On average, it’s a 28-day cycle. 1) FOLLICULAR Phase - Day 1 to Day 14 - Go hard! Do high intensity workouts, lift heavy and perform total body strength training. - Your body is more tolerant to pain and muscles recover quicker. - Your body uses Carbs as its main source of fuel. - You can increase your carb intake slightly especially after an intense workout. 2) LUTEAL Phase - Day 14 to Day 28 - I like to call this phase the “roller coaster” phase. - this is when all the PMS symptoms start to hit: you might crave for sugar or high fat food, you have bigger appetite, you feel sluggish, you have trouble sleeping, your body retains more water, you feel bloated and you might suffer from mild cramps too. - Your body turns to FAT AS FUEL instead of carbs. - Good news, your body is now burning FAT rather than carbs or glycogen. Focus on steady pace cardio to get the most out of your workout. - This is also when the muscle breakdown increases, hence it takes longer for you to recover from your workout. So choose moderation workout that’s less intense. - You might lose your motivation BUT power through your workout, as much as possible, eat well and you WILL feel better. We are all very different and it takes time to really understand your body. If you have this knowledge, you’re able to take advantage of the hormonal benefits and overcome the challenges by adjusting and changing your workout routine and also food intake. Again, remember this is a guideline as there is very limited research in regards to training with your menstrual cycle. Polycystic Ovary Syndrome(PCOS) is the most common female endocrine disorders and is now recognized to be a Metabolic Syndrome.5% to 10% of women in their reproductive age suffers from this disorder. Women with PCOS have a number of conditions that may include irregular menstrual cycles, an increase in facial and body hair,increased weight, hyperinsulinemia and infertility. Diagnosis is made on basis of clinical,laboratory and ultrasound findings. PCOS is diagnosed based on the inability to release an egg from the ovaries on a regular(monthly) basis,increased male hormone levels and/or an increase in hair in the midline of the body(hyperandrogenism), and polycystic appearing ovaries on ultrasound. Lack of ovulation in women with PCOS results in continuous exposure of their uterine lining (endometrium) to estrogen. This may cause excessive thickening of the uterine lining and excessive bleeding. The bombardment of estrogen without ovulation may lead to eventual uterine cancer or pre-cancer. Metabolic syndrome along with hyperinsulinemia is more common in obese women with PCOS. This condition is characterized by abdominal obesity, cholesterol abnormalities,hypertension and insulin resistence/diabetes mellitus.Each of these increases the risk of heart disease.Obesity is noted in over 70% of women with PCOS. Diet and exercise that result in weight loss improve the frequency of ovulation, enhance the ability to get pregnant,lower the risk of Diabetes and lower androgen levels in many women with PCOS. A majority of women with PCOS have decreased sensitivity to insulin due to increased weight. Higher levels of Insulin are needed to keep sugar levels under control in overweight PCOS patients. The resulting high levels of Insulin may contribute to excessive production ofmale hormones(such as testosterone) and can lead to problems with ovulation. Regular exercise, weight loss and improved nutrition are very important. Behavioural change should be the first line of therapy for an overweight woman with PCOS. Women with PCOS are at a greater risk of complications during pregnancy , including miscarriage, diabetes(gestational diabetes),preterm birth, and increased fetal and neonatal death. Before getting pregnant, doctors have a few suggestions for women to make sure their health is in good shape. Dr. Kevin Fleischman, an OB/GYN on the medical staff of Lee Health, says keeping your health on track is important, especially before pregnancy. “Obesity is a big problem. There’s an increase of obese people and people with a higher BMI are now getting pregnant and these are high risk pregnancies.” He advises women to consider their health first before getting pregnant. “We do like women to be as healthy as they can at the beginning of pregnancy, so for instance if someone is overweight we encourage them to exercise and lose weight before they conceive, that way they’ll have a much healthier pregnancy.” Maintaining a healthy weight before pregnancy will prevent problems during pregnancy like high blood pressure, gestational diabetes, preeclampsia, and hypertension. It will also lower the mother’s risk for a C-section. “Gestational diabetes can cause big babies, and the scariest things we deal with is called shoulder dystocia which means the head comes out but the shoulders get stuck because the stomach and the shoulders are out of proportion larger than the head. And that can cause a lot of problems for the baby so we like very good blood sugar control throughout the pregnancy,” said Dr. Fleischman. Before getting pregnant, he recommends women develop healthy eating habits and regular physical activity. Also, monitor your blood pressure and cholesterol. “Sometimes it just takes exercising and losing a few pounds and sometimes it’s three or four percent of their body weight,” said Dr. Fleischman. Having a healthy body weight before pregnancy can help mothers have a healthy pregnancy and delivery.

