no health problems. Risks of HPV in Men. Some of the 30 or so types of HPV associated with genital cancers can lead to cancer of the anus or penis in men. The risk of anal cancer is about 17 times higher in sexually active gay and bisexual men than in men who have sex only with women. Men who have HIV (human immunodeficiency virus) are also at higher risk of getting this cancer. Other types of HPV virus rarely cause cancer in men, but they do cause genital warts. At a given point in time, about 1% of sexually active men in the U.S. will have genital warts. Male HPV in men: The Symptoms of hpv in men. The types of high-risk HPV in men that can cause cancer rarely present any symptoms in men or in women. Genital warts are the first symptom you may see with low-risk HPV strains that cause warts but not cancer. Tests for HPV in Men. To diagnose genital warts in men, the doctor will visually check a man's genital area to see if warts are present. Some doctors will apply a vinegar solution to help identify warts that aren't raised and visible. But the test is not foolproof. Sometimes normal skin is mistakenly identified as a wart. There is no routine test for hpv in men to check for high-risk HPV in men strains that can cause cancer. However, some doctors are urging anal Pap tests for gay and bisexual men, who are at higher risk of anal cancer caused by HPV in men. In an anal Pap test, the doctor collects cells from the anus, and then has them checked for abnormalities in a lab to check hpv in men. Treatments for HPV in Men. There is no treatment for asymptomatic HPV in men infection. Instead, doctors treat the health problems that are caused by the HPV virus in men. When genital warts in men (hpv) appear, a variety of treatments can be used for hpv in men. The patient can apply prescription creams at home. Or a doctor can surgically remove or freeze off the hpv warts. Early treatment of hpv warts in men is discouraged by some doctors because genital warts in men can go away on their own. It can also take time for all hpv warts to appear. So a person who treats hpv warts as soon as they appear may need another treatment later on to treat hpv in men. Anal cancer which is caused by hpv in men can be treated with radiation, chemotherapy, and surgery. HPV is the most common of all STIs. Men and women who are sexually active are vulnerable to complications of HPV, including genital warts. Treatment is key in managing this infection. What are the symptoms of genital warts? Genital warts are transmitted through sexual activity, including oral, vaginal, and anal sex. You may not start to develop warts for several weeks or months after infection. Genital warts are not always visible to the human eye. They may be very small and the color of the skin or slightly darker. The top of the growths may resemble a cauliflower and may feel smooth or slightly bumpy to the touch. They may occur as a cluster of warts, or just one wart. Genital warts on males may appear on the following areas: penis, scrotum, groin, thighs, inside or around the anus. Genital warts in females may appear on the following area: inside of the vagina or anus, outside of the vagina or anus, cervix. Genital warts may also appear on the lips, mouth, tongue, or throat of a person who has had oral sexual contact with an infected person. Even if you cannot see genital warts, they may still cause symptoms, such as: vaginal discharge, itching, bleeding, burning. If genital warts spread or become enlarged, the condition can be uncomfortable or even painful. What causes genital warts? Most cases of genital warts are caused by HPV. There are 30 to 40 strains of HPV that specifically affect the genitals, but just a few of these strains cause genital warts. The HPV virus is highly transmittable through skin-to-skin contact, which is why it’s considered an STI. In fact, HPV is so common that the Centers for Disease Control and Prevention (CDC) says that most sexually active people get it at some point. However, the virus doesn’t always lead to complications such as genital warts. In fact, in most cases, the virus goes away on its own without causing any health problems. About 14 million new hpv cases are added each year. For many, the infection will go away on its own. In rare cases, HPV is a potentially serious risk factor for certain kinds of cancer. What are the symptoms of HPV in men? There are more than 100 types of HPV. Approximately 40 types are sexually transmitted. Each HPV type is numbered and categorized as either a high-risk or low-risk HPV. Low-risk HPVs can cause warts. They generally produce little to no other symptoms. They tend to resolve on their own without any long-term effects. High-risk HPVs are more aggressive forms of the virus that may require medical treatment. Sometimes, they can also cause cell changes that may lead to cancer. Most men with HPV never experience symptoms or realize that they have the infection. If you have an infection that won’t go away, you may begin to notice genital warts on your: - penis. - scrotum. - anus. Warts may also occur on the back of your throat. Cervical cancer is the most common HPV-related cancer in women. Throat cancer is the most common HPV-related cancer in men. How is HPV in men diagnosed? Due to the high correlation between cervical cancer and HPV, much effort has gone into creating tools to diagnose HPV in women. Currently, there are no approved tests to detect HPV in men. Some people may carry and possibly spread the virus for years without ever knowing. If you do notice any HPV-related symptoms, it’s important to report them to your doctor. You should see your doctor immediately if you notice any abnormal skin growths or changes in your penile, scrotal, anal, or throat areas. These may be early signs of cancerous growths. Treating HPV in men: There’s currently no special cure for HPV in men. However, most health problems that are caused by HPV are treatable. If you develop genital warts, your doctor will use a variety of topical and oral medications to treat the condition. Left untreated, polyps may become cancerous. Several factors increase your risk of developing colon and rectal cancer, including aging (above 50 years), some types of bowel diseases, family history, obesity, smoking, excessive alcohol intake, a sedentary lifestyle, Type 2 diabetes and regular intake of processed foods or red meats. African-Americans are at a greater risk of colon cancer than people of other races. As it can be difficult to treat colon cancer after it spreads to nearby areas, it is important to know what the early symptoms are. This can help you seek early treatment and give you a better chance in recovery. Here are the top 10 warning signs of colon cancer you shouldn’t ignore. 1. Constipation Constipation is an important sign of cancer in the colon. A 2011 study published in the Asian Pacific Journal of Cancer Prevention highlights the link between constipation and colorectal cancer risk. An earlier 2004 study published in the European Journal of Cancer supported the hypothesis that constipation or laxative use increases the risk of colon cancer. A tumor present at the far end of the colon can make it very difficult to eliminate waste products, thereby causing constipation. If you persistently have fewer bowel movements per week, without any prior problem of constipation, consult your doctor to find out the exact cause. 2. Diarrhea If you suffer from diarrhea for more than a couple of weeks, it may be an early symptom of colon cancer. When a tumor partially obstructs the bowel, it can cause alternating constipation and diarrhea due to leakage of liquid stool. You may also experience frequent gas, abdominal pain, nausea and vomiting. Plus, a tumor may irritate or narrow the lining of the intestine. It is important to consult your doctor when you have diarrhea that lasts more than a few days, as it can lead to dehydration, drain your body of nutrients and signal other serious problems, such as cancer. 3. Blood in Stools Most often, blood in the stool is due to piles (hemorrhoids), where the veins in the back passage become fragile and cause a little bleeding during a bowel movement.