Normally it depends on a hormone called LH or a Luteinizing Hormone which is secreted by the pituitary which gives a signal to the ovary to release the ovum. This LH increases slightly just before the ovulation. Once this increase happens the ovulation happens after 24 to 36 hours. This LH hormone is present in the urine. so it is basically a urine test. Take an ovulation kit, put a drop of urine, see whether it is showing positive result. If it is negative please repeat it after a day or two. Once it shows positive that means you are going to ovulate in another 24 to 36 hours. So you will have to time your sex during that time because the semen is active in the uterus for 24 to 48 hours. If the ovulation happens after 24 hours when it reaches the uterus the egg is active only for 12 to 24 hours. So by the time the egg reaches the uterus semen will already be there waiting for fertilization. That is one of the easy and simplest method to find out whether you are ovulating or not and how to time your sex during ovulation. Suppose you don’t want to get your ovulation kicked and you want to depend on some simple method the second one is called as a bbt chart or a basal body temperature chart. This is a chart where every day early morning your temperature is measured by a thermometer and put it in a graph. Normally just before ovulation there will be decrease in temperature by half a degree and then it rises a little more. If you keep doing you will come to know when there is a decrease in the temperature and there is a rise. Then you know this is the time you are going to ovulate and you should plan your sex during that time or 4-5 days. It all depends on the accuracy with which you record your temperature and you put it in a graph. The third much more simpler method is normally the cervical mucus i.e. the discharge from the cervix is very thin after periods. It starts becoming thicker during ovulation. So when you test the cervical mucus in case the mucus becomes sticky that means that is the time you are going to ovulate. These are the simple methods. More reliable would be like to go to a doctor, get a follicular monitoring that is done by an ultrasound scan. Doctor will do the follicular scan starting from the eighth day of your periods to see the size of your follicle. The follicle will have to grow up to 18 to 20 millimetres. That means your egg is ready to be released. More reliable method by which you can plan your sex and plan for a pregnancy.

Dr Randy Morris MD-The BOARD CERTIFIED fertility expert with weekly TTC tips on InfertilityTV Women with PCOS have more difficulty getting pregnant. The main reason is that they don’t ovulate. Getting women with PCOS to ovulate can be tricky. Luckily, there are a number of options A common reason why women with PCOS don’t ovulate is insulin resistance. Methods to reduce insulin resistance can often get ovulation to occur. Three ways to reduce insulin resistance are weight loss, exercise and medications. The most commonly used medication in PCOS patients to reduce insulin resistance is called metformin. Fertility medications are also used to induce ovulation. Most people are familiar with a fertility pill called Clomid. However, many women with PCOS do not respond to Clomid. A better choice is another pill called letrozole. Injections of fertility medications can also be used to get ovulation to occur but be careful, there is a much greater risk for multiple pregnancy with injections. Finally, surgery on the ovary can be used to induce ovulation. This surgery is performed through laparoscopy and uses a laser or electric current to vaporize the cells in the ovary that are responsible for some of the hormone abnormalities in PCOS. If a woman can get pregnant but keeps having miscarriages or still birth, it still means infertility and PCOS is a hormonal disorder causing enlarged ovaries with small cysts on the outer edges. Polycystic Ovary Syndrome, or PCOS, is a hormonal disorder that affects 5-10% of all women. It is one of the most under-diagnosed diseases in the United States and is a leading cause of infertility in women. Most women with PCOS don't even know they have it and are not diagnosed until they seek help getting pregnant. In women with PCOS, the ovaries may make more androgens, the dominant male hormone, than is normal. The high levels of androgens affect the development and release of eggs, preventing ovulation and resulting in infertility. Although the exact cause of PCOS is not known, it is thought to be genetic and may run in families. Environmental factors such as diet also play a role in its development. Symptoms of Polycystic Ovary Syndrome can vary from woman to woman. Due to the increase of certain hormones, such as testosterone, some women may develop certain characteristics, such as excess facial or body hair, a deeper voice, or thinning hair. Other, more common symptoms of PCOS include irregular ovulation and menstruation; increased acne on the face, chest and back or oily skin; weight gain or obesity; depression; and infertility. Women with PCOS may also have other health problems that are linked to weight gain, such as diabetes, high blood pressure, and high cholesterol. In order for a woman to be diagnosed with Polycystic Ovary Syndrome, she must have at least two of the following symptoms: irregular menstruation; signs of increased male hormone levels such as acne or excess hair growth in certain areas; or ovaries that appear polycystic on an ultrasound examination. There isn't one specific test for PCOS. To make a diagnosis, your doctor will review your symptoms, medical history, physical exam and perform tests to rule out any other conditions that might explain your symptoms. It is important to know that, although the name for this condition might suggest that women with PCOS have ovarian cysts, this is not the case. In this condition, the so-called cysts refer to the many tiny egg follicles that usually are detected by ultrasound examination. Surgical removal of these follicles is rarely indicated. Although treatments for Polycystic Ovary Syndrome do not cure the disease, they can help improve the symptoms. Oral contraceptive pills may be prescribed to correct the hormonal imbalances. They may also help regulate menstrual cycles and decrease acne and excess hair growth. For women who are overweight or obese, diet and exercise to maintain a normal body weight will ease many of the symptoms and improve the chances for a happy outcome. Women who have PCOS generally do not ovulate, which can cause infertility. There are multiple treatment options for women who have PCOS and do not ovulate regularly. These options include oral medications, injectable medications, surgery, and sometimes IVF. A reproductive endocrinologist will evaluate each patient individually and together with the patient they will decide which treatment is best in each specific scenario.
